As per the first information report (FIR), the accused did not provide helmets or other safety equipment to the deceased while employing them to dig a well. They died after inhaling gas from the well, it stated. A case was registered against Jashabhai Keraliya, Janak Aniyaria, Khimjibhai Saradia, and Kalpesh Parmar, the official said. Bodies of the deceased labourers have been sent for post-mortem, he added. Earlier in February, three workers died after inhaling toxic gas that emanated after a blast triggered by gelatin sticks for an <keyword id="19932636" type="General" weightage="20" keywordseo="illegal" source="keywords">illegal</keyword> mining operation in the district.

<ul class="summary-list"><li>The National Transportation Safety Board has more concerns about Boeing's planes.</li><li>It said at least 40 airlines outside the US may be operating aircraft with risky rudder parts.</li></ul>The National Transportation Safety Board (NTSB) has flagged fresh concerns with Boeing's 737 planes, saying that at least 40 airlines outside the US may be operating aircraft with faulty components.The NTSB issued a <a target="_blank" class="" href="https://www.ntsb.gov/news/press-releases/Pages/NR20240926.aspx">press release</a> on Thursday, saying it had issued urgent safety recommendations to Boeing and the Federal Aviation Administration (FAA).The release said that the NTSB was investigating a February 6 incident with a United Airlines Boeing 737 Max 8 plane, which experienced "stuck" rudder pedals while landing at Newark Liberty International Airport in New Jersey.The Board said that its investigators tested a rollout guidance actuator — one of the rudder control components from the affected airplane — at the component's manufacturer, Collins Aerospace.When tested with another identical unit from a separate airplane, both actuators failed. Moisture was detected in both units.Collins Aerospace later found that the sealed bearings on the units had been incorrectly assembled, "leaving the unsealed side more susceptible to moisture that can freeze and limit rudder system movement," per the release.The NTSB said that Boeing's 737 flight manual instructs pilots facing a jammed rudder to overpower the system by applying "maximum force." But the Board warned that such force on faulty rudders could "unintentionally cause loss of control or departure from a runway."NTSB Chair Jennifer Homendy wrote a September 30 <a target="_blank" href="https://www.ntsb.gov/news/Documents/Boeing%20Urgent%20Safety%20Recommendations%20ltr%20to%20FAA%20Administrator.pdf">letter</a> to the FAA's administrator, Michael Whitaker, saying she was concerned that the FAA "did not take this issue more seriously until we issued our urgent safety recommendation report." She added that she had been made aware the "FAA has been downplaying the urgency of this issue, maintaining that the units are no longer in service."Homendy said that Boeing had said that it had received 353 affected actuator units since February 2017.Per Homendy's letter, 271 affected actuators may now be installed on aircraft in service operated by at least 40 foreign air carriers. She added that 16 units "may still be installed on US-registered aircraft." Homendy also slammed Boeing for failing to inform United Airlines that faulty actuators were installed in the 737 planes it had delivered to them."We are concerned of the possibility that other airlines are unaware of the presence of these actuators on their 737 airplanes," Homendy wrote."Not making operators fully aware of the installed systems and equipment on the airplanes delivered to them is unacceptable and cannot continue to be tolerated," she added.<h2 id="83df9e8e-4477-4580-94cb-f7372a1e0d82">A bad look for Boeing</h2>Guy Gratton, an associate professor of aviation and the environment at Cranfield University, told BI that component faults reflect poorly on Boeing."It's another instance of national authorities in the USA (the Federation Aviation Administration, and the National Transportation Safety Board) stepping in to address safety issues that Boeing themselves should have already identified and rectified," Gratton said. "I'd anticipate their customers worldwide being disappointed in that."However, he added that he thought this issue was not as serious as Boeing's past safety issues, like the January 5 incident when the door plug on an <a target="_blank" rel="" href="https://www.businessinsider.com/alaska-boeing-737-max-9-door-blowout-company-response-earnings-2024-1">Alaska Airlines passenger plane</a> — a 737 Max 9 model — detached during a flight, forcing pilots to make an emergency landing."This fault could not in itself lead to loss of an aircraft, and even an aeroplane going off the side of a runway, whilst extremely embarrassing and expensive, would be unlikely to lead to loss of life," Gratton said.He added: "I'd still be content to get into a Boeing 737, and I do — but it does continue to show the company as being troubled, and its products less trustworthy than those of the obvious competitors."<h2 id="c23a3048-4c95-4c5b-8c89-bc8188352509">Latest blow to an already beleaguered Boeing</h2>Boeing shares dropped 2.74% on Monday. It's also worth noting that the NTSB probe is the latest setback for the company. Their <a target="_blank" class="" href="https://www.businessinsider.com/why-boeing-737-max-planes-have-issues-2024-1">Max family has been plagued by safety concerns</a> since the two crashes of its 737 Max jetliners in 2018 and 2019 that killed a total of 346 people.And following the Alaska Airlines door plug incident, the FAA <a target="_blank" class="" href="https://www.faa.gov/newsroom/faa-halts-boeing-max-production-expansion-improve-quality-control-also-lays-out-extensive">barred</a> Boeing from expanding production on Max plane models until quality and safety issues were addressed.A string of <a target="_blank" class="" href="https://www.businessinsider.com/boeing-whistleblower-says-he-saw-holes-drilled-wrongly-on-787s-2024-6">whistleblowers</a> have also alleged this year that the company is lax with safety and quality control. Some bombshell allegations were publicized in a <a target="_blank" rel="" href="https://www.hsgac.senate.gov/wp-content/uploads/2024.06.17-PSI-Majority-Staff-Memorandum.pdf">sprawling 204-page report released on June 17</a> by the Senate subcommittee that investigated Boeing's safety and quality practices.These accounts include testimony from Sam Mohawk, a quality assurance inspector for Boeing, who alleged that the <a target="_blank" rel="" href="https://www.businessinsider.com/boeing-whistleblower-faulty-plane-parts-may-be-on-new-aircraft-2024-6">company lost track of hundreds of faulty 737 parts</a> and ordered staff to conceal improperly stored plane parts so FAA inspectors would not see them.Merle Meyers, a former Boeing quality manager, said that Boeing's manufacturing team regularly tried to retrieve bad parts from a "reclamation" area even after they were thrown out.Boeing declined to comment on BI's queries, saying it would defer to the NTSB on the investigation. The FAA didn't immediately respond to a request for comment from BI, sent outside business hours.

Boeing's 737 safety mess just got worse

The UK-based group also accorded two stars to <keyword id="21402083" type="General" weightage="20" keywordseo="Honda-Amaze" source="keywords">Honda Amaze</keyword> for adult and zero stars for child occupant safety. It also gave three stars in adult safety and five stars for child occupants to <keyword id="20774736" type="General" weightage="20" keywordseo="Kia-Carens" source="keywords">Kia Carens</keyword> after the latest round of crash tests. The highest score on a vehicle crash test is five stars, while the lowest score is zero stars. "We were disappointed to see Mahindra's one star rating for the Bolero Neo. This falls well below the safety performance we have come to expect from the manufacturer," Global NCAP Secretary General Alejandro Furas said in a statement. Mahindra &amp; Mahindra said the company is committed to deliver vehicles that ensure the safety and satisfaction of customers and users. Bolero Neo has always complied with safety regulations that have been introduced over time and continues to be fully compliant with the latest Indian safety standards, it said. "As we are constantly innovating and improving our vehicles to exceed safety regulations, we want to assure our customers and stakeholders that Mahindra has significantly enhanced safety features in all our recent launches," the auto major stated. Models like Thar, XUV700, XUV300, and Scorpio-N have been recognised with high safety ratings of 4- and 5-star by the Global NCAP, reflecting the company's ongoing commitment to safety. Furas said there has been an improvement in Kia's rating for the Carens since its original test when the model scored a zero rating for adult occupant protection. "However, we were expecting a better performance in the retest given the Carens is fitted with six airbags as standard," he added. On Honda Amaze, Furas stated: "Regrettably Honda did not take the opportunity to update the Amaze with more safety features which explains its disappointing score." Commenting on the development, Honda Cars India said the South Africa spec second generation Amaze has already been tested as four star by GNCAP in 2019. "The latest test basis new protocol shows that the total score is of five-star level. "However, mainly due to the requirement of certain equipment like electronic stability control and side curtain airbags, it resulted in lower rating," the automaker said in a statement. Global NCAP accords low safety rating to Bolero Neo, Amaze

As a former airline safety professional and current frequent flyer, I was surprised after watching the <a target="_blank" href="https://www.youtube.com/watch?v=BaN5sEgwF7k">JAL flight safety video</a>.I have yet to fly Japan Airlines, but <a target="_blank" href="https://www.insider.com/economy-class-airlines-compared-best-worst-2023-10">I have flown on dozens of other carriers</a> — both in the US and internationally — and it was refreshing to see how specific the evacuation instructions were.Of particular interest was the emphasis on <a target="_blank" href="https://www.businessinsider.com/japan-airlines-passengers-survived-because-abandoned-baggage-burning-plane-tokyo-2024-1">passengers not taking any luggage with them</a> if they need to evacuate. The JAL video underlines how this can slow deplaning and damage the evacuation slide. And the information is paired with a clear visual of the consequences of not following crewmember instructions.It appears the passengers on the <a target="_blank" href="https://www.businessinsider.com/japan-jet-crash-passenger-says-people-stayed-calm-fire-spread-2024-1?r=US&amp;IR=T">JAL flight abandoned their luggage</a> just as instructed. That discipline, supported by the training and actions of the flight crew, likely helped save everyone's life."All passengers and crew members on our flight successfully performed an emergency evacuation," JAL said in a recent statement. And I doubt passengers will either.The video, released on Thursday, is the latest in United's series of entertaining briefings that attempt to combine required safety and fun into a heavily regulated part of flying.The mini-movie follows a shiny blue ball as it snakes its way through lively and colorful sets that focus on key safety information, like <a target="_blank" href="https://www.businessinsider.com/latam-boeing-787-plunged-reminder-always-wear-seatbelt-turbulence-safety-2024-3">seatbelt usage</a> and how to use the oxygen mask. In a press <a target="_blank" href="https://www.prnewswire.com/news-releases/united-airlines-debuts-new-onboard-safety-video-302147075.html">release</a>, United said the video was designed with the goal of capturing the focus of passengers clearly and succinctly in hopes the creative scenes and transitions will help people pay more attention to its emergency instructions — especially <a target="_blank" href="https://www.businessinsider.com/frequent-flyer-programs-aadvantage-loyalty-program-competition-for-couples-2024-3">frequent flyers </a>who have likely memorized the spiel by now."The safest safety video is one that people will want to watch even on your 45th viewing," United's creative director, Meg Mitchell, who oversaw production, told <a target="_blank" href="https://www.npr.org/2024/05/16/1251669132/united-rebooted-safety-video-distracted-passengers">NPR</a>. "People start to tune out. And so we wanted something that felt like you could watch over and over and over again and still want to pay attention to."However, the blue ball isn't just rolling slowly across the screen — it's bouncing across windows, knocking down dominoes, and being launched across seats. All the while, the crewmembers are simultaneously giving safety instructions and demonstrating the equipment.<h2 id="933ee0cc-159f-4b8a-9c89-cda3030a878a">I found the theatrics to be distracting</h2>As someone who worked in airline safety before becoming an aviation journalist, I understand United is using entertainment to get people to focus on the video instead of their phones.Despite good intentions, the video's in-your-face theatrics are just as distracting.Throughout the entire video, I found myself focusing more on where the ball was going next in its winding web of tracks rather than on the core safety message.For example, the ball overshadowed information like where the life jackets are located and how to use the exit doors. The seatbelt presentation was interrupted by a close-up of the ball instead of what the camera should focus on — the flight attendant giving the safety instructions.There was simply a lot going on, and I can't imagine passengers — particularly first-time flyers or those who only travel once or twice a year — absorbed the important safety elements.The video's launch comes shortly after the airline faced <a target="_blank" href="https://www.businessinsider.com/united-airlines-faces-tighter-scrutiny-after-several-safety-incidents-2024-3">heightened FAA oversight</a> after a string of safety incidents earlier this year.In a statement to Business Insider on Thursday, <a target="_blank" href="https://www.businessinsider.com/boeing-likely-not-to-blame-recent-safety-events-united-2024-3">United pointed to its collaboration with the agency</a> on the video, saying it believes it is one "passengers will actively watch.""We worked closely with the FAA throughout the production process to ensure the video aligned to their standards, culminating with the FAA's approval of the video," the airline said.<h2 id="151ac5cc-134a-4fec-9c0b-ceb08ce886f7">United and Japan Airlines' safety videos are polar opposites</h2>United's video piggybacks on a similar discussion I wrote about in January after a <a target="_blank" href="https://www.businessinsider.com/japan-airlines-fire-how-passengers-managed-to-escape-burning-plane-2024-1">Japan Airlines Airbus A350 caught fire in Tokyo</a>.<div type="image" title="Screenshot from Japan Airlines safety video shows an animated passenger wearing a life vest" height="612" width="1088" msid="110187162" hostid="261" sizebytes="17092"></div>All 379 passengers on board that plane survived, and the <a target="_blank" href="https://www.businessinsider.com/japan-airlines-jal-safety-video-watch-how-helped-save-lives-2024-1">airline's no-nonsense safety video likely helped</a>.Instead of humor and Hollywood-style sets, JAL takes a straightforward approach to its safety instructions, providing clear and concise information without any extra pizzazz that could distract from the core content.Notably, its warning to leave luggage behind during evacuations included a visual explanation of how to go down the slide — including arms crossed and heels off — and showed the consequences of failing to follow instructions.United, as well as many other Western carriers, lack this level of detail in their videos.Don't get me wrong; I do think there is room for creativity in an airline safety video so long as it clearly conveys the message — but, in my opinion, United's got lost in the theatrics this time.

However, in what many people call a miracle, all 379 <a target="_blank" href="https://www.businessinsider.com/japan-airlines-fire-how-passengers-managed-to-escape-burning-plane-2024-1">passengers and crew aboard the A350 jetliner survived</a>.The aircraft only had three of its eight emergency exit doors available for the evacuation, which took about 18 minutes in total.An investigation into the cause of the crash is underway. Still, it will likely be months before Japanese regulators release their preliminary findings, and it's impossible to know for sure the root cause before then.But information is already starting to emerge, with safety experts trying to piece together what happened. Former JAL pilot and aviation safety analyst Hiroyuki Kobayashi told <a target="_blank" href="https://www.youtube.com/watch?v=ttQ0ImRgI-0">Reuters</a> the cause was likely pilot error.A published air traffic control transcript said the <a target="_blank" href="https://www.businessinsider.com/japan-airlines-crash-transcript-coast-guard-captain-2024-1?mrfhud=true">Dash-8 pilot did not have clearance</a> to enter the active runway. According to the recordings, the instruction was to hold short of the runway, not enter, though the Dash-8 pilot — the one person in the plane who survived the collision — told investigators he believed he had clearance to take off.While this may look like a straightforward case of pilot error or negligence, a core causation model in aviation safety called the "Swiss cheese model" could tell a different story.<h2>The JAL air crash was likely caused by several smaller errors</h2>Air crashes are rarely caused by a single factor, but rather are a series of individual failures that collectively lead to an incident or accident.This is commonly illustrated through the "Swiss cheese" model. The "holes" represent the failures, and the slices of cheese represent the layers of protection put in place to prevent those failures from turning into a catastrophe.Using the method, safety professionals can more easily identify and address safety gaps before an event occurs, whether it be related to human error, technology, procedures, or everything in between."Airlines can be accused of cost-cutting in various areas, like putting more seats on airplanes and cutting back on amenities and services," travel analyst Henry Hardevelt told Business Insider. Fatigue, the hypothetical second hole, is commonly attributed to <a target="_blank" href="https://www.businessinsider.com/are-pilot-fatigue-rules-making-travel-delays-worse-2014-1">pilot error</a>.For the pilots of the A350, it's likely investigators will ask if they saw the Dash-8 on the runway during landing. However, the nighttime conditions — another possible hole — could have made it more difficult for them to spot the turboprop and avoid a collision.<div type="image" title="Swiss cheese" height="683" width="1024" msid="106596362" hostid="261" sizebytes="28888"></div>The earthquake may turn out to be a contributing factor in the JAL accident as well. "I'd be interested in what role that played in the accident and how the coast guard operates out of Haneda airport."The string of hazards doesn't stop there, though. Many large global airports have <a target="_blank" href="https://www.businessinsider.com/japan-jet-crash-runway-warning-lights-reportedly-werent-working-2024-1">"stop bar" lights</a> that indicates to pilots if a runway is clear. They're intended to stop planes from entering an active runway even if they think they have clearance.On the day of the accident, however, the lights at the intersection where the Dash-8 entered the runway were malfunctioning, according to a <a target="_blank" href="https://twitter.com/AviationSafety/status/1742858937379012661?s=20">notice to airmen</a> that went into effect on December 27. <ul class="summary-list"><li>Japan is still coping with the aftermath of a powerful earthquake earlier this month.</li><li>With its history of natural disasters, Japan is one of the countries best prepared for emergencies.</li></ul>Japan's Noto Peninsula is reeling from its most powerful earthquake in a century, but the death toll is remarkably low.The 7.6 magnitude earthquake struck the peninsula in central Japan on January 1, violently rattling buildings, triggering landslides, starting fires, and even forcing land upward to create new beaches on the coast.Such quakes can be incomprehensibly deadly. When a pair of tremors of similar magnitude <a target="_blank" href="https://www.businessinsider.com/turkey-syria-earthquake-death-toll-disaster-of-the-century-2023-2">hit Turkey and Syria</a> last year, the disaster resulted in at least 56,000 deaths. In 2010 in Haiti, a <a target="_blank" href="https://www.businessinsider.com/why-haiti-gets-earthquakes-tectonic-plates-2021-8">7.0 earthquake</a> and its aftershocks killed up to 300,000 people. In 2005, a 7.6 quake killed at least 79,000 <a target="_blank" href="https://www.businessinsider.com/massive-earthquake-strikes-afghanistan-2015-10">in Pakistan</a>.As of Friday, however, the death toll from Japan's latest temblor was 94 people, <a target="_blank" href="https://www.reuters.com/world/asia-pacific/japan-accept-earthquake-relief-us-only-nikkei-2024-01-05/">Reuters</a> reported.It's no coincidence. Just look at the <a target="_blank" href="https://www.businessinsider.com/photos-show-damage-of-japans-2011-disaster-2016-3">mega-disaster Japan experienced</a> in 2011, much bigger than this latest quake. That March, a 9.0-magnitude earthquake <a target="_blank" href="https://www.businessinsider.com/r-special-report-tsunami-evacuees-caught-in-30-billion-money-trap-2014-10">triggered a tsunami</a> and a nuclear power-plant meltdown.While over 18,000 people died, the <a target="_blank" href="https://www.worldbank.org/en/news/feature/2021/03/11/learning-from-megadisasters-a-decade-of-lessons-from-the-great-east-japan-earthquake-drmhubtokyo">World Bank</a> noted that Japan's history of preparedness helped many evacuate and likely saved lives.The situation on the Noto Peninsula is dire, to be sure. The January 1 quake may have been the strongest one felt in the region in over 100 years, according to <a target="_blank" href="https://affiliate.insider.com?h=78c307dc166559a9feb42cd4ed0c915313e37c21a03e940f84ae266fbd02e1d3&amp;postID=6597228fa62cdd3ed8f380bd&amp;postSlug=japan-earthquake-safety-disaster-preparedness-2024-1&amp;site=bi&amp;u=https%3A%2F%2Fwww.nature.com%2Farticles%2Fd41586-024-00010-1">Nature</a>. Over 200 people are unaccounted for and 30 villages remain inaccessible, while the rubble-ridden Wajima city is short of food and water for roughly 11,000 evacuees sheltering there, <a target="_blank" href="https://www.reuters.com/world/asia-pacific/japan-quake-death-toll-rises-73-search-survivors-enters-fourth-day-2024-01-04/">Reuters reported</a>.<a target="_blank" href="https://www.businessinsider.com/earthquake-osaka-japan-2018-6">Japan's quake resilience</a> isn't perfect, but experts say the nation is doing some things right: investing in infrastructure, enforcing strict building codes, and training its citizenry."Japan has lessons that they can give to everyone," Daniel Aldrich, a professor of political science and public policy at Northeastern University who studies disasters and civil society, told Business Insider.<h2>Japan invests in its buildings</h2>Japan sits on four tectonic plates, making it an extremely <a target="_blank" href="https://www.businessinsider.com/ap-magnitude-66-earthquake-hits-northern-japan-near-major-city-2018-9">seismically active country</a>. The plates' constant grinding can cause 1,500 earthquakes a year. As a result, Japan has been honing its earthquake preparedness for many decades.After a series of natural disasters following World War II, Japan enacted the Disaster Countermeasures Basic Act in 1961, which increased its investment and planning around earthquakes and floods.Japan now has some of the strictest building codes of any country in the world, <a target="_blank" href="https://www.nytimes.com/2011/03/12/world/asia/12codes.html">The New York Times</a> reported after the 2011 earthquake. Through decades of studying how the country's temblors affect its buildings, Japanese engineers have homed in on quake-resilient construction.Japan designs new buildings to rock side to side with an earthquake's movement, rather than staying still and letting that movement stress the structure.In tall buildings, giant pads made of rubber and steel help absorb some of the shaking from earthquakes, as do hydraulic shock absorbers. Many older structures were retrofitted to withstand disasters better.However, sometimes surviving the quake itself isn't enough. For example, BBC News reported that many older wooden structures in Wajima went up in flames after a fire broke out following the 2024 earthquake.Of course, Japan's wealth gives it an advantage over other quake-stricken countries. However, some of its seismic safety is more about mindset than money.<h2>Japan maintains a culture of emergency preparedness</h2>September 1 is Disaster Prevention Day in Japan. Officials designated the day in 1960 to <a target="_blank" href="https://www.businessinsider.com/disneyland-tokyo-eeyore-performer-stays-character-during-earthquake-warning-2024-1">prepare citizens for emergencies</a>.When he lived in Japan, Aldrich said he and his neighbors ran comprehensive drills for the day each year, practicing tourniquets, slings for broken arms, and how to spray house fires with a hose."Wherever you are in Japan, everyone has those drills," including children, Aldrich said. The government constantly communicates their importance," Aldrich said, adding, "Are you ready? What have you done? Do you know your neighbors? Can you help in a disaster? The government also recommends people keep an emergency kit ready at all times.That's crucial to survival because affected regions can be cut off from emergency services for days after a disaster, just as much of the Noto Peninsula is now.In those cases, Aldrich said, your neighbors are your first responders. In over a decade of researching disasters, he's found that social relationships are a major predictor of survival. He said that community earthquake drills teach people what to do and help them trust their neighbors.In 2007, the Japan Meteorological Society launched its "Earthquake Early Warning System" (EEWS). The J-Alert system can also broadcast to TVs, radios, and cellphones.Tokyo even has a <a target="_blank" href="https://www.theguardian.com/world/2023/jun/02/japan-vending-machines-to-automatically-offer-free-food-if-earthquake-hits">radio-equipped vending machine</a> that sounds an alarm to park-goers.<div type="image" title="Workers stand on a road where a car is stuck on a broken road after an earthquake in Japan" height="1500" width="2000" msid="106584887" hostid="261" sizebytes="286896"></div>Warnings come through a few seconds to a few minutes before the quake hits, giving people some time to get to a safer location. They're also crucial for rail travel. Japan Railways trains automatically halt when the earthquake detection system kicks in.In 2022, a bullet train derailed after a 7.4-magnitude quake, but none of the 78 passengers and crew were seriously injured, <a target="_blank" href="https://english.kyodonews.net/news/2022/03/f2bf62a8bb81-quake-hit-bullet-train-services-to-fully-resume-around-april-20.html">Kyodo News</a> reported. The train slowed significantly from its typical speed of up to 186 miles per hour, according to <a target="_blank" href="https://www.japantimes.co.jp/news/2022/03/18/national/tohoku-train-derailment-shinkansen/">The Japan Times</a>.<h2>Japan installs resilient infrastructure</h2>Much like its buildings, Japan has the experience and the money to fortify its infrastructure against earthquakes.During that <a target="_blank" href="https://www.businessinsider.com/japanese-economic-disaster-impact-2011-3">2011 disaster</a>, rapid response teams were able to quickly repair roads to help move supplies and assistance to affected areas. In one case, a torn-apart road took only six days to fix, <a target="_blank" href="https://www.theguardian.com/world/2011/mar/24/japan-disaster-reconstruction-road-recovery">The Guardian</a> reported.<div type="image" title="Japan earthquake" height="1286" width="1715" msid="106584890" hostid="261" sizebytes="133282"></div>A 2021 World Bank report said the country learned several lessons from the 2011 disasters, from improving water supply infrastructure to fortifying backups to the undersea cables that power the internet.Aldrich said the biggest lesson was in revamping nuclear plant safety regulations.After the 2011 Fukushima meltdown, Japan was slow to restart using nuclear power. The Japan Times reported that 10 of its 33 reactors are in operation after being shut down over a decade ago.Experts are also concerned about the possibility of a very large "megaquake" occurring sometime in the next four decades, per <a target="_blank" href="https://www.theguardian.com/world/2023/jun/02/japan-vending-machines-to-automatically-offer-free-food-if-earthquake-hits">The Guardian</a>. In 2020 a Japanese government panel <a target="_blank" href="https://www.thesun.ie/news/5352026/japan-predicts-mega-earthquake-100ft-tsunami-obliterate-fukushima-plant/">warned</a> that another large tsunami could inundate the <a target="_blank" href="https://www.businessinsider.com/japan-release-fukushima-nuclear-disaster-radioactive-wastewater-treated-safe-2023-8">Fukushima Daiichi Nuclear Power Plant</a> at any time.<h2>The double disaster of a tsunami</h2><div type="image" title="A massive tsunami wave rushing over trees and houses in Natori, Miyagi Prefecture in northeastern Japan in 2011" height="1500" width="2351" msid="106584891" hostid="261" sizebytes="643428"></div>One of the reasons an earthquake in Japan can be so dangerous is the possibility of tsunamis.During the 1980s and '90s, Japan constructed seawalls to hold back gigantic waves. But in 2011, a <a target="_blank" href="https://www.npr.org/2011/07/05/137611026/what-went-wrong-in-fukushima-the-human-factor">40-foot tsunami</a> rushed over the 19-foot seawall at the Fukushima plant. The plant suffered a meltdown as a result.Despite a long history of disasters, the country wasn't fully prepared for the tsunami that developed following the 2011 earthquake, according to a <a target="_blank" href="https://www.sciencedirect.com/science/article/abs/pii/S2212420912000027">2012 report</a>.In some areas, buoys outfitted with sensors monitor for tsunamis. As with earthquake alerts, the Japan Meteorological Agency uses TV, radio, and mobile phones to distribute the warnings. Sirens also sound, often giving people several minutes of warning that a wave may strike.In contrast, the 2004 Indian Ocean tsunami hit a number of islands with no formal warning system and resulted in over 200,000 deaths, per <a target="_blank" href="https://www.noaa.gov/jetstream/2004tsu_max">NOAA</a>.According to the World Bank, historical knowledge may have helped save lives during Japan's 2011 tsunami. Some survivors knew how far waters could encroach due to stone markers residents had placed after earlier tsunamis. Some of the monuments are hundreds of years old.

Japan takes earthquake safety seriously. Here's how its culture of preparedness keeps so many people alive.

<ul class="summary-list"><li>A Department of Labor investigation found the operators of 14 Subway restaurants violated labor laws.</li><li>Teenage employees suffered burns after operating dangerous equipment, the DOL found.</li></ul>The operators of 14 Bay Area Subway restaurants "endangered children" by making its teenage employees as young as 14 use dangerous equipment and work unlawful hours, an investigation by the US Department of Labor's Wage and Hour Division found.The employers also didn't pay their employees regularly, issued "hundreds" of bad wage checks, and stole tips left by customers, according to the DOL.The US District Court for the Northern District of California ruled on Wednesday that John Meza and his wife Jessica must either sell or shut down their businesses by November 27. Such consent orders are rare, the DOL saidThe Mezas and their limited liability corporations have also been ordered to pay back $475,000 in minimum wage, overtime, tips, and an equal amount in liquidated damages to 184 workers, and $150,000 in damages.They, alongside their site manager Hamza Ayesh, must also pay back $12,000 in punitive damages, related to "retaliatory conduct," after they threatened teenage employees who had raised concerns and coerced them not to cooperate with investigators. Subway did not reply to a request for comment made outside of normal business hours. The Mezas and Ayesh also didn't respond to requests for comment.The defendants' lawyer, Arkady Itkin, said in an emailed statement: "The parties have reached settlement. Some <a target="_blank" href="https://www.businessinsider.com/israel-gaza-hamas-palestinian-million-evacuate-24-hours-issues-order-2023-10">Palestinians</a> traveled on foot and mule-drawn carriages. Others were fortunate enough to have a car or hitch a ride on a bus or a truck. However, it appeared that the trek to safety was not so safe. An eyewitness who previously <a target="_blank" href="https://www.businessinsider.com/israel-airstrike-kills-women-children-fleeing-evacuation-route-northern-gaza-2023-10">spoke to Insider</a> and verified multiple witness reports said that airstrikes killed scores of civilians utilizing a safe route to escape bombing in the north. The <keyword id="20025616" type="General" weightage="20" keywordseo="palestinian-health-ministry" source="keywords">Palestinian Health Ministry</keyword> said 70 people died. The <keyword id="19976425" type="General" weightage="20" keywordseo="israel-defense-forces" source="keywords">Israel Defense Forces</keyword>, which is carrying out its most intensive air campaign against Gaza, denied that its munitions killed the civilians, the <a target="_blank" href="https://www.theguardian.com/world/2023/oct/14/gaza-civilians-afraid-to-leave-home-after-bombing-of-safe-routes?CMP=share_btn_tw">Guardian reported</a>. <a target="_blank" href="https://www.businessinsider.com/israel-wipes-out-hamas-another-group-will-take-its-place-2023-10">Hamas</a>, the militant group that governs the strip, said Israel was to blame. Now, hundreds of thousands of Palestinians are crowded in the southern parts of the region, as food, water, fuel, electricity, and medical equipment are in both high demand and short supply. This is due to the restriction of resources that Israel has put on the area, as human rights groups beg for <a target="_blank" href="https://www.businessinsider.com/gaza-israel-airstrikes-trapped-in-a-humanitarian-disaster-2023-10">humanitarian corridors</a> to provide much-needed aid. Insider spoke with one Gaza resident, <keyword id="23580744" type="General" weightage="20" keywordseo="amjad-shabat" source="keywords">Amjad Shabat</keyword>, who fled Gaza City with her family, about what it's like to live near the <a target="_blank" href="https://www.businessinsider.com/israel-bombed-only-crossing-allowing-people-flee-gaza-palestine-egypt-2023-10">Rafah</a> border crossing. This interview has been edited for length and clarity. Who did you leave with? Are you safe right now? I used to live in Gaza City. I left with my husband, my daughter, my sister, and my mother to my in-laws. So we're staying at the house of my in-laws near the Rafah border that is to the south of the Gaza Strip. When did you leave? The evacuation process started the morning of, I believe, Wednesday. And we left in the afternoon of that day. So yes, upon the calls of Israeli occupation forces to leave the northern Gaza strip to the south. This was a massive evacuation process. What was the process of leaving like? Very, very, very scary. Very frightening. We left already under the massive airstrikes that have been targeting Gaza for three days, four days at that time. Israel only kept one main street that links the northern of Gaza to the south, which is Salah al-Din Road. Along 15-20 kilometers, you saw a series, a line of cars, trucks, and any transportation that could take people. I was very scared and sad at the same time because this all brought the memories of 1948 when the Palestinian people had to leave their villages. <div type="image" title="A gray car filled with people, including a child is sitting on the window. I have been wearing the same clothes since Wednesday. We were lucky that my father-in-law had a car, so he came from Rafah to Gaza, which takes him two hours … He came and he took us out of the city. We actually were lucky because many of our friends and many of our colleagues couldn't leave Gaza until now because they couldn't find a ride. What was the drive like? Very scary. At one moment, my father-in-law was about to hit the side of the road because he couldn't see. We literally piled up inside the car. Thirteen people in a car with a capacity of five people, you couldn't see both sides. And because of this massive crowd of people leaving Gaza into the south, people were very stressed and scared. I believe they were less focused on driving than on running away from the continuous airstrikes. The massive and the terrifying airstrikes. There were voices of airstrikes and bombing when we traveled, but to be honest, not exactly where we've been. I kept hearing the voices. They were in the background but not in front of my eyes. Have you had any updates on your home in Gaza City? Do you even know if it's still standing? I don't know. I just recently bought my flat. My husband and I had been saving for years to have the top flat in the center of Gaza City. We left everything. So I really don't want it to be destroyed. Right behind the neighborhood I used to live in, it was completely destroyed. And all across the road, destruction is everywhere, wherever we go. The most important and the most terrifying thing that we face is the scarcity of basic resources, such as water and food. We've run out of bread today. Three of my brothers-in-law had to stay in front of the bakery for three hours where they couldn't obtain any loaf of bread because when the time came for them to buy their bread, there wasn't any more bread. We are out of water, so we can't use the toilet, we can't wash our clothes or wash the dishes. We keep the freshwater we can drink. No electricity, of course. No internet. I'm lucky to have neighbors who have a private generator, and they have something like cellular data, and they connect for me. What is it like near the Rafah border? I've been to one of our schools. <keyword id="19970301" type="General" weightage="20" keywordseo="unrwa" source="keywords">UNRWA</keyword> is the UN institution that's responsible for feeding and finding work and helping the refugees of Palestine. They're turning their schools into shelters to help thousands of families. At a school with a capacity of 1,000 people, between 2,000 and 4,000 people are literally piling up. Others are in serious shock because they run away under the massive airstrikes where buildings may be demolished in front of their eyes. Others, they can't focus their eyes. They can't speak. I went to interview one of them to see how are the people's mental health. They refused to speak with me. Men had to sleep outside, and it's getting colder and colder. The streets are full. The city where were we evacuated to is a very small city. And the infrastructure, of course, is not suitable to hold this number of people. People who are there have run away without medicines. People with chronic diseases are very frightened of any health crisis that they may face because they don't have their medicines. People with diabetes, with blood pressure, with a history of heart disease. They're very afraid.

Alexis Ohanian, a cofounder of Reddit alongside Steve Huffman and Aaron Swartz, <a href="https://edition.cnn.com/videos/business/2023/05/13/alexis-ohanian-artificial-intelligence-government-regulation-ai-reddit-wallace-sot-contd-wtcw-vpx.cnn" target="_blank" rel="noopener">spoke with CNN's Chris Wallace</a> in an interview about AI. Wallace played a montage of clips showing senators interrogating tech leaders about the technology they develop with little information or education about how it works. In <a href="https://www.youtube.com/watch?v=n2H8wx1aBiQ" target="_blank" rel="noopener">one clip</a> a senator can be seen asking Meta CEO Mark Zuckerberg how its business model is sustainable if users don't pay for the services.
