New York [US], April 5 (ANI): The <keyword id="10561122" type="General" weightage="20" keywordseo="bird-flu-pandemic" source="Orion">bird flu pandemic</keyword> with the potential to be "<keyword id="21202249" type="General" weightage="20" keywordseo="100-times-worse" source="Orion">100 times worse</keyword> than COVID" may be on the horizon after a rare human case was discovered in Texas, experts have warned, the New York Post reported on Thursday. The <keyword id="21202246" type="General" weightage="20" keywordseo="H5N1-avian-flu" source="keywords">H5N1 avian flu</keyword> has spread rapidly since a new strain was detected in 2020 affecting wild birds in every state, as well as in commercial poultry and backyard flocks. But the recent cases detected in mammals, with cattle herds across four states becoming infected and on Monday, federal health officials announced that a dairy worker in Texas caught the virus. "This virus [has been] on the top of the pandemic list for many, many years and probably decades," Suresh Kuchipudi, a <keyword id="95867" type="General" weightage="100" keywordseo="bird-flu" source="Orion">bird flu</keyword> researcher from Pittsburgh, said at a recent panel discussing the issue, the New York Post reported citing the Daily Mail. "And now we're getting dangerously close to this virus potentially causing a pandemic." He noted that the <keyword id="565908" type="General" weightage="20" keywordseo="H5N1-virus" source="Orion">H5N1 virus</keyword> has already been detected in species throughout the world and "has shown the ability to infect a range of mammalian hosts, including humans." "So therefore, in my view, I think this is a virus that has the greatest pandemic threat [that is] playing out in plain sight and globally present," Kuchipudi said. John Fulton, a pharmaceutical industry consultant for vaccines and the founder of Canada-based BioNiagara who organized the meeting, also expressed his concerns, the Mail reports, the New York Post reported. "This appears to be 100 times worse than COVID -- or it could be if it mutates and maintains its high case fatality rate," he said. "Once it's mutated to infect humans, we can only hope that the [fatality rate] drops." Around 52 per cent of humans who have contracted H5N1 since 2003 have died, according to the World Health Organization. For comparison, COVID currently kills less than 0.1% of those it infects -- though at the start of the pandemic, the fatality rate was around 20 per cent. Symptoms of the bird flu are similar to those of other flus, including cough, body aches and fever, the New York Post reported. Some people may not develop noticeable symptoms, but others can develop severe, life-threatening pneumonia. The dairy worker in Texas who was infected has reported "eye redness (consistent with conjunctivitis)" as their only symptom, the Centers for Disease Control and Prevention noted. "The patient was told to isolate and is being treated with an antiviral drug for [the] flu," the CDC said. The agency and "the whole US government is taking this situation very seriously," CDC Director Mandy Cohen told the Washington Post. She maintained that the virus does not pose much of a risk to the general public, noting that the dairy worker was in direct contact with infected cattle, and the US Department of Agriculture has said there are currently no changes that would make it more transmissible to humans. "While cases among humans in direct contact with infected animals are possible, this indicates that the current risk to the public remains low," the department said in a statement over the weekend. But the fact that the virus has been found in cattle could mean it is starting to mutate, Cohen told the Washington Post. "We had not seen avian flu in cattle prior to last week. That is new," she said. "It is a reservoir for [the] virus to circulate and potentially change." If the virus does mutate enough to infect humans, it could spread rapidly, the European Food Safety Authority warned on Wednesday, according to Fox News. "If avian A [H5N1] influenza viruses acquire the ability to spread efficiently among humans, large-scale transmission could occur due to the lack of immune defences against H5 viruses in humans," the Food Safety Authority said. To prevent any potential spread, the US is already testing components used to create a vaccine for the virus, and two candidate vaccine viruses appear to be well-matched to protect against H5N1, the Washington Post reports. White House press secretary Karine Jean-Pierre also said the Biden administration is monitoring the threat. Why 4 new bird flu developments have experts worried about the virus

However, a source in the Union health ministry told PTI that they are all sub variants of <keyword id="20115417" type="General" weightage="20" keywordseo="JN1" source="keywords">JN1</keyword> and there is no associated increase in hospitalisation and severe cases. "So there is no reason for concern or panic. The mutations will keep happening at a rapid pace and this is the natural behaviour of viruses like SARS-CoV2," the source said. The source further said that the INSACOG surveillance is sensitive and is able to pick up the emergence of any new variant and samples are also picked from hospitals in a structured manner to detect any change in the severity of disease due to virus. Data compiled by the Indian SARS-CoV-2 Genomics Consortium (INSACOG) showed that 34 cases of KP.1 have been found across seven states and UTs with 23 cases registered from West Bengal. The other states are Goa (1), Gujarat (2), Haryana (1), Maharashtra (4) Rajasthan (2) and Uttarakhand (1). As many as 290 cases of KP.2 have been registered with Maharashtra reporting the highest number at 148, according to the data. The other states and Union territories are Delhi (1), Goa (12), Gujarat (23), Haryana (3), Karnataka (4), Madhya Pradesh (1), Odisha (17), Rajasthan (21), Uttar Pradesh (8), Uttarakhand (16) and West Bengal (36). The Ministry of Health (MOH) said the estimated number of COVID-19 cases in the week of May 5 to 11 rose to 25,900 cases, compared with 13,700 cases in the previous week. The average daily COVID-19 hospitalisations rose to about 250 from 181 the week before. The average daily intensive care unit (ICU) cases remained low at three cases, compared with two cases in the previous week. The MOH said that to protect hospital bed capacity, public hospitals have been asked to reduce their non-urgent elective surgery cases and move suitable patients to transitional care facilities or back home through Mobile Inpatient Care@Home, an alternative inpatient care delivery model that offers clinically suitable patients the option of being hospitalised in their own homes instead of a hospital ward. Ong urged those who are at greatest risk of severe disease, including individuals aged 60 years and above, medically vulnerable individuals and residents of aged care facilities, to receive an additional dose of the COVID-19 vaccine if they have not done so in the last 12 months. Ong said that if the number of COVID-19 cases doubles one time, Singapore will have 500 patients in its healthcare system, which is what Singapore can handle. However, if the number of cases doubles a second time, there will be 1,000 patients, and "that will be a considerable burden on the hospital system", he pointed out. "One thousand beds is equivalent to one regional hospital," Ong said. "So, I think the healthcare system has to brace ourselves for what is to come." There are no plans for any form of social restrictions or any other mandatory kind of measures for now, as COVID-19 is treated as an endemic disease in Singapore, he said, adding that imposing additional measures would be a last resort. Ong said that with Singapore being a transport and communications hub, it will be one of the cities to get a wave of COVID-19 earlier than others. "So, COVID-19 is just something that we have to live with. Every year, we should expect one or two waves," he said. The ministry added that since COVID-19 vaccination started in 2020 to 2021, the vaccines have consistently been proven to be safe and effective in protecting individuals from severe illness. Billions of doses have been administered globally, and safety monitoring internationally has shown that the vaccine is safe, it said. There have also been no long-term safety concerns with COVID-19 vaccination, and adverse effects from vaccines, including the mRNA vaccines, have all been observed to occur shortly after vaccination, the ministry added. "If it keeps spreading in animals, then it is eventually going to cause problems for humans, either because we don't have food because they've got to start exterminating flocks, or because it starts to make a jump in humans," Adams, <keyword id="6366244" type="General" weightage="20" keywordseo="who" source="Orion">who</keyword> served under former President Donald Trump and was on the administration's initial COVID-19 task force, told Business Insider. "The more it replicates, the more chances it has to mutate." Though he agrees with the CDC's <a target="_blank" href="https://www.cdc.gov/flu/avianflu/inhumans.htm">assessment</a> that the current risk to humans is low, Adams fears the US is repeating many mistakes it made in the early days of COVID-19. <h2>Weak messaging with no clear leaders</h2>Who is in charge of an animal pandemic in the US? The CDC? The US Department of Agriculture? The FDA? The answer is, sort of, all of them. That decentralized responsibility could be behind the lack of widespread, clear public messaging so far. For example, Adams says he hasn't changed anything about his diet, since pasteurization and proper cooking procedures should kill any live virus present. But he isn't sure everyone is getting the message. That means asymptomatic spread could be flying under the radar. "An animal can't tell you, 'Hey, I feel a little under the weather today.' "We need a testing strategy that is proactive and allows true surveillance, and not reactive." The USDA took a step forward on Wednesday, ordering that all lactating dairy cows must be tested for H5N1 before they're moved across state lines and that all positive test results must be reported. To me, it looks like they very much don't want to scare the public and spook the economy in an election year," Adams said. <h2>Business vs. public health</h2>Just like the lockdowns of COVID-19 were devastating for the restaurant and hospitality industries, a crackdown on avian flu can be devastating to the chicken industry. Even before that, just testing the flock can slow down production. "We're seeing the same tension between business interests and public health interests," Adams said. What's more, many of the workers who handle chickens and cattle are undocumented immigrants. That can make them and their bosses hesitant to call in authorities over diseased animals. Many vulnerable groups were hesitant to report illness in the early COVID days, too, including migrant workers and people who didn't have sick leave from work. "My concern is we keep making the same mistakes over and over again," Adams said. "Because we keep focusing on the wrong things instead of focusing on the root causes."

London, Headaches, memory problems, and fatigue -- commonly known to continue post a Covid-19 infection -- may be a result of inflammation in the rest of the body and not because SARS-CoV-2 infects the brain, claims a new research. Early on in the pandemic, researchers surmised that direct infection of the brain could be the cause behind these neurological symptoms. Many studies have also proposed the effect of <keyword id="19896627" type="General" weightage="20" keywordseo="covid" source="keywords">Covid</keyword> virus on the brain symptoms. But a study by researchers at the Charite- Universitatsmedizin Berlin in Germany has now produced evidence to support the new theory. "We took that as our hypothesis at the start, too. But so far, there has been no clear evidence that the coronavirus can persist in the brain, let alone proliferate," said <keyword id="20621224" type="General" weightage="20" keywordseo="dr-helena-radbruch" source="keywords">Dr Helena Radbruch</keyword>, head of the Chronic Neuroinflammation working group at the Department of Neuropathology at Charite. "For that, we would have needed to find evidence of intact virus particles in the brain, for example. Instead, the indications that the coronavirus could infect the brain come from indirect testing methods, so they aren't entirely conclusive," <keyword id="20621226" type="General" weightage="20" keywordseo="radbruch" source="keywords">Radbruch</keyword> added. According to a second hypothesis, the neurological symptoms would instead be a kind of side effect of the strong immune response the body deploys to defend against the virus. For the study, the team of researchers analysed various areas of the brain in 21 people who died in hospital settings, typically in an ICU, due to severe coronavirus infection. For comparison, the researchers studied nine patients who died of other causes after treatment in intensive care. First, they looked to see whether the tissue showed any visible changes and hunted for any indication of coronavirus. Then they conducted a detailed analysis of genes and proteins to identify the specific processes that had taken place inside individual cells. Like other teams of researchers before them, the Charite scientists found coronavirus genetic material in the brain in some cases. "But we didn't find neurons infected with SARS-CoV-2," Radbruch noted. "We assume that immune cells absorbed the virus in the body and then travelled to the brain. They're still carrying the virus, but it doesn't infect cells of the brain. So coronavirus has invaded other cells in the body, but not the brain itself." Still, the researchers did note striking changes in molecular processes in some cells of the brain in those infected with Covid-19: For example, the cells ramped up the interferon signalling pathway, which is typically activated in the course of a viral infection. "Some neurons evidently react to the inflammation in the rest of the body," said Prof. <keyword id="20621225" type="General" weightage="20" keywordseo="christian-conrad" source="keywords">Christian Conrad</keyword>, head of the <keyword id="20621227" type="General" weightage="20" keywordseo="intelligent-imaging" source="keywords">Intelligent Imaging</keyword> working group at the Berlin Institute of Health at Charite (BIH) and one of the principal investigators in the study. "This molecular reaction could be a good explanation for the neurological symptoms we see in Covid-19 patients. For example, neurotransmitters emitted by these cells in the brainstem could cause fatigue. That's because the brainstem is home to groups of cells that control drive, motivation, and mood." Further, the team found that the neurons' reaction to the inflammation is temporary, as shown by a comparison of people who died during an acute coronavirus infection with those who died at least two weeks afterward. The molecular changes are most evident during the acute infection phase, but they do normalise again afterward -- at least in the vast majority of cases.

Now, the expanded CDC guidance suggests that documented serious post-Covid conditions include cardiovascular, pulmonary, neurological, renal, endocrine, haematological, and gastrointestinal complications as well as death. "For decedents, who had a previous SARSCoV-2 infection and were diagnosed with a post-Covid condition, the certifier may consider the possibility that the death was due to long-term complications of Covid-19, even if the original infection occurred months or years before death," according to the updated Vital Statistics Reporting Guidance. In some cases, survival from Covid can be complicated by pre-existing chronic conditions, especially those that result in diminished lung capacity, such as chronic obstructive pulmonary disease (COPD) or asthma. These medical conditions do not cause Covid-19, but can increase the risk of contracting a respiratory infection and death, said the CDC guidance. Long Covid has also been associated with reduced brain oxygen levels, worse performance on cognitive tests and increased psychiatric symptoms such as depression and anxiety, according to new research from the University of Waterloo in Canada. At least 65 million people are thought to have long Covid, according to a latest review of 200 Covid-19 studies published in Nature Reviews Microbiology journal. The US-based researchers estimate that the condition is present in 10 per cent of more than 651 million Covid-19 cases that have been documented worldwide. Covid virus can have lasting effects on nearly every organ for years: CDC

Researchers have identified a protein found in superbugs that enhances SARS-CoV-2 replication by 10- to 15-fold. Global data shows nearly 10 per cent of severe Covid-19 cases involve a secondary bacterial co-infection - with <keyword id="401097" type="General" weightage="80" keywordseo="staphylococcus" source="Orion">Staphylococcus</keyword> aureus, also known as staph A, being the most common organism responsible for co-existing infections with SARS-CoV-2. The study, published in the journal iScience, revealed that the addition of a "superbug" - methicillin-resistant Staphylococcus aureus (MRSA) - into the mix could make the Covid-19 outcome even more deadly. While the mystery of how and why the combination of these two pathogens contributes to the severity of the disease remained unsolved, the researchers identified a protein - IsdA - found in all strains of staph A which boosts SARS-CoV-2 replication by 10- to 15-fold. The findings of this study are significant and could help inform the development of new therapeutic approaches for Covid-19 patients with bacterial co-infections. Interestingly, the study also showed that SARS-CoV-2 did not affect the bacteria's growth. This was contrary to what the researchers had initially expected. Who further stated that it needed more data to understand this COVID19 variant and the extent of its spread, but the number of mutations warrants attention. Recently, the latest variant, EG.5 (also known as Eris), has grabbed the attention of public health experts around the world and is becoming a dominant strain in countries including the United States and Britain. The variant is causing about 17 per cent of the new Covid-19 cases in the country compared to 16 per cent for the next most common lineage, XBB.1.16, according to the latest estimates from the US Centers for Disease Control and Prevention, CNN reported. The new variant is a spinoff of the XBB recombinant strain of the Omicron family. According to the WHO, all viruses, including SARS-CoV-2, the virus that causes COVID-19, change over time. Most changes have little to no impact on the virus's properties. "We're trying to right-size the appropriate message to make sure people continue to acknowledge the health impacts of COVID."<h2 id="3c719499-920d-46e7-b315-35ec4a1cdc50">The long COVID scare</h2>Relaxed attitudes are not necessarily a bad thing, according to doctors."I think it's to be expected as we learn more about the virus over these four years and as we've developed these other COVID countermeasures to address infection," said Dr. Jessica Bender, a primary care doctor at Harbor View Medical in Seattle and the co-director at the university's long COVID clinic.On the one hand, people are generally not getting as sick as they once did when ill with COVID, medical professionals told BI. On the other hand, this summer seems to have brought yet another COVID surge with a spike in cases.People don't typically equate respiratory illnesses with the warmer months, but an increase in summer socializing combined with the general decrease in preventive measures is likely fueling the uptick, Bender said.For an unlucky subset of patients, a COVID diagnosis — even in this new relaxed era — could mean <a target="_blank" class="" href="https://www.businessinsider.com/long-covid-survivor-its-wrecked-my-life-2022-1">long-term challenges.</a>Doctors define long COVID as any infection related to the virus that is present for at least three months, said Chopra, who treats patients at the UW long COVID clinic. According to the CDC, about 17 million people <a target="_blank" class="" href="https://www.cdc.gov/nchs/covid19/pulse/long-covid.htm">reported</a> having long-term COVID-19 in March 2024.For these people, the pandemic is still very real."When I treat people with long COVID, they are always masked," Chopra said.<h2 id="c71342ed-0973-4d09-978b-dae3bd11fc3e">The future of COVID</h2>As doctors look to the <a target="_blank" class="" href="https://www.businessinsider.com/red-cross-says-world-is-dangerously-unprepared-for-next-pandemic-2023-1">future</a> of COVID, little is for sure."We always say the one thing that is predictable about COVID is its unpredictability," Chow said.But medical professionals told BI they are optimistic that even as the virus continues to evolve, the next few years will continue to look like the past year, with small seasonal surges and hopefully less intense illness.While lax attitudes may be a welcome sign of a cautious new normal, doctors said it's important to remember the lessons we've collectively learned from COVID. The precautions that proved useful against the virus protect people from other illnesses, too.Just because the world has adapted to COVID-19, medical professionals say, doesn't mean we should drop the hygiene, <a target="_blank" class="" href="https://www.businessinsider.com/air-circulation-coronavirus-how-to-judge-ventilation-systems-2020-8">ventilation</a>, and masking lessons learned along the way."We've learned so much about how we can protect ourselves," Chow said. "It's hard to see people leave some of those precautions behind."Doctors recommend people gauge their individual risk, as well as the risk of those close to them when considering which precautions to take in this new COVID age. Addressing a press briefing in Geneva, WHO Director-General Tedros Adhanom Ghebreyesus said that the WHO Covid-19 Emergency Committee will discuss next month the criteria for declaring an end to the Covid-19 emergency, Xinhua news agency reported. "We're hopeful that at some point next year, we will be able to say that Covid-19 is no longer a global health emergency," he said. He added, however, that the <keyword id="8949905" type="General" weightage="20" keywordseo="SARS-CoV-2-virus" source="Orion">SARS-CoV-2 virus</keyword>, the culprit behind the Covid-19 pandemic, will not go away. Meanwhile, Maria Van Kerkhove, technical lead of the WHO's Health Emergencies Program, warned waves of infection and re-infection would continue around the world, as the number of weekly new deaths reported by countries still hovers between 8,000 to 10,000. Mike Ryan, executive director of the WHO Health Emergencies Program, warned that the world still does not know how the SARS-CoV-2 virus will evolve in the future, and such uncertainties add to risks. Before the WHO chief ends the Covid-19 emergency, a balance needs to be stricken between the virus -- including its impact and unpredictability -- and "whether or not we have dealt with the vulnerabilities and the resilience issues in our health systems," Ryan said.
