There is no country in the world in terms of "margin protection" better than <keyword id="3658" type="General" weightage="50" keywordseo="india" source="Orion">India</keyword> where technology is extensively used to protect clients' securities, <keyword id="63783" type="General" weightage="50" keywordseo="securities-and-exchange-board" source="Orion">Securities and Exchange Board</keyword> of India (<keyword id="35210" type="General" weightage="100" keywordseo="sebi" source="Orion">SEBI</keyword>) chairperson Madhabi Puri <keyword id="1040391" type="General" weightage="80" keywordseo="buch" source="Orion">Buch</keyword> said on Friday. The SEBI chief said that foreign investors often tell her that she was fortunate to have in the country a culture of data and technology, which is making the role of the capital market regulator effective. "There is huge technology change in the entire market. It has never been done anywhere in the world ever...and we did it in India. So, today in terms of margin protection - the protection of client's securities - there is nobody better in the world than India," Buch said in her lecture on 'Data and Technology in the Capital Market'. The event was organised by the Indian Institute of Management-Bangalore to mark its 49th Foundation Day celebrations on Friday. Explaining how data is used for surveillance to check malpractices during trading, the SEBI chairperson said the study of algorithm and alerts make it easy to study the trading pattern of people and understand that this is 'insider trading'. SEBI has also embedded a QR code which, Buch said, was quite radical. When it is scanned, one can see a video clip, which showed the 'Delta' positions of the particular individual whose trading pattern is reflected. "You look at that film, and you say, 'this is insider trading. This cannot be anything else'. So, we use data in our surveillance," the SEBI chief explained. The regulatory body is also using technology to check 'front running' in the mutual fund market. She, however, said SEBI too had some setbacks because the regulation has not kept pace with technology but it is trying to fix the problem in the next few months. The SEBI chief said the board is now using 'Suptech' or Supervisory Technology as well to inspect and make sure that everybody's following the rules. Citing an example from the mutual fund industry, she said SEBI started collecting data from all the 44 mutual fund houses operating in the country. As the data started coming in, SEBI told the industry that they are going to develop 80 algorithms and explained to them the logic and the interpretation of SEBI laws in the light of the algorithm emanating from the data. According to Buch, many of her colleagues were quite upset as they were against revealing the algorithm and questioned the objective behind it. Buch, however, said she gave an explanation that satisfied them. "The objective of the regulator is not to catch people doing bad things but to prevent them from doing bad things. So, we declared that we are now going to make this 80 algos," she explained. Today, SEBI conducts inspections every quarter, which otherwise could be done on a daily basis, because the board wanted to give the industry time to find their own mistakes and fix them instead of showing up every day, Buch said. "Now, this is only the beginning because this is the easy stuff. So, what's the tough stuff? The tough stuff is where you have to try and find violations of the spirit of the law, and not the letter of the law," the SEBI chief said. On Sunday, Medanta Hospital had put out a health bulletin stating that the veteran leader's condition was quite critical and that he was on life-saving drugs.

At the same time, for vascularisation, a flap of muscle from the chest was transplanted and the muscles of the hand were repaired. Simultaneously, to fix the broken humerus mid shaft, the orthopaedic team decided to do internal bone fixation," he explained. Doctors re-attach severed hand of MBBS student in UP

<keyword id="66" type="General" weightage="100" keywordseo="delhi" source="Orion">Delhi</keyword> hospitals are witnessing a spike in cases of the H3N2 virus that triggers symptoms like fever, cold and body aches, but in certain cases, leaves behind persistent cough making patients extremely weak, doctors said. There has been a nearly 150 per cent rise in patients coming to OPDs with such complaints, they said. Elaborating on the possible reasons behind the surge, Dr Viny Kantroo, Consultant, Respiratory, Critical Care and Sleep Medicine Consultant at Indraprastha Apollo Hospitals, said the outbreak could have been triggered due by seasonal change, mutation of the virus and the economy being fully open. "Children are going to school and they are transmitting it to the elderly. A lot of cross-country travel is happening. In the last two years, <keyword id="8764262" type="General" weightage="80" keywordseo="covid" source="Orion">Covid</keyword> was the dominant virus and there were restrictions but with the relaxation of norms and return of normalcy, these outbreaks are being observed," she opined. Delhi government's LNJP Hospital has set up a 20-bed isolation ward in the emergency block for such patients. In accordance with the ICMR guidelines, we have done so and also stocked up on medicines. A 15-doctor team has been constituted to monitor the patients, said a senior doctor. Dr <keyword id="5518347" type="General" weightage="80" keywordseo="rajiva-gupta" source="Orion">Rajiva Gupta</keyword>, Senior Consultant - <keyword id="329004" type="General" weightage="80" keywordseo="internal-medicine" source="Orion">Internal medicine</keyword> at the CK Birla Hospital, Gurugram and Delhi, said the ILI (Influenza-like illness) cases have seen a spike in the last few days. "For instance, if we were seeing two to three patients in OPDs last month, this month there is one-and-a-half times rise. The symptoms are fever, discomfort, cold, and body aches. In some cases, patients also experience abdominal discomfort, diarrhoea, and even fullness in ears," he added. The senior doctor said the treatment approach changes lightly when the patient has comorbidities and stressed that in those cases, they ask the family members to monitor the BP, pulse, oxygen saturation levels and consciousness levels. "If these levels fluctuate, the patient needs hospitalisation," he stressed. Explaining the current strain, Dr Vikas Deswal, Senior Consultant, Internal Medicine, Medanta, Gurugram said the influenza virus is the most prevalent virus that affects our respiratory system and is present in three different types: A, B, and C. "Among these, subtype A is the most commonly found. One subtype of the influenza A virus is H3N2, which produces symptoms similar to other flu viruses such as cough, fever, cold, sore throat, fatigue, muscle pain, and respiratory complications, especially in children under two years, the elderly, and those with other medical conditions," he said. Both the doctors said in some cases, it has been observed that patients have persistent coughs even after the fever subsides. "One of my patients said he was having a persistent cough and could not attend meetings because of it. Patients also develop extreme weakness due to it," said <keyword id="6356160" type="General" weightage="80" keywordseo="gupta" source="Orion">Gupta</keyword>. Deswal cautioned against letting down a guard at this time and said since viruses mutate over time, it is important to take precautions. "Antibiotics are not effective in treating viral infections such as H3N2 and can be harmful, so people should avoid taking them, especially if they have H3N2. "The virus spreads through droplets and direct contact, so it is recommended to follow precautions such as avoiding touching surfaces, washing hands, following Covid-appropriate behaviour, practising social distancing, wearing masks, and avoiding going out if experiencing viral symptoms. Additionally, it is important for people to get vaccinated against influenza every year," he said. Delhi hospitals witnessing spike in H3N2 cases, say doctors

 The front-runners to acquire Hyderabad-headquartered hospital chain are <keyword id="34499" type="POSITION" weightage="80" keywordseo="private-equity">private equity</keyword> group Abraaj, and a consortium of PD Hinduja Hospital, Everstone Group and even South African healthcare chain Netcare and Bain Capital, Singapore's Temasek Holdings and Baring Private Equity Asia. 
 
They have submitted offers and the final negotiations are underway with the CARE Hospitals Group, which is owned and managed by Quality CARE India Ltd and runs 17 hospitals across nine cities in India. 
 
Sources told Economic Times that majority of the offers have valued the chainbetween Rs 1500 crore - Rs 1800 crore inclusive of its debt. 
CARE Hospitals’ founders developed Asia’s first indigenous <a href="http://www.businessinsider.in/searchresult.cms?query=coronary" target="_blank">coronary stent</a>. The chain plans to add 300 to 400 beds over the next 18 months. 
 
However, <a href="http://www.businessinsider.in/searchresult.cms?query=healthcare+industry" target="_blank">healthcare industry</a> experts have a mixed reaction about the prospects of CARE. 
 
"It is a fully juiced out asset and doesn't throw out cash like a Medanta or Apollo. It is also largely based in Tier II cities and have smaller operations while its flagship property is in Hyderabad, a fiercely competitive market," an investment banker, following this trade, told ET.

CARE Hospitals get buyout offer of Rs 1,800 crore from PE majors

Doctors in India are grappling to diagnose and treat unexplained and persistent symptoms of <keyword id="9814327" type="General" weightage="100" keywordseo="long-covid" source="Orion">long Covid</keyword> patients due to limited guidelines, whereas researchers have flagged inadequate studies on the condition. With the <keyword id="19896311" type="General" weightage="20" keywordseo="world-health-organization" source="keywords">World Health Organization</keyword> declaring an end to <keyword id="19896627" type="General" weightage="20" keywordseo="covid" source="keywords">Covid</keyword> as a global <keyword id="416511" type="General" weightage="20" keywordseo="health-emergency" source="Orion">health emergency</keyword> in May last year, focused efforts are underway around the world to estimate the burden of long Covid among the population. The condition refers to the set of lingering symptoms affecting varied body parts and persisting well beyond the acute Covid infection period, including cough, muscle and joint pain, fatigue, brain fog and difficulty in focusing. The viral disease is caused by the SARS-CoV-2 virus. While studies have suggested that about a third of those moderately or severely infected are likely to suffer from long Covid, region-wise though, incidence could vary. A study by researchers, including those from <keyword id="19988007" type="General" weightage="20" keywordseo="harvard-medical-school" source="keywords">Harvard Medical School</keyword>, US, estimated that 31 % of the once-infected people in North America, 44% in Europe, and 51% in Asia, have long Covid, which is "challenging the healthcare system, but there are limited guidelines for its treatment". It was published in the International Journal of Infectious Diseases in September. In India, however, studies on long Covid are few and far between. One such study by Maulana Azad Medical College in New Delhi, conducted from May 2022 to March 2023 on 553 patients who had recovered from Covid, found that about 45% had lingering symptoms, persistent fatigue and dry cough being the most common. "There is limited exploratory research on the long Covid syndrome with scarce data on long-term outcomes," the authors wrote in the study published in the journal Cureus in May this year. Understanding the long-term effects of the virus is important for developing management strategies, optimising healthcare delivery, and providing support to recovered Covid patients in the community, they said. Dr Rajesh Sagar, Professor of Psychiatry, All India Institute of Medical Sciences (AIIMS), New Delhi, said, "Looking at the current state of long Covid studies in India, it is too premature to say that we understand the condition well enough to know how to diagnose or treat it." Animesh Samanta, assistant professor at School of Natural Sciences in <keyword id="19897094" type="General" weightage="20" keywordseo="shiv-nadar-university" source="keywords">Shiv Nadar University</keyword>, Greater Noida, said, "While studies in India highlight the growing recognition of neurological complications in long Covid patients, more focused research on neuroinflammation is needed." Doctors, too, have reported a rise in patients complaining of symptoms that they did not have pre-Covid. "People who never had <keyword id="6465125" type="General" weightage="20" keywordseo="asthma" source="Orion">asthma</keyword> in the past, post-Covid, with every viral infection, they get a long cough, shortness of breath and wheezing, which require the use of <keyword id="31257" type="General" weightage="20" keywordseo="inhalers" source="Orion">inhalers</keyword> or nebuliser," senior consultant Dr Neetu Jain, who runs a post-Covid care clinic at Pushpawati Singhania Hospital and Research Institute, New Delhi, said. Dr Arun Garg, chairman, Neurology and Neurosciences, Medanta-The Medicity, Gurugram, said he was noticing a spike in stroke cases among young patients not suffering from known risk factors such as diabetes, hypertension and obesity. "Similarly, we are seeing more cases of encephalopathy (swelling of the brain) without reason and having a confused state of mind following one or two days of fever. Their MRI scans show no changes. These patients have increased significantly after Covid," he said. In the absence of medical guidelines to diagnose long Covid, doctors are having to resort to broad, non-specific tests and questionnaires to gauge a patient's 'quality of life'. Studies have shown that the fatigue experienced in long Covid is similar to that in cancer patients, with a quality of life similar to patients of Parkinson's disease. "We really do not have any test to diagnose long Covid, even though it is definitely a clinical diagnosis. We diagnose long Covid for people who had at least moderate to severe infection, following which they could never regain the quality of life pre-Covid. Checking for inflammatory markers like C-reactive proteins (CRP) can support the diagnosis," Dr Jain said. "Other than routine blood tests that measure inflammation, we do antibody tests to look for direct markers. In many of these patients, we are finding rare antibodies which are very new to us and were not there pre-Covid," Dr Garg said. Inflammation persisting despite recovery from acute Covid infection is thought to lie at the heart of long Covid. However, tests to measure this specific immune response are lacking, even as researchers have been working in this direction worldwide. One such effort comes from Shiv Nadar University, where a team led by Samanta has developed a fluorescent probe capable of detecting inflammation in brain cells that can arise due to Covid infection. The probe measures nitric oxide levels in brain cells, especially in human microglia cells, where increased NO levels are linked to the SARS-CoV-2 infection. Microglia are immune cells in the brain that fight disease and help maintain brain health. Lysosomes within microglia, which help clear foreign disease-causing agents like the SARS-CoV-2 virus, produce nitric oxide as part of an immune response to infection. The probe detects nitric oxide produced in lysosomes in response to infection and thereby allows for a measurement of inflammation levels. This examination method can provide "qualitative information on infection status", said Samanta, corresponding author on the study published in the journal Analytical Chemistry in American Chemical Society. He explained that patients with pre-existing conditions such as Alzheimer's disease, Parkinson's disease or multiple sclerosis (an autoimmune disorder) could exhibit prolonged neuroinflammation and loss of brain cells following Covid infection. While the probe has shown efficacy in cell cultures, animal studies would need to be done, before testing it in humans, Samanta said. Looking at the World Health Organization International Clinical Trials Registry Platform, the study by Harvard Medical School had found that 587 clinical studies were conducted on long Covid, of which about 53% (312) were testing potential treatments. Most of these were found to be conducted in the US (58), followed by India (55) and Spain (20). The trials looked at interventions including physical exercise, psychotherapy, and pharmacological ones such as paxlovid and fluvoxamine. However, "to date, only 11 of these 312 studies have published their results that were not confirmative," the researchers wrote. The team called for studies to look into sleep disorders which were rarely included in the registered clinical studies. Further, interventions targeting the biological processes responsible for long Covid are needed but currently lacking, they said.

Doctors struggle to treat long Covid patients in India; researchers point to inadequate studies

The study conducted by a team led by Dr <keyword id="140987" type="General" weightage="50" keywordseo="arvind-kumar" source="Orion">Arvind Kumar</keyword> from Medanta hospital observed that an increasing number of patients treated in the outpatient clinic were non-smokers of a relatively younger age group. Details of patients who underwent treatment between March 2012 and November 2022 were analyzed to assess the demographic details of these patients and chart a trend. A total of 304 patients were analyzed retrospectively. In the study, the age at presentation, gender, smoking status, stage of disease at the time of diagnosis and type of <keyword id="33225" type="General" weightage="80" keywordseo="lung-cancer" source="Orion">lung cancer</keyword> were recorded and analyzed, besides other parameters. Speaking about the study, the Chairman of the Institute of Chest surgery, Chest Onco-surgery and <keyword id="1825127" type="General" weightage="50" keywordseo="lung-transplantation" source="Orion">Lung Transplantation</keyword> at Medanta hospital Gurugram, Dr Arvind Kumar said, "I am shocked at the alarming rise in <keyword id="49477" type="General" weightage="100" keywordseo="lung" source="Orion">Lung</keyword> cancer cases and occurrences in younger individuals, non-smokers, and women. Lung cancer is a dreaded-disease, with one of the lowest 5-year survival rates". Dr Kumar further stated, "While conventional wisdom says that smoking is the main cause, there is strong evidence now, that points to the increasing role of air pollution in the rising incidence of lung cancer". Speaking about the initiative by the Medanta hospital with regards to Lung Cancer initiatives, Dr Kumar asserted, "#BeatLungCancer is a humble initiative by Medanta to increase disease awareness, promote screening for early detection, and provide inspiration and support through stories of strength of other patients". Dr Naresh Trehan, Chairman and Managing Director, Medanta, stated that "Findings of this study which indicate a rising incidence in earlier unaffected population groups is a matter of concern". "An overall increase in the incidence of Lung Cancer in both men and women was observed. In men, Lung Cancer is leading in men in terms of prevalence and mortality, while in women it has jumped to No 3 (as per Globocan 2020) from No 7 (as per Globocan 2012) over 8 years", mentioned in the study. "Nearly 20% of patients were found to be less than 50 years of age. The trend showed Lung Cancer developing in Indians about a decade earlier than in their western counterparts. Nearly 10 per cent of all patients were less than 40 years of age with 2.6% in their 20s", the study mentioned. In the study, it has been found that 50% of patients were non-smokers, "Nearly 50% of these patients were non-smokers. Of this, 70 per cent of patients were in the younger-than-50-years age group and 100 per of patients under 30 years of age were Non-smokers." The incidence of Lung Cancer was rising in women, who accounted for 30 per cent of the overall patient load and all were non-smokers. In the past, this figure was much lower (as per Globocan 2012), revealed by the study. The study further stated that 80 per cent of patients were diagnosed in the advanced stage of the disease when complete treatment is not possible, and the intent of treatment gets restricted to palliative. In nearly 30 per cent of cases, the patient's condition was initially misdiagnosed as Tuberculosis and treated for many months leading to a delay in definitive diagnosis and treatment, the study indicated. The majority of patients were present with Adenocarcinoma against <keyword id="15597191" type="General" weightage="50" keywordseo="squamous-carcinoma" source="Orion">Squamous Carcinoma</keyword>, which dominated earlier reports. Notably, Adenocarcinoma forms when cells lining the outside of the lungs become cancerous, whereas Squamous Carcinoma impacts the cells that line the surface of airways. Former is known to have relatively poorer outcomes. The study indicates that in the coming decade we are very likely to see an increase in the number of non-smoking Lung Cancer patients of the female gender in the younger age group. This risk group is very different from the earlier dominant at-risk demographic of older men who smoke tobacco. According to the study, "there is an urgent need to raise awareness about the risk of Lung Cancer across different sections of the society for appropriate actions. Effective measures to reduce tobacco consumption and control air pollution will help control the rising graph of Lung Cancer cases.
