The Income Tax Department on Friday issued a notification allowing hospitals and nursing homes to accept cash payments of ₹2 lakh and more for the treatment of coronavirus infected patients. The Central Board of Direct Taxes (CBDT), that frames policy for the department, issued the notification. It said hospitals, dispensaries, nursing homes, COVID care centres or similar other medical facilities providing <keyword id="8918466" type="UNKNOWN" weightage="20" keywordseo="covid-19-treatment">COVID-19 treatment</keyword> to patients are being "specified for the purpose of section 269ST of the Income Tax Act". This section restricts any person from receiving an amount of ₹2 lakh or more in cash, from a person in a day, in respect of a single transaction or in respect of transactions relating to one event or occasion from a person. It was introduced by the Union government in 2017 as a measure to curb black money. The notification said the latest provision is being made for a period between April 1-May 31 "on obtaining the PAN or Aadhaar of the patient and the payee and the relationship between the patient and the payee" by such hospitals or COVID care centres. Officials said the move was aimed to remove the hardship being faced by relatives and caregivers of coronavirus infected patients who visit the hospitals for treatment. The country is grappling with the onslaught of a massive COVID-19 wave with a record over 4.14 lakh new infections and 3,915 deaths being reported in a span of 24 hours, as per the Union health ministry data updated on Friday. But now, most people who get hospitalized with COVID-19 could wind up with a hefty bill. A <a href="https://www.healthsystemtracker.org/brief/most-private-insurers-are-no-longer-waiving-cost-sharing-for-covid-19-treatment/">new report</a> from the same group found that among the two largest insurers in each US state, 72% no longer waive costs for COVID-19 treatment.KFF calculated that the average patient hospitalized with pneumonia (which requires a similar treatment to COVID-19) winds up responsible for around $1,300 in out-of-pocket costs - assuming they have an employer-provided plan.The US government requires private insurers to make COVID-19 tests and vaccines free to the public, but treatments were never part of that mandate. So some states - Massachusetts, New Mexico, Idaho, Michigan, Minnesota, and Rhode Island - enacted their own requirements that insurers waive out-of-pocket treatment costs.But even in other states, private insurers have covered treatment costs during the pandemic for the most part - either as a courtesy to patients or a preemptive measure in case the federal government mandated it, KFF found.Then in November, insurers started to change course. By April 2021, roughly half of the 102 large health plans that KFF examined had stopped waiving COVID-19 treatment costs. Now, just 29 of those plans are waiving costs. Ten of those waivers are set to expire by the end of October, and another 12 will likely expire by the end of 2021.<div type="image" title="Louisiana hospital covid" height="4024" width="6048" msid="85515548" hostid="261"></div>The analysis concluded that insurers "may no longer face political or public relations pressure to continue waiving costs for COVID-19 treatment" now that vaccines are widely available.But COVID-19 hospitalizations are skyrocketing again in the US: On average, daily hospitalizations have risen seven-fold in the last two months, from around 1,600 per day to 11,700 per day. That's similar to levels recorded in February, before vaccines were widely available.Hospitals are now <a href="https://www.businessinsider.com/louisiana-hospital-capacity-failure-covid-2021-8">approaching capacity</a> in Kentucky, Louisiana, and Georgia, among several other states. "We are in the process of further improving our home isolation protocol. "We are in the process of revising the rates," Rane said, adding that despite the revision and the capping of prices, patient-to-patient costs may still vary with the nature of treatment given to the Covid-19 patients. Last week, the Goa government had capped the prices for Covid-19 treatment in private hospitals with patients availing treatment in a general ward having to pay 12,000 per day, treatment in twin-shared rooms was capped at Rs 15,000 and while fees for admission of a Covid-19 patient in a private room was set at Rs 25,000 per day. Price for utilising the ICU facilities had been capped at Rs 25,000 per day. Goa to revise home isolation protocol and fees for COVID-19 treatment

New Delhi, June 15 () Apex healthcare industry body NATHEALTH on Monday said it has urged the Delhi government to mandate the hospitals to publish their treatment charges for COVID-19. In a representation to Delhi Health Minister Satyendar Kumar Jain on 'COVID Package Pricing', NATHEALTH also recommended the appointment of an ombudsman at zonal level to inspect the complaints of overcharging. The representation highlights the unprecedented challenges being faced by private healthcare providers in COVID-19 treatment, it added. "Pricing is a factor of quality of infrastructure, level of services, medical protocol followed, brands of medicines used, skill and salaries of doctors and staff and more importantly the medical outcomes," NATHEALTH said. The focus of treatment protocols in COVID-19 times has undergone a significant shift and hospitals need to equally ensure that healthcare workers are not infected while treating patients and hospitals themselves don't become hotspots, it added. This responsibility has thus led to increase in cost of care, both for COVID-19 and non-COVID-19 patients , the statement said. The private hospitals under NATHEALTH have said they fully support the need for transparency of pricing to ensure that patients are not overcharged. Participants also started the treatment within five days of developing symptoms, so those who have been sick for longer than that may not be eligible.Merck's trial also looked exclusively at unvaccinated people, so it's not yet known whether the treatment will be recommended for those who get breakthrough cases after getting vaccinated.<h2>Why is a COVID-19 pill necessary?</h2>So far, the FDA has fully approved just one treatment for COVID-19: the antiviral remdesivir, which is administered via injection. That drug has been found to <a href="https://www.nejm.org/doi/full/10.1056/nejmoa2007764">shorten recovery time</a> for hospitalized patients, but it's not a treatment for mild or moderate COVID-19.The FDA also authorized the emergency use of <a href="https://www.businessinsider.com/tennessee-vaccinated-denied-monoclonal-antibody-treatment-2021-9">monoclonal antibodies</a>, drugs that help keep people with mild or moderate symptoms from developing severe COVID-19. These come in two forms, infusions or injections - both of which are administered at hospitals or clinics. Though monoclonal antibodies are free to the public, they can cost the government <a href="https://www.npr.org/sections/health-shots/2020/10/28/928841997/government-signs-deal-for-covid-19-treatments-from-eli-lilly">more than $2,000 per dose</a>.So drugmakers have been working to develop cheaper, less invasive treatments.Merck's fits the bill: The drug would cost the government around $700 per treatment (though it would also be free to Americans). That full regimen consists of 40 pills - four capsules twice a day for five days. The drug will likely be available as a prescription at pharmacies.But it's not a replacement for vaccines, which still offer the highest chance of avoiding hospitalization or death.<div type="image" title="Capsules of molnupiravir, an antiviral drug developed by Merck and Ridgeback Biotherapeutics that could treat COVID-19" height="2002" width="3000" msid="86974087" hostid="261"></div><h2>Will the pill be easy to get?</h2>The US government has purchased enough of Merck's pill to treat 1.7 million people, and the company already started manufacturing the drug. Merck hopes to produce 10 million treatment courses by the end of the year, much of which would go to other countries.Merck has signed licensing deals with Indian manufacturers to help deliver the drug to most low- and middle-income countries, assuming the pill gets authorized in those places. The company plans to vary the price of the treatment by country.<h2>How does the pill work inside the body?</h2>Merck's pill belongs to a class of antiviral drugs called nucleosides, which can block a virus from replicating inside cells. This particular drug creates mutations in the part of its genetic code that the virus uses to replicate. Once enough of its code is changed, the virus dies out, preventing a patient's symptoms from getting worse."The virus essentially mutates itself to death," Richard Plemper, a virologist at Georgia State University, recently <a href="https://www.nature.com/articles/d41586-021-02783-1">told Nature</a>.<h2>Are there any side effects?</h2>People in Merck's trial reported a similar number of side effects regardless of whether they got the real drug or a placebo (40% of placebo recipients had side effects, compared with 35% of those who got the pill)."We're very comfortable that the drug will be safe if used as intended," Daria Hazuda, Merck's vice president of infectious diseases discovery, said in a press call earlier this month.This story has been updated with new information.
