She ultimately sought the help of a counselor. It was not until her most recent pregnancy that she also started to use an AI chatbot on the fem-tech app Soula, which she said helped with reducing depressive symptoms and reassuring her about her mental health."It tells me: 'Welcome back, LeRonika. Your reactions are natural,'" said Francis, who is based in New Iberia, Louisiana.The AI chatbot called "Dua" also sent her some videos that she could watch about self-hugging, a practice that could reduce stress levels."I feel like Dua is there more than anything. It really showed me: OK, this is what you could do. Make sure you make time for yourself, too, because that's important as well," said Francis.Soula was co-founded by Natallia Miranchuk and Andrei Kulik, who conceptualized a 24/7 AI doula that could close the gap in mental health support for mothers during pregnancy and postpartum periods. Rather than replacing medical providers, apps such as Soula aim to increase broader access to maternal healthcare. Healthcare providers said that these apps could also help reduce the burden on the healthcare system, where wait times can be long and <a target="_blank" href="https://www.businessinsider.com/new-york-nurses-strike-labor-union-staffing-shortage-covid-2023-1">workers face burnout</a>.Business Insider spoke with OB/GYN doctors and doulas, who largely welcomed the rise in AI chatbots because they can make information more accessible to patients.According to data obtained from Sensor Tower, Soula now has more than 35,000 downloads. The startup has fundraised over $750,000 and is backed by investors such as Andrey Mikhaylyuk, the former vice president of product at <a target="_blank" class="" href="https://www.businessinsider.com/unicorn-pitch-deck-flo-health-raises-200-million-series-c-2024-7">Flo Health</a>, a period tracking app.Miranchuk told BI that her experiences as a single mother of two and her work as a former maternity photographer inspired her to develop a supportive app for women. AI therapy apps such as <a target="_blank" href="https://www.businessinsider.com/wysa-mental-health-startup-raises-fresh-vc-funding-expand-coverage-2021-5">Wysa</a> and Woebot received over 1 million downloads, the data said.Corenia Smith, director of the Birth Justice Collaborative in Minnesota, said that while popularizing tools like chatbots can help broaden healthcare access, it is important to avoid a "one size fits all" and ensure that AI can reflect the communities it serves."There are cultural practices that exist and have existed for thousands of years that people still use," Smith said. "A human could talk about all their experience and what they've seen and give referrals. While there's Medicaid coverage for doulas in several states, not all private health insurance plans cover these expenses. <a target="_blank" href="https://www.sciencedirect.com/science/article/pii/S0266613821003120">Long wait times and socioeconomic inequality</a> also contribute to increased levels of stress and lack of healthcare usage among mothers and pregnant people."Traditionally, it's been something that's been available only to folks with a lot of money and a lot of resources. The people that benefit the most from doula care are those that might not trust the healthcare system," said Dr. Devon Rupley, an attending physician and OB/GYN specialist at NYC Health. For $22 per hour, she helped improve the chatbot by using it "hand-in-hand" during her pregnancies, adding in her expertise.While asking questions on Google could yield more informational responses, Pacheco said mothers would want something that provided "more of that reassurance that Google wouldn't."For Armani Grant, a Soula user who became pregnant at 18, Google felt overwhelming. The chatbot allowed her to ask questions that she wouldn't feel comfortable asking her mother. Pacheco said many women she's worked with reported similar experiences."They might even feel incompetent to ask the question, which no woman should ever feel, but with my history of working with people, they definitely tend to revert to: I'm sorry, I'm asking this question for some reason, and that might just be because of the societal pressure that more medicalized doctors have put on people that they don't give the time and day to us," Pacheco said.The app isn't meant to completely replace a doula. Moreover, retention rates of AI healthcare chatbots can be low, said Johannes Eichstaedt, a computational social scientist and assistant professor of psychology at Stanford University. "Yes, breastfeeding and having children younger may help to reduce your risk of breast cancer. But that alone isn't why rates are increasing," she said, adding that lifestyle and environmental factors also play a role in the increased number of cases. But only one in four US infants were exclusively breastfed for more than six months in 2019, a 2023 study published in Frontiers in Public Health found.The risk of breast cancer decreases by around 4% for every 12 months of breastfeeding, according to Breast Cancer UK.A possible explanation is that breastfeeding reduces a person's total number of lifetime menstrual cycles. For the study, published in The <keyword id="22729707" type="General" weightage="20" keywordseo="journal-of-clinical-endocrinology-and-metabolism" source="keywords">Journal of Clinical Endocrinology and Metabolism</keyword>, the authors looked at results from 20 cross-sectional studies across nine countries. Data from close to 29,000 women with PCOS and over 2.5 lakh women not having the condition were included in the analysis. "Individuals with PCOS had higher odds of any eating disorder, which persisted in studies where PCOS was diagnosed by Rotterdam criteria," the authors wrote. According to the Rotterdam criteria, women are diagnosed with PCOS when they have two of these three key features -- increased numbers of ovarian follicles containing immature eggs (called polycystic ovaries) seen on ultrasound, slightly higher levels of testosterone or related symptoms like excess body hair, and irregular or no menstrual periods. Looking at the body mass indices (<keyword id="20014829" type="General" weightage="20" keywordseo="BMI" source="keywords">BMI</keyword>) of women, the researchers found that both women with normal and those having a higher weight had a higher risk of disordered eating, suggesting that the risk of developing eating disorders did not depend on BMI. "Our findings emphasise the importance of screening women with PCOS for eating disorders before clinicians share any lifestyle advice," Cooney said. "The lifestyle modifications we often recommend for women with PCOS -- including physical activity, healthy diet and behaviour modifications -- could hinder the recovery process for eating disorders. Health care providers need to be vigilant about screening for eating disorders in this population," Cooney said.

This was only one year after Spain had granted the island the power to self-govern. But after Spain lost the war, they ceded control of Puerto Rico.At the time, <a target="_blank" href="https://archive.org/details/reportoncensusof00unitiala/page/40/mode/2up?view=theater">Puerto Rico's population</a> was just under 1 million. Most were living in poverty. It wasn't uncommon for families to spend the majority of their income on food. Many people worked for only cents per day harvesting sugarcane. Others owned farms or worked inland on coffee and tobacco fields."Imagine that about 90% of your income is spent on food, and you're still malnourished. There's no money for clothes. Shoes were basically non-existent among the peasantry. "So imagine a doctor in Puerto Rico in the 1930s telling a woman from the peasantry that she should have or recommend that she get sterilized. There is no way for them to say no."Sterilization or "<a target="_blank" href="https://www.thenation.com/article/society/ourmotherstoo-reckoning-with-my-abuelas-coerced-sterilization/">La Operación</a>" as it was called, was one of the only options for birth control women had available to them in the early 20th century. In 1950, the average Puerto Rican woman had given birth to an average of <a target="_blank" href="https://www.jstor.org/stable/2134751?read-now=1&amp;seq=1#page_scan_tab_contents">5.2 children</a> by the age of 50. On one side were the Catholic Church and Puerto Rican nationalists, who opposed sterilization. The Catholic Church took their position on birth control based on religious grounds. The nationalists feared that the US as a colonial power had plans of physical and cultural genocide of Puerto Rico.On the other side of the sterilization debate was the US federal government, the Puerto Rican government, public health officials, and private elite, who held the belief that the underdevelopment of the island was due to poverty and in order to develop the island, the population had to shrink.Even within the ranks of those who supported sterilization or birth control, there were progressive and conservative factions. Progressive factions supported some amount of birth control and sterilization for those living in poverty, but they didn't agree with the genetic inferiority argument that many conservative eugenicists believed. Progressive groups wanted to institute birth control, but they also wanted to strengthen education, combat crime, and develop welfare benefits for poorer families, Dr. Laura Briggs, Professor at the University of Massachusetts Amherst, wrote in "<a target="_blank" href="https://www.ucpress.edu/book/9780520232587/reproducing-empire">Reproducing Empire</a>."Dr. Iris López, a sociology professor and director of Latin American Studies at City College of New York and author of the book "Matters of Choice: Puerto Rican Women's Struggle for Reproductive Freedom," said most people who were a part of the birth control movement held a progressive ideology."The sterilization in Puerto Rico had to do more with the whole overpopulation ideology versus the general view of eugenics that people are targeted, genetically inferior, or unfit to reproduce," López said. While today it may be possible for women to undergo surgical reversal to try to become pregnant, the reversal process was not pioneered until the <a target="_blank" href="https://www.tubal-reversal.net/blog/history-of-tubal-reversal-surgery/">1970s</a>. For most Puerto Rican women, sterilization was permanent.Because of the colloquial language physicians used telling women they needed to get their "tubes tied," some women were under the <a target="_blank" href="https://www.jstor.org/stable/20628205">misconception</a> that the procedure would be <a target="_blank" href="http://palante.org/Women%27s%20Position%20Paper.pdf">reversible</a>.As López writes in her book, while sterilization in many cases was not forced onto Puerto Rican women, many women felt they did not have an alternative because of government policy and the social conditions they were subjected to. Some factories may have also <a target="_blank" href="https://journeys.dartmouth.edu/apulidolats3/2018/04/22/reproductive-genocide-a-look-into-sterilization-in-puerto-rico-and-east-los-angeles/">encouraged</a> their employees to be sterilized to reduce turnover.While <a target="_blank" href="https://pubmed.ncbi.nlm.nih.gov/12279870/">projected rates</a> of sterilization on the island between 1930 and 1970 vary somewhere between 30-40%, López said there were higher rates of sterilization amongst married women. A 1982 <a target="_blank" href="https://pubmed.ncbi.nlm.nih.gov/3259513/#:~:text=21%25%20of%20these%20women%20expressed,being%20consulted%20about%20the%20operation.">survey</a> found that 42% of married women interviewed said they had been sterilized.The sterilization law and eugenics board were <a target="_blank" href="https://www.jstor.org/stable/2633177">repealed</a> in Puerto Rico in 1960, but high rates of sterilization continued on the island until the late 1980s, according to a Family Planning Perspectives study released in <a target="_blank" href="https://www.jstor.org/stable/2135873?origin=crossref">1992</a>.Today, the effects of the economic development plan that led to the birth control movement reverberate on the island. Puerto Rico's population is <a target="_blank" href="https://academic.oup.com/gerontologist/article/62/7/964/6607773">aging rapidly</a>, and people have continued to <a target="_blank" href="https://www.census.gov/library/stories/2019/09/puerto-rico-outmigration-increases-poverty-declines.html">move away</a> from the island in recent years in the wake of Hurricane Maria and as the economic situation worsens. The average number of births per woman was <a target="_blank" href="https://data.worldbank.org/indicator/SP.DYN.TFRT.IN?locations=PR">0.9 in 2021</a>."The Puerto Rican age, on average, is a huge problem," Franqui-Rivera said. "Even when the labor market was so good, incredibly good, it was a tough labor market."

In recent years, we've seen some outspoken <keyword id="20958966" type="General" weightage="20" keywordseo="female-athletes" source="keywords">female athletes</keyword> used as props to push political agendas, most notably Megan Rapinoe, the women's soccer player who became a target for her outspoken hatred of Donald Trump, and Brittney Griner, the WNBA player who was wrongfully imprisoned in Russia for most of 2022. Clark is different. She, herself, does not talk politics. She shoots threes and dishes dimes. She attends her contractual media interviews, and <a target="_blank" href="https://www.wsj.com/sports/basketball/caitlin-clark-nike-deal-415ee8ec">fields offers from the world's biggest shoe brands</a> who have been trying to partner with her since she was at school. But it's getting tougher for Clark (and the rest of the WNBA) to focus on work alone. Fouls that wouldn't even be a footnote in NBA coverage — fouls that other women across the WNBA commit and take every game — are suddenly talking points. Male sports commentators who have never given the WNBA airtime are dedicating<a target="_blank" href="https://www.the-sun.com/sport/11661889/angel-reese-skip-bayless-undisputed-caitlin-clark-wnba/"> multiple </a><a target="_blank" href="https://awfulannouncing.com/espn/stephen-a-smith-players-jealous-caitlin-clark.html">segments</a> to Clark's Black opponents, calling them jealous bullies who are out to hurt her. Matt Leinart, a former football player-turned-media personality, <a target="_blank" href="https://x.com/MattLeinartQB/status/1802403432076296466">said</a> on X that the WNBA should suspend Reese for an unintentional foul because it was "bad for the game." ESPN's Pat McAfee, a football pundit, compared Clark to Eminem in an inflammatory rant, <a target="_blank" href="https://www.si.com/wnba/pat-mcafee-caitlin-clark-controversial-rant">saying</a> that people don't want to give her credit because she's white. Many Black players, including Clark's own teammates, <a target="_blank" href="https://x.com/_Breezy_Briii/status/1798537220628107660">spoke up</a> about experiencing excessive racist abuse on social media in recent weeks. When <a target="_blank" href="https://www.nytimes.com/athletic/5563352/2024/06/14/caitlin-clark-wnba-indiana-fever-dijonai-carrington-basketball/">asked</a> about her name being used "as a weapon in the culture wars dividing the country," Clark initially side-stepped the question. "I don't see it. That's not where my focus is," she said. "My focus is here and on basketball. That's where it needs to be, that's where it has been, and I'm just trying to get better on a daily basis." Some WNBA fans and players took issue with that response. And their compensation falls short, too. (Two weeks later, Nike said A'ja Wilson, a Black two-time MVP who has been with Nike since she was the top draft pick in 2018, will have a signature shoe in 2025.) But this moment, which has been building for decades, is being undercut by commentators and brands who are fixated on Clark. All these projections — from Clark's supporters and detractors — infantilize every player on the court. She is strong, and played the best game of her WNBA career so far on Sunday. She's working hard to diversify her game, to adjust to the next level, on and off the court. In one of her most recent interviews, Clark got more direct than ever. "Everybody in our world deserves the same amount of respect," Clark told James Boyd of <a target="_blank" href="https://www.nytimes.com/athletic/5564661/2024/06/14/caitlin-clark-agenda-comments/">The Athletic</a>. "The women in our league deserve the same amount of respect, so people should not be using my name to push those agendas." She's growing. It's time for the rest of us to do the same.
