Coronavirus hits Mumbai's Jaslok hospital, 31 nurses, 5 doctors test positive

Spinocerebellar ataxia (SCA), also known as spinocerebellar atrophy or spinocerebellar degeneration, is a progressive, degenerative, genetic disease found in the <keyword id="781" type="PERSON" weightage="60" keywordseo="Agarwal">Agarwal</keyword> community in India. 
 It is a rare disease that afflicts only the Agarwal community and has intrigued doctors for nearly a decade since the definite gene test for its diagnosis has become available. Surprisingly, it is more common among people in the Agarwal community in Bengal compared to those in <keyword id="41646" type="COMPANY" weightage="60" keywordseo="Haryana">Haryana</keyword> or <keyword id="6355588" type="ORGANIZATION" weightage="20" keywordseo="Rajasthan">Rajasthan</keyword>. 
 
Where is it found? 
 Around 43 types of SCA have been detected around the world. In India, types 1, 2, 3 and 12 of the disease are the most common. While the other three are more commonly found, <keyword id="3882528" type="UNKNOWN" weightage="20" keywordseo="SCA-12">SCA-12</keyword> is yet to be detected outside the particular community in Bengal. 
 
What does the disease do? (Symptoms) 
 
SCA-12 starts with a mild tremor of the hands and gradually affects walking. Eventually, it leads to significant loss of limb co-ordination, eye movement co-ordination and slurred speech. 
 
Diagnosis 
 “The disease can be diagnosed when the patient is in his/her 30s or later. It also has a probability of 50 percent in the Agarwal community,” said Dr. <keyword id="3882526" type="PERSON" weightage="20" keywordseo="paresh-doshi">Paresh Doshi</keyword>, Director of Neuro Surgeon, <keyword id="6412249" type="ORGANIZATION" weightage="100" keywordseo="Jaslok-Hospital">Jaslok Hospital</keyword> &amp; Research Centre. 
 
• Develop an action tremor of the upper extremities in mid-life 
• A mild gait abnormality and more prominent parkinsonian features 
• Psychiatric disorders and dementia in some of the old individuals. 
 
The Story of a 57-year old woman 
 
A 57-year old housewife (who has 3 kids); resident of Kharghar was leading a good normal healthy life until she started showing some symptoms like involuntary nodding of head in 2003. Gradually her symptoms started increasing, she started having tremors in her right hand which then progressed to left. Gradually she started having difficulty while walking, would walk with support in 2014, and so she avoided parties as travelling to places became difficult for her. 
 
Treatment 
 
A team of doctors has successfully performed world’s first surgery for tremors of different etiology with good success since surgery for SCA -12 tremors had never been attempted before in the world. On careful review of the literature and available data the team of doctors at a hospital in Mumbai (Jaslok Hospital) decided to use a novel target for the control of these tremors. 
 
“There is no treatment to this disease, only symptoms can be controlled. This makes life much less miserable and worth living for patient/s,” quoted Dr. Doshi.

However, snacking between meals is also important for diabetics and it helps maintain post meal sugar level, Dr Chandalia explained. “In our practice, we have almost found 55%-60% of diabetic patients to be obese or overweight. We have to give them a little bit lower calories during the day — 500 calories less than what they are eating and we have to divide the meals very well because if the same amount of calories is consumed in only two big meals, then the post-meal sugar management will become difficult,” he added. Dividing meals is very important for diabetics, and a good practice in general too. Having more than two meals and a few snacks during the day helps in dividing the intake of calories, carbohydrates and protein and therefore, management becomes a lot easier. However, the snack has to be healthy. Taking a dim view of abusive behaviour towards healthcare professionals amid the nationwide lockdown, the Indian Medical Association (<keyword id="8507" type="ORGANIZATION" weightage="20" keywordseo="IMA">IMA</keyword>) has called for observance of a 'White Day' on Wednesday and a nationwide 'Black Day' protest on Thursday. The IMA wrote a letter on Monday to all its current and past office-bearers to issue directions to different hospitals and health workers to light candles on hospital premises as a symbol of protest and vigil on Wednesday. "Let not our white become red," the IMA warned, and demanded enactment of a central law through an ordinance for the protection of healthcare professionals and guarantee for a safe workplace. "If the government fails to enact a central law on violence against doctors and hospitals even after a white alert, the IMA will observe a 'Black Day' on Thursday," it said. The association said that all doctors in the country will sport black badges on Thursday as a mark of protest. "A decision on the further course of action will be taken if suitable steps are not taken by the government even after 'Black Day'. The 'White Alert' is only a warning," said the association. the IMA said that it had so far observed the utmost restraint and patience in spite of extreme provocations. "Doctors have been abused, beaten up, denied entry to their residences. Obstruction in cremation is the last straw that the IMA can bear," Dr Rajan Sharma, IMA President, said in the letter. "If dignity is denied even in death, our patience and restraint lose their value. We demand a special central law against violence against doctors, nurses, healthcare workers and hospitals through an ordinance," Dr RV Asokan, Honorary Secretary General of the IMA, added. One woman had returned from the UK, and another one had a travel history to Dubai. The total number of cases in Mumbai currently stands at 8. India saw a steep rise in the number of infected cases after 28 new cases were reported from 10 states on Wednesday (March 18), taking the total number of infected people to 184. Maha government setups more quarantine facilities With the number of coronavirus cases touching 47 in the state, Maharashtra's anti-coronavirus battle became even fiercer with more testing and quarantine facilities. This is besides the all-out efforts implemented to avoid the challenge of keeping the crowd at bay in public areas and at workplaces. State Health Minister Rajesh Tope announced that from Thursday, additional coronavirus testing labs would start operating at KEM Hospital and Kasturba Hospital in Mumbai and BJ Medical College in Pune. In the next few days, a total of eight more laboratories shall be opened, including at Haffkine Institute in Mumbai. They will test around 250 samples daily. In Mumbai, the Municipal Corporation of Greater Mumbai (MCGM) permitted several private hospitals to start isolation wards with 88 beds. They include Hinduja Hospital (20), Kokilaben Dhirubhai Ambani Hospital (17 beds), Lilavati (15), Raheja (12), Jagjivan Ram WR (10), Jaslok (5), Bombay Hospital (4), HN Reliance (2), Gurunanak (2) and St. Elizabeth (2). After BMC, MCGM orders closing of businesses The Commissioner of the Municipal Corporation of Greater Mumbai (MCGM), Pravin Pardeshi, ordered shutting of all shops, business establishments etc. on alternate days to reduce crowds. The order will be implemented after the concerned ward heads submit details to the MCGM. After making repeated appeals for 'work from home', the Municipal Corporation of Greater Mumbai has now warned of punitive action against those private companies which fail to implement 50% staff norms or disallow work from home. CM Thackeray makes some strict decision Mumbai local trains and retail shops CM Uddhav Thackeray said that the number of suburban trains and buses would be reduced, standees won't be permitted and those seating would be kept at a safe distance as precautionary measures. Thackeray urged 17 million people of Mumbai and nearly 12 crore population of Maharashtra, not to panic or stock up essentials as there is sufficient supply. However, Thackeray said the government plans to implement shift hours for retail shops/markets in different urban localities. The state already elapsed into the partial shutdown with all schools, colleges, malls, theatres, cinemas, gyms and swimming pools closed till March 31. Mumbai crackdown on black marketers with a hefty fine Meanwhile, in Mumbai, the Minister for Food &amp; Civil Supplies, Rajendra Shingane, swooped on pharmacies after complaints of black marketeering of masks, sanitisers etc. He issued instructions to deal with such wilful offenders in the sternest manner. The MCGM started imposing a hefty fine of ₹ 1,000 for people found spitting in public. At the same time, the Bhiwandi Police in Thane slapped a case against Arihant Mattresses for making false claims of 'anti-coronavirus' beddings. Actions are also being initiated at various levels against those spreading fake messages on social media. While delivery of non-essential items was expected to resume after April 20, the government has issued a new notification that prohibits the sale of non-essential items on <keyword id="18635" type="CITY" weightage="20" keywordseo="Amazon">Amazon</keyword>, <keyword id="6354742" type="COMPANY" weightage="20" keywordseo="Flipkart">Flipkart</keyword> and other e-commerce platforms till the lockdown is lifted. On April 16, the Ministry of Home Affairs<a href="https://www.businessinsider.in/business/startups/news/good-news-now-you-order-non-essentials-on-amazon-paytm-mall-snapdeal-and-others/articleshow/75180220.cms"> issued revised guidelines</a> allowing e-commerce platforms to deliver non-essential items from April 21. Amazon, Flipkart cannot deliver non-essential items like mobile phones, TVs during lockdown

<ul><li><keyword id="18635" type="CITY" weightage="20" keywordseo="Amazon">Amazon</keyword> India has partnered with Brihanmumbai Municipal Corporation to deliver essential supplies in Mumbai’s containment zones.</li><li>Amazon will deliver orders to a designated delivery point in the vicinity of the containment zone. </li><li>Volunteers will then deliver the packages to the customer’s doorstep.</li><li>The entire list of containment zones are listed below.</li></ul>Amazon India has partnered with Brihanmumbai Municipal Corporation to deliver essential supplies to the <keyword id="8999150" type="UNKNOWN" weightage="20" keywordseo="containment-zones-in-mumbai">containment zones in Mumbai</keyword>. So far, e-commerce players were not allowed to function in containment zones at all. “Our collaboration with BMC is intended to support those in the containment zones to access essential items conveniently and safely,” said Prakash Rochlani, Director, Last Mile Operations, Amazon India. During Lockdown 3.0, Amazon has also started delivering non-essential orders to green and orange zones in the country. However, the metropolitan cities of India that are the top markets for e-commerce platforms are in the red zone where only essentials can be delivered. How will it function Amazon will deliver orders to a designated delivery point in the vicinity of the containment zone, from where volunteers will further deliver the packages to the customer’s doorstep. Consumers will have to place their orders on the Amazon app, and select their delivery location in the containment zone. Volunteers from the containment zone will then pick up the packages and deliver it to the user's doorsteps. <ul><li>The Brihanmumbai Corporation (BMC), the Mumbai’s civic body identified over 383 containment zones in Mumbai. </li><li>The Ministry of Health defines COVID-19 containment zones as the area where several positive cases of Coronavirus are found. </li><li>The BMC also prepared an <a href="http://stopcoronavirus.mcgm.gov.in/insights-on-map">interactive map for Mumbaikars</a> to identify Coronavirus containment zones and information related to Coronavirus isolation zones. </li><li>Mumbai accounts for 686 active Coronavirus cases out of 1,135 in Maharashtra. </li></ul>The BMC extended the containment zones from 241 to 383 on April 14. Here is a list of extended containment zones containment zones in Mumbai . The authorities have also said that it will disallow entry and exit into those societies to contain the virus. What are containment zones? The Ministry of Health defines COVID-19 containment zones as the area where several positive cases of Coronavirus are found. “The cluster containment strategy would be to contain the disease within a defined geographic area by detecting cases in the nascent stage, breaking the chain of transmission and preventing its spread to new areas. This would include geographic quarantine, social distancing measures, enhanced active surveillance, testing all suspected cases, isolation of cases, quarantine of contacts and risk communication to create awareness among the public on preventive public health measures,” said the Ministry of Health. The difference between a hotspot and a containment zone is that the former are not seized but largely monitored for suspected cases. However, when a place is declared as containment zones, the areas are completely sealed, and no outdoor or indoor movements are allowed until permitted by the authorities.
