Metaverse for mental healthcare? Experts ‘cautiously optimistic’ after WEF report

New Delhi, Apr 15 () As the government extends the lockdown, non-governmental organisation Smile Foundation on Wednesday said it plans to reach out to 40,000 families in the next one month across 14 states for providing psychological counselling through tele-calling. A pilot phase of tele-counselling, which started a week ago in Howrah, Kolkata, scanned about 250 patients a day and the programme is now going national, it said. "With the lockdown being extended, our beneficiaries need us more than ever to be there for them. Through this directly connecting with beneficiaries, we aim to create a sense of security and also help in addressing their fears," Smile Foundation co-founder and executive trustee Santanu Mishra said in a statement. Suicidal tendencies, fears and anxieties have gripped the poor and migrant labour populations who have been the worst affected by the sudden decision of lockdown by the government to contain the spread of coronavirus, he said. Smile Foundation, which works on primary healthcare, has drawn out a plan to reach out to these marginalised populations and help them fight their fears and anxiety in this period of lockdown through mental health counselling. In the coming days, the state governments will be scaling up screening operations in coronavirus-hit states namely Maharashtra, Delhi-NCR, Rajasthan, Tamil Nadu, Andhra Pradesh, Telangana, Karnataka, Jharkhand, Orissa, Chhattisgarh, and Uttar Pradesh. "Over the next couple of weeks, trained counsellors, doctors, nurses from Smile Foundation will be making several thousand calls to all beneficiaries within our network and partner organisations to ease out their angst caused by the spread of the deadly coronavirus," Smile Foundation Health Lead Satnam Singh said. Last week, the Foundation started its first phase of ration distribution reaching out to 40,000 families across 10 states. Over the coming weeks, the foundation aims to reach out to another 1,10,000 families across 19 states and help them meet their daily nutrition needs, as well as sanitation needs like masks, soaps, sanitary napkins and sanitisers, the statement added. COVID-19 lockdown: Smile Foundation plans mental health tele-counselling in 14 states

Analysis shows that children in the age group of 0-14 years are less affected by <keyword id="8763838" type="UNKNOWN" weightage="20" keywordseo="covid-19">COVID-19</keyword>, Union Health Minister Harsh Vardhan informed the Lok Sabha on Friday. He said in a written reply that infection among children is mild and most of them remain asymptomatic. "Hence no specific action plan has been formulated in the current response phase to minimise the adverse effects of COVID-19 on such children," he said. Very rarely a severe illness, in form of multi-system inflammatory syndrome, has been identified, Vardhan said, adding this happens usually three to six weeks after the infection. The Department of Pediatrics, AIIMS, is documenting the long term effect of COVID-19 infection in children, he said. The minister was responding to a question on whether the government had formulated any action plan to control and minimise the adverse effects of COVID-19 in children aged 0-14. For appropriate management of COVID-19 cases, a three-tier arrangement of health facilities has been implemented -- <keyword id="8995444" type="COMPANY" weightage="20" keywordseo="covid-care-centre">COVID Care Centre</keyword> with isolation beds for mild or pre-symptomatic cases, Dedicated COVID Health Centre (DCHC) with oxygen supported isolation beds for moderate cases and Dedicated COVID Hospital (DCH) with ICU beds for severe cases, he said. Guidelines on clinical management of COVID-19 have been issued, Vardhan said. They also provides for management of complications. On whether the government has made any assessment of adverse effects of COVID-19 on <keyword id="6354774" type="ORGANIZATION" weightage="20" keywordseo="Mental-health">mental health</keyword> of children and has provided psychological support through healthcare professionals, Vardhan said the <keyword id="263716" type="COMPANY" weightage="20" keywordseo="ministry-of-education">Ministry of Education</keyword> has initiated a programme called 'manodarpan' covering a wide range of activities to provide psycho-social support to students, teachers and families for mental health and emotional wellbeing. The standard operating procedure for health and safety protocols for reopening of schools issued by the Ministry of Education has incorporated guidelines for promoting emotional well-being of students and teachers, the minister said. The <keyword id="210071" type="COMPANY" weightage="20" keywordseo="Ministry-of-Health">Ministry of Health</keyword> set up a 24/7 helpline through NIMHANS, Bangalore, on March 29, 2020 to provide psycho-social support to the affected population, divided into different target groups, Vardhan said. Two central mental health institutions -- namely LGB Regional Institute of Mental Health, Tezpur and Central Institute of Psychiatry, Ranchi -- along with various states and UTs were also on-boarded to extend the services in regional and local languages. He said guidelines and advisories on management of mental health issues, catering to different segments of the society, were issued. Merely 8% of the respondents knew if their health insurance covers mental health. </li><li>India witnesses nearly 28% of global suicides despite accounting for 18% of the population worldwide.</li></ul>As India observes the <keyword id="57200" type="ORGANIZATION" weightage="20" keywordseo="World-Mental-Health-Day">World Mental Health Day</keyword> on October 10, a recent study of “Access to care, awareness &amp; attitudes towards mental health in India” says that nearly one in six people suffer from some form of mental health issues. Yet, the country lacks awareness on mental health and related disorders. Over half of the Indians are unlikely to recognise the symptoms of mental disorders and people suffering from mental illness, shows the analysis from the study published by Cosmos Institute of Mental Health and Behavioural Sciences (CIMBS), and World Federation for Mental Health (WFMH). “Surprisingly, even amongst those who had close interactions with a person with mental illness, 35% thought mental health issues were uncommon suggesting conversations on mental illness remain taboo. This could possibly be explained by 41% of those who had a person with mental illness amongst their near and dear ones feeling that other people were insensitive and likely to ridicule a patient,” said Dr Sunil Mittal, who was the principal investigator of the study. The National Mental Health survey of 2016 predicts that roughly 13.7% lifetime prevalence of mental illness among Indians. And, over 150 million are in need of active intervention. However, merely one in every four individuals felt that the government provided adequate support to persons with mental illness. “The nature of the caregivers’ role is invisible and complex, much like mental illness itself. The entire journey is laden with fears, insecurities, trauma, isolation and dysfunctionality. To add to it, the new laws do not cater to the concerns and needs of caregivers and their voice,” said Mitali Srivastava, Senior Psychologist at CIMBS. Lack of awareness and suicides Even when they aware, Indians do not know how to cure themselves. Merely 8% of the respondents knew if their insurance covered mental health. India witnesses nearly 28% of global suicides despite accounting for 18% of the population worldwide. Yet, almost 30% people did not consider suicide to be associated with mental illness — which explains the less likelihood of intervention. “There is a need for well-thought guidelines for a participatory approach to encourage non-governmental and private entities, and address concerns regarding medico-legal issues in mental health service delivery. Without which, it is difficult to bridge the huge treatment gap that exists today. Also, innovation and adoption of technology like tele-medicine need to be promoted,” said lawyer Mrinal Kanwar. However, a definite plan on how this will pan out for all corners of India remains to be seen. Moreover, digital will also be leveraged in the set-up and operation of tele-mental health centres, providing the necessary focus and access to mental health services to the Indian masses. This comes as one of the most welcome move as it’ll contribute infinitely to the nation’s long-term growth.
