Doctors in India are grappling to diagnose and treat unexplained and persistent symptoms of <keyword id="9814327" type="General" weightage="100" keywordseo="long-covid" source="Orion">long Covid</keyword> patients due to limited guidelines, whereas researchers have flagged inadequate studies on the condition. With the <keyword id="19896311" type="General" weightage="20" keywordseo="world-health-organization" source="keywords">World Health Organization</keyword> declaring an end to <keyword id="19896627" type="General" weightage="20" keywordseo="covid" source="keywords">Covid</keyword> as a global <keyword id="416511" type="General" weightage="20" keywordseo="health-emergency" source="Orion">health emergency</keyword> in May last year, focused efforts are underway around the world to estimate the burden of long Covid among the population. The condition refers to the set of lingering symptoms affecting varied body parts and persisting well beyond the acute Covid infection period, including cough, muscle and joint pain, fatigue, brain fog and difficulty in focusing. The viral disease is caused by the SARS-CoV-2 virus. While studies have suggested that about a third of those moderately or severely infected are likely to suffer from long Covid, region-wise though, incidence could vary. A study by researchers, including those from <keyword id="19988007" type="General" weightage="20" keywordseo="harvard-medical-school" source="keywords">Harvard Medical School</keyword>, US, estimated that 31 % of the once-infected people in North America, 44% in Europe, and 51% in Asia, have long Covid, which is "challenging the healthcare system, but there are limited guidelines for its treatment". It was published in the International Journal of Infectious Diseases in September. In India, however, studies on long Covid are few and far between. One such study by Maulana Azad Medical College in New Delhi, conducted from May 2022 to March 2023 on 553 patients who had recovered from Covid, found that about 45% had lingering symptoms, persistent fatigue and dry cough being the most common. "There is limited exploratory research on the long Covid syndrome with scarce data on long-term outcomes," the authors wrote in the study published in the journal Cureus in May this year. Understanding the long-term effects of the virus is important for developing management strategies, optimising healthcare delivery, and providing support to recovered Covid patients in the community, they said. Dr Rajesh Sagar, Professor of Psychiatry, All India Institute of Medical Sciences (AIIMS), New Delhi, said, "Looking at the current state of long Covid studies in India, it is too premature to say that we understand the condition well enough to know how to diagnose or treat it." Animesh Samanta, assistant professor at School of Natural Sciences in <keyword id="19897094" type="General" weightage="20" keywordseo="shiv-nadar-university" source="keywords">Shiv Nadar University</keyword>, Greater Noida, said, "While studies in India highlight the growing recognition of neurological complications in long Covid patients, more focused research on neuroinflammation is needed." Doctors, too, have reported a rise in patients complaining of symptoms that they did not have pre-Covid. "People who never had <keyword id="6465125" type="General" weightage="20" keywordseo="asthma" source="Orion">asthma</keyword> in the past, post-Covid, with every viral infection, they get a long cough, shortness of breath and wheezing, which require the use of <keyword id="31257" type="General" weightage="20" keywordseo="inhalers" source="Orion">inhalers</keyword> or nebuliser," senior consultant Dr Neetu Jain, who runs a post-Covid care clinic at Pushpawati Singhania Hospital and Research Institute, New Delhi, said. Dr Arun Garg, chairman, Neurology and Neurosciences, Medanta-The Medicity, Gurugram, said he was noticing a spike in stroke cases among young patients not suffering from known risk factors such as diabetes, hypertension and obesity. "Similarly, we are seeing more cases of encephalopathy (swelling of the brain) without reason and having a confused state of mind following one or two days of fever. Their MRI scans show no changes. These patients have increased significantly after Covid," he said. In the absence of medical guidelines to diagnose long Covid, doctors are having to resort to broad, non-specific tests and questionnaires to gauge a patient's 'quality of life'. Studies have shown that the fatigue experienced in long Covid is similar to that in cancer patients, with a quality of life similar to patients of Parkinson's disease. "We really do not have any test to diagnose long Covid, even though it is definitely a clinical diagnosis. We diagnose long Covid for people who had at least moderate to severe infection, following which they could never regain the quality of life pre-Covid. Checking for inflammatory markers like C-reactive proteins (CRP) can support the diagnosis," Dr Jain said. "Other than routine blood tests that measure inflammation, we do antibody tests to look for direct markers. In many of these patients, we are finding rare antibodies which are very new to us and were not there pre-Covid," Dr Garg said. Inflammation persisting despite recovery from acute Covid infection is thought to lie at the heart of long Covid. However, tests to measure this specific immune response are lacking, even as researchers have been working in this direction worldwide. One such effort comes from Shiv Nadar University, where a team led by Samanta has developed a fluorescent probe capable of detecting inflammation in brain cells that can arise due to Covid infection. The probe measures nitric oxide levels in brain cells, especially in human microglia cells, where increased NO levels are linked to the SARS-CoV-2 infection. Microglia are immune cells in the brain that fight disease and help maintain brain health. Lysosomes within microglia, which help clear foreign disease-causing agents like the SARS-CoV-2 virus, produce nitric oxide as part of an immune response to infection. The probe detects nitric oxide produced in lysosomes in response to infection and thereby allows for a measurement of inflammation levels. This examination method can provide "qualitative information on infection status", said Samanta, corresponding author on the study published in the journal Analytical Chemistry in American Chemical Society. He explained that patients with pre-existing conditions such as Alzheimer's disease, Parkinson's disease or multiple sclerosis (an autoimmune disorder) could exhibit prolonged neuroinflammation and loss of brain cells following Covid infection. While the probe has shown efficacy in cell cultures, animal studies would need to be done, before testing it in humans, Samanta said. Looking at the World Health Organization International Clinical Trials Registry Platform, the study by Harvard Medical School had found that 587 clinical studies were conducted on long Covid, of which about 53% (312) were testing potential treatments. Most of these were found to be conducted in the US (58), followed by India (55) and Spain (20). The trials looked at interventions including physical exercise, psychotherapy, and pharmacological ones such as paxlovid and fluvoxamine. However, "to date, only 11 of these 312 studies have published their results that were not confirmative," the researchers wrote. The team called for studies to look into sleep disorders which were rarely included in the registered clinical studies. Further, interventions targeting the biological processes responsible for long Covid are needed but currently lacking, they said.

This resurgence is a sobering reminder that despite our pandemic response, TB has continued to claim lives, especially in low- and middle-income countries (LMICs), where resources are often stretched thin. According to the latest <keyword id="19896311" type="General" weightage="20" keywordseo="World-Health-Organization" source="keywords" meta.keywordsubtype="org" meta.entityname="World Health Organization" meta.hostid="261" meta.entityid="World Health Organization">World Health Organization</keyword> (<keyword id="19895006" type="General" weightage="20" keywordseo="WHO" source="keywords">WHO</keyword>) report, approximately 8.2 million people were diagnosed with TB in 2023, marking the highest number of cases recorded since global monitoring began in 1995. <h2><keyword id="19895063" type="General" weightage="20" keywordseo="India" source="keywords">India</keyword> and Indonesia</h2>The recent spike in TB cases is largely driven by increases in two countries — India and Indonesia. Together, they accounted for 45% of the global rebound in TB diagnoses between 2021 and 2023, underscoring their critical role in the worldwide fight against TB. Three other countries, the Philippines, Nigeria, and Pakistan, also made significant contributions, collectively accounting for another 32% of the increase. This means that more than three-quarters of the global resurgence in TB can be traced to these five countries alone. One key reason for these increases is the “backlog” effect created by COVID-19 disruptions. During the pandemic, healthcare systems in many countries, including India and Indonesia, had to shift focus to COVID-19, which delayed TB diagnosis and treatment. In 2023, as these countries ramped up recovery efforts, many previously undetected TB cases surfaced, creating an apparent spike in reported numbers. This trend highlights the importance of sustainable healthcare infrastructure capable of handling simultaneous health crises. <h2>Gaps in TB case detection</h2>Despite the daunting increase in TB cases, some countries have made notable progress in <keyword id="23741990" type="General" weightage="20" keywordseo="treatment-coverage" source="keywords">treatment coverage</keyword>. Among the 30 high-burden TB countries, those with the highest levels of treatment coverage in 2023 included Brazil, India, Mozambique, Papua New Guinea, Sierra Leone, Uganda, and Zambia, with coverage rates surpassing 80%. India’s <keyword id="21052657" type="General" weightage="20" keywordseo="national-tuberculosis-elimination-program" source="keywords">National Tuberculosis Elimination Program</keyword> (NTEP) has undoubtedly played a pivotal role in this, aiming to eradicate TB by 2025 — five years ahead of the global target. By focusing on expanded testing, improved diagnostic tools, and more accessible treatment options, India has achieved high treatment coverage levels despite the challenges. <div type="Article" hostid="261" msid="114732986" title="Air Quality Alert: Nearly 30% of asthma cases tied to fine particulate matter exposure from bad air" mstype="2" mssubtype="0"></div>However, a significant treatment gap remains. The difference between estimated TB incidence and actual diagnoses still affects millions, especially in countries with large populations like India, Indonesia, Pakistan, China, and Myanmar. These five nations together account for about 50% of the global gap in reported TB cases, with India alone responsible for 16% of the discrepancy. <h2>India’s unique position in the global TB fight</h2>As the nation with the largest share of global TB cases, India has a unique and critical responsibility in addressing the disease. The high TB burden in India is tied to several factors, including undernutrition, poverty, and limited access to healthcare in rural areas. Moreover, with 26% of all TB cases globally, India’s battle against TB will significantly impact the global fight. The government’s financial support for TB-affected households also aims to reduce the economic burden of treatment. However, challenges remain, especially with <keyword id="22004277" type="General" weightage="20" keywordseo="multidrug-resistant-TB" source="keywords">multidrug-resistant TB</keyword> (MDR/RR-TB). While treatment success rates for MDR/RR-TB have reached 68%, diagnosing and treating all drug-resistant cases is an ongoing challenge. In 2023, India’s coverage of preventive treatment for TB among people living with HIV and close contacts of TB patients also saw improvements, but underfunding continues to limit the full scope of TB elimination efforts. <h2>Global funding gaps</h2>Global funding shortages for <keyword id="21039793" type="General" weightage="20" keywordseo="TB-prevention" source="keywords">TB prevention</keyword> and treatment are a significant hindrance to progress. Only $5.7 billion of the targeted $22 billion in funding was met in 2023, and for LMICs — where nearly all of the TB burden lies — the available funding falls well short of their needs. The United States remains the largest bilateral donor, and contributions from the Global Fund to Fight AIDS, Tuberculosis, and Malaria are crucial. However, these funds are not enough to meet essential TB service requirements, leaving healthcare systems under-resourced. <div type="Article" hostid="261" msid="114646545" title="Doctors struggle to treat long Covid patients in India; researchers point to inadequate studies" mstype="2" mssubtype="0"></div>Even in 2022, only one-fifth of the $5 billion target for research funding was achieved, as per the report. Moreover, many of the drivers of TB are related to social factors, such as undernutrition, smoking, alcohol use, and diabetes, making it imperative to address these root causes. For instance, almost half of the TB-affected households globally face what the WHO calls "catastrophic costs" in seeking treatment. This burden is particularly severe in India, where low-income families often forgo treatment due to financial constraints. "This is one of the first times that we are seeing evidence of efficient and sustained mammalian-to-mammalian transmission of highly pathogenic avian influenza H5N1," said Diego Diel, a virology expert who has written extensively about the topic. As per the new report, significant amounts of the virus has now been identified in the mammary glands of infected cows, meaning their milk itself has the ability to infect others. Researchers have even documented cases where cats and a raccoon became infected, likely from consuming the raw milk from infected cows. Although pasteurisation has shown the ability to kill the virus, the newfound ability to jump from cows to other mammals remains particularly troubling, especially considering how reliant we are on cow products such as beef and milk. <h2>Human cases and <keyword id="22448349" type="General" weightage="20" keywordseo="pandemic-potential" source="keywords">pandemic potential</keyword></h2>If you’re feeling comfortable in the thought that the virus has some ways to go before humans need to be concerned, you might want to think again. The US Department of Agriculture has funded H5N1 testing programs to detect and manage outbreaks early. Enhanced biosecurity measures and quarantine protocols will undoubtedly be critical to prevent the virus from spreading to pandemic levels. However, this is a task much easier said than done for developing countries, who often lack the infrastructure and resources needed for such robust surveillance and response. Many of these countries have limited access to advanced diagnostic tools, vaccines, and antiviral treatments. For as long as we can remember, <keyword id="19935846" type="General" weightage="20" keywordseo="science" source="keywords">science</keyword> and <keyword id="19969367" type="General" weightage="100" keywordseo="religion" source="keywords">religion</keyword> have been at a clash on what constitutes the truth. While centuries of scientific pursuit have quelled the debate on some matters, many other unsolved topics — such as the creation of our universe — still remain a hotbed of conflict among the two groups. Oftentimes, science is misrepresented or outright rejected in service of deeply ingrained convictions. It's a scene all too common: the outspoken aunt in family WhatsApp groups championing religious doctrine over scientific consensus, or the insidious infiltration of <keyword id="4874838" type="General" weightage="20" keywordseo="political-propaganda" source="Orion">political propaganda</keyword> casting doubt on established facts. But what factors truly drive this rejection? A groundbreaking study has delved into this intricate web of religion, tolerance, and science denial with the following hypothesis: individuals who are intolerant of other religions are more inclined to reject scientific tenets. This is based on the idea that such groups may view science as a competing belief system rather than a complementary avenue of understanding. In order to measure “religious tolerance”, the research settled on using local <keyword id="21506716" type="General" weightage="20" keywordseo="religious-diversity" source="keywords">religious diversity</keyword> as an indicator. The rationale being that tolerance thrives in areas with religious diversity, which fosters an environment where differing viewpoints are embraced rather than shunned. They then looked at how these variously religiously diverse groups engaged with science, as evidenced through the efforts of seven studies. One such study surveyed Hindus in India, Muslims in Pakistan and Christians in the United States. It found that groups that espoused <keyword id="21389640" type="General" weightage="20" keywordseo="intolerance" source="keywords">intolerance</keyword> towards other religions often denied science as well. Not only did these intolerant participants disagree with science when it conflicted with their own religion, but they also expressed a general disapproval of scientific recommendations, such as in the administration of <keyword id="20849375" type="General" weightage="20" keywordseo="COVID-vaccinations" source="keywords">COVID vaccinations</keyword>. In fact, another compelling study found that during the tumultuous days of the early COVID-19 pandemic, religious intolerant counties in the United States socially distanced less, even after accounting for the percentage of religious adherents within each county. These groups were also less likely to get the COVID vaccine, highlighting the complicated relationship between <keyword id="21506718" type="General" weightage="20" keywordseo="religious-tolerance" source="keywords">religious tolerance</keyword> and public health compliance. On the contrary, the study found that countries boasting higher religious diversity often emerged as champions of scientific literacy. Scores on the Program for International Student Assessment — a test that measures science and math performance among 15-year-olds — soared in nations where religious plurality thrived. Furthermore, the World Values Survey showed that areas with less religious diversity were more likely to assert that religion is a better guide to uncovering the truth than science. In a world divided by strong opinions and closed-off groups that reinforce their own beliefs, this study urges us to welcome diversity. By embracing different perspectives, we can appreciate faith and reason, moving beyond any potential limitations imposed by politics and religion. India carried out 15 lakh COVID-19 tests on Jan 6, but that is not enough

New Delhi [India], April 23 (ANI): India reported over 10,000 COVID cases for the fifth straight day with 10,112 <keyword id="7571551" type="General" weightage="20" keywordseo="new-infections" source="Orion">new infections</keyword> in the last 24 hours, according to the Union Minister of Health and Family Welfare on Sunday. The country saw a little dip from Saturday's COVID cases tally. India recorded 12,193 on April 22, 11,692 on April 21, 12,591 on April 20, 10,542 on April 19, 7,633 on April 18, and 9,111 on April 17. According to the Union Health Ministry, the country's active cases increased to 67,806 which was 67,556 yesterday. Presently recovery rate stands at 98.66 per cent, the Health Ministry stated further. As many as 9,833 patients recovered from the disease in the last 24 hours taking the overall count of people cured of the infection to 4,42,92,854, according to the Ministry. Under the nationwide vaccination drive, India has so far administered 220.66 crore Covid vaccine doses (95.21 cr second doses and 22.87 cr precautionary doses), of which 1,947 doses were administered in the last 24 hours. The total number of tests conducted in the last 24 hours is 1,43,899, the health ministry stated. In wake of the spike in the number of COVID-19 cases across the country, Dr PK Mishra, the Principal Secretary to the Prime Minister earlier chaired a high-level meeting on Wednesday to assess the situation. The PMO informed in a statement that the focus of the meeting was on the status of preparedness of health infrastructure and logistics, drugs, vaccination campaign in the country and the key necessary steps as a response to the recent surge in Covid cases. "Dr PK Mishra stressed that the time tested 5-fold strategy of Test-Track-Treat-Vaccination and adherence COVID Appropriate Behaviour must continue to be implemented and equally crucial to promote awareness within the community regarding COVID appropriate behaviour and caution citizens against complacency," the PMO statement read. During the meeting, a comprehensive presentation was made by Rajesh Bhushan, the Secretary of Health and Family Welfare providing an overview of the global COVID-19 situation. The presentation also provided an overview of the Genome Sequencing of different variants since January 2023 and noted the proportion of circulating variants in India. The status of vaccination was discussed, followed by drug availability and infrastructure preparedness across the country, the PMO stated. As per directions of the Prime Minister, a nationwide mock drill for the assessment of functional infrastructure was conducted and the status of the mock drill was presented to participants. Further, the expenditure of the COVID-19 vaccination programme and budget provisions for drugs and vaccine raw materials were also reviewed. (ANI)

She is here to attend the third G20 Health Working Group meeting that is being held from June 4 to 6. "Further, India can share lessons on how to create an enabling environment, including investments in human and physical capital, as well as its learnings in moving beyond generic industry to expanding research and development innovator industry," Prasad said. The G20 more broadly can call upon its industry to ensure manufacturers include licensing and technology transfer for essential and emergency health supplies to different regions to ensure equitable access. And, secondly, low-and middle-income country stakeholders need to be involved from the onset to ensure access partnerships take into account the context and needs of the countries they aim to support, she said. "Thirdly, vaccines alone are not enough. We need a toolkit with many interventions including tests, treatments, personal protective equipment and ancillary supplies," she said. "Related to that, efforts need to be multi-sectoral considering the cascading effect of a health emergency - water, sanitation, hygiene and infection prevention control measures are equally important," Prasad stated. Manufacturing of medical countermeasures needs to be geographically diversified to ensure resilience against global shocks, reduced lead times during an outbreak, and increase overall regional and global health security. "We need more investments in research and development for priority pathogens including those with limited market size and limited R and D investments and to create more incentives for industry to engage and increase risk-sharing between public and private sectors," Prasad said. Strengthening health systems needs to be an integral part of preparedness, including strengthening primary health care systems at the community level, she said, adding this will ensure systems are better prepared to deliver novel medical countermeasures (MCMs) when they become available. "Community engagement is essential to reduce hesitancy and increase trust and uptake in MCMs," she stated. On critical components of the global MCM coordination, she said, "We have the opportunity to ensure the world emerges from the Covid pandemic stronger than before." The Access to COVID-19 Tools Accelerator - ACT-A - partnership is "an example of what is possible if we work together. Building on the collaboration and lessons learnt from ACT-A we are now in a position to reinvigorate efforts to ensure more equitable global health outcomes," she said. "We cannot let 'peacetime' between pandemics and largescale health emergencies result in complacency. For the next pandemic, the world will need a diversified vaccine and product portfolio, expanded production spread across the world, and stronger and more resilient health systems," she said. Equity needs to be at the heart of access to medical countermeasures, and the emphasis of global collaboration needs to prioritise reaching the vulnerable, the hardest to reach and the most underserved, including women and children, she said. According to the release, 1,10,522 tests were conducted in the last 24 hours, taking the total number of tests conducted so far to 92.14 crores. It stated further that a total of 1,396 recoveries were made from the virus in the last 24 hours, taking the total number of recoveries to 4,41,68,321. It stated that 220.65 cr total vaccine doses (95.20 cr Second Dose and 22.86 cr Precaution Dose) have been administered so far under Nationwide Vaccination Drive. Further, India's active caseload currently stands at 13,509 with a rate of 0.03 per cent per day and with a recovery rate currently standing at 98.78 per cent. It stated that the weekly positivity rate stands at 1.71 per cent. Meanwhile, Delhi Health Minister Saurabh Bhardwaj called an emergency meeting on Thursday afternoon after 300 fresh Covid-19 cases were recorded in the Capital on Wednesday. "Health Minister Saurabh Bhardwaj will hold a meeting with the officers and specialist doctors associated with the health department this afternoon," officials said. The Special Secretary, Health Department, Director of General Health Services, Nodal Officer for Oxygen and Testing and Medical Directors of several hospitals including Lok Nayak Jai Prakash Narayan (LNJP) will be present in the meeting. The country recorded new 2151 Covid cases between Tuesday and Wednesday with a daily positive rate of 1.51 per cent, an official statement said on Wednesday, adding that 11,336 doses of vaccines were administered in the last 24 hours. Meanwhile, Delhi recorded 300 fresh Covid-19 cases and two deaths on Wednesday, marking a sharp uptick in infections over the last 24 hours. The Ministry of Health and Family Welfare issued revised guidelines for Covid-19 in the wake of the surge of cases in the past week across the country. "Antibiotics should not be used unless there is clinical suspicion of bacterial infection. The possibility of coinfection of COVID-19 with other endemic infections must be considered. Systemic corticosteroids are not indicated in mild disease," revised guidelines said. As per the revised Covid-19 guidelines, maintain "Physical distancing, indoor mask use, hand hygiene, Symptomatic management (hydration, antipyretics, antitussive) Monitor temperature and oxygen saturation (by applying a SpO probe to fingers) Stay in contact with the treating physician. "Seek immediate medical attention if difficulty breathing, High-grade fever/ severe cough, particularly if lasting for more than 5 days. A low threshold is to be kept for those with any of the high-risk features," guidelines discussed and prepared in January said. Additionally, in moderate or severe diseases at high risk of progression, the guidelines recommend, "Consider Remdesivir for up to 5 days (200 mg IV on day 1 followed by 100 mg IV OD for the next 4 days)".
