A weekly intake of a drug, approved to treat Type 2 diabetes, can provide substantial and sustained reductions in body weight, according to a study. The experimental drug, called <keyword id="13799068" type="General" weightage="80" keywordseo="tirzepatide" source="Orion">tirzepatide</keyword>, developed by US pharma major Eli Lilly has also been hailed as an alternative to <keyword id="6403263" type="General" weightage="80" keywordseo="bariatric-surgery" source="Orion">bariatric surgery</keyword>. Tirzepatide combines synthetic mimics of two hormones known as GLP-1 and GIP that our guts naturally release after we eat to make us feel full. These naturally occurring hormones also help control blood sugar. In May, the US Food and Drug Administration approved tirzepatide, under the trade name Mounjaro, for the treatment of Type 2 diabetes. Currently, Tirzepatide is not available for weight loss, but Eli Lilly is aiming to get an updated timeline from the FDA this year. The drug, which is currently available in an injectable form, "is a welcome relief," Dr. Anil Arora, chairman, Institute of Liver, Gastroenterology, Pancreaticobiliary Science at Sir <keyword id="237993" type="General" weightage="50" keywordseo="ganga-ram-hospital" source="Orion">Ganga Ram Hospital</keyword>, New Delhi, told IANS. "The combination of the two hormones go and tell the brain that your stomach is full or you have enough reserves and you don't need to eat more. So that restricts your appetite that makes you burn more calories which are residing in your body unutilised helping you lose weight," he added. In the study, published in the <keyword id="401051" type="General" weightage="50" keywordseo="new-england-journal-of-medicine" source="Orion">New England Journal of Medicine</keyword>, an international team, including from Yale University, randomly split 2,539 overweight or obese participants into four equal groups. One group was offered a self-administered placebo injection once a week for 72 weeks, while the other three groups were offered either 5 mg, 10 mg or 15 mg of tirzepatide. They were also recommended 150 minutes of exercise a week. At the end of the 72-week period, participants given 5 mg of tirzepatide each week lost an average of 16.1 kg, those given 10mg lost an average of 22.2 kg, and those given the 15 mg an average of 23.6 kg. Those given a placebo injection lost an average of 2.4 kg. Those who took the highest of three doses lost as much as 21 per cent of their body weight compared to those who took placebo. One disadvantage of people who undergo bariatric surgery is that they tend to regain weight. But here with the drug, one can tweak the hormones, which are taking the signals from the intracellular tract to tell the brain that you are hungry, Arora said. Importantly, this "type of therapy does not interfere with the normal structure of the body" unlike endoscopic surgery or bariatric surgery, where the size of the stomach is decreased, he noted. Weight loss of 15 per cent to more than 20 per cent also has the potential to change the way doctors treat diabetes. A previous study of tirzepatide in diabetes showed that half the participants who were early in the course of their diabetes went into remission while taking the drug. "That is a potential game changer in how we think about therapy for people with Type 2 diabetes," Dr. <keyword id="14129116" type="General" weightage="50" keywordseo="robert-gabbay" source="Orion">Robert Gabbay</keyword>, chief scientific and medical officer for the American Diabetes Association, was quoted as saying to USA Today. However, what's not known, Gabbay said, is whether remission achieved this way will reduce the typical complications of diabetes, which can include cardiovascular disease, nerve and kidney damage and limb amputations. New diabetes drug significant in fight against obesity, reveals new study

Tirzepatide, sold under the brand name Mounjaro, has attracted attention for dramatic weight loss results, including a <a href="https://www.insider.com/weight-loss-drug-helped-people-lose-20-bodyweight-study-tirzepatide-2022-4">clinical trial that found patients lost 20% of their body weight in 16 months while using it</a>.The study results are a game-changing level of weight loss typically unheard of without weight loss surgery, Dr. Robert Gabbay, chief scientific and medical officer for the American Diabetes Association, told <a href="https://www.usatoday.com/story/news/health/2022/06/04/diabetes-drug-could-game-changer-obesity-new-study-shows/7490446001/">USA Today.</a>Despite its potential, there are a lot of unknowns about the future of the drug, including access for patients, according to an obesity medicine expert.Here's how tirzepatide works, how its prescribed, and possible downsides, including side effects, cost, and insurance issues.<h2>It's not yet approved for weight loss, but doctors can prescribe it</h2>One of the most popular existing weight loss drugs, called <a href="https://www.insider.com/weight-loss-drug-semaglutide-called-game-changer-for-obesity-2021-12">semaglutide,</a> originated as a diabetes medication, and was <a href="https://www.businessinsider.com/weight-loss-obesity-drug-fda-approved-wegovy-2021-6">FDA-approved to treat obesity in 2021</a>, becoming so popular it caused shortages. A similar story may unfold with tirzepatide: although it hasn't been approved by the FDA for weight loss, treatment providers anticipate that will change, based on research showing its potential for patients with obesity. "We do expect that it will be approved for the treatment for obesity," endocrinologist and obesity medicine specialist <a href="https://www.atlantaendocrine.com/about">Dr. Scott Isaacs</a> told Insider, although it's not clear how soon. In the meantime, patients can still access the drug. The lack of FDA approval for weight loss means manufacturers aren't able to market the drug for weight loss, Isaacs said. Since tirzepatide is already FDA-approved for use with diabetes, doctors are able to prescribe it to patients, even for a different purpose, if they think it will help.<h2>It helps reduce appetite, similar to drugs like semaglutide</h2>Tirzapetide works in the same way as other diabetes medications used to treat obesity, according to Isaacs."It's more similar than different to other drugs," he said. It acts on a hormone called GLP-1, which controls appetite, so patients taking the drug feel more full. Tirzapetide uniquely also acts on another hormone related to insulin too. Some <a href="https://www.businessinsider.com/diabetes-drug-obesity-lilly-tirzepatide-2021-7">research has claimed that tirzepatide is more effective for weight loss than other medications</a>, based on a clinical trial showing more pounds lost. But Isaacs said evidence is inconclusive, since the study was designed to focus on diabetes, not weight loss, and didn't use the strongest available dose of semaglutide."It's considered more effective but how much more, no one really knows," he said. As with similar drugs, tirzepatide can cause side effects like nausea. <h2>It may be difficult to afford long-term</h2>One of the biggest questions about tirzepatide right now is if it will be affordable and accessible to patients who need it over time. Typically, the high cost of weight loss drugs can be a deterrent to patients. Semaglutide, for instance, has been listed at a <a href="https://www.insider.com/Semaglutide-diabetes-drug-shortages-linked-to-tiktok-weight-loss-trend-2022-6">price of $1,349 for a month's supply for obesity treatment</a>.A majority of insurance plans don't cover weight loss medications, and those that do require extensive paperwork, according to Isaacs."It's a huge burden to prove patients qualify for the drug," he said. Tirzepatide is gained popularity as a weight loss prescription because of a discount that maked the drug available at $25 for a one to three months supply, according to the <a href="https://www.mounjaro.com/savings-resources#resources">manufacturer's website</a>. Now <a href="https://www.fiercepharma.com/pharma/eli-lilly-clamps-down-mounjaro-type-2-diabetes" target="_blank" rel="noopener">that discount</a> is only available to those with type 2 diabetes.The catch with weight loss medications like tirzepatide and semaglutide is that patients have to continue taking the medication to maintain the weight loss. "We do want to use the drugs long-term because we know if they stop taking them, the weight comes back on," Isaacs said. However, the discount is slated to expire next year, so patients taking tirzapetide will need to find another means to afford the drug. "There's a lot of unknowns. We do our best, and there's hope things will change," Isaacs said. "I'm very up front with patients but I do worry come June next year."
