By Ananya Sengupta <p>New Delhi, Jun 22 () Neetu Sharma spent a sleepless night in the Railways' COVID-care coach stationed in Uttar Pradesh's Mau town, not because of fear of the disease or even the heat but because there were too many mosquitoes. </p><p>She and five members of her family, including her 11-month-old daughter Vanshika, were among the first group of 59 suspected COVID-19 patients to be admitted to any of the 960 modified coaches deployed as Level 1 COVID care centres across five states. </p><p>¨I spent the entire time worrying that if not coronavirus, my daughter will definitely contract malaria or dengue. Her face and her feet are covered with bites and she was crying all the time," Sharma said on Monday, recounting her experience in the COVID-customised coach on the night of June 20 (Saturday). </p><p>"There was also no supply of milk for the children. We had to get milk from our home in Ranipur which is a two-hour drive away," she told over the phone from Mau. </p><p>Swarms of mosquitoes picked on them through their one night stay, the 32-year-old complained. </p><p>All six members of the Sharma family tested negative and were among the eight who were discharged on Sunday. </p><p>According to Sharma, while there were three colour-coded dustbins in the coach, no one came to clean them when they got stuffed with food plates and leftovers. </p><p>However, Sharma and her husband Krishnakant Sharma said they give full marks to the Railways for managing to turn around the coaches and provide so many amenities. But they urged them to try a little harder to address the obvious issues. </p><p>¨There is no point getting one disease while you are being treated for another right? With rains just around the corner, the railways need to address this issue as soon as possible, otherwise they will have to start testing for malaria and dengue before they release patients from here," said Sharma. </p><p>Her husband added that though the Railways needed to be prepared for small children, the preparations overall were excellent given the many restrictions. </p><p>¨I am used to travelling by trains, but when I entered this one, I was taken aback at the sight of the modifications. The stay was comfortable, except for the mosquitoes which were everywhere. The mosquitoes came from under the beds and they should spray repellent under the seats," he said. </p><p>While the Railways has installed mosquito nets on the windows, the Sharmaś said the door of the coaches were kept open all day, facilitating the entry of all kinds of bugs. </p><p>Krishnakant Sharma was admitted to the facility in Mau -- which is among the 372 such coaches deployed across Uttar Pradesh -- after he returned with his family from Mumbai. While their samples were collected at a centre in Mau, they waited for the results while they were in the isolation ward of the Railways. </p><p>Equipped with handheld showers, mosquito nets, bio-toilets, power sockets, oxygen cylinders and more, the COVID-care coaches have it all to keep patients comfortable. But controlling the heat in the steel bodied non air-conditioned coaches has been a huge challenge, officials said. </p><p>At the Mau station, officials said they had kept industrial pedestal fans and stacks of hay for the roof ready to beat soaring temperatures. </p><p>However, Krishnakant Sharma said the weather was quite cool the night of their stay and the fan was enough for them to be comfortable. </p><p>¨I don't know what we would do if the temperature had soared. But I have to say that unlike the reputation of the toilets in trains, the bathroom as well as the toilets in these coaches were clean and it was very convenient to use, even for my elderly mother," he said. </p><p>Krishnakant Sharma, who works at a furniture store in Mumbai'ś Virar area, also had a suggestion for the state government -- please serve more food for the adults and arrange for more milk for the babies. </p><p>There were three other babies in the group and the milk provided was just not enough. </p><p>¨I think the local government was serving the food. It was okay since I cannot demand home food, but just one-and-a-half litres of milk for four babies is definitely not enough. An 11-month-old baby cannot survive on this. At the time we were there, there were three more babies in the coaches. The only thing that they managed to eat was biscuits," he said. </p><p>In an effort to augment the government's efforts to build infrastructure to deal with the spread of coronavirus, the Railways has converted 5,321 non-AC sleeper class ICF (older design) coaches and developed them into COVID Care Level 1 centres for patients with mild symptoms or those suspected to have the infection.  </p>Suspected and confirmed cases will be kept in separate coaches, officials said. But six months later, the pandemic hit and derailed their plans.</p><p>Around then, Tanguy cofounded <a target="_blank" href="https://www.astungkaraway.com/">Astungkara Way</a>, a regenerative travel company. That spurred the couple's decision to put down more permanent roots in Bali, so they started looking for land to build on.</p><p>During their first year in Bali, the couple lived in Kerobokan, an area sandwiched between the bustling, tourist-filled neighborhoods of <a target="_blank" href="https://www.businessinsider.com/gen-x-couple-builds-family-home-bali-sell-empty-nester-2024-6">Canggu</a> and Seminyak.</p><div type="image" title="A pathway leading to the front door of the house. It took us two weeks to clean everything up," Lucie said.</p><p>She added that back in 2020<em>,</em> there was only one local village in the area, although things have changed since.</p><p>"We were like the last point before the rice paddies started and we were promised that it would stay that way," Lucie said. But two months later, a new 160-home development broke ground on the land just beyond their property.</p><h2 id="2e81bc46-8c04-48db-af76-1efbf1733ce9">Building a low-impact house</h2><p>In local measurements, their land spans 15 are — about 16,145 square feet.</p><p>The couple says they paid 4.5 million Indonesian rupiah per are each year for a 25-year lease, which amounts to about 1.687 billion Indonesian rupiah in total, or about $103,500 in today's currency.</p><div type="image" title="The living area and sofa." height="854" width="1139" msid="111047005" hostid="261" sizebytes="157520"></div><p>Building the home took about four months, and they estimate they spent about $70,000 on the build.</p><p>The couple's house is surrounded by their lush outdoor garden. When you have a big concrete villa, the impact of those is colossal," Tanguy said.</p><p>Even the home's interiors are fairly minimal, with each piece of furniture designed by the couple and made by local craftsmen.</p><p>"Maybe we're getting older, we're less attached — and this is also what the idea of coming here was about, to be <a target="_blank" href="https://www.businessinsider.com/shannen-doherty-90210-declutter-downsize-breast-cancer-mom-2024-4">less attached to material things</a> and more attached to how we feel, how we live, how we are as a couple, as parents, and just focusing more on life," he said.</p><p>Now, even with an infant son in tow, they have a slower pace of life.</p><p>Every day, they spend time as a family and walk their three dogs<strong> </strong>along the beach, which is less than 10 minutes away on foot. The couple also has two cats, and they rear chickens and grow their own produce in the garden.</p><div type="image" title="A little play area for the baby." height="854" width="1139" msid="111047010" hostid="261" sizebytes="114724"></div><p>"The only thing that we didn't think about when we were creating the house was that we were going to have a baby, so it's not very baby-proof," Lucie added.</p><p>That said, the layout can be easily adjusted because of the house's design and materials.</p><p>"If we want to add two more rooms, we can still do it within the space," she said. "We can improve it as we grow according to our needs."</p><h2 id="482b5f10-b856-495e-bee0-8f6d7d8f90de">Be part of the solution, not the problem</h2><p id="482b5f10-b856-495e-bee0-8f6d7d8f90de">Tanguy acknowledges that mass tourism has greatly impacted Bali and its natural ecosystems. He points to the <a target="_blank" href="https://www.nationalobserver.com/2024/04/29/news/tipping-point-tourism-sustainability-bali-water-shortages#:~:text=Over%2065%20per%20cent%20of,water%20in%20a%20few%20years.">island's ongoing water crisis</a> and the destruction of Bali's natural landscapes for materials as examples.</p><div type="image" title="The garden." height="854" width="1139" msid="111047003" hostid="261" sizebytes="260668"></div><p id="74819f83-4ea4-476f-86fd-c5b1e0786f20">"I think people should move here if they want to contribute, but if they are here to just extract value from the people, from unregulated resource management, then they should reconsider," Tanguy said. "It's a pretty fragile part of the world."</p><p id="74819f83-4ea4-476f-86fd-c5b1e0786f20">He added that there are plenty of eco-tourism alternatives that are now available on the island.</p><p id="74819f83-4ea4-476f-86fd-c5b1e0786f20">"There are a lot of hotels that are trying to do better, who are growing their own food, doing wastewater treatment or sourcing their food locally," Tanguy said. "If you come to Bali, go there and support. REUTERS/Ricardo Moraes/File Photo" data-mce-source="Thomson Reuters" data-mce-caption="Athletes attend a training session at 2016 Rio Olympics Games' Canoe Slalom Circuit as Brazilian Army soldiers take part in a simulation of decontamination of multiple victims during a a training in Rio de Janeiro" /><p class="source"><span>Thomson Reuters</span></p><p class="caption">Athletes attend a training session at 2016 Rio Olympics Games' Canoe Slalom Circuit as Brazilian Army soldiers take part in a simulation of decontamination of multiple victims during a a training in Rio de Janeiro</p></div><p></p></div></div>The World Health Organization isn't in favor of canceling or postponing the 2016 Olympics.</p><p>In a new statement released Tuesday, the organization said "there is a very low risk of further international spread of Zika virus" because of the games, but women who are pregnant are advised not to attend the games or <keyword id="6354692" type="ORGANIZATION" weightage="60" keywordseo="Travel">travel</keyword> to areas with ongoing Zika transmission. </p><p>Here's the WHO's rationale (emphasis ours):</p><p style="padding-left: 30px;">"The Committee concluded that there is a very low risk of further international spread of Zika virus as a result of the Olympic and Paralympic Games as Brazil will be hosting the Games <strong>during the Brazilian winter</strong> when the intensity of autochthonous transmission of arboviruses, such dengue and Zika viruses, will be minimal and is intensifying vector-control measures in and around the venues for the Games which should further reduce the risk of transmission."</p><p>Essentially, because it will be the winter in Brazil, there won't be as much local transmission of Zika and other viruses, including dengue by mosquitoes. So, the WHO noted, it shouldn't be much of a problem as long as there are good prevention measures in place. </p><p>Zika, which is transmitted mainly <a href="http://www.businessinsider.com/could-genetically-engineered-mosquitos-stop-zika-2016-1">via mosquitoes</a>, has been spreading around the Americas over the last year. Once infected with Zika, only about 20% of people ever show <a href="http://www.cdc.gov/zika/symptoms/index.html">symptoms</a>, which most commonly include fever, rash, joint pain, and red eyes. There is no vaccine or treatment available for the virus. One reason Zika is troubling is because <a href="http://www.businessinsider.com/cdc-zika-causes-birth-defects-including-microcephaly-2016-4">it is a cause</a> of birth defects including microcephaly (a condition where the baby's head is abnormally small) in babies whose mothers have had Zika. The virus also been linked to a neurological condition called <a href="http://www.businessinsider.com/what-is-guillain-barre-syndrome-2016-2">Guillain-Barré Syndrome</a>.</p><p>The WHO did recommend that pregnant women should not travel to areas with Zika transmission, and, since the virus can be sexually transmitted, to refrain from having sex with their partners who travel to areas with Zika. The WHO doesn't want to postpone the Olympics because of Zika - here's the reason why

<p><div class="KonaFilter image-container display-table float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/56d477492e5265bb008ba39d-3413-2560/rtx21lkj.jpg" alt="Rio Olympics Brazil" data-mce-source="Pilar Olivares/Reuters" /><p class="source"><span>Pilar Olivares/Reuters</span></p><p class="caption">A sign in preparation for the Olympics in Brazil this August.</p></div><p></p></div></div>Mounting cases of the Zika virus, a predominantly mosquito-borne virus that's drawn concern primarily because of its <a href="http://www.businessinsider.com/what-is-microcephaly-2016-2">potential links to birth defects</a> in babies whose mothers have had symptoms, has lead to widespread concern over the coming Olympic games, which will take place in Rio de Janeiro.</p><p>Last week, the CDC advised preganant women to <a href="http://www.reuters.com/article/us-health-zika-olympics-idUSKCN0VZ32K">consider skipping the games</a> altogether.</p><p>Still, Zika isn't the only danger to the region.</p><p>Another major concern is dengue, another mosquito-borne infection that sickens as many as <a href="http://www.cdc.gov/dengue/">400 million people a year</a>. It affects many of the same regions <a href="http://www.businessinsider.com/dengue-could-be-the-surprise-culprit-making-zika-worse-2016-2">where Zika is now spreading</a>, especially Brazil. </p><p>"By no means has dengue diminished," Dr. Gary Nabel, the Chief Scientific Officer of Sanofi told Business Insider <a href="http://www.businessinsider.com/sanofi-cso-gary-nabel-on-infectious-diseases-and-vaccines-2016-2">in a recent interview</a>. "It's just as prevalent as it's been the past few years."</p><p>Zika, which typically has low-level symptoms - if they occur at all - likely aren't as debilitating as dengue, which can develop into more serious symptoms including <a href="http://www.who.int/mediacentre/factsheets/fs117/en/">severe bleeding and trouble breathing</a>. </p><p>"I think for example were I going to Brazil and I were an athlete in the Olympic games, I'd have as much cause to worry, maybe more cause to worry [about dengue] at least in the immediate term." </p><p>Those symptoms could make competing and even watching the games incredibly difficult, said Nabel.</p><p>"If you get dengue when you're trying to compete, I doubt you're going to do much of anything," he said. </p><p>It's not only mosquito-borne diseases that present a problem. Last summer, <a href="http://www.businessinsider.com/rios-filth-is-already-spoiling-the-2016-summer-olympics-2015-7">an Associated Press investigation</a> pointed out the health problems with Rio de Janeiro's water, coming to the conclusion that the water was so contaminated that athletes risked becoming violently ill due to the viruses and bacteria lurking around. Those competing in water sports like rowing <a href="http://www.theguardian.com/sport/2015/dec/07/rio-2016-olympic-games-ioc-meeting">have been advised</a> not to splash or swim in the water where they were competing.</p><p>The CDC's <keyword id="6354692" type="ORGANIZATION" weightage="60" keywordseo="Travel">travel</keyword> recommendations for the Olympics and Zika only covered women who are pregnant or trying to become pregnant. People headed to Brazil for the Olympics have more to worry about than just Zika

REUTERS/Ueslei Marcelino" data-mce-source="Thomson Reuters" data-mce-caption="Guilherme Soares Amorim, 2 months, who was born with microcephaly, gets his head measured by his mother Germana Soares, at her house in Ipojuca" /><p class="source"><span>Thomson Reuters</span></p><p class="caption">Guilherme Soares Amorim, 2 months, who was born with microcephaly, gets his head measured by his mother Germana Soares, at her house in Ipojuca</p></div><p></p></div></div>Scientists are working quickly to answer some pressing questions about Zika and its link to a birth defect called microcephaly in which babies are born with abnormally small heads.</p><p>One of the biggest unanswered questions: When are pregnant mothers most at risk for passing the virus along to their unborn children?</p><p>For their <a href="http://www.cdc.gov/mmwr/volumes/65/wr/mm6509e2er.htm?s_cid=mm6509e2er_w#contribAff">report released Tuesday</a>, researchers from the Ministry of Health in Brazil and the Pan American Health Organization analyzed 574 cases of microcephaly in search of more evidence that might help them understand the links between Zika, microcephaly, and a symptom of the virus that occurred during pregnant women's first trimester - rashes and fevers. </p><p>States in Brazil that had locally-transmitted Zika had 2.8 cases of microcephaly per 10,000 live births, while states without active Zika transmission had 0.6 cases of microcephaly per 10,000 live births. This difference in microcephaly rates adds to evidence that could eventually prove Zika is the cause of the birth defect (though they have not yet done so). It could also help clarify when in pregnancy that fetal infection occurs.</p><p>The Zika virus is predominantly transmitted via mosquitoes, though cases of sexual transmission <a href="http://www.businessinsider.com/sexually-transmitted-zika-is-more-common-than-we-thought-2016-3">are increasingly reported</a>. Only about one in five people ever experience symptoms once infected with Zika, which include fever, rash, joint pain, and red eyes.</p><p>The rising number of microcephaly cases in places with active Zika transmission makes the virus especially troubling for pregnant women living in or traveling to these areas. Scientists just got more evidence linking Zika to microcephaly in the first trimester of pregnancy

Here's what they know about each one:</p><ul><li>In two women who tested positive for Zika, their only contact with the virus came from sex with a sick male partner who had traveled to an area with local Zika transmission, or areas where the disease is traveling via mosquito.</li><li>Four more women have tested positive for Zika in preliminary tests but are still waiting for tests to confirm they have Zika.</li><li>The other eight are still under investigation. </li></ul><p><span style="line-height: 1.5em;">In all of the above cases, the sick travelers were men who had reported symptoms of Zika within two weeks of when the female partner's symptoms started. Although all cases of sexual transmission so far have come from male partners, interestingly, the CDC added, "At this time, there is no evidence that women can transmit Zika virus to their sex partners; however, more research is needed to understand this issue."</span></p><p>The Zika virus is typically not dangerous to the average person: only about one in five people ever experience symptoms. However, its links to a birth defect called <a href="http://www.businessinsider.com/what-is-microcephaly-2016-2">microcephaly</a>, in which babies are born with abnormally small heads, make these reports of sexual transmission troubling for pregnant women.</p><h2>For that reason, here are the CDC's guidelines on sexual transmission for pregnant women:</h2><ul><li>Pregnant women and their male sex partners should discuss potential Zika exposure with the woman's healthcare provider. This includes any history of Zika-like infection.</li><li>Men with a pregnant sex partner who have traveled or live in <a href="http://www.businessinsider.com/zika-virus-local-transmission-cases-2016-1">areas where the virus is being locally transmitted</a> should consistently and correctly use condoms or abstain from sex during the pregnancy.</li><li>Generally, men who have traveled or live in areas with local Zika transmission who are worried about sexual transmission should consider using condoms or abstain. </li><li>The CDC isn't sure how long the risk of sexually transmitted Zika lasts, but said they are looking into answering that question. Zika virus tends to persist for about a week in blood, but it's unclear how long it persists in other bodily fluids like semen or saliva. But it's easy to shrug it off as the rose-tinted perspective of a couple luxuriating in an abundance of gifts: intelligence, love, family, success and more money than King Midas.</p><p>I recently spent the day at the Bill &amp; Melinda Gates Foundation, walking a mile in the billionaires' shoes, so to speak, and talking with some of the managers and directors who run various programs there. </p><p>Hearing about their work was inspiring and gave me a newfound hope for the future, just like Bill's.</p><p>And the reason is simple. These people are taking on some of the world's hairiest, most complex and seemingly intractable problems. </p><p>And they are winning.</p><h2>Death to polio and malaria</h2><p>Gates' father, Bill Gates, Sr., was the first to run the family charitable foundation in the late '90s. The foundation has morphed into a much bigger entity over time, especially after it became the main focus of Bill and Melinda Gates themselves.</p><p><div class="KonaFilter image-container display-table float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/55b7924d2acae716008ba9b6-1886/gettyimages-181317443.jpg" border="0" alt="Bill Gates, father, Bill Gates Sr." data-mce-source="Brian Ach/Getty Images" data-mce-caption="Bill Gates with father Bill Gates, Sr." data-link="http://www.gettyimages.com/detail/news-photo/bill-gates-sr-and-bill-gates-are-seen-during-the-the-lasker-news-photo/181317443" /><p class="source"><span><a href="http://www.gettyimages.com/detail/news-photo/bill-gates-sr-and-bill-gates-are-seen-during-the-the-lasker-news-photo/181317443">Brian Ach/Getty Images</a></span></p><p class="caption">Bill Gates with father Bill Gates, Sr.</p></div><p></p></div></div>The elder Bill Gates' presence still hangs large at the Foundation's visitor's center, which is a cross between a techie interactive kids' museum, a community educational facility, and a place to learn about the Foundation's work.</p><p>A video of Gates' dad introduces himself, and explains that because of the "other fellow" - that is, his famous son - he's known as "Senior." </p><p>In its early days, the foundation focused mostly on local charity, but turned its attention to global health after Bill Gates Jr. read an article on children in poor countries dying from preventable illnesses.</p><p>He sent the article to his dad with a note that said: "Dad, maybe we can do something about this."</p><p><span>Those words are now plastered in giant letters in the visitor's center.</span></p><p><span>T</span>he foundation has since become a major force in global health. It joined a global effort to eradicate polio in 2007. In 1988 there were an estimated 350,000 case of polio worldwide. To them, it's proof that hard problems like distributing vaccines to poor, remote children can be solved by smart people and repeatable systems. </p><p>With Bill and Melinda as their leaders, the people at the foundation are stunningly smart with impressive credentials: I met neuroscientists, biologists and infectious disease specialists. </p><p>With polio almost over, the foundation now wants to end malaria.</p><p><div class="KonaFilter image-container display-table float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/5c6737c52628982c15595d92-1500/gates-foundation-mosquito nets.jpg" border="0" alt="Gates Foundation mosquito nets" data-mce-source="Business Insider/Julie Bort" data-mce-caption="Gates Foundation mosquito nets" /><p class="source"><span>Business Insider/Julie Bort</span></p><p class="caption">Gates Foundation mosquito nets</p></div><p></p></div></div>Like the others I met, Philip Welkhoff, the director of the Malaria team, has a personal interest in the mission - and impressive credentials. He grew up in Haiti and had malaria himself as a kid. He became a rocket scientist, then earned his PhD in computational neuroscience from Princeton and turned his attention to global health.</p><p>He loves his job because he's saving lives.</p><p>"All the work that the international community does together saves about 500,000 people from malaria every year compared to the early 2000s. That's a half million lives every single year," he told me. </p><p>The current challenge is to reach the most remote locations, which the foundation has expertise in doing, and containing outbreaks. No malaria vaccine is rolled out yet, but the foundation is having a lot luck by increasing the distribution of mosquito-killing bed nets.</p><p>But even a limited outbreak can turn into a bigger infection once mosquitoes start transmitting the illness, and people move from place to place. And so, malaria is even more complicated to eradicate than polio.</p><p>Still, there's steady progress, and the goal is to reduce malaria deaths 90% by 2030 compared to their 2015 level.</p><h2>The toilet team</h2><p>Outside of malaria, one of the projects that gets a lot of attention is Bill Gates' off-the-grid toilet project, which asks scientists to rethink the toilet.</p><p>Gates launched the<a href="https://docs.gatesfoundation.org/Documents/Fact_Sheet_Reinvent_the_Toilet_Challenge.pdf"> Reinvent the Toilet</a> challenge in 2011, asking researchers to come up with toilets that could sanitize human waste with no water, electricity, sewer or septic system. The toilets were to clean the waste and reclaim the water to safe drinking water standards, as well as to harvest other nutrients for other uses, a game-changer to those living without sanitation.</p><p>Some of those original teams went to the nines on their designs. One team even held a "smell summit" working with perfume makers who recreated latrine smells. This to help them test their methods for countering such smells. That work involving taking whiffs of those lovely scents.</p><p>The director of the toilet project is Brian Arbogast - though his real title is "director, Water, Sanitation, and Hygiene," and his job involves more than just toilets. He left the company to become a cleantech investor, and has been on the boards of various clean-water entities and charities. He worked closely with Bill Gates for decades before joining the foundation in 2013. </p><p>When the toilet project was first started, no one knew if it could really be done, Arbogast said. In 2015, though, one inventor produced a system called the Omni Processor. It's not a toilet, but an off-grid fecal sludge treatment plant that outputs purified water. One day it may also output electricity.</p><p>A working prototype has been operating in Dakar, Senegal in Africa for a few years. Arbogast believes the tech will eventually influence sanitation in the developed world, including green buildings, septic systems and remote, off-the-grid cabins.</p><p><span>The latest version has been licensed to three commercial companies: the US company created by the tech's inventor, Sedron Technologies, as well as a company in China and one in India.</span></p><p><span><div class="KonaFilter image-container display-table"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/5c673ad20d15f50e063cd756-1174/peter janicki and bill gates water.png" border="0" alt="Peter Janicki and Bill Gates water" data-mce-source="YouTube/GatesNotes" data-mce-caption="OmniProcessor inventor Peter Janicki pours Bill Gates a glass of water processed from human fecal sludge." data-link="https://www.youtube.com/watch?v=bVzppWSIFU0&amp;t=25s" /><p class="source"><span><a href="https://www.youtube.com/watch?v=bVzppWSIFU0&amp;t=25s">YouTube/GatesNotes</a></span></p><p class="caption">OmniProcessor inventor Peter Janicki pours Bill Gates a glass of water processed from human fecal sludge.</p></div><p></p></div></div></span></p><h2><span>Poop hats and toilet water</span></h2><p>Bill Gates famously drank the sewage water produced from the <a href="https://www.youtube.com/watch?v=bVzppWSIFU0">Dakar Omni Processor in 2015. </a>And there are lots of stories at the foundation and in the Gates household about that water.</p><p>"Two weeks before [Bill drank], I had to drink from it, as did most of the team," Arbogast told us.</p><p>Melinda Gates told us that her daughter made her drink that water, too. Bill Gates drove around with a 2-liter bottle of it in his car for weeks, and every time someone got in the car, their daughter made them drink some.</p><p>"I<span>f we got sick - which we didn't - it would not have not been from the water, but because we're all passing it around,"  Melinda Gates laughed.</span></p><p><span>Gates spread the love around, too: In 2015, he <a href="https://www.businessinsider.com/bill-gates-jimmy-fallon-poop-water-2015-1">tricked Jimmy Fallon</a> into drinking that water on the air, as well. </span></p><p>And there are actual toilets in development, too, with models on display (but not available for use) at the foundation's visitor's center. </p><p><div class="KonaFilter image-container display-table float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/5c6739502628981de0110629-800/poop emoji hat.jpg" border="0" alt="Poop emoji hat" style="color: #222222;" data-mce-source="Amazon" data-mce-caption="The Poop emoji hat is a symbol of honor for the Gates water sanitation team. It drew in 1,200 attendees, and loads of exhibitions for new sanitation tech.</p><p>Jim Yong Kim, the president of the World Bank, keynoted the event - but Gates stole the show by <a href="https://www.businessinsider.com/bill-gates-wants-world-to-move-past-flush-toilets-2018-11">bringing a jar of poop on stage</a> to use as a visual aid.</p><p>The group wears their association with poop as a badge of honor, Arbogast says.</p><p>He keeps a hat that looks like a poop emoji in his office, and someone on his team often gets to wear it like a crown.</p><p><span>"The poop hat gets passed on for a job well done," he said.</span></p><h2>Worldwide clout</h2><p>Dr. Kathy Kahn is a scientist who funds agricultural projects. Kahn is having success with projects that improve how <a href="https://www.npr.org/sections/thesalt/2019/01/03/681941779/scientists-have-hacked-photosynthesis-in-search-of-more-productive-crops">plants do photosynthesis</a>. These plants grow faster using far less water. It's a promising field for arid countries, as well as for growing food adapted to climate change.</p><p>Kahn came to the foundation after working at the <span>U.S. Department of Agriculture. She said one of the things that makes her work here different is the foundation's name recognition. </span></p><p><span>While other nonprofits toil in obscurity, the Gates Foundation carries prestige. She gets a seat at the table with worldwide policy makers and the top scientists in her field. It's a scientist's dream, she says. </span></p><h2><span>In awe of the Gates'</span></h2><p>One of the themes we heard repeatedly was how much employees appreciated their access to Bill and Melinda Gates. They are heavily involved, eagle-eyed, ask penetrating questions and they work as a team, each bringing different perspectives. </p><p><div class="KonaFilter image-container display-table float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/5a05bfcf3dbef4b5748b4c86-1335/ap110602119379.jpg" border="0" alt="Bill Gates Melinda" data-mce-source="Elaine Thompson/AP Images" /><p class="source"><span>Elaine Thompson/AP Images</span></p></div><p></p></div></div>Progress meetings with them can feel exhilarating and nervewracking, like any employee feels about presenting to their CEO.</p><p>But there's a general sense of good humor, too. </p><p>If a technical issue occurs, say with the videoconferencing system, and an employee can't immediately fix it, Bill Gates can do it without blinking an eye. There's nothing like having one of the fathers of the PC industry in the room when tech support is needed.</p><p>Another theme was just about the thrill of having Bill or Melinda Gates get excited about the employee's projects or work. </p><h2>Helping whole countries get healthier</h2><p>Of all the stories, Dr. Jean Kagubare's was one of the most moving.</p><p>Born in Rwanda, his parents fled the country when he was a baby. He became a doctor and returned to Rwanda right after the genocide of the 1990s. He ran several national health programs and one the country's biggest hospitals before moving to the US and getting his PhD in health systems at John Hopkins. For instance, the foundation has funded a telemedicine app that helps doctors treat far-flung patients in villages without forcing them to travel to a healthcare facility.</p><p>Prior to this program, global healthcare workers would sort of parachute in to a country and focus on their one area - distributing vaccines or providing prenatal care, he explained. Those programs still exist, but now they are coordinated in a more holistic way, and his program measures total healthcare improvement, rather than just progress in one area.</p><p>"<span>Bill Gates asks the question all time: Is our money going to where it should be? Are the right people getting help?" he said. </span></p><p><span>All of this means that Kagubare is helping far more people across the globe, and especially in his home country, than he could when he worked as a doctor.</span></p><p>"<span>I was in Rwanda two months ago and talked to people and patients there. I spent an uplifting day at the Bill & Melinda Gates Foundation and discovered what it's really like to work there

In many parts of the developing world, <span>public health workers are working hard to contain the spread of many of these illnesses - primarily by increasing peoples' access to the shots and pills </span><a href="http://www.businessinsider.com/young-people-more-likely-to-be-anti-vaccine-than-adults-2015-2">we so often take for granted</a><span>.</span></p><p>New, revolutionary treatments have also had a big effect, but gains in this area have stalled in recent decades. <a href="http://www.nobelprize.org/nobel_prizes/medicine/laureates/2015/">Three scientists</a> who were <a href="http://www.nature.com/news/anti-parasite-drugs-sweep-nobel-prize-in-medicine-2015-1.18507">awarded the Nobel Prize on Monday</a> are credited with coming up with <a href="http://www.nytimes.com/2015/10/06/science/william-c-campbell-satoshi-omura-youyou-tu-nobel-prize-physiology-medicine.html">two drugs</a> that have<span> "radically changed [that] situation." </span></p><p><span>They are </span><span><span>Avermectin, a new drug whose derivative, Ivermectin, has come close to wiping out river blindness and has slashed the incidence of lymphatic filariasis, and Artemisinin, a drug that has dramatically reduced the death rates from malaria.</span></span></p><h2>Artemisinin to fight Malaria</h2><p>More than <a href="http://www.who.int/gho/malaria/epidemic/deaths/en/">half a million people</a> - many of them children - are killed by Malaria each year. Over the past 15 years, however, many of those deaths have been prevented, <span>according to a <a href="http://www.businessinsider.com/3-innovations-saved-663-million-people-from-malaria-since-2000-2015-9">recent </a></span><a href="http://www.businessinsider.com/3-innovations-saved-663-million-people-from-malaria-since-2000-2015-9">WHO and UNICEF report</a><span>. </span></p><p>And while simple solutions, including bed nets treated with insecticide, appeared to make the most difference in bringing down incidence rates, the Nobel-prize winning drugs, known as <a href="http://www.who.int/malaria/areas/treatment/overview/en/">artemisinin-based combination therapies</a>,<span> also accounted for a large chunk of the total Malaria cases that were prevented.</span></p><h2>Ivermectin to battle filariasis and river blindness</h2><p><div class="KonaFilter image-container display-table float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/5553ad9569bedd09364446fe-700-482/2778292954_d88e9d4691_o.jpg" alt="lymphatic filariasis elephantiasis" data-mce-source="Wikimedia Commons" data-link="http://de.wikipedia.org/wiki/Elephantiasis" /><p class="source"><span><a href="http://de.wikipedia.org/wiki/Elephantiasis">Wikimedia Commons</a></span></p></div><p></p></div></div>Lymphatic filariasis and river blindness are infections caused by parasites. </p><p>Filariasis can be insidious: People are often first infected with the parasites that cause it as children, but the damage these parasites causes <a href="http://www.who.int/mediacentre/factsheets/fs102/en/">can remain invisible until an infected person is an adult</a>. Around that time, people with filariasis can start to develop elephantiasis, a condition that involves parts of the body swelling to massive proportions.</p><p>Using <a href="http://www.who.int/mediacentre/factsheets/fs102/en/">medicines like the one that just won the Nobel Prize</a>, the WHO hopes to totally wipe out the illness by 2020. These treatments work by clearing baby parasites from the blood and stopping them from spreading to new mosquitoes. When this is done in an area repeatedly for about five years, <a href="http://www.who.int/wer/2010/wer8538a.pdf?ua=1">it can wipe out the disease</a>.</p><p><span>River blindness can develop silently as well, but many sufferers develop symptoms within days or weeks of infection. The parasitic worms that cause it are carried by s</span><span>mall, black flies living near rivers across Sub-Saharan Africa and South America</span><span>. </span></p><p><span>Once they get inside the skin, the worms mate and reproduce. Each day, one worm can spawn another thousand baby worms. As they <a href="http://www.cartercenter.org/health/river_blindness/index.html">spread throughout the body of an infected person</a>, the parasites infect the skin and eyes, where they can cause blindness, skin discoloration, intense itching, and rashes.</span></p><p><span>Public health workers are working to eliminate the disease - or at least limit its spread - by giving out a medicine called <a href="http://www.cartercenter.org/health/river_blindness/index.html">Mectizan, which kills the parasite larvae in the body</a> (and can prevent blindness) and blocks the virus from spreading to others. They've also had some <a href="http://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0011476/">success using Ivermectin</a>, the treatment that just won the Nobel, to help<span> prevent damage to the front of the eye in infected people.</span></span></p>

These Nobel-winning drugs are poised to help wipe out 2 of the world's most horrific illnesses

Now competing with April Ross, she is yet again a favorite to capture gold on the sand this summer.</p><p>With the Olympics a little more than five months away, Business Insider caught up with Walsh-Jennings to discuss her career, her training, the problems facing Rio, and more.</p><p>[<em>Lightly edited for length and clarity</em>]</p><p><strong>Business Insider: </strong><span style="font-weight: 400;">You are coming off a recent surgery on your shoulder, the fifth of your career. I first wanted to ask how you are feeling, and how the surgery is affecting your training leading up to Rio.</span></p><p><strong>Kerri Walsh-Jennings: </strong><span style="font-weight: 400;">Yeah, so I had my surgery September 10, and I feel like it was like three years ago. [Laughs.] At some point you're like, gosh, this rehab is never going to end, and then all of a sudden I'm full strength and I'm about to go to my first competition. So I feel really great. My support system, in my surgeon, my P.T., my pilates, my weightlifting, everything that I'm doing to make my body and my mind stronger is just, you know, has been on point. So I feel very prepared for the journey to qualify, and the journey to wear the gold medal.</span></p><p><strong>BI: </strong><span style="font-weight: 400;">What is your typical training day like? How do you prepare to compete on a surface like sand?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">Well there's only one way to prepare for playing beach volleyball, and that's by getting in the sand. But offseason is really important because generally I'm staying off the sand and getting my feet under me, and it's really important for my body to know what it feels like to have sound footing under me. And then at some point I go back on the sand to get my sand legs. Because it takes a good month for my legs to catch up with everything, with the displacement and all that stuff. So right now we're training on the beach six days a week for practice, and that's generally about two and a half hours. And then I'm doing pilates three times a week, I do a program called Fast Twitch twice a week, basically my lifting and strength training, I'm working with a spine specialist, which has been a new addition for me since my surgery to really get some great support for my shoulder, and I really love the way my body feels. It takes the pressure off my lower back and kind of just shares duties, really, with my body.</span></p><p>And then I'm doing mental training as well. So, you know, body, mind, and spirit - everything is being addressed, every single day. Generally I'll have three training sessions a day. </p><p><strong>BI:</strong><span style="font-weight: 400;"> Wow, it certainly sounds like you are training around the clock. And you also have three children. How do they factor into all of this? My mother is my biggest role model and she showed me how to do it right. She was very present in our lives and has a great career of her own. Same with my father. So I knew it was possible going in to stay closely connected to my children while also chasing big dreams. And it's given me great perspective. It makes me really focused and efficient, which - I was focused before I had babies but I wasted a lot of time. Prior to having babies I thought I thought I was so busy and now I realize just how ignorant I was. [Laughs.] Because, you know, I had </span><em><span style="font-weight: 400;">nothing</span></em><span style="font-weight: 400;"> but time.</span></p><p>But they've made me better. They hold me more accountable for who I am and who I aspire to be, and they make me want to be better. And that's not just as a mommy, but as a woman and as an athlete.</p><p><strong>BI: </strong><span style="font-weight: 400;">What is your nutrition like as an Olympic athlete?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">I</span> <span style="font-weight: 400;">want to be the best role model I can be for my family. I want my husband and I to be the ones our kids look to for guidance, to be the great role models that I had with my parents growing up, so for as hard as we work, I want our kids to see us having fun. I want our kids to know that we have to feel our bodies. And nutrition is a huge part of that. I want to be efficient and effective in everything I do. I start every day with a protein shake, with my vanilla Almond Breeze, and my kids start their day the same way as well. [Nutrition] is a huge piece of the puzzle. It's not just about working hard and being physical. It's about being smart and mindful of what's going in and out of our bodies. And also taking the time to enjoy things, and to appreciate the process.</span></p><p><span style="font-weight: 400;"><div class="KonaFilter image-container display-table float_left"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/56d5c3232e52651a008ba87c-1280-853/gettyimages-150008048.jpg" alt="Kerri Walsh-Jennings" data-mce-source="Cameron Spencer/Getty" /><p class="source"><span>Cameron Spencer/Getty</span></p></div><p></p></div></div></span></p><p><strong>BI: </strong><span style="font-weight: 400;">The Zika virus has been the hot-button issue in the build-up to Rio. When you won the gold medal in London in 2012, you were newly pregnant. How do you think female athletes should prepare for Rio, knowing what we now know about Zika?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">For me, I think educating myself has been huge. I feel like the way it's presented in the media is that if you got to Brazil you're flipping a coin on your health. I don't think it has to be that way. If I were wanting to have a baby right after the Olympics I would take precautions, and then when the Olympics were over I would get tested to make sure I didn't have the virus in me, and then I'd go for it. I would take all the precautions, I'm going to bring my natural remedies, keep the mosquitoes away naturally and I'm going to bring my DEET. I'm going to do my part. And again, it's something, when a baby is involved you don't want to take any risks. So I would just exercise a lot of caution and do my part and then get tested and go for it.</span></p><p><strong>BI: </strong>A<span style="font-weight: 400;">re there other worries you or other Olympic athletes have about Rio?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">Well we're going to Brazil. And every time I've gone to Brazil I've gotten sick upon return. You know, it's just a different situation there. And I take every precaution - eating cooked foods and staying away from tap water, brushing my teeth with bottled water - and yet I still manage to get sick. So I'm just going to stay on point, bring my probiotics. Ideally we're going to stay at an American hotel. We just want to take every precaution. Because if you get sick out there, and you're playing in that hot sun and you get depleted, it's going to be a problem. So that would be the only other thing I think that athletes on a whole are going to be worried about. But again, we have to do our part and take every precaution and be smart.</span></p><p><span style="font-weight: 400;"><div class="KonaFilter image-container display-table float_left float_right"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/56d5c42b2e526543008ba67b-1280-853/gettyimages-150007261.jpg" alt="Kerri Walsh-Jennings Misty May" data-mce-source="Cameron Spencer/Getty" /><p class="source"><span>Cameron Spencer/Getty</span></p></div><p></p></div></div></span></p><p><strong>BI: </strong><span style="font-weight: 400;">At the 2000 Olympics you competed with Team USA's indoor volleyball team. And then at the 2004 Olympics you switched to beach volleyball. What prompted that switch?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">At the point when I switched from indoor to beach I had been playing indoor for 12 years. And, to be honest, to make a living indoors you have to go overseas. I am such a family girl and just wanted to be home, so that didn't appeal to me. Misty May was looking for a partner, I was looking to stay at home, and the beach just came calling. And mostly I stuck with it because I loved the challenge of it, but also just the autonomy of it. It's two on two, just you and your partner, you're not one of the herd. And the lifestyle is unbeatable.</span></p><p><strong>BI: </strong><span style="font-weight: 400;">I'm curious to hear more about partner dynamics. You and Misty May were obviously so dominant for so long, and part of that dominance seemed to have something to do with how close you two were. How did you two establish such a solid relationship?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">I think from day one we just had a synergy about us. We had a flow. And then we had twelve years to build off that. I firmly believe that when you have a foundation of respect, love of what you're doing, and a shared vision, so much is possible. Misty and I had something magical, and I think at the basis of that was so much trust. Especially in the really competitive situations where it really felt do-or-die, we were unbreakable. We always stuck together, even if we lost we were together. And that's my mission with April. We're working so hard, but it's something you cannot force. It's a process, and April and I are well into that process. And it's going to be really fun to go and do it our way. But trust is hugely important on any team, no matter how big or small it is.</span></p><p><span style="font-weight: 400;"><div class="KonaFilter image-container display-table"><div><div class="image on-image"><img src="https://static-ssl.businessinsider.com/image/56d5d5892e5265bc008ba7a8-1280-855/gettyimages-149857683.jpg" alt="Kerri Walsh-Jennings" data-mce-source="Ryan Pierse/Getty" /><p class="source"><span>Ryan Pierse/Getty</span></p></div><p></p></div></div></span></p><p><strong>BI:</strong><span style="font-weight: 400;"> Having already played in four Olympics and won three gold medals, you are obviously a pretty popular athlete, and yet your sport gets little attention outside of the Olympics. How much of a responsibility do you feel you have in growing the sport of beach volleyball?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">I take my role as an ambassador for the sport, and as a role model for boys, girls, mommies, daddies - whoever it is - very, very seriously. I know the impact my role models have had in my life, and I'm in a really beautiful position to be able to be that for others. As far as my sport is concerned, my mission is to make it as big as it deserves to be. We've been growing, it's just not as noticeable. The NCAA picked beach volleyball up to be a championship sport, and it was the fastest test sport that's been adopted. That's a really big deal for our sport because that just means the USA system is going to have a feeder system from the college system.</span></p><p><strong>BI: </strong><span style="font-weight: 400;">You have the chance to become the second woman after Lisa Leslie to win a gold medal in four consecutive Olympics in a team sport. How much are you thinking about that as you prepare for Rio?</span></p><p><strong>KWJ: </strong><span style="font-weight: 400;">April and I still have to qualify, but what a great opportunity. If and when we do [qualify], we're going after gold and we're going after nothing less. The legacy will take care of itself as long as we do our job to the best of our ability. And I have so much faith in us and I mean I honestly feel like my job since I was 22 has been to be an Olympian. That's a very privileged job and I love it with all my heart. I'm planning on playing my best volleyball to date throughout 2016 and certainly during the Olympics.
