COVID-19 vaccine trial: Pfizer expects results by October end

Pune, June 9 () Serum Institute of India, the world's largest vaccine manufacturer, is investing USD 100 million on a potential COVID-19 vaccine being developed at Oxford University, according to a company official. The Pune-based company has partnered with AstraZeneca, a British biopharma giant, to ensure equitable supply of the AZD1222 vaccine doses to India as well as low-and-middle-income countries. "Our manufacturing facility is ready and we plan to start production in two months. We are spending more than USD 100 million for this facility. Till the trials are completed successfully for safety and efficacy, vaccines will not be distributed either in India or anywhere else in the world," Adar Poonawalla, CEO, Serum Institute of India (SII), said. "Having said that we will start making a few millions of doses and stockpiling it at personal risk," added Poonawalla, while plying to a question on the firm's plans to move ahead in terms of production of the doses. To a question on how much doses India is likely to get in the first phase and by when they would be available, he said that it is a bit early to comment on a specific number. "However, if the vaccine trials succeed, India will gain access to the doses as it will also be the requirement of the Government of India. And we are certain that everybody will respect if the substantial volumes go to India," he added. Earlier, Poonawalla, on tie-up with the British drug major had said, "SII is delighted to partner with AstraZeneca in bringing this vaccine to India as well as low-and-middle-income countries." Over the past 50 years, SII has built significant capability in vaccine manufacturing and supply globally. "We will work closely with AstraZeneca to ensure fair and equitable distribution of the vaccine in these countries." Talking about SII's endeavour to develop a vaccine with USA based firm Codagenix, Poonawalla recently said that at present, the firm is undergoing pre-clinical trials (animal trial phase) and hope to progress to the human trial phase by September/October. "The aim is to make the vaccine over the next 1.5-2 years to help combat the novel coronavirus. With the combined efforts of our partner Codagenix, we are optimistic that we will be able to provide a viable and effective vaccine for mass use," he had said. The Phase 1 study will enroll up to 40 healthy adult volunteers in Philadelphia and Kansas City, where screening of potential participants has already begun, said the company. The first dosing is planned for Monday. Each participant will receive two doses of INO-4800 four weeks apart, and the initial immune responses and safety data from the study are expected by late summer. Additional preclinical trials, including challenge studies, will continue in parallel with the Phase 1 clinical trial, according to the company. Once injected into muscle, the RNA self-amplifies - generating copies of itself - and instructs the body's own cells to make copies of a spike protein found on the outside of the virus. US halts AstraZeneca COVID-19 vaccine production at Baltimore plant where 15 million J&J vaccines were ruined

The clinical trials which started on April 27 in <keyword id="9481138" type="LOCATION" weightage="20" keywordseo="shangqiu-county">Shangqiu county</keyword> in China's Henan province have shown the vaccine to be effective and safe, said CNBG. The trials were designed as randomised, double-blind and placebo-controlled studies. This is not the first Covid-19 vaccine candidate from CNBG, which is affiliated to the state-owned China National Pharmaceutical Group (Sinopharm). In the middle of this month, another institute under CNBG in Wuhan announced the results of phase-1 and Phase-2 clinical trials of an inactivated Covid-19 vaccine candidate it developed. These trials, which started on April 12, also involved 1,120 volunteers and the results revealed a good safety record, with no cases of severe adverse effects found in the clinical trials. India has leveraged its manufacturing capacity to pre-order 600 million doses of potential <keyword id="8904563" type="UNKNOWN" weightage="20" keywordseo="covid-19-vaccine">Covid-19 vaccine</keyword> and negotiating for another billion doses, says a new global anaylysis of advance market commitments (AMC) for vaccine candidates against the coronavirus. The total number of these vaccine doses, if they are approved to be utilised in the course of time, should be enough to vaccine half of the country's over 1.3 billion population given the fact that most of the vaccine candidates against the coronavirus require two doses. The analysis of advance market commitments for experimental Covid-19 vaccine till October 8 by US-based Duke Global Health Innovation Center noted that international initiatives such as COVAX and other alliances are critical to ensuring equitable allocation of vaccines for low and middle-income countries. Current models predict that there will not be enough vaccines to cover the world's population until 2024, said the report by the Center's Launch and Scale Speedometer, an initiative that identifies hurdles to delivering health innovations to low-income countries. However, the analysis showed that the number of confirmed potential Covid-19 vaccine doses that India has ordered is second only to the US which already has made deals for 810 million doses and negotiating for another 1.6 billion doses. "Several middle-income countries, including India, also have robust vaccine development programs and are putting forward vaccine candidates, though these are not as far along in the process as the leading candidates from high-income countries" said the report. "If any of the candidates from middle-income countries achieve regulatory approval, the landscape will likely shift significantly," it added. For countries without manufacturing or vaccine development capacity, those with the infrastructure to host clinical trials, such as Peru, have used that as leverage to negotiate purchase deals, the report added. Overall, the analysis showed that even before any vaccine candidates are approved for market, confirmed purchases cover 3.8 billion doses, with another five billion doses currently under negotiation or reserved as optional expansions of existing deals. The report comes at a time when nearly 200 Covid-19 vaccine candidates are moving forward through the development and clinical trials processes at unprecedented speed, more than ten candidates are already in Phase 3 large-scale trials and several have received emergency or limited authorisation. For the study, the researchers aggregated and analysed publicly available data on vaccine procurement and manufacturing to track the flow of procurement and better understand global equity challenges. India is planning to start the distribution of Covid-19 vaccine first to the healthcare workers followed by other groups to be added "sequentially". The distribution drive would take place over a year. For this, the states and union territories would form a three-tier system as per the Centre's directions. "Zydus has already manufactured clinical good manufacturing practice (GMP) batches of the vaccine candidate and plans to initiate the clinical trials in July 2020 across multiple sites in India in over 1,000 subjects," the company said. ZyCoV-D, developed at the company's Vaccine Technology Centre in Ahmedabad, has now received permission from the Drug Controller General of India (DGCI) - Central Drugs Standard Control Organisation (CDSCO) to initiate phase I/II human clinical trials in India, Cadila Healthcare said in a regulatory filing. In animal studies the vaccine was found to elicit a strong immune response in multiple animal species like mice, rats, guinea pigs and rabbits. The antibodies produced by the vaccine were able to completely neutralise the wild type virus in virus neutralisation assay indicating the protective potential of the vaccine candidate, it said. It further said no safety concerns were observed for the vaccine candidate in repeat dose toxicology studies by both intramuscular and intradermal routes of administration. In rabbits, up to three times the intended human dose was found to be safe, well tolerated and immunogenic. Zydus said, with its ZyCoV-D, it has successfully established the DNA vaccine platform in India using non-replicating and non-integrating plasmid carrying the gene of interest making it very safe. The company claimed that the vaccine candidate has no vector response and with absence of any infectious agent, the platform provides ease of manufacturing the vaccine with minimal biosafety requirements. It is also known to show much improved vaccine stability and lower cold chain requirements making it easy for transportation to remotest parts of the country, it said. "Furthermore, the platform can be rapidly used to modify the vaccine in couple of weeks in case the virus mutates to ensure that the vaccine still elicits protection," it added. The University of Melbourne-led research team, which worked with <keyword id="10337292" type="COMPANY" weightage="20" keywordseo="the-fiji-ministry-of-health-and-medical-services">the Fiji Ministry of Health and Medical Services</keyword>, said the results underline the importance of vaccines and how they can potentially reduce the impact of viruses such as Covid-19. Published in the journal 'Lancet Regional Health - Western Pacific', the first study looked at Fiji's national rotavirus vaccine programme five years after it became the first independent Pacific island country to introduce the vaccine in 2012. According to the researchers, rotavirus is severely contagious and the most common cause of diarrhoeal disease among infants and young children. It can even cause death in extreme cases. After taking the vaccine, morbidity and mortality due to rotavirus and all-cause diarrhoea in Fiji fell in those aged two months to 55 years. Rotavirus diarrhoea admissions at the largest hospital among children aged under five fell by 87 per cent. These reductions were most likely due to the vaccine as rotavirus diarrhoeal outbreaks remained blunted for the five years after vaccine introduction. At the same time, in 2012, the Fiji government introduced a routine infant immunisation schedule for ten-valent pneumococcal conjugate (PCV10) vaccine using three primary doses and no booster dose. Published in 'The Lancet Global Health', this second study looked at hospital admission rates for children with pneumonia at three Fiji public tertiary hospitals. Five years after the vaccine was introduced, hospital admissions for all-cause pneumonia had fallen for children aged 24-59 months. Mortality was down by 39 per cent among children aged two to 24 months who were admitted to hospitals with all-case pneumonia, bronchiolitis and asthma. The study showed the effect of PCV10 vaccine and supported its introduction for children in other low and middle-income countries in that region. "These results provide supportive evidence of the probable benefits of PCV10 in reducing pneumonia in children in Fiji," said study lead researcher Fiona Russell from the Murdoch Children's Research Institute and <keyword id="6200092" type="COMPANY" weightage="20" keywordseo="The-University-of-Melbourne">the University of Melbourne</keyword>.
