US CDC asks states to prepare for distribution of potential COVID-19 vaccine by November 1

New data revealed by <keyword id="195728" type="COMPANY" weightage="20" keywordseo="Gilead-Sciences">Gilead Sciences</keyword> has shown that its antiviral drug <keyword id="8775508" type="UNKNOWN" weightage="20" keywordseo="remdesivir">remdesivir</keyword> reduces death risk of severe Covid-19 patients by as much as 62 per cent when compared with standard care alone. This is an important finding that requires confirmation in prospective clinical trials, Gilead said on Friday about the results presented at the virtual COVID-19 conference as part of the 23rd International AIDS Conference. The analysis included 312 patients treated in the Phase-3 SIMPLE-Severe study and a separate real-world retrospective cohort of 818 patients with similar baseline characteristics and disease severity who received standard of care treatment in the same time period as the SIMPLE-Severe study. Patients were primarily located in North America (92 per cent, remdesivir cohort vs. 91 per cent, standard-of-care cohort), Europe (5 per cent vs. 7 per cent) and Asia (3 per cent vs. 2 per cent). The analysis demonstrated that remdesivir treatment was associated with significantly improved clinical recovery and a 62 percent reduction in the risk of mortality compared to standard of care. Findings from the comparative analysis showed that 74.4 per cent of remdesivir-treated patients recovered by Day 14 versus 59 per cent of patients receiving standard of care. The mortality rate for patients treated with remdesivir in the analysis was 7.6 per cent at Day 14 compared with 12.5 per cent among patients not taking remdesivir. "This comparative analysis provides valuable additional information regarding the benefit of remdesivir compared with standard of care alone," Susan Olender of Columbia University Irving Medical Center said in a statement. "While not as vigorous as a randomized controlled trial, this analysis importantly draws from a real-world setting and serves as an important adjunct to clinical trial data, adding to our collective understanding of this virus and reflecting the extraordinary pace of the ongoing pandemic." The results of this comparative analysis add to the previously presented National Institute of Allergy and Infectious Disease (NIAID) randomized, double-blind, placebo-controlled study in hospitalized patients with COVID-19, which showed that remdesivir shortened time to recovery by an average of four days as compared to placebo -- 11 vs. 15 days. In the NIAID study, patients taking remdesivir trended toward lower mortality compared with those in the placebo group, but this result did not reach statistical significance --7.1 percent vs. 11.9 per cent. Due to the current public health emergency, the US Food and Drug Administration (FDA) has issued an Emergency Use Authorization for remdesivir for the treatment of hospitalised patients with severe Covid-19. "These data presented at the Virtual COVID-19 Conference shed additional light on the use of remdesivir in specific patient populations, including those that may be susceptible to higher rates of COVID-19 infection, as well as others that are particularly vulnerable, including children and pregnant and postpartum women," said Merdad Parsey, MD, PhD, Chief Medical Officer, Gilead Sciences. A <a href="https://www.nber.org/papers/w27432">recent study</a> concludes that, although lockdowns did contribute to the economic impact, the vast majority of our current economic pain is caused by Americans taking precautions against getting sick. The recent reduction in consumer spending and restaurant reservations in Texas, Arizona, and Florida shows the impact of this.<div type="image" title="Screen Shot 2020 07 02 at 8.36.35 AM" height="620" width="948" msid="76758040" hostid="261"></div>One of the simplest and most effective ways for the country to control the virus and preserve as much of the economy as possible would be to <a href="https://markets.businessinsider.com/news/stocks/goldman-sachs-national-mask-mandate-save-gdp-hit-economic-impact-2020-6-1029355543">adopt universal mask-wearing</a>. Alas, our President continues to refuse to order or even recommend this. — HBTrump still thinks he can choose between emphasizing the virus or the economy, Republicans share in that delusion.As Blodget suggests, most Americans know that the pandemic and the economy are inextricably linked, and they're showing it with their wallets. Starting on June 12, when the virus started to surge again in states like Texas and Florida, consumer confidence started to trend down substantially, according to <a href="https://twitter.com/MorningConsult/status/1277680735844007936?s=20&amp;utm_campaign=wp_the_finance_202&amp;utm_medium=email&amp;utm_source=newsletter&amp;wpisrc=nl_finance202rc=nl_finance202">Morning Consult.</a>In other words, Americans understand that it's not safe to engage in the same economic activity they used to during the pandemic. We have a demand problem stemming from the virus' very existence.Unfortunately, this appears to be lost on Trump and his aides, according to <a href="https://www.cnn.com/2020/07/01/politics/donald-trump-coronavirus-response/index.html">CNN</a>. White House Chief of Staff Mark Meadows and Jared Kushner reportedly want Trump to stop talking about the virus and instead emphasize the economy. They are operating under the delusion that this is a real choice, as if the pandemic is not what is crippling the economy. The Republican party at large shares this fantasy. Take, for example, the debate over <a href="https://www.businessinsider.com/gop-trump-administration-threatening-throw-us-economy-off-a-cliff-2020-6">continuing enhanced unemployment benefits</a>for Americans out of work. It's an extra $600 on weekly <keyword id="6358907" type="UNKNOWN" weightage="60" keywordseo="unemployment">unemployment</keyword> benefit checks that is set to expire at the end of the month. Republicans want this supplement to fade out because some Americans are making more money staying at home than they were going to work.We'll set aside what that says about the state of wages for many working Americans, especially <a href="https://www.washingtonpost.com/outlook/2020/05/21/essential-workers-pay-wages-safety-unemployment/">essential workers who make less than the benefit</a>. This also reveals that Republicans don't understand the problem at hand. Republicans think they have a choice between sending Americans back to work in a pandemic and continuing to pay them to stay home. They don't. If the pandemic is not under control (it is not), businesses will continue to shut down, employees will be scared to go to work, and our demand problem will persist. Many workers simply won't be needed.The government should be paying people to stay home in these circumstances. It's better for the economy if people can still pay their bills, so Americans aren't trapped in debt and real damage isn't done to the financial system when this is all over. It's also easier to implement social distancing if people stay home, which will help slow the spread of the virus. So Trump and Republicans are looking at this wrong. They think they have a choice between the virus and the economy. Actual demand in the economy is still far from normal levels — the CEO of Delta Airlines said this morning that the airline expects to carry <a href="https://twitter.com/e_russell/status/1278661955688058881">less than a fifth</a> of the number of passengers this weekend that flew last July 4th weekend — so some of these re-hirings may be temporary.Also, the number of "permanent" job losses <a href="https://twitter.com/NickTimiraos">rose</a> in June. And initial unemployment claims in the past week <a href="https://twitter.com/DanielAlpert/status/1278661376093945862">were still extremely high</a>. Insider contributor Dan Alpert notes further that the rate of "underemployment" remains extreme and that most of the new jobs created in June were <a href="https://twitter.com/DanielAlpert/status/1278672224761905154?s=20">crappy low-wage jobs</a>. Let's look at 1918...</h3>As debates rage about whether we should prioritize health or the <keyword id="34412" type="ORGANIZATION" weightage="60" keywordseo="economy">economy</keyword> — and about when and how to reopen — we should keep this in mind. Studies of <a href="https://www.businessinsider.com/when-will-life-get-back-to-normal-coronavirus-pandemic-2020-4">the "Great Influenza" of 1918 </a>concluded that cities that adopted "non-pharmaceutical intervention" measures earlier and kept them in place for longer did better, both health-wise and economically. Specifically, they had fewer deaths and their economies recovered faster.In other words, in 1918, it wasn't health or the economy. It was health and the economy. <a href="https://jamanetwork.com/journals/jama/fullarticle/208354">Different cities approached the 1918 epidemic differently</a>, as a 2007 study from Howard Markel, Harvey Lipman, and J. Alexander Navarro shows. The varying experiences include a warning about what could happen if we reopen too early.St. Louis, Denver, and San Francisco, for example, adopted "distancing" measures early, which helped mitigate the initial epidemic wave. All three cities, however, then relaxed their measures early… and then saw second waves. In St. Louis and Denver, these second waves were actually worse than the first. As the second waves took hold, the cities re-implemented distancing measures, but it was too late.<div type="image" title="St. Louis flu deaths 1918" height="1322" width="1374" msid="75326962" hostid="261"></div><div type="image" title="Denver flu deaths 1918" height="1398" width="1384" msid="75326965" hostid="261"></div>The same thing <a href="https://www.businessinsider.com/what-san-francisco-can-learn-spanish-flu-pandemic-coronavirus-1918">happened in San Francisco</a>, as Insider's Katie Canales shows in a vivid photo essay.In 1918 — just as in 2020 — San Francisco was one of the first cities to shut down, and it got the epidemic under control. However, five weeks later, with cases declining, the city declared victory too early and reopened too fast. (Literally. As Katie reports, "a whistle blew and people poured out into the streets in celebration, tossing their masks in the process.")A few weeks later, cases started climbing again. San Francisco tried to shut down again, but many frustrated citizens resisted the orders. The second wave <a href="https://www.nationalgeographic.com/history/2020/03/how-cities-flattened-curve-1918-spanish-flu-pandemic-coronavirus/">continued for months, as this chart from National Geographic shows</a>.Meanwhile, other cities took a different approach, keeping their initial restrictions in place for longer. New York City issued basic <a href="https://jamanetwork.com/journals/jama/fullarticle/208354">isolation and quarantine rules earlier</a> than other cities and then kept them in place until its death rate returned to a very low rate. The city saw only one major wave of infections, and its total death rate was at the lower end of the US range.Importantly, despite having its initial rules in place for longer, New York's restrictions were only in place for about half the total time as those in St. Louis and Denver. The rush to reopen in those cities, in other words, led to second shutdowns that lasted about as long as the first.<div type="image" title="5ea099e8a34b3c0e215a3827" height="1307" width="1365" msid="75326968" hostid="261"></div>Another lesson from the 1918 experience is that restrictions are not "all or nothing." There are different measures and approaches, of varying degrees of severity. So as some states begin to roll back some restrictions, we should probably stop acting as though the decision is binary. — HB<h3>How coronavirus spread in a restaurant</h3>While we're thinking about when and how to reopen, here's a fascinating study of how coronavirus spread in a restaurant. It holds both good and bad news for restaurant reopenings.As Insider's Bill Bostock reports, a letter from researchers in China that was published by the CDC traced <a href="https://www.businessinsider.com/how-restaurant-air-conditioning-gave-nine-people-covid-china-2020-4">an infection that spread one afternoon in late January among three families</a> sitting near each other in a large restaurant in Guangzhou.During this meal, a woman who had just arrived from Wuhan and was still asymptomatic apparently spread the virus to nine other diners nearby. The woman developed symptoms later that day. The others did within the next week to 10 days.<div type="image" title="An annotated diagram showing the location of the AC in the restaurant in Guangzhou, China." height="676" width="901" msid="75326972" hostid="261"></div>It's frightening—and <a href="https://www.businessinsider.com/air-conditioning-spread-coronavirus-restaurant-can-service-industry-open-again-2020-4">it's bad news for restaurants</a>—to realize that one asymptomatic person can spread the virus to nine other people eating nearby. But the study also contained additional insights. Everyone who got infected in the restaurant was in the same "air flow" of an air-conditioner. Other diners nearby, despite being just as close to the infected person, did not get sick. The professor and journalist Zeynep Tufekci <a href="https://twitter.com/zeynep/status/1251556084424347649?s=20">theorizes that this </a>supports the idea that the coronavirus spreads via "micro-droplets" but does not fully "aerosolize." Regardless, this study should sober up anyone hoping to eat out or open their restaurant anytime soon. Yes, wearing masks would probably help. But it's hard to eat while wearing a mask. — HB<h3>Hydroxychloroquine isn't a miracle drug. Here's what that mistaken hope cost us. </h3>Hope, like toilet paper, is in short supply these days. So it's not surprising that we cling to any seeming bit of good news. But there's a cost to misplaced hope, and we're seeing it with the deflated enthusiasm for the drug hydroxychloroquine. Remember <a href="https://www.businessinsider.com/hydroxychloroquine-trump-coronavirus-wonder-drug-does-it-work-2020-4">hydroxychloroquine</a>? Just two weeks ago, President Trump and a choir of others were touting it as a "gamechanger" for COVID-19, pointing to claims by a French doctor — let's be blunt, an<a href="https://www.thedailybeast.com/didier-raoult-is-the-bs-artist-behind-trumps-miracle-drug"> eccentric, Trumpish French doctor</a>— that he had cured 100% of his Covid patients with it. Fox News went gaga over this apparent wonder drug. So did Dr. Oz. Doctors began prescribing it. The FDA gave emergency approval to use it on American COVID-19 patients. And tests of it alone, and in conjunction with the antibiotic azithromycin, were started. Alas, so far, no good news. On Tuesday, the National Institutes for Health <a href="https://www.forbes.com/sites/rachelsandler/2020/04/21/nih-panel-recommends-against-using-drug-combination-touted-by-trump-outside-clinical-trials/#18a0c26c4b40">recommended against </a>using hydroxychloroquine in COVID-19 patients outside clinical trials, citing a potential heart risk. The same day, a<a href="https://www.medrxiv.org/content/10.1101/2020.04.16.20065920v1.full.pdf"> retrospective study of 368 veterans</a> found higher death rates in patients treated with hydroxychloroquine and no reduction in the risk of ending up on a ventilator. This followed a French study also showing no benefit to using the drug for COVID-19. And Trump, Dr. Oz, and Fox have <a href="https://www.cnn.com/2020/04/22/media/fox-news-hydroxychloroquine-reliable-sources/index.html">all finally stopped hyping it</a>.It's still possible that hydroxychloroquine might help against COVID-19. There haven't been any randomized controlled trials disproving its effectiveness. But there also hasn't been any clear evidence that it works.So what's the harm in all the hype about it? As Trump said, "What do you have to lose?" Well, several things.The optimism fanned by Fox News and Trump had costs. It caused <a href="https://www.lupus.org/resources/hydroxychloroquine-plaquenil-coronavirus-covid19-questions-answers#">a shortage of the drug</a> for the lupus and arthritis patients who need it. Hydroxychloroquine overdoses doubled over the same period last year. An Arizona man died when he self-medicated by taking a related chemical used to clean fish tanks. The hydroxychloroquine hype also distorted medical care around the country for a few critical weeks. The president threw the weight of the federal government — and an FDA emergency action — into a drug that may not help at all, much less be a miracle.It's not that we shouldn't be testing hydroxychloroquine — of course we should! — but the presidential and media hype caused an over-investment of time and mental energy, and took effort and intellect away from testing other therapies. The hydroxychloroquine hype machine also spent down one of most precious commodities we have: public trust. Every miracle cure that raises — then dashes — hope increases skepticism, and makes it less likely that Americans will trust the doctors and public health authorities later, when we're asked to get a vaccine or take a prophylactic medicine. The latest casualty of the President's brief hydroxychloroquine obsession appears to be Dr. Rick Bright, the director of the Biomedical Advanced Research and Development Authority, who says he was <a href="https://www.politico.com/news/2020/04/22/hhs-ousts-vaccine-expert-as-covid-19-threat-grows-201642">ousted after resisting pressure to hype and fund it</a>. The Administration disputes this characterization. — DP<h3>While we're paying attention to the pandemic… </h3>The Republican-led Senate Intelligence Community released a unanimous report concluding that the intelligence community's investigation into Russia's interference in the 2016 election was<a href="https://www.nytimes.com/2020/04/21/us/politics/russian-interference-senate-intelligence-report.html"> analytically sound, justified by the evidence, and conducted professionally</a>. "The committee found no reason to dispute the intelligence community's conclusions," said chair Sen. Richard Burr, a Republican from North Carolina. During the Mueller investigation and impeachment, President Trump said the investigations were corrupted by "deep state" interference, malevolent manipulation by President Obama, and overreliance on the Steele dossier. The Senate committee found none of that. — DP<h3>The more things change, the more politics stays the same...</h3>"Even now, nearly two months after disaster first struck, the federal response remains inadequate and disorganized. Americans are rightly angry about this failure of government." Marking the 10th anniversary of the Deepwater Horizon oil well disaster in the Gulf of Mexico, John Dickerson of 60 Minutes excavated <a href="https://twitter.com/jdickerson/status/1252338519504121858">some of the Republican (and Democratic) responses</a> to the Obama administration's handling of it. They sound surprisingly familiar.What did now-Vice President Mike Pence say back then, for example? "The American people want answers...The American people know this was a slow response." It would later be discovered that the March 6 high school game was also the event in which five Indiana residents are believed to have contracted the novel coronavirus and later died due to complications with COVID-19.Warren Central fan Roscoe Taylor, Lawrence North High School basketball assistant coach Jim DeSalle, Warren Central fan Charles Johnson, North Central High School Athletic Director Paul Loggan, and Lawrence North fan Larry Rush, all attended the sectional semifinals.Several of their loved ones told USA Today that they believed the game could have been where they contracted the virus. "I feel like that's probably where we got it," Kay Johnson, the widow of one of the five victims, told USA Today. "Too many have gotten sick who were there. The sad thing is we just didn't know. We didn't have the information to go on."The morning of the game the <a href="https://www.coronavirus.in.gov/">Indiana State Health Department reported its first coronavirus related case</a>.
