"Because I made the informed choice to get vaccinated, there will be a 94.5% reduced risk, I'll get COVID-19. In May, <a href="https://www.cnn.com/videos/health/2020/04/15/coronavirus-vaccine-timeline-acfc-vpx.cnn">Corbett told CNN</a> a vaccine could be ready for the general public by spring 2021. Now, <a href="https://www.deseret.com/u-s-world/2020/12/29/22204426/covid-19-vaccine-vaccination-numbers-total">2.1 million Americans</a> have already received their first doses of a vaccine from either Pfizer or Moderna. <a href="https://covid.cdc.gov/covid-data-tracker/#vaccinations">According to the Centers for Disease Control and Prevention</a>, more than 11 million doses of the vaccine have been distributed. However, public health experts have been warning for months that the legacy of medical racism in the US might stand as a barrier to equitable vaccination across the US. It's an issue Corbett has been calling attention to, as she tries to instill trust in the vaccine by posting about it on social media. "This overarching mistrust of the medical institution, in general, is something that is being highlighted now because of the dire circumstances of which we're in," <a href="https://www.cnn.com/2020/12/16/health/kizzmekia-kizzy-corbett-interview-moderna-vaccine-gupta/index.html">Corbett told CNN</a>. "But it is not news to me, because I'm Black and I have a Black family and I am well-read on the history of injustice when it comes to medicine in the Black community."<h2>Corbett wants to rebuild trust between Black Americans and a medical system that discriminates against communities of color </h2>According to a Pew Research survey released December 4, while <a href="https://www.npr.org/sections/coronavirus-live-updates/2020/12/04/943213216/survey-finds-asian-americans-are-racial-or-ethnic-group-most-willing-to-get-vacc">63% percent of white Americans</a>, 83% of English-speaking Asian Americans, and 61% of Latinx Americans report they will get the vaccine, only 42% percent of Black Americans report they will receive the vaccine. A <a href="https://www.cnn.com/2020/12/15/health/survey-covid-vaccine-kaiser-monitor/index.html">Kaiser study released December 15</a> found 35% of Black Americans say they will probably or definitely not receive the vaccine. Experts, <a href="https://www.bet.com/news/national/2020/07/29/covid-19-vaccine-black-people-weary-dr-anthony-fauci-interview.html">including Dr. Anthony Fauci</a>, point to centuries of racist medical abuse, from the <a href="https://www.bese.com/how-puerto-rican-women-made-birth-control-possible-at-the-expense-of-their-health/">forced sterilization of women of color to develop birth control</a> to the <a href="https://www.theguardian.com/us-news/2017/jun/24/tuskegee-syphilis-study-victims-trump-opposes-memorial">Tuskegee syphilis experiment,</a> that have caused many Black Americans to be wary of medical institutions. Anticipating the disparity, both Pfizer and Moderna intentionally tried to increase the number of Black participants in the trial phases of their vaccines, <a href="https://www.businessinsider.com/covid-19-vaccine-black-americans-phase-3-trials-2020-9">Insider's Taylor Ardrey</a> previously reported. "The first step is that scientists and physicians and vaccine developers and et cetera, is to understand that the onus of this problem is not on [communities of color] and their distrust; it is on us and our level of trustworthiness. Internal emails obtained by Insider show the agency has "exhausted" its vaccine supplies after vaccinating 3,000 of its employees.<ul><li>What we don't know: It's not clear how many NIH employees who worked directly on the vaccine have received the shot. We do know that NIH Director Francis Collins, Dr. Anthony Fauci, and Kizzmekia Corbett, the lead scientist on the vaccine project, have all received it.</li></ul>Not all agencies are created equal: "The Centers for Disease Control and Prevention decided last year that the workers at the Bureau of Prisons, the Indian Health Service, the State Department, the Defense Department, and the Department of Veteran's Affairs should be first to get vaccinated."<ul><li>Where those efforts stand: "At the VA, 68% of workers were vaccinated as of February 10. FDA approval for Ervebo finally <a href="https://www.fda.gov/news-events/press-announcements/first-fda-approved-vaccine-prevention-ebola-virus-disease-marking-critical-milestone-public-health">arrived in December 2019</a>.Offit predicts a coronavirus vaccine will be distributed widely in the US on similar grounds."I honestly don't think it will be FDA-licensed when it rolls out," Offit told Business Insider. "It will be the first time a modern vaccine was distributed to the public without FDA licensure."He said he could see people getting a coronavirus shot at their local pharmacy, much like we do the seasonal flu shot.<h2>'There will be fights about who gets priority'</h2>Arthur Caplan, director of New York University's Division of Medical Ethics, said the development of a successful coronavirus vaccine poses serious moral dilemmas.Manufacturers may <a href="https://www.statnews.com/2020/05/01/infect-volunteers-with-covid-19-in-the-name-of-research-a-proposal-lays-bare-a-minefield-of-issues/">use human challenge trials</a>, for example, in which healthy volunteers consent to take the vaccine and are then deliberately exposed to the virus. That would eliminate the need for placebos but puts participants at risk.It also means that tens of thousands of people worldwide would be immunized before a vaccine is available for general use in the US."There's this idea that the vaccine is a get-out-of-jail-free card," Caplan told Business Insider. "But the reality is that we'll see the biggest ethical challenge the world has ever seen."WHO director-general Tedros Adhanom <a href="https://www.who.int/dg/speeches/detail/who-director-general-s-opening-remarks-at-the-launch-of-the-access-to-covid-19-tools-accelerator">has prioritized</a> equitable access to any successful vaccine."Past experience has taught us that even when tools are available, they have not been available equally to all," Adhanom said. Internally, governments would prioritize healthcare workers and populations with serious risk factors — the elderly, immunocompromised, and individuals with underlying health conditions.Caplan also emphasized focusing on populations that might otherwise be overlooked."There will be fights about who gets priority," he told Business Insider. "It makes sense to say, well, where's the worst impacted area? Or where is it likely to spread the most from?"Among those potential hot zones, Caplan said, are "slums, refugee camps, and prisons."<h2>How the US may roll out a coronavirus vaccine</h2>Outside of the first batch of essential workers, the US is unlikely to see a federal decree about the order in which people get vaccinated, Caplan said."Governors may get into it," he said, adding that they may not follow advice from medical experts. "A lot of them aren't paying any attention to recommendations about closing down right now, right?"Lawmakers may push for immunizing those in industries supporting America's food supply, Offit said, some of which have been <a href="https://www.usatoday.com/in-depth/news/investigations/2020/04/22/meat-packing-plants-covid-may-force-choice-worker-health-food/2995232001/">hard-hit by the virus</a>.The CDC can make recommendations about immunizing the elderly and immunocompromised first, but it would be up to providers to follow those guidelines.
