The hospitals across West Bengal, both public and private, are overflowing with <keyword id="6360092" type="General" weightage="20" keywordseo="dengue" source="Orion">dengue</keyword> patients. According to an official in the state health department, 800-900 cases are being reported on an average daily. This could send West Bengal to the top of the list among the states in the country where dengue cases have been reported, in just over a month. According to the figures released by the National Center for Vector Borne Diseases Control (NVBDCP), West Bengal had reported only 239 cases till August 31. In the 40-odd days since, the number of cases in the state has surpassed 20,000, the state health department admits. Apart from Kolkata, most cases are from districts like North 24-Parganas, Howrah, Hooghly, Murshidabad, Siliguri and Darjeeling. "Lack of proper sanitation, clogged drainage systems, accumulation of rain water due to unplanned urbanisation are some of the reasons behind this spurt. There has certainly been some negligence on the part of civic bodies entrusted with monitoring the situation and combating the threat. Sufficient intervention was not carried out and efforts weren't made to prevent the accumulation of water near human settlements. The presence of cattle sheds, encroachments and the like in urban and semi-urban areas added to the mess," the official in the state health department said. There was hardly any activity by the civic bodies over the last fortnight due to Durga Puja celebrations across the state. State government offices -- including civic bodies -- have remained closed since October 1 and will open from Monday. Health officials hope that measures will be taken on a war-footing thereafter to prevent further spread as hospitals have started running out of space, despite opening temporary wards for the dengue patients. West Bengal's record hasn't been too good, so far as combating the disease is concerned. In 2015, it recorded fourth highest figures after Delhi, Punjab and Haryana. In 2016 and 2017, West Bengal was at the top of the list. No records were available for the next two years from the state. In 2020, West Bengal was again in second position after Punjab. Doctors say that more than the figures, people's suffering is what matters. Dengue cases in Bengal shot up by 20,000 in a month

While Delhi and many other states including Uttar Pradesh and West Bengal have seen a sudden spurt in <keyword id="64895" type="General" weightage="20" keywordseo="dengue-cases" source="Orion">dengue cases</keyword> in the last couple of days, government data showed that the number of <keyword id="6360092" type="General" weightage="20" keywordseo="dengue" source="Orion">dengue</keyword> cases has recorded a significant rise in the last few years. Moreover, states like Uttar Pradesh, Punjab and Rajasthan witnessed the maximum number of cases of dengue during 2021 among all the states. Uttar Pradesh, Punjab and Rajasthan recorded 29750, 23389 and 20749 dengue cases respectively during 2021. Delhi also recorded a significantly higher number of cases at 13089 during the same year. A total of 1.93 lakh cases were reported from all across the country in 2021. Government data revealed that 2020 had witnessed a remarkably lower number of 44,585 cases probably due to the Covid related restrictions in most parts of India as against 1.57 lakh cases in 2019. This year, a considerable number of dengue cases have been reported from Delhi and many cities of Uttar Pradesh. As per reports, UP has so far registered over 2,200 dengue cases, with the state capital Lucknow reporting over 300 cases. Similarly, Delhi has also recorded nearly 1000 cases. Officials said that October can be the most vulnerable period for the spread of dengue in Delhi and recent spells of rain have only added to the woes. Recently, the Delhi government asked hospitals to reserve 10-15 per cent beds for patients of vector-borne diseases. Government data showed that 346 deaths were reported across the country due to dengue in 2021 as against 56 deaths in 2020 and 166 deaths in 2019. The central government intervenes whenever there is a sudden rise in dengue cases. Reports said that a six-member team has been sent to Uttar Pradesh very recently by the <keyword id="83655" type="General" weightage="20" keywordseo="Union-Health-Ministry" source="Orion">Union Health Ministry</keyword> to collaborate with the state authorities in taking public health measures for dengue management in the districts of Firozabad, Agra and Etawah. The National Centre for Vector Borne Diseases Control (NCVBDC) is the nodal agency under the umbrella of the National Health Mission for Vector Borne Diseases (including dengue) prevention and control in the country. The NCVBDC has issued multiple advisories to the States/UTs for detection and control of outbreaks, preparedness and capacity building to take effective measures for vector control, involvement of the non-health sector, ensuring availability of drugs, diagnostics, insecticides, equipment and to fill up the vacant positions of entomologists. Moreover, training has been imparted to doctors on case management and to entomologists on integrated vector control at the central and state level. As per a parliament reply on August 2, 2022, free diagnostic facilities through 769 Sentinel Surveillance Hospitals and 17 Apex Referral Laboratories have been made functional. 1.92 lakh IgM tests have been provided by the Centre. It said that the NCVBDC provides technical guidelines for prevention, control, and management of dengue cases in States/UTs. This study included more than 7,000 participants from the US, aged between 18 and 100, who were assessed before the pandemic (from 2014 onwards), early in the pandemic in 2020, and then later in the pandemic in 2021 or 2022. The researchers found significant declines in extroversion, openness, agreeableness and conscientiousness in 2021 and 2022 compared before the pandemic. Aman Khandelwal, a 23-year-old pursuing his CA finals, said that he had started having a lot of mood swings lately. He had also noticed his concentration level had gone down significantly, affecting his preparations for his exams in November. “Before Covid, my concentration level was much higher than what it is now. And this is making me more irritated and frustrated all the time. I’ve become so moody lately and this is hampering my studies a lot,” Khandelwal said. According to a WHO report, there was at least 25% rise in depression and anxiety cases within young people in 2020, as compared to 2019. However, doctors say this is not limited to teenagers and young adults alone. “Middle-aged adults between 30-64 years also showed changes while the oldest group of adults showed no statistically significant changes in their behaviour. These changes, whether they are temporary or permanent, are not known yet,” Dr Vachna told Business Insider India. India prides itself in being a galloping economy with great ideas and cultural diversity. 
 
However, it also stands out for its colossal neglect of some basic factors---such as health. 
 
For instance, Bill Gates recently made an embarrassing comparison of India with Ethiopia. The African country, he said, was reportedly terrible 10 years ago. However, it has become very good today, and continues to improve due to efficient surveillance systems. Worryingly, the world’s problem areas include parts of India, Pakistan, Nigeria and the DRC, he said. 
 
The <keyword id="102649" type="ORGANIZATION" weightage="20" keywordseo="Gates-Foundation">Gates Foundation</keyword> 
 To streamline processes, the Bill and <keyword id="142497" type="ORGANIZATION" weightage="20" keywordseo="Melinda-Gates-Foundation">Melinda Gates Foundation</keyword> recently launched the Child Health and Mortality Prevention Surveillance Network, or CHAMPS. It is putting some special focus on improving the structure and getting systems into place, said Bill Gates, to <a href="http://www.theatlantic.com/health/archive/2015/05/bill-gates-child-health-mortality-prevention-surveillance-network/392549/">The Atlantic</a>. 
 
For instance, it is not clear why neonatals die, yet their project is trying to probe the drivers of mortality for all children. The Gates announced that they are investing $75 million in a number of surveillance sites expected to collect information "about how, where and why children are getting sick and dying.” The plan will be spread first through six nations in Africa and South Asia, depending on field workers to take the autopsies of children who have died, especially in “beefed-up laboratories.” 
 
Disease surveillance system 
 The Integrated Disease Surveillance Programme (IDSP) was launched in India with the support of the World Bank in November, 2004, so that the country could detect and respond to disease outbreaks quickly. In March 2010 to March 2012, World Bank funds were made available for <keyword id="2858752" type="UNKNOWN" weightage="20" keywordseo="Central-Surveillance-Unit">Central Surveillance Unit</keyword> as well as nine identified states (Uttarakhand, Rajasthan, Punjab, Maharashtra, Gujarat, Tamil Nadu, Karnataka, Andhra Pradesh and West Bengal). The other 26 states were funded only by local budgets. 
 
The Surveillance Programme will continue well into the 12th Plan (2012-17) under the National Health Ministry, with an outlay of Rs 640 crore from the domestic budget. 
 
The World Bank’s focus was to strengthen the system for epidemic-prone diseases, improve central monitoring and coordination, improve state and district surveillance focusing on nine states, gear up and prevent influenza epidemics. 
 
Findings of the project 
 
The project seemed to have detected a few parameters for progress in surveillance as well as in identification and response, related to IDSP and also assessed the infrastructure and systems in place. A number of future goals have been evolved to improve the public health information and intervention. 
 
Only recently, it was found that ratings for the Integrated Disease Surveillance Project (IDSP) for India were just moderately satisfactory. The survey factored the risks to development outcome, bank and borrower performance. 
 
Unfortunately, Non-Communicable Diseases (NCDs), which should be a core, important public health function in India became part of the surveillance survey very late. The NCD risk factor was thus delayed and was improved only in some provinces, not countrywide. Ultimately, the country plans to integrate the NCD factor into the national programme. 
 
Inequalities 
 
The solutions to fight epidemics and diseases are distributed unevenly among different groups and communities, impacting socially and economically sensitive groups. Unfortunately, the inequitable system of distribution in India is glaringly evident in the implementation of the system. 
 
Take TDAP (Tetanus, diphtheria and pertussis), for instance. At first, the programme was not implemented with efficiency. It was restructured only in 2010 in the nine “best performing states”, selected on the basis of their performance as well as administrative and reporting efficiency. It was an ironical conclusion, as the states that needed support lagged behind the most, yet got the least central assistance. 
 
The twin outbreaks of dengue and chikungunya were a major public health concern and pain point a few years ago. The Aedes mosquito-borne chikungunya, with more than a million suspected cases in India, could be controlled by general hygiene and sanitation, but the disease was not shaken off due to the problems in the sanitation, hygiene and environment. 
 
Amazingly, the health minister at the time, Anbamoni Ramadoss, actually commented that the health ministry can't do much in the job! “(We need) involvement of general public, the associations and the civil society," he said. 
 
Efficient implementation 
 
The centre cannot wash off its responsibilities, but the execution is too erratic. For instance, Andhra Pradesh has noted that a special sanitation drive was launched, including massive disease surveillance, to improve the vaccination system in its state. In Gujarat, traditional medicines have been harnessed to fight the disease. 
 
The surveillance system reveals that a number of measures are being identified and put in place--for instance, deployment of trained personnel, appropriate technology as well as efficient administrative services. Establishing extensive laboratory networks are needed for careful investigation. 
 
So far, the ratings of the system have been just moderately satisfactory. Hence, the nation should gear up and streamline the administration, which is crying out for attention.
