<ul class="summary-list"><li>The US surgeon general is pushing for a warning label on social media about teen mental-health risks.</li><li>A comparison to cigarette warnings is tempting, but social media might be more like explicit lyrics.</li></ul>US Surgeon General Vivek Murthy is recommending a warning label on social-media sites cautioning that "social media is associated with significant mental-health harms for adolescents."This label would warn teens and their parents of the risk of harm from using social media. In an <a target="_blank" href="https://www.nytimes.com/2024/06/17/opinion/social-media-health-warning.html">op-ed for The New York Times</a>, Murthy writes, "The mental health crisis among young people is an emergency — and social media has emerged as an important contributor."Murthy references the warning label on tobacco products, which is viewed as a public-health success story, helping reduce tobacco use significantly since the 1970s.But is social media really comparable to cigarettes? And could a warning label actually make an impact? Sure, apps like TikTok and Instagram can be addictive and harmful. However, there are a few really basic differences that I think are worth considering.First of all, unlike social-media apps, cigarettes are aimed at adults (yes, they have also been stealthily marketed to teens), and they are legally sold only to adults (yes, obviously, teens are getting their hands on them anyway). The warning label is for everyone: Cigarettes pose a health risk to anyone who smokes.Secondly, there's little hope that tobacco companies will be inspired to make cigarettes healthy — there is no healthy version of an American Spirit. But there is hope that social media could be less harmful. There could be all sorts of changes, small and large, that make social platforms safer for kids. Big Tech platforms could make these changes if spurred by regulation, negative media coverage, advertiser boycotts, or simply their belief that it's the right thing to do.Speaking of regulation, the decline in cigarette use over the past 50 years has been aided by smoking bans in offices, planes, restaurants, bars, and other public places. These bans resulted from a series of state, local, and federal regulations.So far, the government's approach to social-media regulation has been somewhat chaotic. A few bills exist, like the Kids Online Safety Act, <a target="_blank" href="https://www.congress.gov/bill/118th-congress/senate-bill/1291">Protecting Kids on Social Media Act</a>, and an overhaul of the existing COPPA laws on data privacy and advertising to kids online.There's also a bunch of state attorneys general who have filed lawsuits over social-media harms to kids, and individual states have crafted their own laws, like a <a target="_blank" href="https://apnews.com/article/social-media-tech-kids-new-york-82779a3040385dfc26c6ee15f490234e">new bill in New York</a> that would ban "addictive" algorithmic feeds for teens. Meta has said it would welcome regulation (meaning regulation that it likes) and has also suggested that it would like the responsibility of age gating and parental consent to download social apps to be shifted over to Apple's and Google's app stores.Perhaps instead of thinking of social media like cigarettes when it comes to warning labels, there's another analogy that might help us make more sense of this moment: the parental-advisory stickers on music albums with explicit lyrics.Social media is maybe closer to music than cigarettes, at least in terms of the First Amendment (there's a far more obvious speech implication than the right to rip heaters in the airport).In the mid-1980s, a group called the Parents Music Resource Center, which included a handful of influential wives of senators like Tipper Gore, was incensed over songs like Prince's "Darling Nikki" and Judas Priest's "Eat Me Alive" and urged record labels to attach warnings to albums containing explicit lyrics. The pressure campaign culminated in a media spectacle when John Denver, Frank Zappa, and Dee Snider of Twisted Sister spoke before Congress to defend their art.Ultimately, no law was passed — the parental-advisory labels you still see on albums today resulted from self-regulation by the record industry to avoid actual regulation: a compromise between the Recording Industry Association of America and the PMRC. (This <a target="_blank" href="https://www.jstor.org/stable/853600?mag=parental-advisory-the-story-of-a-warning-label&amp;seq=1">1999 article from the journal Popular Music</a> is a fun read if you're interested in diving deeper into the history of the warning labels.)A surgeon general's warning label that pops up when you open Instagram is probably going to be annoying, but it can't really hurt. So, hey, why not? I'm not going to sit here and say a government effort to help teen mental health is bad. Go for it.But I am skeptical of how effective it will actually be, and how much political capital might be wasted on this instead of the hard, unfun work of coming up with regulations on social platforms that are actually effective.

Social media isn't like cigarettes, and a warning label won't help much

The government has made it mandatory for <keyword id="5293670" type="General" weightage="20" keywordseo="ott-platforms" source="Orion">OTT platforms</keyword> to display an anti-tobacco health warning as a prominent static message at the bottom of the screen during the period of a tobacco product being displayed or used in a programme, the Rajya Sabha was told on Tuesday. The government notified the Cigarettes and other Tobacco Products (Prohibition of Advertisement and Regulation of Trade and Commerce, Production, Supply and Distribution) Amendment Rules, 2023 on May 31, Minister of State for Health SP Singh Baghel said in a written reply. According to the amended rules, every publisher of online curated contents displaying tobacco products or their use has to display anti-tobacco health spots, for a minimum 30 seconds each at the beginning and middle of the programme. They have to display anti-tobacco health warning as a prominent static message at the bottom of the screen during the period of display of the tobacco products or their use in the programme, and also display an audio-visual disclaimer on the ill-effects of tobacco for a minimum 20 seconds each in the beginning and middle of the programme, Baghel said in his written reply. Moreover, all compliance is ensured as per Rule (6) of the Amendment Rules, 2023 through an inter-ministerial committee consisting of representatives from the Ministry of Health and Family Welfare, Ministry of Information and Broadcasting and Ministry of Electronics and Information Technology.

Now, OTT platforms must display anti-tobacco warnings onscreen

<ul class="summary-list"><li>A study found higher rates of emphysema among marijuana smokers compared to tobacco smokers of the same age. </li><li>The study author told Insider her research raises concerns that marijuana smoking is not safer than cigarettes. </li></ul>A small study found higher rates of <a href="https://www.mayoclinic.org/diseases-conditions/emphysema/symptoms-causes/syc-20355555">emphysema</a>, a lung condition that causes shortness of breath, among <a href="https://www.insider.com/can-you-die-from-marijuana-thc-overdose-2022-11">marijuana smokers</a> compared to tobacco smokers of the same age. The study also suggests marijuana and tobacco use together could be more harmful than tobacco use by itself. <a href="https://med.uottawa.ca/radiology/people/revah-giselle">Dr. Giselle Revah</a>, a cardiothoracic radiologist at The Ottawa Hospital and the study's lead author, looked at chest CT scans at Ottawa Hospital from 2005 to 2020 and identified 56 patients who reported using marijuana.The majority of <a href="https://www.insider.com/guides/health/mental-health/marijuana-effects">marijuana</a> users — 50 out of the 56 patients — said they also smoked cigarettes. She compared them to 33 tobacco-only smokers and 57 non-smokers. The tobacco-only smokers' ages skewed higher because Revah collected these patient chest CTs through her hospital's <a href="https://www.insider.com/woman-shoulder-pain-lung-cancer-spread-symptoms-2022-9">lung cancer</a> screening event, which was open to patients over 50 who self-reported as heavy smokers. Marijuana smokers in her sample tended to get a chest CT for reasons unrelated to emphysema. When the radiologist matched tobacco-only smokers to marijuana smokers of the same age, marijuana smokers had higher rates of emphysema: 93% (28 out of 30) compared to 67% of similarly-aged tobacco smokers. The radiologist found marijuana users overall — including younger people who hadn't been exposed to as much smoke — had significantly higher rates in particular of paraseptal emphysema, a rare form of the condition that damages tiny ducts which connect the lung's air sacs.The way marijuana smokers use the drug might damage air sacs. Marijuana users tend to take deep breaths and hold smoke in for longer, causing pressure changes that can irritate the lung's air sacs, Revah told Insider."The main message of the whole study is there's this public perception that marijuana is safe; people believe that it's safer than cigarettes," Revah said. "And this study raises concerns that maybe marijuana's not as safe as everyone thinks it is, and suggests that ultimately we need more robust research before we can make sweeping conclusions."The paper sheds light on the under-researched health effects of marijuana. Literature on the chest CTs of marijuana smokers is sparse, Revah said, since Canada only legalized the drug in 2018. The US has not legalized cannabis nationally, and getting<a href="https://www.science.org/content/article/cannabis-research-database-shows-how-us-funding-focuses-harms-drug"> funding for marijuana research</a> involves cumbersome legal steps. Lung doctors told Insider more research is needed on the health effects of <a href="https://www.insider.com/guides/health/what-does-marijuana-do-to-your-heart">marijuana use</a>. <a href="https://wexnermedical.osu.edu/find-a-doctor/philip-diaz-md-2020">Dr. Philip Diaz</a>, a pulmonary disease physician at the Ohio State University Wexner Medical Center who was not involved in the study, said because most marijuana smokers in the study were cigarette smokers, it's possible smoking both marijuana and cigarettes increases risk for lung damage. But Diaz stressed the results of the small study should not be overstated."You don't want to dilute the fact that it's really the cigarette smoking that's the problem," Diaz told Insider. "I think all you could say is there could be some increased risk if you do both."Revah said she is working on a prospective study that asks patients how much marijuana they use, and hopes a larger study will confirm her findings. <a href="https://www.lung.org/media/meet-our-experts/al-rizzo">Dr. Albert Rizzo</a>, the chief medical officer at the American Lung Association, told Insider scientists and doctors need longer, in-depth studies on the long term health effects of marijuana, especially as the drug quickly <a href="https://www.businessinsider.com/legal-marijuana-states-2018-1">becomes legalized in states across the US</a>. "I think this study's a good one in trying to show or support the fact that airway use of marijuana leads to problems, emphysema being among them," Rizzo told Insider. "Smoking marijuana is not safe, and we don't know really what the long term effect of smoking marijuana is."

"We have defenses in our upper airway to trap larger particles and prevent them from getting down into the lungs. These are sort of the right size to get past those defenses," Hill said. "When those particles get down into the respiratory space, they cause the body to have an inflammatory reaction to them."<h2>What is the risk of wildfire smoke?</h2>Fine-particle air pollution can cause inflammation in the lungs and reduce heart function – lasting effects similar to smoking cigarettes or exposure to diesel exhaust, <a href="https://www.nytimes.com/2022/08/09/well/live/fire-smoke-lung-cancer.html" target="_blank" rel="noopener">the New York Times reported</a>. Poor health outcomes. That's why.The Centers for Disease Control and Prevention is now warning people who smoke both vapes and cigarettes about the negative health effects. "Dual use of tobacco products is a health concern because it may result in greater exposure to toxins and worse respiratory outcomes than using either product alone," the CDC said in a <a href="https://www.cdc.gov/nchs/data/databriefs/db475.pdf" target="_blank" rel="noopener">newly released study</a> on e-cigarette (or vape) use in adults in 2021. Use of both products was highest in adults aged 18 to 44, the CDC found. Vapes have previously been marketed as a way to quit smoking cigarettes. A <a href="https://www.cnbc.com/2019/09/05/juul-accused-of-illegally-advertising-vaping-as-a-way-to-quit-smoking.html" target="_blank" rel="noopener">past campaign</a> for Juul, an e-cigarette company that was federally <a href="https://www.businessinsider.com/juul-labs-vape-agrees-pay-438-million-probe-teen-marketing-2022-9" target="_blank" rel="noopener">investigated for its marketing to young people</a>, encouraged people to "make the switch" to e-cigarettes. It seems some of those people just ended up smoking both."The Food and Drug Administration has not found any e-cigarette to be safe and effective in helping smokers quit. The fact is, e-cigarettes are tobacco products too," reads a <a href="https://www.lung.org/quit-smoking/e-cigarettes-vaping/quit-dont-switch" target="_blank" rel="noopener">statement</a> from the American Lung Association, a nonprofit group. Among all American adults, the CDC study found that 10.2% smoked only cigarettes, 3.2% smoked only e-cigarettes, and 1.3% smoked both. E-cigarette use was greatest in young adults aged 18 to 24, where 11% reported using them. Most adults in that age category reported that they never smoked cigarettes. "Despite this, the percentage of adults aged 18-24 who were dual users of e-cigarettes and cigarettes was similar to the percentage among adults aged 25-44 (1.8% compared with 2.0%)," the study found. Meanwhile, adults aged 45 and over largely preferred cigarettes only, reporting the lowest percentages of e-cigarette use or dual use.

The Biden administration wants all cigarettes to have minimal or nonaddictive levels of nicotine, sources told The Journal, but it could take several years for the regulations to take effect. Insider contacted the Food and Drug Administration for comment.Nicotine is an addictive chemical found in cigarettes and other tobacco products. The FDA began a wideranging review of <a href="https://www.fda.gov/tobacco-products/ctp-newsroom/fdas-comprehensive-plan-tobacco-and-nicotine-regulation">tobacco and nicotine regulation in 2017</a> in a bid to tackle addiction, protect young people and help smokers to quit. While cigarettes are known to cause cancer, heart <keyword id="6361612" type="UNKNOWN" weightage="60" keywordseo="disease" source="Orion">disease</keyword>, and lung disease, the FDA said the thousands of other components in cigarette smoke rather than <a href="https://www.fda.gov/tobacco-products/health-effects-tobacco-use/nicotine-why-tobacco-products-are-addictive">nicotine itself were to blame</a>. The most recent data from the <a href="https://www.cdc.gov/tobacco/data_statistics/fact_sheets/fast_facts/index.htm#:~:text=Cigarette%20smoking%20is%20responsible%20for,or%201%2C300%20deaths%20every%20day.">Centers for Disease Control and Prevention</a> (CDC) show that more than 480,000 deaths in the US each year are caused by cigarette <keyword id="12427629" type="UNKNOWN" weightage="60" keywordseo="smoking" source="Orion">smoking</keyword>, with more than 41,000 deaths related to secondhand smoke exposure, or passive smoking.In the US each day, about 1,600 youths smoke their first cigarette and nearly 200 start smoking every day, per the CDC. It also found that flavored tobacco products <a href="https://www.cdc.gov/tobacco/data_statistics/fact_sheets/youth_data/tobacco_use/index.htm#:~:text=Flavorings%20in%20tobacco%20products%20can%20make%20them%20more%20appealing%20to%20youth.&amp;text=In%202021%2C%2080.2%25%20of%20high,tobacco%20product%20during%20that%20time.">are popular with young people</a>.Meanwhile, the UK is mulling <a href="https://www.theguardian.com/society/2022/jun/09/smoking-age-england-should-rise-year-each-year-government-review-says#:~:text=The%20age%20at%20which%20people,effort%20to%20make%20smoking%20obsolete">increasing the smoking age</a> by 12 months every year until no one can no longer buy it. The FDA has been focusing on lowering nicotine levels since the 1990s as it attempts to crack down on cigarette sales to minors, but the <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1117833/">Supreme Court </a>had found the FDA wasn't permitted to do so at the time.In 2009, the <a href="https://www.fda.gov/tobacco-products/rules-regulations-and-guidance/family-smoking-prevention-and-tobacco-control-act-table-contents">Family Smoking Prevention and Tobacco Control Act </a>gave the FDA the authority to regulate the manufacture, distribution, and marketing of tobacco products including lowering nicotine levels, based on scientific evidence.The FDA also proposed a ban in April on <a href="https://www.fda.gov/news-events/press-announcements/fda-proposes-rules-prohibiting-menthol-cigarettes-and-flavored-cigars-prevent-youth-initiation">menthol cigarettes and flavored cigars</a> that aimed to stop young people taking up and significantly reduce tobacco-related diseases and deaths.
