<p></p><div class="KonaFilter image-container display-table"><div><div class="image on-image"></div><p></p></div></div><ul><li><strong>A London Tramlink service derailed last year, with fatal results.</strong></li><li><strong>Rail crash investigators released a video reconstruction on Thursday.</strong></li><li><strong>It showed the tram leaving the tracks at 78 km/h in a 20 km/h zone.</strong></li><li><strong>Investigators said driver Alfred Dorris may have been in a "microsleep" at the time.</strong></li><li><strong>Dorris is being investigated by police over a potential manslaughter case.</strong></li></ul><p><br>Rail accident investigators have released a video showing exactly how a "catastrophic" tram crash unfolded last year, killing seven people.</br></p><p>The UK's Rail Accident Investigation Branch (RAIB) published animated footage showing how a London Tramlink service to Croydon derailed on November 9, 2016.</p><p>It concluded that the driver lost control of the vehicle, and suggested that he might have done so because he was having a "microsleep" - a technical term for a brief period of involuntary unconsciousness.</p><p>The video shows the tram approaching a tight left-hand curve after leaving a series of connected tunnels in the Sandilands area of Croydon.</p><p>It approaches the curve at high speed, and starts to tip onto its right side as it proceeds. The tram carriage then tips over completely, and skids to a halt on the other side of the tracks.</p><p>The impact killed seven people, left 19 people with serious injuries and 43 more with minor injuries. The driver was one of those with minor injuries.</p><p></p><div class="KonaFilter image-container display-table"><div><div class="image on-image"></div><p></p></div></div><p>The tram was travelling at 78 km/h - close to its maximum speed of 80 km/h - when it left the tunnels.</p><p>At that point the tram ought to have slowed to 20 km/h to successfully navigate. But it kept on at high speed, leading to it spiralling off the tracks.</p><p>Describing the crash, <a href="https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/665491/R182017_171207_Sandilands.pdf">the full report said</a>: "This catastrophic accident was the worst to occur on a British tramway for more than 90 years."</p><p>It concluded that&#xa0;there were "insufficient safety measures" in place, and recommended 15 steps to help prevent future accidents.</p><p></p><div class="KonaFilter image-container display-table"><div><div class="image on-image"></div><p></p></div></div><p>The report did not blame anybody for the accident, a task RAIB officials said is beyond its remit.</p><p>RAIB said: "A possible explanation for this loss of awareness was that the driver had a microsleep, and that this was linked to fatigue.</p><p>"Although it is possible that the driver was fatigued due to insufficient sleep there is no evidence that this was the result of the shift pattern that he was required to work."</p><p>Officers from the British Transport Police have questioned the driver, named in the media as 43-year-old Alfred Dorris, on suspicion of manslaughter.</p><p>A police spokesman told Business Insider they last spoke to Dorris in October. Previously, most crude produced in the United States was moved by pipeline from Cushing, Oklahoma.  </p><p>The increase in crude by rail is great for the industry - it can get the massive amounts of crude now being produced where it needs to go using infrastructure that already exists.</p><p>But it has its downsides. The ramp-up of crude-carrying trains has exposed <a href="http://www.businessinsider.com/crude-oil-train-derailments-2015-3">the dangers of crude-carrying train derailments</a>. They existed before, but there wasn't enough being shipped for it to pose a big risk. </p><p>Reuters' John Kemp explains the problem this poses for the railroads and the oil industry: </p><p style="padding-left: 30px;">Negotiations between the railroads, oil producers, refiners and the U.S. government about new crude-by-rail regulations and tank car safety standards boil down to the question of how to balance the safety imperative of withdrawing older and less secure tank cars as soon as possible against the commercial imperative of keeping them in service for longer to maintain tank car availability and keep the oil flowing.</p><p>The problem is that the risks are real, but minute for any one shipment. And the rewards are easy to calculate.</p><p>Safety seems to be making some gains, though. <a href="http://www.businessinsider.com/r-north-dakota-launches-oil-rules-hoping-to-curb-us-rail-disasters-2015-4">Reuters reported Wednesday</a> that many of the gases in North Dakota crude that can make crude-by-rail crashes at higher risk of explosions will need to be removed before the oil is shipped.</p><p>It's a North Dakota rule, but Reuters' Ernest Scheyder writes that, "i<span style="line-height: 1.5em;">n the absence of concrete regulations from the U.S. Department of Transportation, North Dakota's new rules become the de facto national standard on the treatment of crude before tankcar loading."</span></p>

The amount of oil being shipped by rail has increased 50-fold since 2010

<p>The 41-year-old musician's Cadillac allegedly "struck a Ford pickup truck, causing it to hit a guard rail, crash down an embankment and flip, throwing driver Paul Howard Shoulders from the vehicle," reports <a href="http://www.gossipcop.com/todd-harrell-arrested-vehicular-homicide-3-doors-down-car-crash-accident-dead/" target="_blank">GossipCop</a>.</p>
<p><span>Shoulders,<span> who was not wearing a seatbelt,</span> was pronounced dead at a nearby hospital after <span>Harrell failed field sobriety tests and admitted to cops that he’d been drinking hard cider and taking prescription pills.</span></span></p>
<p><span>As Harrell was being booked into jail, <a href="http://www.huffingtonpost.com/2013/04/21/todd-harrell-arrested-3-doors-down_n_3127101.html" target="_blank">law enforcement officials found</a> <span>a plastic bag concealed in his sock that contained eight Xanax pills, 24 Oxycodone pills and four Oxymorphone pills.</span></span></p>
<p><span><span><span>Harrell has been charged with vehicular homicide, bringing controlled substances into jail, and is currently being held on $100,000 bond.</span></span></span></p>
<p>"We are deeply saddened by the passing of Paul Howard Shoulders, Jr. Our hearts and prayers go out to his family and friends at this difficult time," said a statement released on the <a href="http://www.3doorsdown.com/news" target="_blank">3 Doors Down website</a>.</p>
