The World health Organisation (<keyword id="6366244" type="General" weightage="20" keywordseo="who" source="Orion">WHO</keyword>) released its first-ever report on the devastating global impact of high blood pressure, during the 78th Session of the United Nations General Assembly on Tuesday. The report shows an estimated 188.3 million adults aged 30-79 years live with <keyword id="22984" type="General" weightage="20" keywordseo="hypertension" source="Orion">hypertension</keyword> in India. In order to achieve a 50 per cent control rate, 67 million more people with hypertension would need to be effectively treated. Only 37 per cent Indians have the diagnosed condition of high BP, of which 32 per cent are males and 42 per cent females. Of the 35 per cent undertaking treatment, 35 per cent are females, while 25 per cent are males. Currently, only 15 per cent people have their hypertension under control (19 per cent female, 11 per cent males), the report said. Uncontrolled hypertension is known to cause heart attack, stroke and premature death. In fact, the report showed that 52 per cent deaths in the country due to cardiovascular diseases such as heart attack can be attributed to elevated blood pressure. Globally, hypertension affects one in three adults worldwide. About four out of every five people with hypertension are not adequately treated. But if countries can scale up coverage, 76 million deaths could be averted between 2023 and 2050, the report said. "Hypertension can be controlled effectively with simple, low-cost medication regimens, and yet only about one in five people with hypertension have controlled it," said Dr. Tedros Adhanom Ghebreyesus, WHO Director-General. Ageing and genetics can increase the risk of having high blood pressure, but modifiable risk factors such as eating a high-salt diet, not being physically active and drinking too much alcohol can also increase the risk of hypertension. Lifestyle changes like eating a healthier diet, quitting tobacco and being more active can help lower blood pressure. Some people may need medicines that can control hypertension effectively and prevent related complications. Prevention, early detection and effective management of hypertension are among the most cost-effective interventions in health care and should be prioritised by countries as part of their national health benefit package offered at a primary care level, the report said. "Hypertension control programmes remain neglected, under-prioritised and vastly underfunded. Strengthening hypertension control must be part of every country's journey towards universal health coverage, based on well-functioning, equitable and resilient health systems, built on a foundation of primary health care," the WHO chief said. India can avert 4.6 mn deaths by 2040 with better hypertension control: WHO

A new study by <keyword id="5069783" type="General" weightage="20" keywordseo="Centre-for-Chronic-Disease-Control" source="Orion">Centre for Chronic Disease Control</keyword>, India (CCDC) on cardiovascular risk reduction in South Asia has demonstrated significant long-term benefits of a multi-faceted quality improvement (QI) strategy, comprising of task sharing, and clinical decision support software (CDSS) with electronic health records, in managing type-2 diabetes in India and Pakistan. The study, published in the prestigious journal PLOS Medicine, highlights the sustained impact of QI interventions on <keyword id="21411276" type="General" weightage="100" keywordseo="diabetes-care" source="keywords">diabetes care</keyword> and associated complications over a median follow-up period of 6.5 years. The burden of diabetes Diabetes control remains a global challenge, leading to severe complications such as <keyword id="22908" type="General" weightage="20" keywordseo="heart-disease" source="Orion">heart disease</keyword>, <keyword id="25191" type="General" weightage="20" keywordseo="stroke" source="Orion">stroke</keyword>, <keyword id="16753" type="General" weightage="20" keywordseo="blindness" source="Orion">blindness</keyword> or visual impairment, and kidney and nerve damage. LMICs are often faced with challenges such as inadequate infrastructure and trained healthcare staff with digital literacy. In that respect, the study's findings are crucial for integrating care models sustainably and identifying relevant organizational barriers. Previous studies from high-income countries showed improvements in diabetes care, but their long-term impact on vascular complications and mortality was unclear. This study aimed to address the critical gap in data regarding the long-term effectiveness of QI strategies and their impacts on vascular complications in LMICs. Professor Nikhil Tandon, Department of Endocrinology and Metabolism, All India Institute of Medical Sciences, New Delhi, and senior author of the study explained the relevance of these findings in the context of national efforts to combat non-communicable diseases (NCDs). He said, "The <keyword id="22511956" type="General" weightage="20" keywordseo="National-Programme-for-NCDs" source="keywords">National Programme for NCDs</keyword> [NP-NCDs] operationalized by the Government of India has already a functional portal which provides an electronic health record functionality. Integrating the CDSS in this portal will provide significant incremental benefit to the impact of the National Programme, and bolster governmental efforts to battle the scourge of NCDs including diabetes and hypertension." Speaking about the findings, Prof. (Dr) Dorairaj Prabhakaran, Executive Director, CCDC, and one of the study authors, said, "Through the trial, we've demonstrated that a sustained quality improvement strategy can significantly enhance diabetes care and reduce complications in South Asia. For every 13 patients treated with this strategy, one major adverse cardiovascular event like heart attacks or strokes- or a microvascular event -like eye disease or decline in kidney function- was prevented, showcasing its profound impact on patient outcomes." Professor MK Ali, William H. Foege Distinguished Professor of Global Health, Emory University and one of the key authors of this article, said, "This study is an exemplar of trans-national collaboration for benefits of both, the global south and global north." Study Design and Findings The randomized clinical trial included 1,146 patients with poorly controlled type 2 diabetes across ten outpatient diabetes clinics from January 2011 through September 2019. Participants were randomized to receive either the QI strategy, which involved trained non-physician care coordinators and a clinical decision support software (CDSS) integrated into the electronic health record form, for physicians, or the usual care. Some of the key findings from the study include: <ul><li>At 6.5 years, patients in the QI strategy group were nearly twice as likely to achieve and maintain target levels for HbA1c (&lt;7%), systolic blood pressure (&lt;130mmHg), and LDL-cholesterol (&lt;100mg/dl), compared to the usual care group. </li><li> The incidence of first microvascular complications, such as diabetic eye disease, decline in kidney function, and nerve pain was notably 32% lower in the QI group. </li><li>There was a significant 28% reduction in the combined occurrence of microvascular and macrovascular events in the QI group.</li></ul> Importantly, these benefits were sustained only in clinics that continued the QI strategy for 6.5 years. With diabetes accounting for one in seven deaths in South Asia, the study provides unique insights into the sustainability of QI strategies in achieving diabetes care goals in low- and middle-income (LMICs) and the effects of these improvements on reducing vascular complications and diabetes-related deaths.

The thicker, narrow blood vessels associated with high blood pressure make it easier for plaque to build up on your artery walls. If that plaque accumulation in the arteries leading to the heart, it can cause<a href="https://www.mayoclinic.org/diseases-conditions/coronary-artery-disease/symptoms-causes/syc-20350613"> coronary artery disease</a>, which can result in a heart attack or heart failure. A study in India found that hypertensive adults are <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5682717/">11 times</a> more likely to develop coronary artery disease.</li><li>Heart attack. When the arteries build up with plaque, a blood clot is more likely to form. A study in Massachusetts found that <a href="https://www.sciencedirect.com/science/article/pii/S2213177917303177?via%3Dihub">91% of those who developed heart failure</a> also had hypertension. </li></ul><h2>Hypertension and stroke</h2>There are two main types of stroke: ischemic and hemorrhagic. Ischemic strokes are more common than hemorrhagic strokes (which only make up <a href="https://www.stroke.org/en/about-stroke/types-of-stroke/hemorrhagic-strokes-bleeds">about 13%</a> of stroke cases), but hypertension is strongly related to each type. <ul><li>Ischemic stroke. When blood clots form in arteries that lead to the brain, the result is an <a href="http://www.strokecenter.org/patients/about-stroke/ischemic-stroke/">ischemic stroke.</a> Without ample blood supply to brain cells, or neurons, they will stop working and heavily impair your body's <a href="https://www.heart.org/en/health-topics/high-blood-pressure/health-threats-from-high-blood-pressure/how-high-blood-pressure-can-lead-to-stroke">most vital functions</a>. In some cases, high blood pressure can cause blood vessels to burst. Two had gotten the vaccine from Pfizer, two had gotten the vaccine from Moderna, and two had gotten an <a href="https://www.gavi.org/vaccineswork/indias-covaxin-vaccine-shows-high-efficacy-against-covid-19-infections-phase-3">Indian-made vaccine called Covaxin</a>, the study said. Their infections were confirmed with lab tests and viral sequencing for Delta, it said.Each experienced some common symptoms of COVID-19, including fever, cough, fatigue, and body aches, the study said. Those who'd gotten the Moderna and Covaxin vaccines also lost their sense of smell.One Covaxin recipient and one person who'd gotten Pfizer's vaccine came down with more severe infections, the study said. The latter was a man in his 60s with no known medical conditions that increase the odds of contracting COVID-19 - he was hospitalized and given Regeneron's monoclonal antibody treatment (the same one <a href="https://www.businessinsider.com/what-is-human-fetal-tissue-research-why-is-it-done-2020-10">President Donald Trump received</a>) 10 days after the wedding. The Covaxin recipient, a man in his late 60s (also with <a href="https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-with-medical-conditions.html">no COVID-19 comorbidities</a>), died from complications of COVID-19. The long-term consequence of PCOS is excessive thickening of the inner lining of the uterus, which can lead to endometrial cancer in the long run. The only way to treat this condition is to reduce body weight. “Exercise, weight loss and proper diet are the primary modalities of treatment. Even a 5% weight loss does wonders in terms of treatment. Medicines are given to regularise cycles, but they are not of much use unless lifestyle alterations are made too,” Dr Nampoothiri said. Patients also have to stick to the treatment regime strictly, say doctors. “Second treatment can be through insulin sensitizers to control hormones. Sometimes to control hormones, oral contraceptive pills can be given. World Heart Day 2022: Silent attacks account for 45% of heart attacks in men say doctors

The <keyword id="83655" type="COMPANY" weightage="20" keywordseo="Union-health-ministry">Union Health Ministry</keyword> has issued revised guidelines for <keyword id="8699674" type="UNKNOWN" weightage="20" keywordseo="Home-isolation">home isolation</keyword> for <keyword id="6364395" type="UNKNOWN" weightage="20" keywordseo="patients">patients</keyword> with mild or asymptomatic symptoms. The new revised guidelines said, "Patient under home isolation will stand discharged and end isolation after at least 10 days have passed from onset of symptoms (or from date of sampling for <keyword id="9100396" type="UNKNOWN" weightage="20" keywordseo="asymptomatic-cases">asymptomatic cases</keyword>) and no fever for 3 days. There is no need for testing after the home isolation period is over." As per the revised guidelines, the patient should be clinically assigned a mild/asymptomatic case by the treating Medical Officer."The asymptomatic cases are laboratory confirmed cases not experiencing any symptoms and having oxygen saturation at room air of more than 94 per cent. Clinically assigned <keyword id="9271864" type="UNKNOWN" weightage="20" keywordseo="mild-cases">mild cases</keyword> are patients with upper respiratory tract symptoms (and/or fever) without shortness of breath and having oxygen saturation at room air of more than 94 per cent are recommended for home isolation," the revised guidelines said. It said that a caregiver should be available to provide care on a 24x7 basis and a communication link between the caregiver and hospital is a prerequisite for the entire duration of home isolation.Elderly patients aged more than 60 years and those with co-morbid conditions such as hypertension, diabetes, heart disease, chronic lung/liver/ kidney disease, cerebro-vascular disease shall only be allowed home isolation after proper evaluation by the treating medical officer. Talking about treatment protocols, the revised guideline said, "Patients must be in communication with a treating physician and promptly report in case of any deterioration. If fever is not controlled with a maximum dose of Tab. Paracetamol 650mg four times a day, consult the treating doctor who may consider advising other drugs like non-steroidal anti-inflammatory drugs (<keyword id="805213" type="COMPANY" weightage="20" keywordseo="NSAID">NSAID</keyword>) (ex: Tab. Naproxen 250 mg twice a day)." It further stated that consider Tab Ivermectin (200 mcg/kg once a day, to be taken empty stomach) for 3 to 5 days. Inhalational Budesonide (given via inhalers with spacer at a dose of 800 mcg twice daily for 5 to 7 days) to be given if symptoms (fever and/or cough) are persistent beyond 5 days of disease onset. "The decision to administer Remdesivir or any other investigational therapy must be taken by a medical professional and administered only in a hospital setting. Do not attempt to procure or administer Remdesivir at home," it said. As per the guidelines,a patient under home isolation seeks immediate medical attention if serious signs or symptoms develop which could include difficulty in breathing, dip in oxygen saturation, persistent pain pressure in the chest and mental confusion or inability to arouse. Dubbed "GoRide," the service aims to help non-urgent patients get to their appointments at partnered hospitals and facilities. Ford will launch the project with Beaumont Health, a network with more than 200 facilities. The company currently has 15 transit vans with plans to expand to 60 vans by the end of the year. Ford is entering an increasingly crowded market. Ride-sharing companies Uber and Lyft have made several forays into the NEMT market. For example, Lyft <a href="http://www.businessinsider.com/digital-health-briefing-oscar-health-surpasses-3-billion-valuation-apple-health-app-adds-27-more-hospitals-lyft-expands-its-nemt-footprint-2018-3">teamed</a> up with Acuity Link to expand its NEMT footprint in March, and in February <a href="http://www.businessinsider.com/digital-health-briefing-google-ai-can-predict-heart-attacks-from-eye-scans-lyft-hitch-health-partner-for-medical-transport-nokia-executive-sees-no-path-for-digital-health-2018-2">partnered</a> with Hitch Health. A beta version of the Uber Health platform <a href="http://www.businessinsider.com/digital-health-briefing-uber-launches-ride-sharing-platform-for-hospitals-teladoc-nearly-doubles-revenue-google-sister-verily-plans-insurance-market-play-2018-3">released</a> in July 2017 is already being used by more than 100 healthcare organizations in the US. That's on top of other companies, like Veyo, that are dedicated NEMT services providers. Health systems likely view the growing interest in NEMT as an opportunity to overcome the massive annual loss of revenue caused by missed appointments. In the US, missed appointments cost healthcare providers as much as $150 billion each year, with no-show rates as high as 30%, according to SCI Solutions.<div class="KonaFilter image-container float_left"><div class="image on-image"><a href="http://www.businessinsider.com/intelligence/top-5-trends-shaping-the-future-of-digital-health?IR=T&amp;utm_source=businessinsider&amp;utm_medium=DH_Briefing&amp;utm_campaign=digital-health-top-5-trends-briefing&amp;utm_term=DH_briefing"></a></div></div>However, as non-health specific companies take on NEMT, they're likely to face barriers that could stymie adoption and growth. Further, it's not clear whether ride-sharing has a positive impact on appointment no-shows, <a href="https://jamanetwork.com/journals/jamainternalmedicine/fullarticle/2671405">according</a> to a study published in JAMA Internal Medicine.GOOGLE GLASS IMPROVES HEALTH SYSTEM WORKFLOW: Physicians at California-based health system Sutter Health are using Google's augmented reality (AR) enabled Glasses to cut back on the amount of time they spend on administrative tasks, <a href="https://mhealthintelligence.com/news/with-google-glass-sutter-health-sees-mhealth-success-in-workflows">according</a> to mHealthIntelligence. Google Glass devices are embedded with mHealth tech from Augmedix - a company that makes software for smartglasses. With the physician wearing the Glasses, Sutter's remote scribes can "sit in" on the visit, taking notes in real-time as the physician sees to the patient. Doctors can also use the glasses to pull up patient files and look up health information without having to turn away from the patient. In addition to improving the physician-patient relationship, the tech streamlines the process of note taking, immediately storing it in the electronic health record server. This cuts down on the amount of time physicians spend writing notes after the session and reduces the risk of physician burnout, which can negatively impact productivity and customer relations. The randomized study aimed to determine if the Medisafe smartphone app could improve self-reported medication adherence and blood pressure control among hypertension patients. As per a PWC report on "working towards wellness in India", hypertension and diabetes have the greatest impact on productivity in our country. It estimated that in 2005 the foregone income due to heart disease, diabetes and stroke was $8.7 billion in India. Hence it extolled the business leaders to fight the spread of chronic diseases, and work towards designing wellness programs that are aimed at helping manage chronic problems.

- How does HealthPlix connect employees to doctors real time. 
 
We partner with individual clinics that treat diabetes to use our product. Doctors use HealthPlix app and the platform in their practice. Patients get a subscription for on-going chronic care from their doctor. So far, we do not have a partnership with the corporates. 
 
- Where did the idea of HealthPlix come from? 
 
It was an outcome of a personal frustration of not finding a proper chronic care intervention. I chose diabetes as the problem to solve as it affects 70 million patients. Chronic diabetes care transcends beyond the basic clinic setup. There is an immense need to extend the care to patients with chronic diabetes problems in an on-going manner. Because, the only way to deal with chronic diabetes is to manage it well as it cannot be cured. 
 
In India, the gap between the need and the available approaches to chronic care treatment is significant. And this need can be bridged by using a technology solution. Hence, the idea of HealthPlix. 
 
- How has the experience been so far with Healthplix?

What I mean by this is that, the extent of tech usage for treating the patients is limited only to the diagnostics devices and robot assisted surgeries among others. While these are essential tools for care interventions, the needs of disease specific technology solutions are lacking. In other words, the verticalized tech offerings for specific diseases are missing. 
 
Raghuraj feels employee demands are different today and companies need to frame policies in tune with these demands. He leaves corporates with an advice: The long hours, stressful schedules, and the fatigue of working for more than a decade or two needs more careful attention by companies. They would do well by focusing on focused initiatives aimed at improving the health of their employees, not just fitness (agree that is the first step) programs. These would go a long way in solving the problem of presenteeism and also absenteeism resulting from ill health of the employees.
