Incidence of stroke and related deaths are rising substantially around the world, with <keyword id="19900550" type="General" weightage="20" keywordseo="air-pollution" source="keywords">air pollution</keyword>, <keyword id="21334318" type="General" weightage="20" keywordseo="high-temperatures" source="keywords">high temperatures</keyword> and metabolic risk factors such as high blood pressure and physical inactivity, driving these increases, according to estimates published in The Lancet Neurology journal. Researchers found that the contribution of high temperatures to poor health and early death due to stroke has increased 72% since 1990 and is likely to continue increasing in the future, thereby highlighting how environmental factors can impact growing stroke burden. Further, for the first time, particulate matter or PM air pollution was found to have the same contribution as smoking towards causing a fatal form of brain bleed, according to the researchers forming the <keyword id="20691740" type="General" weightage="20" keywordseo="Global-Burden-of-Disease" source="keywords">Global Burden of Disease</keyword>, Injuries, and Risk Factors Study (GBD) group. The GBD study, the "largest and most comprehensive effort to quantify health loss across places and over time", is coordinated by the Institute for Health Metrics and Evaluation (IHME), University of Washington, US. Globally, the number of people experiencing a stroke for the first time rose to 119 lakh in 2021 -- up by 70% since 1990 -- while deaths related to stroke rose to 73 lakh, which was up by 44% since 1990, making the neurological condition the third leading cause of death after ischaemic heart disease (short supply of blood to heart) and COVID-19. They added that over three-fourth of those affected by strokes live in low- and middle-income countries. According to lead author Valery L. Feigin from Auckland University of Technology, New Zealand, the rapid growth in the number of people affected by stroke strongly suggested that <keyword id="23228151" type="General" weightage="20" keywordseo="stroke-prevention" source="keywords">stroke prevention</keyword> strategies currently used are not sufficiently effective. "New, proven effective population-wide and motivational individual prevention strategies that could be applied to all people at risk of having a stroke, regardless of the level of risk, as recommended in the recent Lancet Neurology Commission on Stroke should be implemented across the globe urgently," said Feigin. The researchers also estimated that stroke-related liabilities attributable to 23 modifiable risk factors, including air pollution, excess body weight, high blood pressure, smoking and physical inactivity, increased from 100 million years of healthy life lost in 1990 to 135 million in 2021. These risk factors are present in huge numbers in Eastern Europe, Asia and sub-Saharan Africa, they said. The authors also acknowledged substantial progress made in reducing the global stroke burden from risk factors linked to poor diet, air pollution and smoking. Poor health resulting from diets high in processed meat and low in vegetables declined by 40 % and 30%, respectively, while that due to PM air pollution and smoking fell by 20 % and 13%, respectively, they found. The results suggested that strategies for reducing exposure to these risk factors over the past three decades such as clean air zones and public smoking bans have been successful, the authors said. They called for implementing and monitoring the evidence-based recommendations set out in the 2023 World Stroke Organization-Lancet Neurology Commission on stroke for drastically reducing the global burden of stroke in the years to come and improving brain health and overall well-being of millions of people around the world. The recommendations include stroke surveillance programmes that monitor indicators of stroke such as incidence, recurrence, death rates and risk factors in a country, and care and rehabilitation services for people affected by stroke.

The <keyword id="22667872" type="General" weightage="100" keywordseo="mpox-outbreak" source="keywords">mpox outbreak</keyword> in <keyword id="19924003" type="General" weightage="20" keywordseo="africa" source="keywords">Africa</keyword> is yet another example of how <keyword id="20040661" type="General" weightage="20" keywordseo="infectious-diseases" source="keywords">infectious diseases</keyword>, perceived to be "someone else's problem", and affecting mainly poor, developing countries, may suddenly pose unexpected global threats. Other examples of <keyword id="22817243" type="General" weightage="20" keywordseo="neglected-diseases" source="keywords">neglected diseases</keyword> include the West Nile, Zika and Chikungunya viruses. Mpox was discovered in 1958 (in captive monkeys, hence the original misnomer "monkeypox") and the first human case was identified in 1970. Then, for decades it was largely neglected by the scientific and public health communities, regarded as an uncommon infection in remote rural areas in tropical Africa, without relevance for the rest of the world. When a massive mpox outbreak hit developed countries in 2022, increased research funding led to a surge in scientific studies. On just one medical search engine, there's been more research produced since April 2022 than in the preceding 60 years. The 2022-23 global mpox outbreak happened despite repeated calls from African researchers for increased global investment in diagnostic, therapeutic and infection prevention tools for mpox. The WHO has now declared the current upsurge of mpox in central Africa a <keyword id="21952478" type="General" weightage="20" keywordseo="public-health-emergency" source="keywords">public health emergency</keyword> of international concern. This is the highest alert level for events that constitute a public health risk to other countries and requires a coordinated international response. Mpox's recent history is yet another reminder that an infectious disease in one corner of the world should not be regarded as someone else's problem, as it can suddenly start to spread fast and far. It also highlights global inequities in resource allocation and access to vaccines, diagnostics and treatments. These were made available in many industrialised countries and helped curb the global outbreak, but are still largely lacking in most of Africa. <h2>The 2022 outbreak: a total surprise</h2>The disease has been renamed "mpox" but the name of the virus, for now, remains "monkeypox" (MPXV). It is closely related to the smallpox virus. MPXV was considered a <keyword id="21311828" type="General" weightage="20" keywordseo="zoonotic-disease" source="keywords">zoonotic disease</keyword> endemic in parts of central and west Africa. It was acquired mainly through close contact with wild mammals, especially handling bush meat, but there was no sustained human-to-human transmission. Only very occasionally were cases seen outside the endemic areas, due to infected travellers or import of infected small mammals. This changed abruptly in 2022: a massive, rapidly evolving global outbreak caused over 99,000 laboratory-confirmed cases in 116 countries. At its peak in August 2022, over 6,000 cases were reported each week. This outbreak came as a total surprise: most cases were reported from non-endemic countries, mostly in men who have sex with men, who had become infected during recent sexual encounters. Even though most cases were clinically not particularly severe and the death toll stands at just over 200, the global outbreak was declared a public health emergency of international concern by the <keyword id="19896311" type="General" weightage="20" keywordseo="world-health-organization" source="keywords">World Health Organization</keyword> on 23 July, 2022. Fortunately, case numbers soon plummeted due to a combination of behavioural changes and vaccination in at-risk groups. Modern vaccines and antiviral drugs with activity against mpox were made available in many affected high-income countries. These had been developed and stockpiled in the US and Europe, mostly in preparation for a potential bioweapon attack using a poxvirus. The global outbreak in 2022 was caused by clade II of MPXV, which is endemic in west Africa and not as virulent as clade I MPXV, which so far has only been seen in the Congo Basin. That first mpox public health emergency of international concern was declared over in May 2023. Clade II MPXV infections are still occurring globally, but the worst seems over - for now. Complacency would be misguided, as illustrated by the current mpox outbreak that is gathering steam. <h2>Upsurge in Africa</h2>The African region is experiencing an upsurge in mpox cases, which started in 2023. As the continent which includes the areas where mpox has been endemic for a long time, Africa now presents a complex mosaic. The current clade I MPXV (formerly called Congo Basin strain) is more virulent than the clade II (west African) strain, resulting in a higher case fatality rate. The ongoing outbreak has its epicentre in South Kivu province, eastern DRC, and has the potential to fuel a large pandemic. It has a distinct epidemiological pattern with sustained chains of human-to-human transmission, often via the sexual route. It may have increased transmissibility (we don't know yet). The virus which causes it is the newly defined clade Ib lineage. It displays mutations that are the hallmark of human-to-human spread that is estimated to have been happening since September 2023. Case numbers are rising rapidly, even though many suspected cases are likely not tested and thus not counted as confirmed. Complicating matters, a commonly used test was found to miss infections with this lineage of the virus. It affects mostly adults. The case fatality rate is higher than it was in the 2022 global outbreak. Already, this outbreak has resulted in mpox cases occurring in several neighbouring countries, including some (like Kenya), with no previous record of mpox. The challenge is enormous. The eastern DRC is an area beset by multiple problems. This includes natural disasters, violence and infectious diseases including measles, cholera and poliomyelitis for the DRC. In recent years the second-largest Ebola outbreak ever took place in the wider area and, despite the availability of vaccines and treatments, posed considerable challenges. <h2>What needs to happen</h2>Equitable access to diagnostic tests, vaccines and antiviral treatments requires political commitment and financial investments. Scientific investigations are needed to learn more about exposure settings, transmission routes and clinical presentations. It's important to find the best ways to make these interventions. There is no doubt that a disease in one corner of the world can suddenly become a global heath threat. It's time the global health system woke up to this reality.

Mpox outbreak in Africa was neglected - it could now turn into the next global pandemic

A new study has uncovered genes that could be related to <keyword id="21415597" type="General" weightage="20" keywordseo="heart-conditions" source="keywords">heart conditions</keyword> and <keyword id="20097972" type="General" weightage="100" keywordseo="Alzheimers-disease" source="keywords">Alzheimer's disease</keyword>, suggesting that the two could have similar origins. Researchers said that evidence increasingly links people having the heart condition with a quicker <keyword id="20097971" type="General" weightage="20" keywordseo="cognitive-decline" source="keywords">cognitive decline</keyword> and a 26% increased risk of dementia, affecting one's memory and thinking, and interfering with daily activities. Alzheimer's disease is the most common form of dementia. <keyword id="20849758" type="General" weightage="20" keywordseo="coronary-artery-disease" source="keywords">Coronary artery disease</keyword> is a condition in which blood flow to the heart is restricted due to narrowing of blood vessels. Even abnormal levels of fat in blood and inflammation could be possible shared risk factors between having coronary artery disease and developing Alzheimer's disease, the researchers at Edith Cowan University, Australia said. However, the processes underlying the link remain unclear. In this study, they employed genetic methods to examine the intricate relationship between heart disease and Alzheimer's disease. The team chose to study seven heart disease-related factors, such as coronary artery disease and angina, and 13 lipids, including cholesterol - both 'good' and 'bad'. Previous studies have long suggested lipids to play a role in the development of Alzheimer's disease, they said. "Our comprehensive analysis reveals a significant and positive global genetic correlation between Alzheimer's disease and three <keyword id="22817116" type="General" weightage="20" keywordseo="lipid-traits" source="keywords">lipid traits</keyword>: LDL ('bad' cholesterol), triglycerides, and total cholesterol," the authors wrote in the study published in the <keyword id="22817118" type="General" weightage="20" keywordseo="International-Journal-of-Molecular-Sciences" source="keywords" meta.keywordsubtype="org" meta.entityname="International Journal of Molecular Sciences" meta.hostid="261" meta.entityid="International Journal of Molecular Sciences">International Journal of Molecular Sciences</keyword>. This suggests that certain lipids, not all, may have a more direct genetic link with Alzheimer's disease, they said. However, many of the lipids that the authors studied were genetically related with coronary artery disease. "Specifically, we found significant genetic correlations between coronary artery disease traits and lipid profiles such as HDL, LDL, triglycerides, and total cholesterol, aligning with the existing literature that underscores the genetic interplay in <keyword id="22817117" type="General" weightage="20" keywordseo="cardiovascular-risk-factors" source="keywords">cardiovascular risk factors</keyword>," they wrote. A new study by <keyword id="5069783" type="General" weightage="20" keywordseo="Centre-for-Chronic-Disease-Control" source="Orion">Centre for Chronic Disease Control</keyword>, India (CCDC) on cardiovascular risk reduction in South Asia has demonstrated significant long-term benefits of a multi-faceted quality improvement (QI) strategy, comprising of task sharing, and clinical decision support software (CDSS) with electronic health records, in managing type-2 diabetes in India and Pakistan. The study, published in the prestigious journal PLOS Medicine, highlights the sustained impact of QI interventions on <keyword id="21411276" type="General" weightage="100" keywordseo="diabetes-care" source="keywords">diabetes care</keyword> and associated complications over a median follow-up period of 6.5 years. The burden of diabetes Diabetes control remains a global challenge, leading to severe complications such as <keyword id="22908" type="General" weightage="20" keywordseo="heart-disease" source="Orion">heart disease</keyword>, <keyword id="25191" type="General" weightage="20" keywordseo="stroke" source="Orion">stroke</keyword>, <keyword id="16753" type="General" weightage="20" keywordseo="blindness" source="Orion">blindness</keyword> or visual impairment, and kidney and nerve damage. LMICs are often faced with challenges such as inadequate infrastructure and trained healthcare staff with digital literacy. In that respect, the study's findings are crucial for integrating care models sustainably and identifying relevant organizational barriers. Previous studies from high-income countries showed improvements in diabetes care, but their long-term impact on vascular complications and mortality was unclear. This study aimed to address the critical gap in data regarding the long-term effectiveness of QI strategies and their impacts on vascular complications in LMICs. Professor Nikhil Tandon, Department of Endocrinology and Metabolism, All India Institute of Medical Sciences, New Delhi, and senior author of the study explained the relevance of these findings in the context of national efforts to combat non-communicable diseases (NCDs). He said, "The <keyword id="22511956" type="General" weightage="20" keywordseo="National-Programme-for-NCDs" source="keywords">National Programme for NCDs</keyword> [NP-NCDs] operationalized by the Government of India has already a functional portal which provides an electronic health record functionality. Integrating the CDSS in this portal will provide significant incremental benefit to the impact of the National Programme, and bolster governmental efforts to battle the scourge of NCDs including diabetes and hypertension." Speaking about the findings, Prof. (Dr) Dorairaj Prabhakaran, Executive Director, CCDC, and one of the study authors, said, "Through the trial, we've demonstrated that a sustained quality improvement strategy can significantly enhance diabetes care and reduce complications in South Asia. For every 13 patients treated with this strategy, one major adverse cardiovascular event like heart attacks or strokes- or a microvascular event -like eye disease or decline in kidney function- was prevented, showcasing its profound impact on patient outcomes." Professor MK Ali, William H. Foege Distinguished Professor of Global Health, Emory University and one of the key authors of this article, said, "This study is an exemplar of trans-national collaboration for benefits of both, the global south and global north." Study Design and Findings The randomized clinical trial included 1,146 patients with poorly controlled type 2 diabetes across ten outpatient diabetes clinics from January 2011 through September 2019. Participants were randomized to receive either the QI strategy, which involved trained non-physician care coordinators and a clinical decision support software (CDSS) integrated into the electronic health record form, for physicians, or the usual care. Some of the key findings from the study include: <ul><li>At 6.5 years, patients in the QI strategy group were nearly twice as likely to achieve and maintain target levels for HbA1c (&lt;7%), systolic blood pressure (&lt;130mmHg), and LDL-cholesterol (&lt;100mg/dl), compared to the usual care group. </li><li> The incidence of first microvascular complications, such as diabetic eye disease, decline in kidney function, and nerve pain was notably 32% lower in the QI group. </li><li>There was a significant 28% reduction in the combined occurrence of microvascular and macrovascular events in the QI group.</li></ul> Importantly, these benefits were sustained only in clinics that continued the QI strategy for 6.5 years. With diabetes accounting for one in seven deaths in South Asia, the study provides unique insights into the sustainability of QI strategies in achieving diabetes care goals in low- and middle-income (LMICs) and the effects of these improvements on reducing vascular complications and diabetes-related deaths.

The latter two are particularly linked with the risk of hypertension, cardiovascular disease, and chronic kidney disease. These findings underscore the necessity for optimal nutrient intake to maintain cardiovascular and kidney health. They also stress the urgency for personalised dietary interventions to mitigate the risks of prevalent non-communicable diseases. Emphasising the need for targeted interventions to address the alarming dietary patterns, Prof. Vivekanand Jha, lead researcher and Executive Director at The George Institute for Global Health, India, said: “A poor nutritious diet is a major risk element for non-communicable diseases (NCD), which are of considerable public health concern. “In India, people eat different foods, so it is important to know exactly what nutrients they are getting to help prevent and manage these diseases. The high salt intake and low potassium intake point to the need to develop interventions targeted to individuals and societies.” In combating the prevalence of non-communicable diseases, researchers have advocated for multifaceted strategies. These include enhanced food labelling to empower healthier choices, reducing salt content in processed foods, and promoting the consumption of potassium-rich fruits and vegetables. In embracing an informed and holistic approach to dining, where taste and nutrition harmonise, the populations can not only savour the essence of North Indian cuisine, but also nurture their well-being with every delectable bite. The findings of the study, conducted on 1,869 cancer patients across the country, were released on the World <keyword id="20775483" type="General" weightage="20" keywordseo="head-and-neck-cancer" source="keywords">Head and Neck Cancer</keyword> Day observed on Saturday. Cancer Mukt Bharat Foundation, a Delhi-based non-profit organisation, conducted the study by collating data from calls received on its helpline number from March 1 to June 30. Dr. Ashish Gupta, a senior oncologist who is heading the Cancer Mukt Bharat Campaign in India, said that India is seeing surge in head and neck cancer cases, especially among young men, due to increased <keyword id="20681485" type="General" weightage="20" keywordseo="tobacco-consumption" source="keywords">tobacco consumption</keyword> and human papillomavirus (HPV) infection. "Around 80-90 pc of oral cancer patients have been found to use tobacco in some form, be it smoking or chewing. Most of the head and neck cancer are preventable, unlike other cancers for which the reason is unknown. It is a preventable cancer that can be prevented with lifestyle modification. "There is a need to raise awareness to quit tobacco and early check-ups for early pickup of the disease," Gupta said. In India, nearly two-thirds of cancer cases are detected late likely due to low adoption of proper screening, he said. Gupta added that the Cancer Mukt Bharat campaign aims to reduce the incidence and impact of cancer on individuals and communities through education and early detection. "If found early in stages one or two, most head and neck cancers are curable in more than 80 pc of patients. In the field of cancer treatment, we get new medicines almost every week which can treat cancer in better way leading to better outcomes and good quality of life. "A combination of surgery, radiation therapy, chemotherapy, targeted therapy, and immunotherapy can be used for treating head and neck cancer. Latest cancer treatment has evolved to prioritize not just curing the disease, but also ensuring a good quality of life for survivors," he said. Under the Cancer Mukt Bharat Campaign, a free national helpline number (93-555-20202) was launched recently, which is operational from 10 am to 5 pm from Monday to Saturday. Cancer patients can call on this number to speak to leading oncologists directly or even do a video call to discuss about their treatment without paying any fees. Head and neck cancer was closely followed by gastrointestinal cancers which was 16 per cent. Fufteen per cent cancers in India re breast cancers and and blood cancers make up 9 per cent of them. These data are in line with the latest GLOBOCAN data -- a database that gives global cancer statistics -- on India. GLOBOCON data is part of International Agency for Research on Cancer's Global Cancer Observatory. India is expected to have 2.1 million new cancer cases by 2040, a big jump from the numbers in 2020, as per a recent GLOBOCAN report. While constant reminders from our teachers and parents to use dustbins might've felt rather irksome initially, they did go a massive way to reduce litter on the streets and in our homes. Now, from rainwater harvesting to waste segregation, more and more households are reshaping their daily routines and work practices to create a more sustainable future. While these concerned communities continue to drive change on the frontlines, their institutional allies aren't being nearly as sincere, research has found. As inspiring as household change may be, big businesses and industries remain the major contributors to <keyword id="6361617" type="General" weightage="100" keywordseo="climate-change" source="Orion">climate change</keyword>. They need to be more forthcoming in adopting more sustainable practices for us to achieve tangible change in our limited time — something that can most effectively be achieved by stricter sanctions. However, a new study has found that governments often take a backseat to these matters, preferring to plan and finance climate change adaptation measures such as incentivising green infrastructure. This rang true even in areas where the government was more involved than usual. “Individuals are primarily focused on changing what they can control: their own behaviours,” said co-author Elphin Tom Joe. “That’s needed, but so is action from institutional actors who can coordinate more broadly impactful adaptation.” Across the globe, individuals and households were doing the most to adapt to climate change on average compared to their governments, the study found. The divide tended to widen further in rural areas, but the trend somehow reversed in high-resource urban areas. However, even in high-income countries such as the United States or the UK, the government primarily suggested mere guidance on climate adaptation actions. Ultimately, it fell upon the individual to switch to more sustainable practices, such as picking flood-prevention measures or drought-resilient seeds. For the not-so-well-off, this can be at great personal and financial cost that they might not be able to manage effectively. Shifting the outdoor working hours to cooler parts of the day might seem like a small change, but it can mean reduced business and a greater burden on the worker's health. Additionally, the authors worry that such individual change, while undoubtedly inspiring, still does little to spur the quintessential institutional change that is needed to create tangible results. “In rural areas, especially in <keyword id="12925" type="General" weightage="50" keywordseo="africa" source="Orion">Africa</keyword> or the global south, individuals are doing what they have the agency to do — changing crops to something more resilient, maybe changing livelihoods from farming to fishing or even migrating,” explains another co-author Christine Kirchhoff. “And, while they will need to continue to take action, since they are the ones feeling the impacts the most, we still need all other actors to contribute to implementation.” The findings of this research have been published in Nature Climate Change and can be accessed <a href="http://dx.doi.org/10.1038/s41558-023-01824-z" data-type="tilCustomLink">here</a>.
