Putin initially <a target="_blank" class="" href="https://www.washingtonpost.com/world/2024/08/18/vladimir-putin-kursk-crisis-reponse/">froze</a> as he has in other crises, but Moscow eventually put the FSB — Russia's primary security and intelligence agency — in charge, leading a host of other groups. War experts <a target="_blank" class="" href="https://www.businessinsider.com/russia-messy-response-ukraine-kursk-shows-weaknesses-still-plaguing-army-2024-8">told BI that it was a surprising move</a> that showed Russia had not fixed long-standing issues with command and control. And, Bohnert said, those security forces are "subordinate to Putin."<h2 id="c00c66f6-4166-464f-a2fc-270dc15b4b6b">Putin in charge</h2>The Russian president has played an unusually direct role in leading the war in Ukraine, at times giving orders himself. US intelligence said in 2022 that <a target="_blank" class="" href="https://www.businessinsider.com/putin-giving-orders-generals-ukraine-battlefield-dysfunction-grows-us-intelligence-2022-9">Putin was giving orders directly to his generals</a> and that that was causing confusion for his military leadership. Because it's very clear Putin never wanted to give any general credit for winning the war in Ukraine."US officials told The New York Times that at the start of the invasion, there was no central war commander on the ground in Ukraine, and decisions were instead made from Moscow.Weeks after the start of the full-scale invasion, Russia put Gen. Aleksandr V. Dvornikov in charge of operations in Ukraine. He was fired from the role weeks later, with the UK Ministry of Defense saying it was highly likely because of the military's "poor performance."Multiple top Russian generals have been arrested during this war on various charges, including accepting bribes.Putin's motives for these arrests are unconfirmed. Occupants of four-wheeled vehicles make up 25% of the deaths and pedestrians account for 21 %. Cyclists represent 5% of the deaths. The remaining 20% include occupants of large vehicles, heavy goods vehicles, and other or unknown user types. "In the WHO Southeast Asia Region, the share of reported road traffic deaths is 46% for users of powered two- and three-wheelers, 12 for occupants of four-wheeled vehicles, 17% for pedestrians, 3% for cyclists and 22% for others. In all these countries, vulnerable road users (pedestrians, motorised two-wheeler users and bicyclists) constitute 66% of the total reported road traffic deaths," the report said. "Drivers or riders of two and three-wheelers constitute the highest proportion among all road user categories in India (45.1%), Maldives (100 %), Myanmar (47%) and Thailand (51.4%)," it added. The WHO noted that no country in the South Asian region had achieved the target of reducing its estimated road traffic deaths by 50 %. However, considerable reduction in the WHO's estimated road traffic deaths occurred in the Maldives and Thailand, where estimated road traffic deaths decreased by 46.2% and 41.7 %, respectively. On the contrary, reported road traffic deaths in the region increased by 15 % in the 2010-2021 period. At the country level, no member state of the Southeast Asia Region with more than 10 million population has reported a decrease in road traffic deaths during this period. "However, it should be noted that the reported number of road traffic deaths in Thailand reduced by 22.9% between 2016 and 2021. The estimated road traffic deaths too increased in Bangladesh, India and Nepal during the period 2010-2021 by 23 %, 2% and 25%, respectively," the report said. Etienne Krug, director of the WHO's Department for Social Determinants of Health, said road traffic crashes were a major crisis of the 21st century. The report reveals that the crisis is huge but we need to understand and act with determination that prevention is possible and needs strict enforcement," he said. <ul class="summary-list"><li>Precision warfare has been a central tenet of American strategy. </li><li>But perceptions that precision weapons are effective is a myth, a retired Army officer argues.</li></ul>America loves smart bombs. Ever since World War II, precision warfare has appealed to what America sees as its strengths: High technology, efficiency and the ability to strike down its enemies with a minimum of harm to innocents.But that's actually a myth argues Amos Fox, a retired US Army lieutenant colonel. Precision-guided munitions, or PGMs, are no more effective than conventional munitions in limiting collateral damage, and in some cases can make the damage worse.Fox calls this the "precision paradox." Or, "the incongruence between precision strike theory and the fervent enthusiasm of precision ideologues," he wrote in an <a target="_blank" href="https://rusi.org/explore-our-research/publications/rusi-journal/precision-paradox-and-myths-precision-strike-modern-armed-conflict">essay</a> for the Royal United Services Institute, a British think tank.Precision warfare is associated today with guided missiles, but the concept dates back to the 1930s, when the US began to embrace high-altitude daylight bombing by heavy bombers such as the B-17 Flying Fortress. Swayed by Italian airpower theorist <a target="_blank" href="https://www.h-net.org/reviews/showpdf.php?id=42478">Giulio Douhet,</a> American planners were convinced they could cripple an adversary by bombing its factories, without the need for a costly ground war.This contrasted with Britain's night area-bombing strategy in World War II that targeted entire German cities. Even if factories weren't hit, residential neighborhoods would be destroyed and workers "de-housed," which was expected to collapse the public's morale. In practice, the distinction between precision and area bombardment proved blurry: bombing through cloudy European skies that obscured targets, while under fighter and flak attack and relying on pencil-on-paper navigation plotting and rudimentary bomb sights meant the majority of American bombs <a target="_blank" href="https://www.airandspaceforces.com/article/1008daylight/">failed</a> to hit their target.PGMs were supposed to solve this problem. Why drop a dozen bombs when a single GPS-guided missile can destroy a bridge or a command post? Fox sees several flaws in modern precision strike theory. For one, "decapitation" strikes intended to defeat an enemy by eliminating its leaders and command posts have not worked. Nor does Fox believe that the precision strike strategy has actually shortened wars.But most significantly, Fox questions the essence of US precision warfare: the belief that smart bombs spare a need for boots on the ground. "Accurate strikes are not equivalent to effective strikes," he wrote. In other words, a strike can land on the intended area, selected based on intelligence, and yet fail to achieve the goal of, say, killing a militant leader or stopping a factory from making more bombs, hence necessitating follow-on strikes. When PGMs don't accomplish the mission, "then precision-based warfighting requires additional strikes and, likely, a subsequent use of land force activities to offset the shortcomings of precision strikes."<div type="image" title="More than half of the buildings in Gaza have been damaged or destroyed in Israel's bombing campaign since the Oct. 7 terror attacks." height="1333" width="2000" msid="110818254" hostid="261" sizebytes="513954"></div>This can actually result in higher civilian casualties than if conventional weapons had been used in the first place because it requires repeated attacks. "If precision strikes are often accurate, but ineffective, and additional strikes or land operations are required to create the effect intended with the initial precision strikes, then precision strategies do not decrease civilian casualties and collateral damage in conflict zones."Fox points to the extensive use of American PGMs during the ferocious battles of Raqqa and Mosul in 2016-2017. In trying to root out heavily fortified Islamic State positions dug into civilian neighborhoods, many buildings were destroyed and thousands killed. Israel faces a similar situation today as it hunts Hamas in Gaza.Thus the paradox: an individual PGM may be more accurate than a dumb bomb. But if a PGM fails to knock out a target — whether due to poor intelligence or the pure chance that reigns on battlefields — more guided weapons have to be launched, thus defeating the whole purpose of precision."Accurate strikes do not inherently mean effective," Fox told Business Insider. "Therefore, more strikes are required when a strike does not effectively accomplish its intended purpose. Thus, in the aggregate, if a PGM isn't 100 percent effective, it can often result in similar outcomes to ballistic artillery, or other non-precision munition employment."Fox doesn't believe that smarter bombs will solve the paradox. "Better PGMs isn't really the problem," he said. "PGMs are currently about as accurate as can be. For that matter, although artillery is an area fire weapon, it is still very accurate."But what about the conflicts in Ukraine and Gaza, in which massive amounts of PGMs are being employed? In Ukraine, both sides are using massive numbers of guided weapons, at a rate that is depleting stockpiles and factory capacity, yet neither side has managed to achieve decisive results. Israel carried out strikes in Gaza against 29,000 targets in the first four months of the war, often with guided weapons, but that has failed so far to destroy Hamas."The lesson is that Hamas is a land force," said Fox. "The inconvenient truth about war is that it still requires a land force to defeat a land force. Precision warfare, which isn't really a thing, augments a land force in defeating another land force. The effects of the fatal bridge collapse in Baltimore will be felt for months

<ul class="summary-list"><li>Federal regulators are investigating Ford's BlueCruise system following two fatal crashes.</li><li>The probe comes amid increased scrutiny of automated driving-assistance systems.</li></ul>Federal auto regulators are looking into Ford Motor's driver-assistance system following two fatal crashes that the safety agency linked to the automation technology.The National Highway Traffic Safety Administration has opened an investigation into Ford's BlueCruise system, according to an <a target="_blank" href="https://www.documentcloud.org/documents/24624131-nhtsa-blue-cruise?responsive=1&amp;title=1">agency filing</a> made public Monday.Both accidents involved a Mustang Mach-E electric vehicle crashing into a stationary car while on a freeway during lightning conditions at night, according to the agency. An initial agency review confirmed that BlueCruise was in use before both collisions.One person died in a February collision in San Antonio, and two people died in a Philadelphia crash in March, <a target="_blank" href="https://apnews.com/article/ford-blue-cruise-crash-investigation-deaths-4429651e132e3702a6a2a6a127aebbc2">The Associated Press reported.</a>A spokesperson for Ford told Business Insider the automaker was working with the NHTSA to support the investigation.The investigation comes as the auto industry ramps up scrutiny of automated driving-assistance systems amid a boom in the new, imperfect technology.Earlier this month, the NHTSA launched a second probe into Tesla's popular driving assistance software, Autopilot, after several accidents involving the system. The agency is investigating whether <a target="_blank" href="https://www.businessinsider.com/tesla-recalls-2-million-cars-as-regulators-say-autopilot-insufficent-2023-12">Tesla's December recall of 2 million vehicles</a> installed with Autopilot had adequately addressed the safety risks.In a recent <a target="_blank" href="https://static.nhtsa.gov/odi/inv/2022/INCR-EA22002-14496.pdf">report, the agency said it investigated 956 Tesla crashes between 2018 and 2023 in which</a> the agency found Autopilot was involved, resulting in 29 fatalities.Tesla has not publicly commented on the new probe. Tesla did not immedatiely respond to a request for comment from BI.The Insurance Institute for Highway Safety tested 14 driving automation systems across nine manufacturers for a March study and found that the "hands-free" devices can <a target="_blank" href="https://www.businessinsider.com/driving-assistance-systems-may-create-more-risks-than-solve-study-2024-3">create new safety risks by allowing drivers to become easily distracted.</a> Nearly all of the systems, including Ford's, scored low <a target="_blank" href="https://www.businessinsider.com/tesla-autopilot-safety-assisted-self-driving-systems-2024-3">marks</a>.Driving assistance systems aren't fully autonomous yet and typically monitor steering, braking, and acceleration on highways, though Ford and other car companies have stressed the importance of driver attention even while the software is in use.The NHTSA investigation into BlueCruise will determine how well the software does various driving tasks and evaluate its camera-based monitoring system.

The department did not respond to Business Insider's request for records.The driver told a Washington State Patrol trooper that he was in Autopilot mode and looked at his cellphone while the Tesla was moving, according to the CNBC report."The next thing he knew there was a bang and the vehicle lurched forward as it accelerated and collided with the motorcycle in front of him," the trooper wrote in a probable-cause document, according to CNBC.On Friday, federal <a target="_blank" href="https://www.businessinsider.com/feds-investigating-tesla-autopilot-recall-after-crashes-kept-happening-2024-4">regulators announced they are looking into whether Tesla's</a> recall of two million vehicles in December was enough to make sure the driver-assist technology stops driver misuse.The National Highway Traffic Safety Administration <a target="_blank" href="https://static.nhtsa.gov/odi/inv/2024/INOA-RQ24009-12046.pdf">announced</a> it was opening an investigation to "evaluate the adequacy" of the December recall to "address misuse, mode confusion, or usage in environments the system is not designed for."The new investigation will look into Model Y, X, S, 3, and Cybertruck vehicles with Autopilot made between 2012 and 2024, according to the NHTSA report.In December, <a target="_blank" href="https://www.businessinsider.com/tesla-recalls-2-million-cars-as-regulators-say-autopilot-insufficent-2023-12">Tesla recalled over two million vehicles</a> to update the Autopilot software. The recall stemmed from a two-year NHTSA investigation that identified at least 13 crashes with at least one fatality and multiple serious injuries. The software update aimed to improve driver awareness by increasing warnings and alerts.The NHTSA document stated that Tesla said "a portion of the remedy both requires the owner to opt in and allows a driver to readily reverse it."It's not known whether the driver involved in the Washington accident had the software update installed in his Tesla.According to NHTSA documents, most newer Teslas have software that would automatically update.In a document <a target="_blank" href="https://static.nhtsa.gov/odi/inv/2022/INCR-EA22002-14496.pdf">published</a> Thursday on the recall query, the NHTSA notes that the term "Autopilot" may lead drivers to think "the automation has greater capabilities than it does and invite drivers to overly trust the automation."Tesla's website states Autopilot mode does not make the vehicle autonomous and drivers must use additional caution and pay attention.Tesla did not immediately respond to a request for comment.

All Nepali air carriers are banned from entering the airspace of any European Union country, <a target="_blank" href="https://transport.ec.europa.eu/document/download/3195fabb-7575-4db9-85b9-ec5a77b481aa_en?filename=air-safety-list-2024-05-30_en.pdf">per the EU Air Safety List</a>.The European Union Aviation Safety Agency bans airlines from operating in the EU for "failure to adhere to the applicable international safety standards."Professor Ron Bartch, chairman and founding director of aviation consultancy firm AvLaw, told Business Insider that one reason for its poor safety record is that the Civil Aviation Authority of Nepal is "not a terribly well-resourced safety regulator."The CAAN did not immediately respond to a request for comment from Business Insider.Nepal's mountainous terrain, unpredictable weather, and remote runways have contributed to the country's reputation as one of the most difficult places to fly.Kathmandu's high altitude makes for a "challenging operating environment," Bartch added, noting that this means a greater distance and an accelerated approach speed are required for takeoff and landing as the air is less dense than at sea level.He also said Nepal's monsoonal conditions affect the country's aviation safety.The deadliest plane crash in Nepal in three decades occurred in January 2023, when at least <a target="_blank" class="" href="https://www.businessinsider.com/folk-singer-hotelier-travel-blogger-among-victims-nepal-plane-crash-2023-1">72 people were killed</a> after a <a target="_blank" class="" href="https://www.businessinsider.com/video-nepal-plane-was-spinning-sharply-before-crash-40-reported-killed-2023-1">Yeti Airlines flight went down near Pokhara</a>, which government investigators later attributed to the aircraft's pilots accidentally cutting off the power.In 1992, all 167 passengers on board a Pakistan International Airlines flight to Kathmandu died when the Airbus crashed on approach to the airport.Since 2000, more than 350 people have died in Nepal in 19 air crashes.
