"Responding to the COVID-19 crisis has put frontline healthcare personnel under a severe strain. Our helpline is an attempt to ease the pressure on hospitals and doctors, enable those in need to get treatment on priority, and equip citizens with useful and accurate medical information," Shah said. He added that the participation from Cognizant employees has expanded the helpline's reach across marginalised communities and areas with limited access to medical expertise. Before joining the helpline team, Cognizant's doctor volunteers took a two-week virtual training from Shah on various aspects of running a helpline, such as gathering information from callers, reading symptoms, and responding to various types of questions. Cognizant has over 2,700 medical professionals globally, including physicians, nurses, and pharmacists who provide critical services such as pharmacovigilance, clinical data management, medical management and claims management, among others, to healthcare and life sciences organisations globally. The US-based company's healthcare and life sciences business revenue was about USD 4.7 billion and accounted for 28% of its overall topline for 2019. Under Cognizant's employee-led volunteering program Outreach, Cognizant employees are supporting several governmental and non-governmental organisations across India on COVID-19 initiatives. The company has about two lakh employees in India. Ramkumar Ramamoorthy, Chairman and Managing Director at Cognizant India said most questions to date on the helpline have been about likely symptoms, self-isolation, diet, hygiene and testing of COVID-19. "Through the helpline, Cognizant volunteers have been able to provide non-prescriptive advice to marginalised and underserved communities as well," he added. Similarly, KONE Elevator India, a fully-owned subsidiary of Finland-based KONE Corporation, said it is offering its cloud technology for free to its customers operating medical facilities. The KONE 24/7 Connected Services technology can better predict, maintain and take action before breakage, thereby ensuring less equipment downtime and fewer faults, a statement said. This preventive maintenance system helps the elevator work seamlessly in hospitals 24/7 and in the current pandemic situation, it is absolutely critical that elevators or escalators run smoothly without any setbacks, it added. In collaboration with IBM Watson, the cloud-based solution uses sensors that collect data on a range of parameters on equipment performance and powerful analytics to identify deviations. If the system identifies the need for maintenance, it alerts a technician immediately. Cognizant's doctors volunteer at Covid-19 helpline and address queries

"But it's not where most people think," he told Insider recently."It exists in a place where I, as a doctor, was not wanting to hurt my opponent because I'm so used to healing people. But learning family medicine became his priority in college."This is kind of my last hoorah of trying to be a professional athlete because I'm aging out of that window," the 32-year-old told us. "Now, being in my prime and being able to be physically fit, smart, sharp, this is my best chance to succeed in a sport I've developed a tremendous passion for."Boxing "is unlike any other sport," said Varshavski. "You have to be incredibly body tough, which means strong, well-conditioned, and mentally tough, as you're in there by yourself."You need to learn to count on yourself which is a lot like life because you have to succeed on your own and ride the hard times. Boxing is exactly that. It's a great reflection of how life is."Varshavski, who beat content creator iDubbbz in an exhibition earlier this summer, said he believes he's succeeding in boxing because of how he lived his life to this point.He told Insider he came "to the United States very poor" from Russia, and lived off of welfare programs. A young Mikhail got bullied as a kid in New York, he said, as he did not know English at the time."I understand what it's like to struggle with that, and overcoming that has really prepared me for the struggles that exist inside that ring," he said.Varshavski, though, said there's a second juxtaposition when it comes to being a fighting doctor."I don't want to send the wrong message to my patients, or my followers. I'm not encouraging them to become professional fighters, but I want to encourage them to stay fit, be healthy, and to take classes. Hit the heavy bag."You can "get into good shape" using boxing, "but you don't have to get hit in the head to enjoy and celebrate the sport," he said.Varshavski is aware of the risks involved when it comes to pro fighting. "What is safe to you is different to what is safe for someone else," he said. "Only you as an individual can make that decision."My patients do all sorts of activities, and I never say not to do it. I simply explain the risks of what happens if they continue doing it. If as cognizant adults they understand those risks, that's it. That's where the conversation ends."If I'm rock climbing, I need to understand I can slip and fall. Sky diving — the parachute might not open. In boxing, you can get your head rung pretty badly," he said.<h2>Being told he's the sexiest doctor alive</h2>Varshavski was finishing his intern year of residency, which he said was the hardest year, when he received an email that he thought was a prank.People magazine told him he was being considered as the sexiest doctor alive for 2015. "It didn't make a lot of sense," he said.Varshavski felt like a nerd through high school. "Especially as someone who loved science and math," he said.Earning that moniker was "random," he said, however, he's since leveraged that title "into getting people to pay attention to health, sickness prevention, and avoiding misinformation."So, which would mean more to him — winning his pro debut this weekend, or being called the sexiest boxer alive?"Oh, pro debut all day long! I can't wait to get that win," he said. "And put that newspaper clipping hung up on the wall, to share that victory with my friends, family, my medical colleagues, and my patients."My patients are even sending me messages on my electronic medical record wishing me luck, which is not what an electronic medical record is for! But I appreciate the good wishes."To any of his patients, or followers, he has one final message."The doctor is about to prescribe a KO — and you should all tune in."

'Sexiest doctor alive' puts his stethoscope down and picks up boxing gloves instead for a fight Saturday

<ul class="summary-list"><li>Experts in San Francisco, Miami, and the UK say many monkeypox cases are being "missed."</li><li>Some patients are receiving false negative monkeypox tests, while others are being misdiagnosed.</li></ul>More than <a href="https://www.cdc.gov/poxvirus/monkeypox/response/2022/us-map.html">7,100 monkeypox patients</a> have been diagnosed in the US since this outbreak began with a single case in mid-May.But Dr. Graham Walker, an emergency physician in San Francisco, says that many more monkeypox patients are not being given the care and attention they deserve to prevent, diagnose, and treat this infectious disease efficiently.Walker says he's seen "several patients" whose monkeypox diagnosis was "missed" by another provider before they landed in his ER in excruciating pain."I had a patient who, I saw their record from another facility, and it said, 'lesions only in the genitals, so unlikely to be monkeypox,'" he told Insider. Other clinicians and scientists in the US, the UK, and West Africa say the same: monkeypox is being widely underdiagnosed. <h2>Some misdiagnosed patients end up in so much pain they can't wear clothes</h2>Across the country from Walker's ER, Dr. Lilian Abbo, an infectious disease specialist at the University of Miami, says she's noticed the same issue. "We have seen patients that have been going up to seven centers: three different urgent cares, dermatologists, and ERs to try to figure out what is going on," she said during a recent Infectious Diseases Society of <a href="https://affiliate.insider.com?u=https%3A%2F%2Fvimeo.com%2F729999731%3Fsid%3Din%257C62d9a412884de613c421403c%257Cbrowser%26utm_campaign%3D3861930%26utm_source%3Daffiliate%26utm_channel%3Daffiliate%26cjevent%3Db9f54d9d14c111ed805a011f0a82b82d%26clickid%3Db9f54d9d14c111ed805a011f0a82b82d">America telebriefing</a>, stressing the need for better awareness of how monkeypox can present.Better awareness can have huge implications for both treatment, and disease containment. It's possible that if close contacts find out early enough in the course of their infection (meaning, in the first 4 days after an exposure) the Jynneos smallpox vaccine may be <a href="https://www.cdc.gov/poxvirus/monkeypox/considerations-for-monkeypox-vaccination.html">used to prevent monkeypox infection altogether</a>.But, "by the time they arrive," many patients who've been misdiagnosed "may have been healed, or some of them may have progression of disease," Abbo said. That progression can be painful. Patients in this outbreak have, in some of the most severe presentations, been unable to use the toilet, because the pain around their anus and genitals becomes so severe. "One of my patients said 'I can't even wear pants or underwear without pain,'" Walker said. <h2>'Textbook' descriptions of symptoms aren't always accurate</h2><div type="image" title="Matt Ford shows monkeypox spots" height="1000" width="2000" msid="93393933" hostid="261" sizebytes="90386"></div>Part of the reason why monkeypox is being underdiagnosed is that <a href="https://www.insider.com/monkeypox-miserable-experience-man-timeline-symptoms-vaccine-2022-7">this disease outbreak isn't operating</a> like the "textbook" monkeypox presentations of the past, with a classical fever, swollen lymph nodes in the neck, and headache all appearing before pox surface on the face and on the hands.Instead, monkeypox lesions may be lodged in the rectum, lymph node swelling may be present only in the groin, and it's not unusual for a single pock to be the only marker of a person's entire infection.A July 21 <a href="https://www.nejm.org/doi/full/10.1056/NEJMoa2207323?query=featured_home">New England Journal of Medicine</a> compilation of 528 case reports from 16 countries around the world establishes some clear patterns for this outbreak. Nearly two-thirds of the patients studied had lesions in their anogenital (private) area, with a majority of patients counting less than 10 lesions on their entire body.Patients were "presenting with symptoms that weren't part of the case definitions," NEJM study author Chloe Orkin told <a href="https://www.insider.com/monkeypox-symptoms-butt-pain-fevers-lethargy-lesions-2022-7">Insider</a>. "The important thing is to help doctors to recognize it."Fever, fatigue, muscle aches and headaches were some of the most common pre-rash signs a monkeypox infection was budding, which also makes it easy for people to confuse the condition with COVID, initially. "Every single patient I've talked to has done a COVID test when they had a fever, and they're like, 'oh, it's not COVID,'" Walker said. Other times, diagnoses are missed because providers don't get a "good, aggressive scrub" of the lesions, he said."I've seen that before, where people have had a test that came back negative and I'm like, 'you have literally every symptom of monkeypox, this looks exactly like all the other monkeypox cases I'm seeing, and we've ruled out other stuff, like herpes, syphilis, whatever.'" <h2>Some people avoid monkeypox diagnosis because it can be stigmatizing, painful, and lonely</h2>Early treatment is key to curb a monkeypox infection and prevent the excruciating pain it causes. The antiviral drug Tecovirimat (TPOXX) can help, as well as prescription painkillers which are stronger than Motrin and Advil.But many patients, fearing stigma and lacking information, are not seeking help.One of Walker's patients had been managing his lesions alone for two full weeks, when he finally decided to seek treatment. "He just wanted to manage it at home. He was worried about the stigma, he said he'd never had an STD before. But the pain was so severe, he finally came in," Walker explained. "There's lots of fear, there's lots of anxiety, and to be honest, there's probably a lot of people who aren't diagnosed yet," he added. Dr. Jason Zucker, an infectious disease specialist at New York Presbyterian hospital, said "even patients with mild disease have been taking it very hard" when they receive a monkeypox diagnosis."In addition to this stigma, patients who are diagnosed then isolate at home for up to four weeks alone," he told journalists on a Zoom call Friday. "It's important to make sure that we're all cognizant of this, that we all work together to reduce stigma, and that we offer patients mental health and other supportive resources after their diagnosis." <h2>A Nigerian doctor spotted this kind of monkeypox in 2017. He hopes people will finally start funding research on this virus.</h2>In Nigeria, where Dr. Dimie Ogoina has been studying monkeypox outbreaks linked to sexual contact <a href="https://pubmed.ncbi.nlm.nih.gov/29619921/">since 2017</a>, he says he's struggled with similar issues of confusion and stigma, which both make it harder to get a sense of the true scope of any outbreak.Often, he suspects, patients with "genital lesions" aren't coming to the hospital to be properly diagnosed, instead, they're just going to the pharmacy for STD treatment. "There are a lot of things that still not known about monkeypox" because "it has been a neglected disease," Ogoina told Insider. "Since the global north is now having cases, I'm sure there will be investments in research, and we will be able to uncover most of these unknowns."

An ER doctor says he's sick of seeing monkeypox patients misdiagnosed, only to end up in the hospital in excruciating pain

<ul><li>The Ordinance seeks amendments to the 125-year-old <keyword id="552772" type="PERSON" weightage="20" keywordseo="Epidemic-Diseases-Act">Epidemic Diseases Act</keyword>, 1897, that protects health workers in the country. </li><li>The Indian government in April approved an ordinance that recognised any crime against health workers in India as a “cognizable and non-bailable” offence. </li><li>It means that offenders can be arrested without a warrant, and no bail will be granted. </li></ul>The Indian government is preparing to introduce over 20 bills for discussion in the monsoon session of the Parliament, which is slated to begin on September 14. Moreover, 11 ordinances will also have to be passed during the session, as most of them face an expiration date by the winter. Among the ordinances to be discussed is Epidemic Diseases (Amendment) Ordinance 2020. The Ordinance seeks amendments to the 125-years-old Epidemic Diseases Act, 1897, that protects health workers in the country. What is the Epidemic Diseases (Amendment) Ordinance 2020? The Indian government in April approved an ordinance that recognised any crime against health workers in India as a “cognizable and non-bailable” offence. It means that offenders can be arrested without a warrant, and no bail would be granted. Offenders can also be fined anywhere between ₹50,000 to ₹2 lakh and will also have to serve a prison term between 3 months to five years, Union Minister Prakash Javadekar had said in April. Moreover, people who damage the vehicles of healthcare workers or clinics will have to compensate by paying twice the value of the damaged property. These cases will be fast-tracked. An investigation against such cases will take place within 30 days, and a decision will be announced within a year. Why the need to amend the Epidemic Diseases Act 1987 now? The Epidemic Diseases Act is a century-old law enacted when Bombay (Mumbai) was hit by Bubonic plague. Over the years, it has been used to prevent the spread of epidemics including H1N1 influenza in 2009, Cholera in 2018 etc. Ever since the pandemic began, several <a href="https://www.businessinsider.in/politics/india/news/indian-doctors-attacked-abused-assaulted-while-fighting-coronavirus/articleshow/74961776.cms">incidences of abusive behaviour</a> against the doctors came to light. People were said to be pelting stones and attacking health workers. At some places, a mob tried to stop a medical team from taking a <keyword id="1754604" type="PERSON" weightage="20" keywordseo="coronavirus">coronavirus</keyword> infected man into isolation. The Indian government then said that it will not tolerate any incident of violence or harassment against the doctors. Since the amendments were made in April and is applicable during the pandemic only Several experts have lauded the ordinance and<a href="https://indianexpress.com/article/opinion/columns/covid-19-pandemic-coronavirus-ima-indian-medical-association-ordinance-ensuring-doctors-safety-6380562/"> asked</a> the government to extend the ordinance to post-COVID period as well. “The ordinance has been brought as an amendment to the Epidemic Diseases Act, 1897, and hence, may become infructuous after the pandemic is declared over. What is the Epidemic Diseases (Amendment) Ordinance — key provisions up for Parliament's consideration

<ul class="summary-list"><li>In the first few weeks of the pandemic, the number of <a href="https://www.businessinsider.com/category/coronavirus">COVID-19</a> patients in the United States has exponentially increased to over 2,500,000 confirmed cases.</li><li>We chat with <keyword id="25205" type="PERSON" weightage="60" keywordseo="doctors">doctors</keyword> in Chicago, New York City and Sacramento, California over the course of three days to hear their stories as they work within the realms of the U.S. <a href="https://www.businessinsider.com/category/healthcare">healthcare system</a>.</li><li>They tell us their experiences working on the frontlines of the <a href="https://www.businessinsider.com/category/coronavirus">COVID-19 crisis.</a></li><li><a href="https://www.businessinsider.com/?hprecirc-bullet">Visit Business Insider's homepage for more stories</a>.</li></ul>Following is a transcript of the <keyword id="21654" type="COMPANY" weightage="60" keywordseo="video">video</keyword>.Tasnim Khan: We're still in the trenches, and not really clear on where that's gonna go.Saquib: The things I'm seeing in the hospital, this is really no joke, and I've never seen anything like it.Ahmed Nadeem: Right now, I have so much to offer them, but you don't know how it's gonna be next two, three weeks.Medha Imam: In the last couple of weeks, the number of <keyword id="1754604" type="PERSON" weightage="60" keywordseo="coronavirus">coronavirus</keyword> patients has increased exponentially to now over 400,000 confirmed cases in the United States. We contacted physicians in Illinois, New York, and California to hear their perspectives. But since things are evolving so quickly, we spoke to them over the course of three days. Here are their diaries.Saquib: This is day No. 1.Nadeem: This is day one.Noel Lasala: Day one in the fight against coronavirus.Saquib: Well, we had our fast patient on the floor die, which was really sad. It's tough, what we're going through. That interaction with, you know, the doctor who was taking care of that patient, and, you know, he just kinda looked at me incredulous and he's just like, you know, "I knew it was possible that was gonna happen, but it just happened so fast." There isn't necessarily set protocols 'cause no one knows how to deal with a pandemic, so we're really figuring a lot of stuff out on the fly.Lasala: Patients today had mostly routine chronic medical issues. Luckily, today was not like three days ago, when we had our first highly suspected <keyword id="8763838" type="UNKNOWN" weightage="60" keywordseo="covid-19">COVID-19</keyword> patient.Nadeem: The greatest challenge was seeing these two patients. One of them was a sickly 64-year-old female, and the other one was a 26-year-old young gentleman. Both of them had similar symptoms. They were a little tough to manage just because we were understaffed today. Not too many people showed up.Khan: We're still in the trenches, and not really clear on where that's gonna go. We evolve day to day; we evolve hour to hour. It's been such a challenge to keep up with the ever-changing recommendations and even sometimes confusion and conflicting recommendations.Saquib: We're just not resourced appropriately for something like this, and right now it's definitely been an uphill battle.Nadeem: You see a lot of people cough a lot, and this 26-year-old is coughing quite a bit. I had about two gowns each time I went to see him, I went to see him four times, so you have about eight gowns, you've got about eight masks, you got eight pairs of gloves. It's very easy to run out of all this equipment that you have, whether it's masks, whether it's ventilators, and whether it's nasal cannulas to give them oxygen. I think the next challenge we're gonna address is the surge that's gonna be coming in the next couple of days. One of the biggest challenges is trying to calm yourself when patients come in because almost everyone is nervous. You're the captain of the ship, so to speak, so you have to really have your whole composure there, you have to really be, not get nervous at all.Khan: The one word that seemed to resonate and seemed to be common, commonly expressed by most of our team was abandonment. Neglect, isolation.Nadeem: The rates of deaths are gonna increase. We know that. We know that from Seattle. Getting prepared for that is a little scary because we're not really quite sure how much equipment we have. Right now I have so much to offer them, but you don't know how it's gonna be next two, three weeks. So when you think about that, it's a little depressing.Khan: We're rationing our tests, so that means that there're not enough. What I have heard from just the phone conversations with health state officials is that the labs just can't keep up with the demand, the testing demand.Saquib: We really need to emphasize social distancing just because we have to limit the spread, be smart, and recognize that this is now a public-health issue and we need to act collectively.This is day two.Lasala: There's been a surge for our city of COVID cases with some moralities, too. Some people have died from it. We continue to fight the fight here in the trenches.Saquib: The ICUs are getting really full. Initially we had put all patients who were very severe in the cardiac ICU, but now that's completely full. They've moved all patients out of the surgical and medical ICUs and created a makeshift ICU, and now those ICUs are being prepped for coronavirus overflow patients. I have never encountered a situation where you've had to create intensive care units on the fly.Khan: We had a high no-show rate today. So we had a lot of people who just didn't come. We converted a lot of our visits to phone consults. On the good side of things, the Health and Human Service Department has waived HIPAA requirements, so we can pretty much use Skype, FaceTime, to do a video visit. For me it's a challenge to tell people to be in the front line, and I as the chief medical officer of my group feel very responsible for people. And if I can't protect my people or my providers, it does present itself as an ethical dilemma to me to tell them to go see patients and they don't have the right protection.Nadeem: Definitely one of my low moments was when I was told that we don't have enough nurses to work the night shift. Some nurses had called in sick, and we had a ratio of about six patients to one nurse, which is actually pretty high.Khan: In a developed country, you would expect to have supplies, you expect to have a system, and our system is just inadequate. We've attended several calls where they've told us to reuse our protective masks.Saquib: Today it was kinda nuts, but we were taking care of one patient who suddenly developed a cardiac arrhythmia out of nowhere. He obviously had coronavirus. And while we're getting in the process of putting pacer pads on him, another patient starts to fall apart, and they had to intubate that patient on the other side of the floor, and you're just kinda looking like, how many shoes can drop all at once?Khan: We've got to find other ways to work better and smarter and reconfigure how we're dong care so that we're not burning ourselves out too quickly, and we've got to be able to sustain the system.Saquib: The ER is doing a better job of admitting only patients who there is a lot of concern, but, just, the numbers keep growing.Nadeem: We have to really protect ourselves. Can't keep up with the guidelines. Can't keep up with the confusion around doing phone consults. Can't keep up with the constantly changing testing. Every minute was accounted for. Every minute was talking to people. Every single minute. You know, not even a chance to go to the bathroom. The greatest challenge of today was keeping providers motivated to stay in place and not panic, was keeping them focused on the fact that we need to be here to support what we expect will be a surge.Nadeem: The greatest challenge today was probably a surge of patients I received between 11:30 and noon. Received about five patients. All the patients had similar symptoms as coronavirus patients. They came with fever, chills, cough, runny nose, and two of them actually had shortness of breath. So it's an issue because we have only two isolation rooms in the ER and we can only accommodate two of those patients at one time.Saquib: As far as the situation in the hospital, it's definitely getting crazier. Our census of patients who either have coronavirus or who are suspected of, you know, has jumped from, I think it was, what, six middle of last week to around 85 today. Existing ICUs are now only for coronavirus patients, which is nuts. I've never seen anything like it. And then on the floor, patients are just falling apart kinda left and right. It's definitely gotten real. And it's progressed faster than I thought it would. She also advised that kids who are sick stay home, emphasizing that it's not just monkeypox, but COVID-19 as well as other respiratory illnesses such as colds and cases of the flu are still circulating. New Delhi, Apr 1 () Founders of Indian startups, members of investment advisory firms and independent advisors have joined hands to launch a Rs 100 crore grant for supporting innovators working on solutions to control COVID-19 pandemic that has claimed thousands of lives globally.The mission of ACT (Action COVID -19 Team) is to provide guidance and resources to startup founders and employees, and empower teams through financial grants and mentoring to create a large-scale impact in the fight against COVID-19.The group will be supported by partner NGOs and industry veterans and will collaborate with government agencies to scale up solutions that show promising results."Investors, industry veterans and founders will collaborate and support startup founders in their preparation and planning as they face significant economic impact caused by COVID-19. It will be a dynamic resource to share best practices with them and enable them to collaborate and co-operate efficiently with business partners and various other external and internal stakeholders," a statement said.A Rs 100 crore ACT Grant will springboard innovators who are working on solutions to overcome COVID-19, it added.The grant is seeking applications from NGOs, SMEs and startups that are working on innovative solutions to combat the effect of COVID-19 in areas like prevention of COVID-19 spread, scaling testing, disease management at home, enhanced support for healthcare workers and hospitals, management of critically-ill patients, and support for mental health."A 25-member team combining the resources of the startup community has been formed to enable rapid evaluation and recommendations on potential projects. A leadership team comprising members of the investment advisory community, founders andindependent advisors has been formed to make grant decisions," the statement said.Further, a mentoring team has been constituted who can monitor and support post-investment for the projects, it added.Corporates have been rushing in to help the government and citizens fight the Covid-19 pandemic in India.IT major Cognizant has also announced an initial USD 10 million philanthropic commitment to support communities around the world in addressing the immediate and long-term impacts of COVID-19.Cognizant and its US and India-based foundations will provide critical resources to strengthen public health systems, education and workforce institutions, and the economic outlook of communities worldwide, the company said in a statement.As part of the USD 10 million commitment, the Cognizant US Foundation will match all Cognizant associate contributions to GlobalGiving's 'Coronavirus Relief Fund' throughout April.In a separate statement, short-video platform TikTok said it is donating Rs 100 crore worth of medical equipment in India.TikTok is donating 4 lakh hazmat medical protective suits and 2 lakh masks to protect doctors and front line medical staff in India, it said in a statement. The ACT (Action COVID -19 Team) initiative has seen participation from venture capital firms like Sequoia Capital India, Accel, Lightspeed, Chiratae Ventures and SAIF Partners as well as individuals like Nandan Nilekani, Vijay Shekhar Sharma (Paytm), Kiran Mazumdar Shaw, Mukesh Bansal (cure.fit), and Abhiraj Bhal (Urban Company). SoftBank and Bnext have also contributed to the grant that is being managed by United Way Worldwide. The group will be supported by partner NGOs and industry veterans, and will collaborate with government agencies to scale up solutions that show promising results.
