Sydney, Aug 15 (The Conversation): The <keyword id="19896311" type="General" weightage="20" keywordseo="World-Health-Organization" source="keywords" meta.keywordsubtype="org" meta.entityname="World Health Organization" meta.hostid="261" meta.entityid="World Health Organization">World Health Organization</keyword> (<keyword id="19895006" type="General" weightage="20" keywordseo="WHO" source="keywords">WHO</keyword>) has declared <keyword id="20965747" type="General" weightage="100" keywordseo="mpox" source="keywords">mpox</keyword> a public health emergency of international concern, after rising cases in the <keyword id="22796094" type="General" weightage="20" keywordseo="Democratic-Republic-of-the-Congo" source="keywords">Democratic Republic of the Congo</keyword> and the potential for further spread. This now triggers a coordinated international response to an extraordinary event and the mobilisation of resources, such as vaccines and diagnostic testing, to curb the spread of this infectious disease. But WHO has not declared mpox a <keyword id="19913581" type="General" weightage="20" keywordseo="pandemic" source="keywords">pandemic</keyword>. Rather, the measures it has triggered are designed to prevent it from becoming one. <h2>What triggered this latest alert?</h2> Mpox, once known as monkeypox, is a viral infection closely related to <keyword id="22362924" type="General" weightage="20" keywordseo="smallpox" source="keywords">smallpox</keyword>. Initial symptoms include a fever, headache, swelling of the lymph nodes and muscle ache. A typical rash follows, mainly on the face, hands and feet. The spread of mpox through certain African countries led the Africa Centres for Disease Control and Prevention to declare earlier this week mpox a public health emergency of continental security. This is the first time the organisation has issued such an alert since it was established in 2017. The situation in the Democratic Republic of the Congo in central Africa has been particularly worrying for more than a year. There are two types or clades of mpox. Clade II, which originates in west Africa, is less severe. It has a fatality rate of up to 1 per cent (in other words, roughly one in 100 are expected to die from it). But clade I, from central Africa, has a fatality rate of up to 10 per cent (up to one in ten die). This compares to a 0.7 per cent fatality rate for the Omicron variant of SARS-CoV-2, the virus that causes COVID. The Democratic Republic of the Congo is seeing large epidemics of the more deadly clade I mpox. Mpox is endemic in some parts of central and west Africa, where the virus exists in animals and can spread to humans. Outbreaks have been increasing, with more human-to-human spread, since 2017. This is partly due to very low levels of immunity to the mpox virus, which is related to the virus that causes smallpox. Mass vaccination against smallpox ceased more than 40 years ago globally, resulting in minimal immunity in populations today against mpox. The WHO designation announced this week relates to the clade I. Not only does this have a higher fatality rate, it has new mutations that enhance spread between people. These changes, and the global lack of immunity to mpox, makes the world's population vulnerable to the virus. <h2>There are two different epidemics</h2> In 2022, an epidemic of mpox swept through non-endemic countries, including beyond Africa. This was a variant of clade II originating from Nigeria, called clade IIb. This was sexually transmitted, predominantly affecting men who have sex with men, and had a low fatality rate. That epidemic peaked in 2022, with vaccines made available to people at risk in high-income countries, but there has been an uptick in 2024. At the same time, large clade I epidemics were occurring in the Democratic Republic of the Congo, but with far less attention. Vaccines were not available there, even in 2023, when there were 14,626 cases and 654 deaths. Mortality was 4.5 per cent, and higher in children. In fact, most cases and deaths in the Democratic Republic of the Congo have been children. This means most transmission there is non-sexual and is likely to have occurred through close contact or respiratory aerosols. However, in 2023 an <keyword id="20877025" type="General" weightage="20" keywordseo="outbreak" source="keywords">outbreak</keyword> in a non-endemic part of the country, South Kivu in the east, appeared to be by sexual transmission, indicating more than one epidemic and different transmission modes in the Democratic Republic of the Congo. By mid-2024, there were already more cases in the country than all of 2023 - more than 15,600 cases and 537 deaths. Testing capacity is low in the Democratic Republic of the Congo, most cases are not confirmed by lab testing, and the data we have are from a small sample of genomic sequences from the Kamituga region of South Kivu. This show mutations to the clade I virus around September 2023, to a variant termed clade Ib, which is more readily transmissible between people. We do not have much data to compare these viruses with the viruses causing cases in the rest of the country. <h2>Mpox is spreading internationally</h2> In the past month, the virus has spread to countries that share a border with the Democratic Republic of the Congo - Rwanda and Burundi. It has also spread to other east African countries, such as Kenya and Uganda. None of these countries have had mpox cases previously. In an interconnected, mobile world, cases may spread to other continents, as mpox did in 2018 from Nigeria to the United Kingdom and other countries. A few travel-related cases between 2018 and 2019 may have led to the large multi-country 2022 clade IIb epidemic. <h2>We have vaccines, but not where they are needed</h2> As the mpox virus and smallpox viruses are related (they are both orthopoxviruses), smallpox vaccines offer protection against mpox. These vaccines were used to control the 2022 clade IIb epidemic. However, most of the world has never been vaccinated, and has no immunity to mpox. The newer <keyword id="19959412" type="General" weightage="20" keywordseo="vaccine" source="keywords">vaccine</keyword> (called Jynneos in some countries and Imvamune or Imvanex in others) is effective. However, supplies are limited, and vaccine is scarce in the Democratic Republic of the Congo. WHO's declaration of mpox as a public health emergency of international concern will help mobilise vaccines to where they are needed. The Africa Centres for Disease Control had already begun negotiations to secure 2,00,000 doses of vaccine, which is a fraction of what is required to control the epidemic in the Democratic Republic of the Congo. <h2>What happens now?</h2> Ultimately, a serious epidemic anywhere in the world is a concern for all of us, as it can spread globally through travel, as we saw with the COVID pandemic. Controlling it at the source is the best measure, and WHO's latest declaration will help mobilise the required resources. Surveillance for spread of this more serious version of mpox is also essential, bearing in mind that many countries do not have the capacity for widespread testing. So we'll have to rely on "suspected cases", based on a clinical definition, to keep track of the epidemic. Open-source epidemic intelligence - such as using AI to monitor trends in rash and fever illness - can also be used as an early warning system in countries with weak health systems or delayed reporting of cases. A further complication is that 20-30 per cent of people with mpox may simultaneously have chickenpox, an unrelated infection that also causes a rash. So an initial diagnosis of chickenpox (which is easier to test for) does not rule out mpox. Effective communication and tackling push-back against public health measures and disinformation is also key. We saw how important this was during the COVID pandemic. Now, WHO will coordinate the global mpox response, focusing on equity in disease prevention and access to diagnostics and vaccines. It is up to individual countries to do their best to comply with the International Health Regulations, and the protocols for how such a global emergency are managed.

He said the virus has caused miscarriages and blindness.An earlier, milder form of mpox — called clade IIb — circulated worldwide in 2022, ultimately <a target="_blank" href="https://www.businessinsider.com/heres-how-the-us-handled-its-first-ever-monkeypox-outbreak-2022-5">infecting more than 32,000 people in the US</a> and killing 58 of them. It prompted the World Health Organization to declare a public health emergency, which ended last year.The new virus is called clade Ib, since it's a <a target="_blank" href="https://www.businessinsider.com/woman-hiv-had-coronavirus-216-days-virus-mutated-2021-6">mutation</a> of an even earlier form of mpox. It first appeared in a remote region of the Democratic Republic of the Congo in September and has spread to towns along the Rwanda border, near Burundi and Uganda.The new clade Ib virus could cause a <a target="_blank" href="https://www.businessinsider.com/bird-flu-avian-influenza-outbreak-us-h5n1-wiping-out-everything-2023-4">global outbreak</a>, experts fear, though they stopped short of warning of a pandemic."The pandemic question is difficult," Trudie Lang, director of the Global Health Network at Oxford University, said in the briefing."I think we need to be really vigilant now and move to try and contain this as quickly as possible in this region," she added. "There's definitely the opportunity for this to get on an airplane."<h2 id="ef566fbb-abea-4b10-aabf-9d64faaf238f">The new mpox strain spreads easily </h2>Mpox spreads through close contact, especially skin-to-skin contact. It can cause flu-like symptoms and a <a target="_blank" href="https://www.businessinsider.com/monkeypox-rash-pictures-when-to-get-checked-2022-5">painful or itchy rash</a> that develops into puss-filled blisters, lesions, and eventually scabs.<div type="image" title="Monkeypox palm of hands" height="459" width="700" msid="111468793" hostid="261" sizebytes="24580"></div><a target="_blank" href="https://www.businessinsider.com/monkeypox-man-hospitalized-couldnt-swallow-spit-feared-death-symptoms-2022-7">Previous strains of the virus</a> have had limited transmission abilities. The clade I virus that's long circulated in the DRC has been mostly associated with spread through families and within households, Land said. The clade IIb virus that went global was mostly sexually transmitted and mostly affected men who have sex with men.The new clade Ib strain seems to do it all.It began spreading through sexual transmission, via the local sex work industry, according to the researchers. However, they said the new virus has also spread within households, between mothers and their children, and there have even been cases of person-to-person spread outside households and without sexual contact.That's "incredibly worrying," Lang said. That's because it has more opportunities to spread than previous strains did.The clade I form of the virus, from which the new strain has emerged, is also more deadly than clade II. According to <a target="_blank" href="https://www.cdc.gov/poxvirus/mpox/about/index.html">WHO</a>, some outbreaks have killed up to 10% of sick people.<h2 id="b8b424dd-af4a-4193-a5ac-002f109faf5a">Researchers only see 'the tip of the iceberg'</h2>It's not yet clear just how deadly the new clade Ib virus is, because researchers aren't sure how many cases there are.Leandre Murhula Masirika, a research coordinator in the local DRC health department, said in the briefing that they had identified more than 600 cases so far."These are the severe cases that make it to hospital, so this will be the tip of the iceberg," Lang said.The WHO's technical lead for mpox, Rosamund Lewis, also raised the alarm last week, saying "There is a critical need to address the recent surge in mpox cases in Africa," according to <a target="_blank" href="https://www.reuters.com/business/healthcare-pharmaceuticals/who-scientists-warn-mpox-strain-congo-2024-06-25/">Reuters</a>.The researchers said they need to better understand the virus to identify the right safety measures and vaccines to contain its spread."We need to first carry out studies of serology and immunology in order to see what kind of vaccination we can propose," Murhula Masirika said.He added that, in the meantime, he wants to give the smallpox vaccine to local sex workers and healthcare workers in hopes that it will confer some immunity to mpox.

Immutep jumps 13% after getting fast-track status for its drug to treat head and neck cancer

The first batch of DRDO-developed anti-Covid drug 2-DG (2-deoxy-D-glucose) was launched on Monday to treat patients suffering from the deadly disease. The medicine comes in powder form and can be taken with water. Defence Minister Rajnath Singh and Health Minister Harsh Vardhan released the first batch of the 2-DG, an anti-Covid-19 therapeutic application of the drug developed by the Institute of Nuclear Medicine and Allied Sciences (INMAS), a lab of <keyword id="152819" type="COMPANY" weightage="20" keywordseo="Defence-Research-and-Development-Organisation">Defence Research and Development Organisation</keyword> (DRDO) in collaboration with Dr Reddy's Laboratories (DRL). Rajnath Singh handed over the anti-Covid drug to his cabinet colleague Harsh Vardhan who handed over it to Delhi All India Institute of Medical Sciences (AIIMS) director Randeep Guleria at an event here. The Defence Minister would be later distributing around 10,000 doses of the drug to a few hospitals in the national capital. The Drugs Controller General of India (<keyword id="172241" type="COMPANY" weightage="20" keywordseo="DCGI">DCGI</keyword>) granted emergency use nod for the drug last week. Clinical trial results have shown that the drug helps in faster recovery of hospitalised patients and reduces supplemental oxygen dependence. Higher proportion of patients treated with 2-DG showed <keyword id="9043621" type="COMPANY" weightage="20" keywordseo="RT-PCR">RT-PCR</keyword> negative conversion in Covid patients. The medicine was found to be safe for Covid-19 patients in phase 2 trials, conducted between May and October last year. It was found to be effective in cutting short the hospital stays of Covid patients and reducing their supplemental oxygen dependence. During the first wave of the pandemic in April last year, INMAS-DRDO scientists conducted laboratory experiments with the help of <keyword id="395778" type="COMPANY" weightage="20" keywordseo="Centre-for-Cellular-and-Molecular-Biology">Centre for Cellular and Molecular Biology</keyword> (CCMB), Hyderabad, and found that this molecule works effectively against SARS-CoV-2 virus and inhibits the viral growth. Based on these results, the Drugs Controller General of India (DCGI) <keyword id="170943" type="COMPANY" weightage="20" keywordseo="Central-Drugs-Standard-Control-Organisation">Central Drugs Standard Control Organisation</keyword> (CDSCO) permitted Phase-II clinical trial of 2- DG in Covid-19 patients in May 2020. The DRDO, along with its industry partner DRL, Hyderabad, started the clinical trials to test the safety and efficacy of the drug in Covid-19 patients. In Phase-II trials (including dose ranging) conducted during May-October 2020, the drug was found to be safe in Covid-19 patients and showed significant improvement in their recovery. Phase-II was conducted in six hospitals and Phase IIb (dose ranging) clinical trial was conducted at 11 hospitals all over the country. Phase-II trial was conducted on 110 patients. DRDO launches a new anti-COVID drug called 2-DG that has helped patients recover faster during trials

Its net interest income or revenues too went up by 34% to ₹3,074 crore from ₹2,288 crore in the comparable quarter. The fee income of this bank which has been banking heavily on its retail business, also went up by 22%. Its deposits and advances too went up by a robust 22% on an annual comparison. “During Q3 FY-20, the Bank witnessed a healthy growth in its topline as well as in operating profits. The Bank also reached a milestone as the Balance Sheet footage crossed ₹ 3 trillion and the advances crossed the ₹2 trillion-mark. PCR (provisioning coverage ratio) has been increased to 53% to strengthen the balance sheet,” said Romesh Sobti, managing director and CEO, IndusInd Bank said. However, the bank’s stock dropped by 2.9% to ₹1,493 as per BSE data at 3 pm. The bank which acquired Bharat Financial in 2017, is banking on its expanded footprint especially in the rural areas to drive growth going ahead. The acquisition turned effective on July last year, allowing the bank to derive full benefits of acquiring the shadow bank. A research note by Nomura says that the acquisition is a moat for the bank. “The asset side franchise has strengthened with BHAFIN, and IIB will undergo a full-fledged extension of the pilots that management believes will enable it to use Bharat Financial’s reach to create a rural touchpoint network that provides significant cross sell opportunity and also improve efficiency in a manpower-intensive MFI business,” said Nomura. The research note also believes that asset side strengths of the bank will be able to offset weaknesses in liabilities. “Bulkiness in liabilities and fees, apart from recent corporate asset quality issues, has been IIB's key weakness. While it is easier said than done, if the bank delivers well on granularizing its liabilities and successfully monetizing rural distribution via BHAFIN, it should drive a sustainable re-rating, in our view,” the note said.

The Reserve Bank of India (RBI) has given people the option to defer payments on their loans, but according to IndusInd Bank — its customers aren’t going down that route. During the conference call, the bank said it received 96% of non-vehicle portfolio repayments and 95% of repayments of its vehicle portfolio in March. Even in the micro-finance portfolio, the lender received 99% of repayments. Going forward, the bank expects retail to account for 60% and corporate to account for 40% of its balance sheet by the end of the next financial year. “Bank’s strategy has shifted towards ‘Retailisation of Balance Sheet’ with focus on improving retail deposit franchisee, increase the contribution of non-vehicle retail loans, and increase the proportion of microfinance business,” according to Philip Capital. Due to a range of dynamic external influences from technology to socioeconomic factors to government policies, these macro trends are poised to continue, changing the insurance industry along with it. Since its inception, insurance industry has grown stronger as the sector adopted itself to increased awareness, innovative products, and more distribution channels. Though there has been a growth in premiums at the industry level, the insurance penetration level in India is still very low as compared to other countries in the world, thereby providing ample opportunity for all insurers in the ecosystem to grow. With over 460 million internet users, India is the second largest online market, ranked only behind China. By 2021, there will be about 635.8 million internet users in India, most of the digital population in India are mobile internet users. This has resulted in people who are adopting more digital mediums to interact and with the backing of the government to push for the digital India movement, the insurance industry is also adopting digital technologies in presence. The use of web-based technologies as an Insurance service delivery medium has added new elements to service dissemination. Escalation in self-service technologies has provided customers with multiple choices for using the services offered by insurers. The online service attributes of ‘convenience’ and ‘ease of use’ are being given priority by insurers for designing web-based services Future of digital insurance New digital platforms support micro duration and atomisation of insurance by item. In a digital era where the customer needs are ever-changing, insurance providers need digital infrastructure to introduce, evaluate, and adopt new products/technologies/business models. Building such infrastructure in-house will be costly and slower. Integrating with third party systems like hospitals to evaluate medical data for underwriting, evaluating the wellness of a policy holder in terms of fitness and his need during the entire customer lifecycle would ensure better products and pricing. Also, use of telematics systems in auto-insurance would change vehicle insurance from class-based pricing to driver behaviour-based pricing. Distribution through E-Tailors, Web aggregators and fintech companies are expected to witness an upward momentum in coming years In addition, Insurers in India are gradually adopting digital medium to provide their services keeping in view the millennials as well as rural penetration. Use of digital medium instead of physical distribution are much more feasible for them to reach out to the end users as well as increase insurance penetration. But fortunately, breast cancer is usually diagnosed as stage I, "because we use mammograms and other imaging tools to diagnose it before a lot of women are even aware that they have something going on," says<a href="https://www.uofmhealth.org/profile/33085/norah-lynn-henry-md-phd" target="_blank" rel="noopener"> Norah Lynn Henry</a>, MD, breast oncology disease lead at the Rogel Cancer Center at the University of Michigan. But in other countries where this type of technology is not as common, breast cancer is diagnosed at a higher stage, she says. <h1>Why breast cancer is staged </h1>Breast cancer stages are used to guide therapy, says <a href="https://www.mskcc.org/cancer-care/doctors/yolanda-bryce" target="_blank" rel="noopener">Yolanda Bryce</a>, MD, a radiologist at<a href="https://www.mskcc.org/cancer-care/doctors/yolanda-bryce" target="_blank" rel="noopener"> Memorial Sloan Kettering Cancer Center</a>. They help oncologists communicate about and help patients understand the type and severity of cancer they have, Henry says. Different stages have different survival rates, but it's important not to get too hung up on the statistics, because situations vary depending on the individual, Bryce says. "There's so much innovation and so much advancement in the field that [a patient's] clinical picture doesn't necessarily look like the published numbers," she says.How quickly breast cancer progresses from one stage to the next varies widely. Some cancers can take a few months to progress, and others can take years, Bryce says. Here's a guide on the different stages of breast cancer and what each stage may mean for you.<h1>Stage 0 breast cancer </h1>Stage 0 is the earliest stage of breast cancer, also called ductal carcinoma in situ (DCIS). It refers to cancer that is <a href="https://www.nationalbreastcancer.org/breast-cancer-stage-0-and-stage-1" target="_blank" rel="noopener">present</a> in the milk duct but has not spread outside of the ducts or lobules into the surrounding breast tissue. You can think of it like little flecks of cancer that haven't formed into a tumor yet, Bryce says. In stage 0, you typically have no symptoms - you can't see or feel a mass yet, she says. For stage 0, the five-year survival rate, meaning the percent of people who live at least five years after the cancer is found, <a href="https://www.nationalbreastcancer.org/breast-cancer-stage-0-and-stage-1" target="_blank" rel="noopener">is 100%</a>. <h1>Stage I breast cancer </h1>In Stage I, the cancer cells have spread to surrounding breast tissue, but they are still contained in a small area. Stage I is divided into <a href="https://www.mskcc.org/cancer-care/types/breast/diagnosis/stages-breast" target="_blank" rel="noopener">stages IA and IB</a>.In stage IA:<ul><li>The tumor measures up to 20 millimeters (mm).</li><li>Cancer has not spread to the lymph nodes.</li></ul>In stage IB:<ul><li>No tumor or a small tumor (less than 20 mm) is in the breast.</li><li>Small clusters of cancer cells are in the lymph nodes.</li></ul>Many people with stage I cancer have no symptoms, but some women feel a mass, Henry says. "For most women, it's found on [a] mammogram," she says. The five-year survival rate is<a href="https://www.komen.org/breast-cancer/facts-statistics/breast-cancer-statistics/survival-rates/" target="_blank" rel="noopener"> 98% to 100%</a> for stage I. Because cancer caught at this stage has a high survival rate, this underscores the importance of getting mammograms, Bryce says. And a sizable number of women do not. In 2018, an estimated 63% of US women 45 and older were up to date with breast cancer screening, according to a 2021 American Cancer Society<a href="https://www.cancer.org/content/dam/cancer-org/research/cancer-facts-and-statistics/cancer-prevention-and-early-detection-facts-and-figures/2021-cancer-prevention-and-early-detection.pdf" target="_blank" rel="noopener"> report</a>. <h1>Stage II breast cancer </h1>In <a href="https://www.mskcc.org/cancer-care/types/breast/diagnosis/stages-breast" target="_blank" rel="noopener">stage II</a> breast cancer, the cancer has grown larger but is still in a limited region of the breast. Like stage I, stage II is broken up into two substages:Stage IIA involves one of the following:<ul><li>There is no tumor or a small tumor (up to 20 mm) in the breast, plus cancer has spread to the lymph nodes under the arm. </li><li>Or there is a tumor of 20 to 50 mm in the breast, but cancer has not spread to the lymph nodes.</li></ul>Stage IIB involves one of the following:<ul><li>A tumor of 20 to 50 mm is in the breast, and cancer has spread to between one and three nearby lymph nodes.</li><li>Or the tumor is larger than 50 mm, but cancer has not spread to any lymph nodes. </li></ul>Women with stage II breast cancer often feel a mass in their breast, and if they have more cancer in their lymph nodes, they may feel a mass under their arm, Henry says. But they still may not feel anything. Other possible symptoms<a href="https://www.cdc.gov/cancer/breast/basic_info/symptoms.htm" target="_blank" rel="noopener"> include</a>:<ul><li>Thickening or swelling of part of the breast</li><li>Irritated, dimpled, red, or flaky skin on the breast</li><li>Pulling in or inversion of the nipple</li><li>Nipple discharge other than breast milk, sometimes bloody</li><li>Any change in the size or the shape of the breast</li><li>Pain in the breast</li></ul>Although pain may be a symptom, it's not common, Bryce says. The five-year survival rate for stage II is<a href="https://www.komen.org/breast-cancer/facts-statistics/breast-cancer-statistics/survival-rates/" target="_blank" rel="noopener"> 90-99%</a>. <h1>Stage III breast cancer </h1>In <a href="https://www.mskcc.org/cancer-care/types/breast/diagnosis/stages-breast" target="_blank" rel="noopener">stage III</a>, the cancer has spread farther in the breast, or the tumor is larger. This stage includes three subcategories: Stage IIIA involves one of the following:<ul><li>Cancer is found in four to nine nearby lymph nodes, with or without a tumor in the breast.</li><li>A breast tumor is larger than 50 mm, and the cancer has spread to between one and three nearby lymph nodes.</li></ul>In stage IIIB, the tumor has spread to the chest wall behind the breast, and:<ul><li>Cancer may have spread to the skin, causing swelling or inflammation.</li><li>Cancer may have broken through the skin, causing a wound.</li><li>Cancer may have spread to as many as nine lymph nodes under the arm or nodes near the breastbone.</li></ul>In stage IIIC, with or without a tumor in the breast, the cancer has spread to one of the following places:<ul><li>Ten or more underarm lymph nodes</li><li>Lymph nodes near the collarbone</li><li>Some underarm lymph nodes and lymph nodes near the breastbone</li><li><a href="https://www.breastcancer.org/symptoms/diagnosis/staging#stage3" target="_blank" rel="noopener">The skin of the breast</a></li></ul>In stage III, the patient may experience similar symptoms as stage II, listed earlier. The five-year survival rate for stage III breast cancer is<a href="https://www.komen.org/breast-cancer/facts-statistics/breast-cancer-statistics/survival-rates/" target="_blank" rel="noopener"> 66% to 98%</a>.Note: For stages 0 to III, the goal is to cure the cancer and to keep it from coming back, Henry says. "The higher the stage, the more likely the cancer is to come back."<h1>Stage IV breast cancer </h1><a href="https://www.mskcc.org/cancer-care/types/breast/diagnosis/stages-breast" target="_blank" rel="noopener">Stage IV</a>, also called metastatic, is the most advanced stage of breast cancer. In this stage, the cancer has spread to nearby lymph nodes and to other parts of the body, such as the lungs, liver, brain, or bones. If the cancer has spread to the bone, patients may have bone pain or back pain, and if it has spread to the liver, patients may have pain on the right side of their abdomen, Bryce says. Some of the changes in the skin and nipple listed earlier under stage II may also occur in stage IV. When the cancer has spread to other, distant parts of the body, the five-year survival rate is<a href="https://www.komen.org/breast-cancer/facts-statistics/breast-cancer-statistics/survival-rates/" target="_blank" rel="noopener"> 28%</a>. "With the treatments that we have available to us today, most patients, unfortunately, are not cured of their cancer" if it is stage IV, Henry says. But, she adds, "We can still treat, we can still manage, we can still help people feel good for as long as possible." Also, many new treatments are emerging. "People are living longer even with stage IV disease," Bryce says. A 2018<a href="https://acsjournals.onlinelibrary.wiley.com/doi/full/10.1002/cncr.31638" target="_blank" rel="noopener"> study</a> found that the effectiveness of breast cancer treatment on survival increased substantially from 1975 to 2015. <h1>Other information about staging</h1>Along with the numerical stages, breast cancer is classified using the American Joint Committee on Cancer<a href="https://www.cancer.org/cancer/breast-cancer/understanding-a-breast-cancer-diagnosis/stages-of-breast-cancer.html" target="_blank" rel="noopener"> TNM system</a>, which takes into account: <ul><li>The size of the tumor (T), </li><li>Spread to nearby lymph nodes (N), </li><li>Spread or metastasis to distant sites (M), </li><li>Estrogen receptor (ER) status, </li><li>Progesterone receptor (PR) status, </li><li>Status called Her2 protein (Her2),</li><li>The grade of the cancer (G), or how much the cells look like normal cells </li></ul>"The grade for breast cancer is how aggressive it looks under the microscope," Henry says. The grade is also assigned a number, with one being the least aggressive and three being the most. Since both the stage and grade are numbered systems, patients sometimes confuse the two, she says. When meeting with a breast cancer patient, Henry says she talks about the stage, the grade, and other factors to advise the best form of treatment. <h1>Insider's takeaway </h1>Breast cancer is classified into five different stages, in part to make it easier for <keyword id="11894571" type="UNKNOWN" weightage="100" keywordseo="health" source="Orion">health</keyword> care providers and patients to communicate about and understand the type of cancer. "Usually, the most common presentation is nothing. You don't feel anything at all. And I think it's important to stress that that's why mammogram is necessary," Bryce says.If you've been diagnosed with breast cancer, don't be afraid to ask your oncologist questions, Henry says. Patients may not fully understand the risks and benefits of a treatment, especially "in the stress and the overwhelming nature of first getting diagnosed with cancer," she says.
