The pharmaceutical giant <keyword id="19910149" type="General" weightage="20" keywordseo="AstraZeneca" source="keywords" meta.keywordsubtype="org" meta.entityname="AstraZeneca" meta.hostid="261" meta.entityid="AstraZeneca">AstraZeneca</keyword>'s <keyword id="20040659" type="General" weightage="20" keywordseo="COVID-19-vaccine" source="keywords">COVID-19 vaccine</keyword> is being withdrawn worldwide after the company acknowledged for the first time in court documents that it can cause a rare and dangerous side effect, according to a report in the British Newspaper 'The Telegraph'. AstraZeneca has announced that the <keyword id="19959412" type="General" weightage="20" keywordseo="vaccine" source="keywords">vaccine</keyword> was being removed from markets for commercial reasons. It further said that the vaccine was no longer being made or supplied, having been superseded by updated vaccines that fight new variants. The application to withdraw the vaccine was made on March 5 and came into effect on May 7. The vaccine can be no longer used in the European Union following the company's decision to withdraw its "marketing authorisation." Similar applications will be submitted in the UK and other nations in the coming months that have given a go-ahead to the vaccine, known as Vaxzevria. In recent months, Vaxzevria has come under scrutiny over a very rare side effect, which causes blood clots and low blood platelet counts. In court documents, AstraZeneca in the High Court in February admitted that the vaccine "can, in very rare cases, cause <keyword id="20218859" type="General" weightage="20" keywordseo="TTS" source="keywords">TTS</keyword>". TTS which stands for <keyword id="21475609" type="General" weightage="20" keywordseo="Thrombosis-with-Thrombocytopenia-Syndrome" source="keywords">Thrombosis with Thrombocytopenia Syndrome</keyword> has been associated with at least 81 deaths in the UK and hundreds of serious injuries. More than 50 alleged victims and grieving relatives have filed a lawsuit against AstraZeneca in a High Court case. AstraZeneca has insisted that the decision to withdraw the vaccine is not related to the case or admission that it can cause TTS and termed the timing a pure coincidence, according to The Telegraph report. "We are incredibly proud of the role Vaxzevria played in ending the global pandemic. According to independent estimates, over 6.5 million lives were saved in the first year of use alone and over three billion doses were supplied globally. Our efforts have been recognised by governments around the world and are widely regarded as being a critical component of ending the global pandemic," The Telegraph quoted AstraZeneca as saying. In a statement, the pharmaceutical giant said, "As multiple, variant Covid-19 vaccines have since been developed, there is a surplus of available updated vaccines", adding that it has led to a decline in demand for Vaxzevria, which is no longer being manufactured or supplied, The Telegraph reported. It further announced its decision to initiate the withdrawal of the marketing authorisations for Vaxzevria within Europe. The company said, "We will now work with regulators and our partners to align on a clear path forward to conclude this chapter and significant contribution to the Covid-19 pandemic." Last week, AstraZeneca reiterated its commitment to patient safety while emphasising the vaccine's overall safety profile. An AstraZeneca spokesperson had stated, "Our sympathy goes out to anyone who has lost loved ones or reported health problems. Patient safety is our highest priority, and regulatory authorities have clear and stringent standards to ensure the safe use of all medicines, including vaccines." This comes in the wake of a recent admission by AstraZeneca, the pharmaceutical company, that its <keyword id="19896627" type="General" weightage="20" keywordseo="covid" source="keywords">Covid</keyword> vaccine Covishield and Vaxzevria "can, in very rare cases, cause Thrombosis Thrombocytopenia Syndrome (TTS)." Despite these rare occurrences, the pharmaceutical company maintained that extensive clinical trial data and real-world evidence consistently support the vaccine's safety and efficacy. AstraZeneca withdraws COVID-19 vaccine worldwide, cites commercial reasons

However, now the company has decided to cut the prices but the debate is far from over. Many people argue that it should be free for all. And many state governments like those Delhi, Bihar, Uttar Pradesh, Goa, West Bengal, Tamil Nadu, Andhra Pradesh, and many others, have already said that the state will bear the cost of vaccination. Adar Poonawalla's Serum cuts Covishield vaccine price for state governments to ₹300 a dose after much backlash

By allowing vaccine makers to choose the pricing based on who is buying the vaccine, the government may give the producer an incentive to direct more vaccines to the more expensive private hospitals, whereas a majority of the country still depends on state-run facilities. “Pricing should be uniform. Both the central and state governments should pay the same price for the vaccine. The government should also cap the price of the vaccine sold at private hospitals. Even the administrative charges should be capped. Otherwise a family of four may end up paying a lot for vaccination,” Doctor Sanjeev Bagai, Chairman and Managing Director of the New Delhi-based Nephron clinic told Business Insider. “Direct benefit transfer is also an option. However, decades of similar policy experiments have taught us that such agreements are inefficient and provide a fertile ground for corrupt practices. The failure of such mechanisms gave birth to the idea of Direct Benefit Transfer (DBT). It could be deployed here as well, given that patients coming in for vaccination are already being asked to submit their Aadhaar (unique identity number) details. Vaccine makers should charge the same price from all customers. Those who access it via government agencies, or any other filter as may be applicable, should be allowed to get the subsidy reimbursed to their bank account. This has been effectively used in transferring other subsidies like the ones for cooking gas and fertilisers. The government’s own estimate is that it has saved over ₹1.78 lakh crore ($25 billion) in subsidy leakage because of DBT. The argument against direct benefit transfer There are two arguments here against direct benefit transfer. One from R Ramakumar, Professor at Tata Institute of Social Sciences, who wants vaccines to be free for all as it is in countries like US, UK and China. The other argument is from Dr Arvind Virmani, an economist and the former Chief Economic Advisor to the government of India, who wants drug prices to be decontrolled totally and do a direct cash transfer on a larger scale and not limit it to vaccines alone. “Supply chain has to be developed. It can’t be done overnight. The data on production and sale of vaccines is transparently available from the producers, for the government, Astrazeneca and others to see. To introduce DBT now, it would mean taking out funding from other budgeted activities like infrastructure construction. Right now, vaccination is working smoothly. Don’t disrupt it,” he told Business Insider. Aside from the fact this may add a bureaucratic layer, Ramakumar’s contention is that by allowing state-run facilities to charge less, and private hospitals to charge more, there is an incentive of corruption in distribution. India already has decades of bad experience. from the public distribution system a.k.a the ration shops, where the subsidised stock is moved out to be sold at a higher price. A cap on prices, and a crackdown on hoarders by making it a criminal act, may be necessary to counter this, even with reimbursement of the vaccine cost via direct transfer to bank accounts. Virmani’s take is that there is a case to be made for both deregulating drug prices, as well as direct cash transfer for a variety of reasons, in India but that can’t be debated in the middle of a pandemic. “All drugs are controlled. We should be discussing decontrol of drugs. That’s not the problem right now. For the last 12 months, I have been saying you can create a direct cash transfer in 4 months. It should still be considered,” he added. If the COVID-19 vaccine prices are too high for some, then why not use direct benefit transfer

<ul><li>India's expert panel has recommended SII’s Oxford-AstraZeneca COVID-19 vaccine dubbed Covishield in India for emergency use with 'restrictions.'</li><li>The SEC recommendation on Covishield has been forwarded to the Drugs Controller General of India (DCGI), which is likely to take a final call soon.</li><li>The Pune-based Serum Institute of India (SII), the world's largest vaccine manufacturer, has tied up with AstraZeneca to manufacture Covishield. </li></ul>An expert panel of India's drug regulator on Friday recommended emergency use approval for Oxford-AstraZeneca vaccine Covishield, being manufactured by the Serum Institute of India, paving the way for the roll-out of the first COVID-19 shot in the country in the next few days.. The SEC recommendation on Covishield has been forwarded to the Drugs Controller General of India (DCGI), which is likely to take a final call soon. The SEC, which earlier had sought additional safety, immunogenicity and efficacy data from SII and Bharat Biotech, deliberated on their applications seeking emergency use authorisation (EUA) for their shots on Wednesday, and met again on Friday to review the matter. While granting the restricted emergency use approval for the Oxford COVID-19 vaccine, the panel imposed certain regulatory provisions, including that the shot is indicated for active immunisation in individuals of 18 years or more to prevent the disease and that it should be administered intramuscularly in two doses at an interval of 4 to 6 weeks. Further, the SII should submit safety, efficacy and immunogenicity data from the ongoing clinical trials in the country and across the globe for review at the earliest. Also, the Pune-based firm should submit the safety data including the data on adverse events following immunisation (AEFI) and adverse event of special interest (AESI) with due analysis every 15 days for the first two months and monthly thereafter till the completion of the ongoing clinical trial in the country. The Pune-based Serum Institute of India (SII), the world's largest vaccine manufacturer, has tied up with AstraZeneca to manufacture Covishield. The approval comes at a time when the Indian government is wary of the new strain of COVID-19, which has been wreaking havoc in the UK. So far 29 cases of the new mutant variant have been detected in India. As compared to Pfizer and Moderna vaccines, the Oxford-AstraZeneca COVID-19 vaccine is <a href="https://www.businessinsider.in/india/news/india-needs-to-ramp-up-its-cold-storage-facilities-to-accommodate-the-distribution-of-the-covid-19-vaccine-within-the-country/articleshow/79422624.cms">more suited to India’s requirements</a> with storage temperature requirements of between two to eight degrees Celsius. Serum’s Covishield vaccine gets expert panel recommendation, paving the way for first authorized COVID-19 shot in India

Nepal's Foreign Minister Pradeep Gyawali is currently in New Delhi to take part in the sixth meeting of Nepal-India Joint Commission. Nepal approves Covishield vaccine for emergency use

Till Thursday 913,219 people out of the island nation's 21 million population had been <keyword id="178509" type="PERSON" weightage="20" keywordseo="vaccinated">vaccinated</keyword>. Vaccination programme has been halted from Wednesday night due to the need to use the available Oxford-AstraZeneca doses for the second jab of those already vaccinated, primary health care and COVID-19 prevention minister Dr Sudarshani Fernandopulle was quoted as saying by the Daily Mirror newspaper on Friday. She said the COVID-19 vaccination programme was temporarily suspended as there was a delay in securing more <keyword id="6409874" type="DOMAIN" weightage="20" keywordseo="astrazeneca">AstraZeneca</keyword> vaccines, the report added. The Serum Institute of India (SII) recently suspended exports of the AstraZeneca vaccine. The WHO has recommended that the second jab must be made available within 12 weeks of the first. The second dose was to be administered from April 19, the report said. After the free gift, Sri Lanka placed an order for more doses of the Indian vaccine. The Oxford-AstraZeneca vaccine, <keyword id="9617748" type="COMPANY" weightage="20" keywordseo="covishield">Covishield</keyword>, has been developed in collaboration with SII. The Pune-based vaccine major has entered into a collaboration with the University of Oxford and AstraZeneca to manufacture the vaccine. The Serum Institute had curtailed exports in the recent weeks. So, Fernandopulle said that Sri Lanka was not able to gain any assurances on the arrival of the next consignment from India in time. This week China donated 600,000 doses of Sinopharm vaccines. However neither the local health regulators or the WHO have approved its emergency use. Officials said the Chinese vaccine will be administered on the Chinese workers in the country. Sri Lanka is also to order 700,000 doses of the Russian Sputnik V vaccine. Sri Lanka temporarily suspends COVID-19 jabs due to delay in securing vaccines from Serum Institute of India, says Health Minister

Fortis Healthcare on Thursday announced that Sputnik V vaccine against Covid-19 will be available at two of its hospitals as part of a limited pilot roll-out from June 19. It will be extended at more Fortis hospitals across 11 cities. The price per dose for Sputnik V (as fixed by the government) including administrative charges is ₹1,145. Oxford’s COVID-19 vaccine: Phase III human trials begin in India

The SII has partnered with Novavax Inc to manufacture its potential Covid-19 vaccine for India and other countries. Under an agreement with the US drug developer, the Pune based drugmaker will develop two hundred crore doses of Novavax's vaccine candidate annually. The drugmaker will also manufacture the antigen component of the vaccine. The SII has also partnered with the US-based Codagenix to manufacture and supply its coronavirus vaccine. The firm's first Covid vaccine is developed from the masterseed of AstraZeneca/Oxford University's vaccine against the disease. It was approved by India's drug regulator on January 3 for emergency use authorization alongwith Bharat Biotech's Covaxin. However, both the drugmakers are being criticised for less transparent data in their clinical trials and receiving the approvals without completing the due process of drug licencing. Commenting on the criticism the vaccine manufacturers have been receiving for getting approvals without completing all the trials, Jadhav said that such chances have been taken earlier as well. "It is not the only time when humanity has taken the chances. Four years back, there was an outbreak of Ebola in Africa and its vaccine by a Canadian pharmaceutical firm which had only completed the phase 1 and undergoing the phase 2 trial, was approved by the <keyword id="43588" type="COMPANY" weightage="20" keywordseo="World-Health-Organization">World Health Organization</keyword> (WHO). The risk paid off and the vaccine helped in controlling the Ebola there," Jadhav added. "In 2009 when the H1N1 pandemic flu struck, we took 1.5 years to develop and receive the approval for the vaccine against it after completing all the stages of clinical trials but the drug manufacturers in the West marketed such products in less than seven months then. Nobody questioned them then. Then why this sudden hullabaloo now?" he queried.
