The bank's Global Markets Institute, led by Amanda Hindlian, warned of "significant" potential risks to the world's largest cities, which are especially vulnerable to more frequent storms, higher temperatures, rising sea levels, and storm surges. Cities generate about 80% of global GDP and are home to more than half of the world's population, a share that Goldman says, citing the United Nations, is projected to reach two-thirds by 2050. About 40% of the global population lives within 100 kilometers of a coast, it says, and 1 in 10 live in areas less than 10 meters above sea level.Goldman highlighted three cities which would be subject to those storm surges and in the future could face harmful flooding - New York, Tokyo, and Lagos. Miami, Alexandria, Dhaka, and Shanghai face major flood risks due to being less than 11 meters above sea level. <div class="KonaFilter image-container display-table"><div><div class="image on-image">Goldman SachsAreas such as Brooklyn and Queens could be affected heavily.</div></div></div>Goldman said that when starting the study they took a broad consensus that human activity, namely emission of greenhouse gasses "is causing the earth to warm in ways that are affecting the climate."Natural ecosystems would be damaged, and risks to human health would rise, as well as pressures on food and drinking water. Agriculture would also be massively affected: "Warmer temperatures and shifting precipitation patterns could reduce yields and nutritional quality as well change growing seasons and agricultural zones around the world."Goldman gave some fairly stark warnings about potential outcomes:<ul><li>More frequent, more intense, and longer-lasting heatwaves. The consequences will affect human health, productivity, economic activity, and agriculture. "Higher surface temperatures could exacerbate the warming process by causing permafrost to melt, releasing further methane and CO2 into the atmosphere."</li><li>Destructive weather events, including storms, winds, flooding and fires. It's not just New York, Tokyo and Lagos. "Other major low-lying coastal or already flood-prone cities include Shanghai, Dhaka, Mumbai and Karachi - each of which has a population of 15 million people or more."</li><li>Changing disease patterns. "Warmer temperatures could cause disease vectors to migrate from the tropics to regions where people have less immunity; this is true not only for viruses like malaria and dengue fever but also for water-borne and food-borne diseases."</li><li>Shifting agricultural patterns and food shortages. "Livestock could be affected by higher temperatures and reduced water supplies. Ocean acidification is likely to put stress on aquatic populations and affect current fishing patterns. Some of these changes are already underway. Some climate scientists, for example, estimate that coral reefs will be all but extinct over the course of the century due to ocean acidification."</li><li>Water. "Half of the world's population will live in water-stressed areas as soon as 2025," Goldman notes, citing the World Health Orgnization. "Even in non-stressed areas, the quality of surface water could deteriorate as more rain and storms drive erosion and the release of toxins. These dynamics could affect everything from the availability of drinking water for people to a shortage of water for livestock and crops (with negative effects for the food supply) to decreases in hydroelectric power generation."</li></ul><div class="KonaFilter image-container display-table"><div><div class="image on-image">Goldman SachsTokyo Bay and Lagos also could suffer as a result</div></div></div>The bank said that all those factors would "affect economic activity, damage infrastructure - from buildings to transportation to water and waste-management systems - and disproportionately harm vulnerable residents.""Despite the uncertainty around the timing and scale of the impact, it may be prudent for some cities to start investing in adaptation now," Goldman says. "Urban adaptation could drive one of the largest infrastructure build-outs in history. Goldman Sachs just released a 34-page analysis of the impact of climate change. And the results are terrifying.

It looks at how climate change is affecting the US now and what the country might look like by the end of the century.The findings are dire.The researchers found that the average temperature in the US rose 1.2 degrees Fahrenheit from 1951-2010, and an additional 2 degrees more is inevitable by 2050. But if we continue business-as-usual (and don't change our energy or agriculture systems to emit less heat-trapping greenhouse gases), the average temperature could go up by as much as 11 degrees by the end of this century. Sea levels on US coasts have already risen about 9 inches, but we could see as much as 6 feet by 2100. About $1 trillion of wealth in coastal real estate could be threatened.Deaths from heat-related causes are projected to increase, as are the frequency and severity of allergic illnesses like asthma. Changing temperatures will also expand the geographic range of disease-carrying insects like ticks and mosquitoes, leaving more people at risk of getting Lyme disease, Zika, West Nile, and dengue. All of these changes, and the many others projected - including more severe storms, changes in growing seasons, and impacts on infrastructure - could majorly hurt the economy. "Annual losses in some economic sectors are projected to reach hundreds of billions of dollars by the end of the century - more than the current gross domestic product (GDP) of many US states," the report says.<h2>What the data shows</h2>Over 300 scientists and other experts from academia, government, non-profits, and the private sector helped write the assessment. Andrew Light, a senior fellow at the World Resources Institute, worked on the chapter focusing on mitigation. What's different about this report from the last one, he told Business Insider, is how much clearer and more precise scientists are able to get about the US-specific consequences of climate change."We can now say with more accuracy that by the end of the century, the difference between the United States in a world where we have achieved something like the goals that we undertook for temperature stabilization in the Paris agreement and one where we don't is tens of thousands of lives lost annually and potentially hundreds of billions of dollars lost annually," he said.<div class="KonaFilter image-container display-table"><div><div class="image on-image">Zachary Fagenson/ReutersFlooding is seen on city streets in Miami Beach, Florida on November 5, 2013.</div></div></div>For example, the report predicts that in the years from 2070-2099, the US will see 20% more precipitation in winter and spring for the north central US, and a 20% decrease in the southwest in spring. And the area burned by lightning-ignited wildfires each year is expected to increase by at least 30% by 2060.But the report doesn't just make projections about changes that may occur decades from now; it attributes trends and disasters that we're already seeing today to climate change as well. "With climate <keyword id="41551" type="ENTERTAINMENT" weightage="100" keywordseo="science">science</keyword> now, we can tell you today how climate change is impacting the world and the United States in particular regions. Light said it's impossible not to conclude that the timing is intended to hide the report from public view."Making an announcement on Friday is called taking out the trash. This is not just taking out the trash, it's trying to burn it, bury it, scatter it to the four winds," he said. "This administration does not want anyone to understand that this report is coming out. They don't want people to come to the absolutely reasonable conclusion that it contravenes everything that this administration is doing on climate change. And that's a shame, because this should not be a partisan issue. This is a public health crisis."With regard to human health, in fact, the report found that the annual health impacts and costs would be approximately 50% lower in a scenario in which we reduce emissions and limit global warming than they would in a business-as-usual scenario.<h2>'There is a growing and increasing market for getting on the right side' of climate change</h2>But despite these ominous predictions, Light said the report gives us reasons for hope as well. "There is abundant evidence that this is not a negative-cost proposition, that you can make tons of money, that there is a growing and increasing market for getting on the right side of pricing the pollution and then selling the alternatives to high-carbon energy sources," he said. That evidence includes the fact that, <a href="https://www.bls.gov/ooh/fastest-growing.htm">according to the US Bureau of Labor Statistics</a>, the two fastest growing professions in the US are in the renewable energy sector: solar photovoltaic installers and wind turbine service technicians.<div class="KonaFilter image-container display-table"><div><div class="image on-image">Deepwater Wind/GEThe Block Island Wind Farm, which began operating in 2016, was the US' first commercial offshore wind farm.</div></div></div>There is also growing interest in the tech community around ways to suck carbon dioxide out of the air. Y Combinator, the largest startup accelerator in Silicon Valley, put out a request in October for companies working on such technologies."This is where markets are going. This is the new set of technologies that people are starting to pay attention to," Kate Gordon, a fellow at Columbia's Center on Global Energy Policy, told Business Insider last month. The Trump administration released a dire new report on climate change that predicts hundreds of billions of dollars in economic losses to come

The WHO has cautioned people across flood-hit areas of Pakistan, precisely the worst affected <keyword id="12961" type="General" weightage="80" keywordseo="sindh" source="Orion">Sindh</keyword> province to be extra careful. Tedros on Saturday highlighted that stagnant water serves as a breeding ground for mosquitoes and spreads vector-borne diseases such as malaria and dengue and urged donors to continue to respond generously to save lives and prevent more suffering . Separately, the WHO Director general tweeted that he was deeply concerned about the potential for a 2nd disaster in Pakistan, spreading disease and threatening lives following the flooding and asked for quick support to tackle the issue. If we act quickly to protect the health system and deliver essential services, we can reduce the impact of this impending health crisis, he tweeted. Situation in Pakistan continues to be grim as the devastating floods have caused havoc in the country, precisely Balochistan and Sindh provinces. At least 1,545 people have been killed due to the floods across Pakistan and an estimated 16 million children impacted. The WHO has immediately released USD 10 million from the WHO Contingency Fund for Emergencies which enabled the global health body to deliver essential medicines and other supplies to the country. In a related development, the <keyword id="6366068" type="General" weightage="50" keywordseo="international-monetary-fund" source="Orion">International Monetary Fund</keyword> (<keyword id="6355408" type="General" weightage="80" keywordseo="imf" source="Orion">IMF</keyword>) on Sunday announced to support flood relief and reconstruction efforts in Pakistan under its current bailout package for the country. The Fund last month restored a stalled economic assistance package and also provided USD 1.1 billion to the country. The IMF's resident representative in the country, Esther Perez Ruiz, said in a statement that the IMF was deeply saddened by the devastating impact of the recent floods in Pakistan and extended its sympathies to the millions of flood victims. We will work with others in the international community to support, under the current programme, the authorities' relief and reconstruction efforts, and especially their ongoing endeavour to assist those affected by the floods while ensuring sustainable policies and macroeconomic stability, the statement said. However, it is not clear how IMF would provide support under the existing Extended Fund Facility (EFF) arrangement, which was signed in July 2019 to provide USD 6 billion to Pakistan during a 39-month period. But the IMF board had approved an extension of the program until end-June 2023 and also enhanced it to about USD 6.5 billion. The Sindh health department, meanwhile, said a total of 2.5 million patients had been treated at different medical camps across the province from July 1 to date, the Dawn newspaper reported. As many as 594,241 patients were treated for skin-related diseases, followed by diarrhea (534,800), malaria (10,702), dengue (1,401), and other diseases (120,745,1), stated a report by the Sindh Directorate General Health Services. The report also showed that 90,398 patients were treated during the last 24 hours, of which 17,919 had diarrhea, 19,746 had skin-related diseases, 695 had malaria and 388 had dengue. Around 92,797 people were treated in the province on September 15, the paper reported. In Khyber Pakhtunkhwa province, the health department said that its officials are already struggling to stem the spread of dengue hemorrhagic fever in flood-hit districts of the province, The Express Tribune newspaper reported. The 33 countries where the climate crisis poses the most risk to children contributed only 9% of global CO2 emissions, while 70% of emissions were attributed to just 10 countries.A <a href="https://www.nature.com/articles/srep20281">2016 study</a> found 20 of the 36 highest-emitting countries were among the least vulnerable to the effects of a warming climate, while 11 of the 17 countries with low or moderate emissions were significantly vulnerable.Some of the countries with low emissions per capita that are at a high risk include Bangladesh, India, Philippines, and multiple countries in sub-Saharan Africa, the Caribbean, and South America.They are facing heat waves, water scarcity, coastal flooding, and water-borne disease, among other hazards, which in turn can lead to poor sanitation, lack of food, scarce income, and deteriorating living conditions. Meanwhile, many of the places currently experiencing these effects lack the infrastructure or resources to mitigate them.Experts and climate activists have said that while countries contributing the most need to reduce emissions, poorer countries are in need of immediate funding to address current challenges.<h2>Heat waves have killed thousands in India in recent years</h2>In the summer of 2019, large swaths of India were <a href="https://www.nytimes.com/2019/06/13/world/asia/india-heat-wave-deaths.html">scorched by temperatures</a> greater than 113 degrees Fahrenheit for nearly three weeks. In some regions, officials closed local schools. Medical authorities <a href="https://www.ndtv.com/india-news/heat-wave-conditions-in-rajasthan-leaves-of-churu-doctors-cancelled-2049388">canceled time off for doctors</a> to ensure hospitals could handle the influx of patients.Four passengers <a href="https://www.indiatoday.in/india/story/kerala-express-passengers-dead-heatwave-uttar-pradesh-jhansi-1546733-2019-06-11">fell ill on board an express train</a> that lacked air conditioning, and died by the time the train reached a station in Jhansi, south of New Delhi."Shortly after we left Agra, the heat became unbearable and some people started complaining of breathing problems and uneasiness. Before we could get some help, they collapsed," a passenger told <a href="https://www.indiatoday.in/india/story/kerala-express-passengers-dead-heatwave-uttar-pradesh-jhansi-1546733-2019-06-11">India Today</a>.By the end of that summer heat wave, <a href="https://www.indiaspend.com/65-indians-exposed-to-heatwaves-in-may-june-2019-july-2019-was-indias-hottest-ever/">more than 200 people had died</a>. The heat wave of 2015 was far worse, with extreme heat causing the deaths of more than 2,000 people. In the past decade, more than 6,000 people in India have died as a result of excessive heat, <a href="https://www.nytimes.com/2019/06/13/world/asia/india-heat-wave-deaths.html">according to government data</a>.<div type="image" title="In this Thursday, May 30, 2019, file photo, children returning from school walk through a dried pond on a hot summer day on the outskirts of Jammu, India. Many parts of India are experiencing extreme heat conditions." height="3723" width="4964" msid="87688718" hostid="261"></div>"Climate change is leading to more frequent, more intense, and longer heat waves around the world," said Olga Wilhelmi, a scientist at the National Center for Atmospheric Research, adding that the link has been well-documented.An estimated 820 million children, more than a third of kids worldwide, are highly exposed to heat waves, according to the UNICEF report."Extreme heat is probably one of the least appreciated weather hazards, but it's actually one of the more deadly weather hazards in the US and worldwide," Wilhelmi said. "Studies show that in the US, somewhere on average between 600 and 1,800 people die from extreme heat every year, but we don't usually read about that in the news."Health complications of extreme heat can vary from dehydration to heatstroke. Heat waves can also exacerbate cardiovascular and respiratory diseases. Groups at a higher risk include older people, children, people with pre-existing conditions, and those who are lower-income and do not have access to air conditioning.Heat waves can also shut down electric grids when they don't have the capacity to handle the increased demand for air conditioning.<h2>Lack of water leads to poverty and hunger </h2>Water scarcity, the lack of adequate access to clean water, is another way people are feeling the climate crisis.While scarcity can be due to institutional failures to deliver water, in many instances it's due to dwindling water sources or growing populations that need more water. More than a third of kids worldwide, 920 million are currently highly exposed to water scarcity. This issue affects kids in various parts of the world including parts of India, South America, Australia, and the Middle East. Pierce said rural areas tend to be poorer and have one water source. Residents are usually already walking long distances to get to that water which may not always be clean. Water isn't just needed for drinking, but for producing and selling agriculture. Water is very much tied to livelihoods. "The scarcity issue makes it harder to do agriculture, which is what most rural populations rely on for livelihoods. So, there's a hunger, inability to grow food and inability to earn any money," Pierce told Insider. "That lack of water leads directly to people starving and that is going to become a lot more common, unfortunately again, in many parts of the world."When those resources dry up, residents would be forced to travel even further or move, further straining resources. While many parts of India are dealing with water scarcity, other regions are also experiencing flooding, Pierce said. However, the flooding comes with other complications and doesn't counter the consequences of water scarcity. <h2>Coastal flooding, which destroys property and resources, is already taking a heavy toll in Bangladesh</h2>In Uddin's home of Bangladesh, rising seas and extreme precipitation have inundated communities with flooding in recent years.The village of Bonnotola, once home to more than 2,000 people, has less than 500 residents left, as the flooding and salt water-tainted soil destroyed many people's homes and livelihoods, the <a href="https://apnews.com/article/climate-science-europe-environment-and-nature-bangladesh-0f2a0a0f463523a162901f1beda23c7e">Associated Press</a> reported.One woman from the village of Gabura told AP everyone used to grow food in their backyards, but that salt water flooding has disrupted the once-fertile lands and freshwater that many relied on."We have water everywhere, but we don't have a drop anymore to drink from ponds or wells," she said.Almost every ocean on Earth is experiencing a sea-level rise, according to William Sweet, an oceanographer at the National Oceanic and Atmospheric Administration.One in 10 children, or 240 million, are currently exposed to coastal flooding, according to the UNICEF report. Sea-level rise is exacerbated by extreme rainfall events, which are also a consequence of warming."The same heating that's causing oceans to rise, ice caps and ice sheets to melt, and oceans to expand, is also enabling more moisture to be held within the atmosphere," Sweet said, adding that the result is frequent heavy rains. "The combined effect of course is flooding with nowhere for that water to go."While the threat of entire cities being underwater is the most commonly talked about result of rising sea levels, there are many significant issues that arise well before a city or property is actually underwater."It's not when you're underwater, it's when the system fails," Sweet said, adding that flooding can overwhelm infrastructure and destroy roadways, stormwater and wastewater systems, and people's personal property.He said by the time areas are actually underwater, people are already gone by then, driven out by the flooding.<div type="image" title="Bangladeshi flood victim man carrying their houses by boat during flood in Kurigram, Bangladesh on July 27, 2019." height="1333" width="1777" msid="87688722" hostid="261"></div>Coastal flooding creates more problems where water infrastructure is lacking, Pierce told Insider."Flooding doesn't typically increase water supply. It can have a direct impact on water-borne illness, especially because there's poor sanitation in a lot of places and when there's flooding, it gets into wherever the waste is being contained or into the open sewers and spreads," he said. <h2>Diseases are becoming more prevalent in sub-Saharan Africa and South America because of the climate crisis</h2>As temperatures rise, cities flood and water becomes scarce, there's also a rise of waterborne illnesses with an estimated 600 million children currently exposed to diseases like malaria and dengue fever, according to the UNICEF report. The impact can be most immediately felt in countries in sub-Saharan Africa and South America. Martin Muchangi, AMREF water and sanitation specialist based in Kenya, told Insider he studies waterborne illnesses across much of sub-Saharan Africa, and that a vast majority of people are dealing with water scarcity which in turn is driving up waterborne diseases.Muchangi said the issue is twofold: changing temperatures pushing things like mosquitoes into more places and increasing pathogens that cause diarrhea illnesses like cholera and typhoid and the lack of water leading to poor hygiene and diseases like scabies or glaucoma. He told Insider the problem is leading to harming both people and the economy. "So now at the end of the day, if you combine the sum total, you realize that there's major economic loss, there's serious effects to the health, there's serious effects to the thriving of children," he said. <h2>Many countries facing the worst of the climate crisis also have the least resources to address it </h2>While nowhere on earth will escape the effects of the climate crisis, some countries have infrastructure and resources that make them more prepared to respond.Pierce told Insider many of the impacts can be mitigated with robust public health initiatives and proper infrastructure. Water filtration systems, for example, can mitigate the risks of water-borne disease.Muchangi and Uddin told Insider the countries they live and work in, however, don't have the funds they need to address the issues they're facing. In Bangladesh, Uddin said funding is needed to direct towards researching ways to remove salt from saltwater and towards building infrastructure that can withstand flooding, including fortifying schools, hospitals, homes, and other essential buildings. Muchangi said that while finding ways to mitigate the immediate impacts of the changing climate is necessary, limiting CO2 emissions would greatly reduce the burden."If we are able to alleviate climate change itself, then the effects of climate change are going to become lesser and we are going to be able to thrive in a better way," he said.

That's one of the dire predictions in <a href="https://nca2018.globalchange.gov/">the climate report</a> that the Trump administration <a href="https://www.businessinsider.com/climate-report-us-economy-will-lose-hundreds-of-billions-2018-11">released on Black Friday</a>. Scientists project that as average temperatures continue to rise, <a href="https://www.businessinsider.com/climate-report-scary-conclusions-and-reason-for-hope-2018-11">the geographic ranges of disease-carrying insects</a> like mosquitoes and ticks will grow, putting more Americans at risk of getting infected with West Nile virus, Zika virus, Lyme disease, and dengue in the coming decades.The report, called the National Climate Assessment, is the fourth in an ongoing series mandated by a 1990 law. It analyzes the possible consequences of various levels of climate change in the US. With respect to West Nile virus, the report says, the US could see about 3,300 more cases each year by 2100 if current greenhouse-gas emission patterns continue. (That's under a scenario in which we keep putting more heat-trapping gas into the atmosphere for the rest of the 21st century.)The annual costs of those illnesses and deaths by 2050: $3.3 billion.The report also looks at a more optimistic scenario, which shows greater reductions in greenhouse gas emissions over the next 30 years, with further decreases after that. Almost everywhere, average annual temperatures will rise. Combined, these factors will likely lead to higher rates of many other infectious diseases in addition to West Nile virus. For example, droughts and dry conditions are known to cause a spike in cases of Valley fever. More cases of the illness, which is caused by a fungus in soil, have already been observed in states like Arizona and California. Valley fever sends people to the hospital in about 40% of cases and causes 75% of infected people to be unable to perform daily tasks for weeks or months. The geographic ranges of pests that carry tropical and subtropical diseases could expand as well.Outbreaks of dengue, for example, have recently been reported <a href="https://nca2018.globalchange.gov/chapter/23/">in southern Texas</a> because the mosquitoes carrying the tropical disease no longer get killed off by low temperatures in the state. <a href="https://nca2018.globalchange.gov/chapter/26/">Alaskans</a>, meanwhile, could get infected with tick-borne illnesses like Lyme at higher rates. A rise in instances of diarrhoeal disease, for example, has been linked to flooding, heavy rainfall, and high temperatures. These factors, and the sanitation issues they cause, could also increase rates of infections like cryptosporidiosis, which causes diarrhea, fever, and stomach pain.In addition to nationwide trends in infectious disease transmission, the report looks at some specific locations that are at risk. In Alaska, for example, environmental changes could reduce residents' access to clean water, since tundra ponds that provide clean water are shrinking due to climate change. Riverbank erosion could also lower water quality, and wastewater treatment systems could get damaged from coastal erosion or storm surges. This might force some Alaskans to use untreated water, reuse water, or ration water, which could cause waterborne infectious diseases to become more common, the report said. Heat waves and high temperatures could also make Americans more vulnerable to germs in water, such as Vibrio bacteria, which can cause deadly infections. <a href="https://nca2018.globalchange.gov/chapter/18/">In the Northeast</a>, shellfish are projected to make more people sick because warmer waters lead to to shell disease in lobsters and other pathogens in oysters.The transmission risk for some diseases could go down because of very high temperatures, the report said, and economic development could also reduce people's risk of getting some diseases. But overall, it's an upward trend. According to the report, the high-emissions, business-as-usual scenario would cause thousands of additional deaths in the US. "In the absence of more significant global mitigation efforts, climate change is projected to impose substantial damages on the US economy, human health, and the environment," the report said. West Nile virus infections could become twice as common in the US within 30 years. Here's why disease-carrying insects are spreading.

India prides itself in being a galloping economy with great ideas and cultural diversity. 
 
However, it also stands out for its colossal neglect of some basic factors---such as health. 
 
For instance, Bill Gates recently made an embarrassing comparison of India with Ethiopia. The African country, he said, was reportedly terrible 10 years ago. However, it has become very good today, and continues to improve due to efficient surveillance systems. Worryingly, the world’s problem areas include parts of India, Pakistan, Nigeria and the DRC, he said. 
 
The <keyword id="102649" type="ORGANIZATION" weightage="20" keywordseo="Gates-Foundation">Gates Foundation</keyword> 
 To streamline processes, the Bill and <keyword id="142497" type="ORGANIZATION" weightage="20" keywordseo="Melinda-Gates-Foundation">Melinda Gates Foundation</keyword> recently launched the Child Health and Mortality Prevention Surveillance Network, or CHAMPS. It is putting some special focus on improving the structure and getting systems into place, said Bill Gates, to <a href="http://www.theatlantic.com/health/archive/2015/05/bill-gates-child-health-mortality-prevention-surveillance-network/392549/">The Atlantic</a>. 
 
For instance, it is not clear why neonatals die, yet their project is trying to probe the drivers of mortality for all children. The Gates announced that they are investing $75 million in a number of surveillance sites expected to collect information "about how, where and why children are getting sick and dying.” The plan will be spread first through six nations in Africa and South Asia, depending on field workers to take the autopsies of children who have died, especially in “beefed-up laboratories.” 
 
Disease surveillance system 
 The Integrated Disease Surveillance Programme (IDSP) was launched in India with the support of the World Bank in November, 2004, so that the country could detect and respond to disease outbreaks quickly. In March 2010 to March 2012, World Bank funds were made available for <keyword id="2858752" type="UNKNOWN" weightage="20" keywordseo="Central-Surveillance-Unit">Central Surveillance Unit</keyword> as well as nine identified states (Uttarakhand, Rajasthan, Punjab, Maharashtra, Gujarat, Tamil Nadu, Karnataka, Andhra Pradesh and West Bengal). The other 26 states were funded only by local budgets. 
 
The Surveillance Programme will continue well into the 12th Plan (2012-17) under the National Health Ministry, with an outlay of Rs 640 crore from the domestic budget. 
 
The World Bank’s focus was to strengthen the system for epidemic-prone diseases, improve central monitoring and coordination, improve state and district surveillance focusing on nine states, gear up and prevent influenza epidemics. 
 
Findings of the project 
 
The project seemed to have detected a few parameters for progress in surveillance as well as in identification and response, related to IDSP and also assessed the infrastructure and systems in place. A number of future goals have been evolved to improve the public health information and intervention. 
 
Only recently, it was found that ratings for the Integrated Disease Surveillance Project (IDSP) for India were just moderately satisfactory. The survey factored the risks to development outcome, bank and borrower performance. 
 
Unfortunately, Non-Communicable Diseases (NCDs), which should be a core, important public health function in India became part of the surveillance survey very late. The NCD risk factor was thus delayed and was improved only in some provinces, not countrywide. Ultimately, the country plans to integrate the NCD factor into the national programme. 
 
Inequalities 
 
The solutions to fight epidemics and diseases are distributed unevenly among different groups and communities, impacting socially and economically sensitive groups. Unfortunately, the inequitable system of distribution in India is glaringly evident in the implementation of the system. 
 
Take TDAP (Tetanus, diphtheria and pertussis), for instance. At first, the programme was not implemented with efficiency. It was restructured only in 2010 in the nine “best performing states”, selected on the basis of their performance as well as administrative and reporting efficiency. It was an ironical conclusion, as the states that needed support lagged behind the most, yet got the least central assistance. 
 
The twin outbreaks of dengue and chikungunya were a major public health concern and pain point a few years ago. The Aedes mosquito-borne chikungunya, with more than a million suspected cases in India, could be controlled by general hygiene and sanitation, but the disease was not shaken off due to the problems in the sanitation, hygiene and environment. 
 
Amazingly, the health minister at the time, Anbamoni Ramadoss, actually commented that the health ministry can't do much in the job! “(We need) involvement of general public, the associations and the civil society," he said. 
 
Efficient implementation 
 
The centre cannot wash off its responsibilities, but the execution is too erratic. For instance, Andhra Pradesh has noted that a special sanitation drive was launched, including massive disease surveillance, to improve the vaccination system in its state. In Gujarat, traditional medicines have been harnessed to fight the disease. 
 
The surveillance system reveals that a number of measures are being identified and put in place--for instance, deployment of trained personnel, appropriate technology as well as efficient administrative services. Establishing extensive laboratory networks are needed for careful investigation. 
 
So far, the ratings of the system have been just moderately satisfactory. Hence, the nation should gear up and streamline the administration, which is crying out for attention.
