Another 95,000 were delayed by more than three hours, and a further 300,000 faced shorter delays.The incident was caused by a software failure. A single flight plan for a journey from Los Angeles to Paris — passing over the UK — contained two waypoints with the same name.This confused the automatic flight planning system, causing both the primary and the backup to shut down within 20 seconds.The system can usually process 900 flight plans an hour during peak periods, but <a target="_blank" class="" href="https://www.businessinsider.com/air-traffic-control-problems-delays-travel-chaos-holiday-weekend-uk-2023-8">air traffic controllers had to process them manually</a> — reducing its capacity to just 60 an hour, the report found.It also identified numerous factors that meant it took longer to identify and rectify the problem.One such factor was that a Level 2 engineer systems expert was on call but couldn't intervene remotely because their login and password "could not be readily verified due to the architecture of the system."It took the engineer 90 minutes to reach the air traffic control site — just over three hours after the meltdown began.All the traffic regulations implemented due to the incident were lifted after a total of nine-and-a-half hours.Delays and cancellations had knock-on effects on airline schedules, causing further issues for the next week. The report estimated that airlines lost £65 million ($82 million) as a result.The report recommended that NATS, the UK's air traffic control provider, has a Level 2 engineer working on-site during busy travel periods.Its 34 recommendations include government legislation, requiring airports and airlines to review the support available for affected passengers, and for NATS to provide earlier notices of possible disruption.UK Transport Secretary Louise Haigh said, "My department will look to introduce reforms, when we can, to provide air travelers with the highest level of protection possible."A <a target="_blank" class="" href="https://www.businessinsider.com/ryanair-ceo-calls-air-traffic-control-boss-resign-flight-chaos-2024-9">NATS</a> spokesperson said in a statement that it welcomed the report and apologized "for the inconvenience passengers suffered because of this very unusual technical incident. Over the 15 months since this incident, we have worked hard to address the lessons from it, and to ensure this particular issue cannot happen again."NATS has implemented most of the 48 recommendations from its own investigation, including "improving our engagement with our airline and airport customers, our wider contingency and crisis response, and our engineering support processes."

The Blue Line followed closely with 20.80 lakh passengers, while the Red Line recorded 8.56 lakh commuters, 8.15 lakh in Pink Line and 7.93 lakh in Violet Line. The Magenta Line saw 6.19 lakh passengers, followed by the Green Line with 4.12 lakh commuters, 81,985 in Airport Line, 57,701 in Rapid Metro, and 50,128, in the Grey Line, as per the data. Thick grey haze continued to choke Delhi for the third consecutive day on Tuesday, with pollution levels remaining alarmingly high at 488 in the 'severe plus' category. To encourage more people to use the metro in this scenario, the Delhi Metro Rail Corporation (DMRC) is running 60 extra trips on weekdays, offering more capacity than usual, an official statement said. The DMRC has recorded its 25 highest passenger journeys since August this year, the statement added. It emphasised that the more people opt for the metro over private vehicles, the greater the reduction in vehicular emissions, which will help improve the air quality of the city and its surrounding areas. In addition, the DMRC has introduced several measures to make travel more convenient for passengers, the statement read. These include the ability to book single or multiple journey tickets through various digital platforms, allowing passengers to plan their journeys anytime and anywhere without the need to stand in queues at the station. It has consistently been among the 100 busiest airports in the world and was ranked <a target="_blank" href="https://www.worldairportawards.com/worlds-top-100-airports-2024/">as the world's 32nd-best by Skytrax in 2024</a>.On December 9, 2022, the morning shift controller arrived at work at 5:15 a.m. to find their night shift counterpart asleep at the console.The worker was discovered lying across two chairs with a blanket covering them, according to <a target="_blank" class="" href="https://www.atsb.gov.au/sites/default/files/2024-09/AO-2022-065%20%28Final%29.pdf">the report</a>. They were still wearing their headset, and the air situation display (ASD) screensaver was on.After ensuring there was no traffic in the area, the station was handed over to the oncoming controller.The report noted that the Brisbane Centre building was cooler at night, and it was considered normal practice for blankets to be used during night shifts.The ATSB's report blamed multiple night shifts in a short space of time, the worker's low workload, and an ineffective risk management system for the incident. It noted that the worker had 10 years of experience as a controller and had been in their current position for three years. However, due to last minute roster changes, they had worked their seventh night shift in nine days and had not taken the recommended rest period in between.There had been no traffic or scheduled flights in the airspace the controller was responsible for and a system supervisor had been on duty at the front desk of the center.However, the report found that cumulative fatigue was not being effectively managed at the center."Likely due to an underlying lack of resources within Airservices Australia, there was an over-reliance on tactical changes to manage the roster," investigators said.The ATSB said it would update its safety procedures and fatigue risk management regulations in response to the incident.Airservices Australia, which runs the Brisbane Centre, has increased the overall number of air traffic controllers and enhanced internal processes since the incident, according to the report. The ATSB will continue to monitor the issue.In the US, fatigue has been a growing safety concern as <a target="_blank" class="" href="https://www.businessinsider.com/airport-collision-close-call-solutions-sleep-runways-air-traffic-controllers-2024-2">shortages of air traffic controllers</a> have driven up the amount of overtime required for those that remain. New Delhi, <keyword id="842113" type="General" weightage="20" keywordseo="Domestic-air-passenger-traffic" source="Orion">Domestic air passenger traffic</keyword> in the country recorded an annual growth of nearly 11 per cent to 1.26 crore in October, according to official data released on Thursday. The traffic stood at 1.14 crore passengers in October 2022 and at 1.22 crore passengers in September this year. The data released by aviation regulator DGCA showed that IndiGo carried 79.07 lakh passengers, garnering a domestic market share of 62.6 per cent in October. The market share was lower compared to 63.4 per cent registered in September. Last month, Air India's domestic market share climbed to 10.5 per cent from 9.8 per cent in September while that of Vistara and AirAsia India (renamed as AIX Connect) declined to 9.7 per cent and 6.6 per cent, respectively, in October. SpiceJet's market share grew to 5 per cent in October from 4.4 per cent in September while that of Akasa Air remained unchanged at 4.2 per cent. "Passengers carried by domestic airlines during January-October 2023 were 1,254.98 lakhs as against 988.31 lakhs during the corresponding period of previous year thereby registering annual growth of 26.98 per cent and monthly growth of 10.78 per cent," the Directorate General of Civil Aviation (DGCA) said. As many as 587 passengers were denied boarding by scheduled airlines in October, according to the data. A total of 30,307 passengers were affected by flight cancellations and 1,78,227 passengers were impacted due to flight delays last month.
