Drug maker Cipla Limited on Sunday announced the launch of experimental anti-viral drug remdesivir under its <keyword id="9441113" type="UNKNOWN" weightage="20" keywordseo="brand-name-cipremi">brand name Cipremi</keyword>. Remdesivir is the only US FDA approved Emergency Use Authorisation (EUA) treatment for adult and paediatric patients hospitalized with suspected or laboratory confirmed COVID-19 infection. In May, US drug maker Gilead Sciences extended a voluntary non-exclusive license to Cipla to manufacture and market Cipla's generic version of remedisvir called Cipremi. The Drug Controller General of India (DCGI) has granted Cipla regulatory approval for restricted emergency use in the country as part of the accelerated approval process considering the urgent and unmet medical need, the company said in a statement. As part of a risk management plan, Cipla will provide training on use of the drug, inform patient consent documents, conduct post marketing surveillance as well as conduct a Phase IV <keyword id="122283" type="PERSON" weightage="20" keywordseo="Clinical-trial">clinical trial</keyword> on Indian patients. "Cipla appreciates the strong partnership with Gilead to bring remdesivir to patients in India. We have been deeply invested in exploring all possible avenues to save millions of lives impacted by Covid-19 pandemic, and this launch is a significant milestone in that direction," said Umang Vohra MD and Global CEO, Cipla. According to a preliminary report from the ACTT-1 (Adaptive COVID-19 Treatment Trial 1) study, a randomized clinical trial conducted with remdesivir on 1,063 patients over 60 centres across US, Europe and Asia demonstrated a faster time to clinical recovery in hospitalised patients as compared to placebo. Most of these patients were on oxygen therapy of which some were receiving high flow oxygen or non-invasive ventilation, and some were on a mechanical ventilator. The mortality rates in the study were 7.1 per cent in those given remdesivir and 11.9 per cent in those who were given placebo. In May, Gilead Sciences signed non-exclusive voluntary licensing agreements with six Indian pharmaceutical companies to boost the supply of remdesivir, the experimental antiviral drug that has shown promise in Covid-19 treatment. These companies were -- Dr. Reddy's Laboratories Ltd., <keyword id="9441116" type="COMPANY" weightage="20" keywordseo="zydus-cadila-healthcare-ltd-">Zydus Cadila Healthcare Ltd.</keyword>, Biocon company Syngene, Hetero Labs Ltd.; Jubilant Lifesciences; and Cipla Ltd. Media reports said that Hetero Labs Ltd has also received regulatory approval from DCGI but the drug maker was yet to make it official. Cipla said it will be commercialising remdesivir through its own facilities and partnered sites. The drug will be supplied through government and open market channels, to ensure equitable distribution. There are no medicines approved by the FDA as safe and effective to treat people in the hospital who have COVID-19. Therefore, the FDA has authorized the emergency use of remdesivir for the treatment of COVID-19 under an EUA, said the agency. Remdesivir is given to patients through a vein one time each day for up to 10 days depending on recommendations of healthcare providers, according to the FDA. The drug may help decrease the amount of the coronavirus in bodies and help patients get better faster, said the agency. US FDA approves emergency use of Remdesivir for COVID-19 patients

Experts of the <keyword id="6364396" type="General" weightage="20" keywordseo="ICMR" source="Orion">ICMR</keyword>'s National Task Force on COVID-19 for the fourth time have unanimously rejected the inclusion of <keyword id="9385138" type="General" weightage="20" keywordseo="anti-viral-drug" source="Orion">anti-viral drug</keyword> <keyword id="10880838" type="General" weightage="20" keywordseo="Molnupiravir" source="Orion">Molnupiravir</keyword> in the coronavirus treatment guidelines saying its "known and unknown harms far outweigh its claimed benefit". Responding to a question over Molnupiravir's inclusion in the treatment guidelines amid doctors prescribing it, ICMR Director General Dr Balram Bhargava said experts have had extensive debate and discussions and reviewed the data available from three trials on the drug on Monday. "We had extensive deliberation and the final conclusion was that Molnupiravir has certain risks that warrant caution in its use. Experts present in the meeting opined that there is a rampant and irrational use of Molnupiravir. Efforts should be made to restrict its use as known and unknown harms far outweigh its claimed benefit," he said on Wednesday. "The currently available synthesised evidence was reviewed and the members unanimously agreed that it does not merit the inclusion of Molnupuravir in the national treatment guidelines for COVID-19," Bhargava said. The emerging evidence will constantly be reviewed, he added. "And lastly it was further highlighted the current clinical window of application appears extremely narrow for Molnupiravir with relevance only to the following --elderly, unvaccinated, with other comorbidities. ICMR experts reject inclusion of Molnupiravir in COVID-19 treatment guidelines for fourth time citing safety concerns

Amid the ongoing resurgence of new Covid-19 cases in India, an expert panel of the Central Drugs Standard Control Organisation on Tuesday approved Merck's anti-viral drug molnupiravir for restricted use in emergency situations. Earlier this year, the Sun Pharma had signed a non-exclusive voluntary licensing agreement with US-based Merck to manufacture and supply a generic version of molnupiravir in over 100 low and middle-income countries (LMICs) including India. The DCGI, based on the review of clinical data of molnupiravir has approved molnupiravir for treatment of adult patients with Covid-19, with oxygen saturation less than 93 per cent and who have high risk of progression of the disease, including hospitalisation or death. "Molnupiravir is an important addition to the portfolio of oral therapies available for treating Covid-19 patients. In line with our consistent efforts to accelerate access to new drugs for Covid-19 treatment, we will make Molxvir available to patients at an affordable price," said Kirti Ganorkar, CEO of India Business, Sun Pharma, in a statement. Ganorkar added that the company is in the process of launching a toll-free helpline to ensure the availability of Molxvir to doctors and patients across India. "Our endeavour is to make the product available in a week's time," she said. The recommended dose of the drug is 800 mg twice a day for five days. The duration of treatment of molnupiravir is much shorter compared to other therapies which is a significant advantage as it reduces the pill burden and enhances compliance. Molnupiravir is an oral anti-viral that inhibits the replication of multiple RNA viruses including SARS-CoV-2. The drug is used for the treatment of non-hospitalised patients with Covid-19 globally. Molnupiravir has been developed by Merck and Ridgeback Biotherapeutics. Beijing, Apr 30 () COVID-19 patients may be at an increased risk of cardiovascular impairment, and should be given proven protective anti-inflammatory therapies to aid survival and recovery, according to a study. Researchers from Peking Union Medical College Hospital in China also highlighted the potential problems with drugs currently being administered to fight the novel coronavirus. The study, published in the journal Frontiers in Cardiovascular Medicine, describes the different ways COVID-19 can trigger serious inflammatory-related cardiovascular problems and provide clinicians with guidance for treating these issues. "We are the first to comprehensively discuss the application of cardiovascular anti-inflammatory treatments for patients severely affected by COVID-19," said Professor Shuyang Zhang, lead author of the research, based at Peking Union Medical College Hospital Beijing, China. "Our study sets out guidance for the selection of specific cardiovascular anti-inflammatory therapies for COVID-19 patients, depending on the severity of disease and a patient's response to therapy," Zhang said. The researchers highlight the known risks to the cardiovascular system of treatments currently being tested on patients with COVID-19. The risk of pneumonia and respiratory distress in COVID-19 patients is well known, but there is increasing evidence of severe cardiovascular problems associated with the disease, they said. "Inflammation plays an important role in the development and complications of cardiovascular diseases and we have seen that COVID-19 patients with greater signs of an inflammatory response are more likely to suffer serious cardiovascular events and are at greater risk of dying," explained Zhang. "We have identified a number of ways that COVID-19 can trigger cardiovascular issues," she said. Zhang noted that the virus could directly infect and cause inflammation of the heart's tissues, aggravate existing cardiovascular problems, or trigger an over-excessive immune response in the body, often referred to as a 'cytokine storm', which leads to the body attacking itself. By examining current promising COVID-19 treatments as well as cardiovascular anti-inflammatory therapies that have been verified in clinical trials with positive results, Zhang and her colleagues highlight potentially effective treatments and suggest ongoing anti-inflammatory treatment to aid recovery. "Many clinical trials have been conducted over the past decade to directly test the feasibility of using different anti-inflammatory agents for cardiovascular protection under various conditions, Accumulating evidence supports their ability to improve cardiovascular outcomes," explained Zhang. "Using current knowledge of cardiovascular anti-inflammatory therapies might be of great value in the management of COVID-19 and we recommend referring to this knowledge and experience in clinical practice and conduct related COVID-19 clinical trials," she said. The researchers warn against using new, pre-clinical drugs for treating COVID-19 because of their unknown efficacy and safety risks. The use of certain anti-viral drugs, some currently under clinical assessment for treating COVID-19, should be used with caution, they said. "Some drugs currently in use for COVID-19 patients such as lopinavir/ritonavir, interferon-alpha, ribavirin, azithromycin, and hydroxychloroquine may actually increase the risk of cardiovascular impairment," Zhang said. "Considering that these drugs may be essential in the clinical management of COVID-19 patients, especially the anti-viral agents, cardiovascular protective strategies are urgently needed to improve the overall prognosis," she added. The US is banking on India’s Hydroxychloroquine medicine — yet experts have their doubts

The study, published in the journal Cell Reports, said that remdesivir potently inhibited the virus which causes Covid-19 in human lung cell cultures and improved lung function in mice infected with the virus. "All of the results with remdesivir have been very encouraging, even more so than we would have hoped," said study author Andrea Pruijssers from the Vanderbilt University Medical Center (VUMC) in the US. "But it is still investigational, so it was important to directly demonstrate its activity against SARS-CoV-2 in the lab and in an animal model of disease," Pruijssers added. These preclinical findings help explain the clinical effect the drug has had in treating Covid-19 patients. Remdesivir has been given to patients hospitalised with Covid-19 on a compassionate use basis since late January and through clinical trials since February. In April, a preliminary report from the multicentre Adaptive COVID-19 Treatment Trial (which included VUMC) suggested that patients who received the drug recovered more quickly. According to the researchers, ongoing clinical trials will determine precisely how much it benefits patients in different stages of the Covid-19 disease. Meanwhile, Pruijssers said, "We also are focusing on how to use remdesivir and other drugs in combinations to increase their effectiveness during Covid-19 and to be able to treat at different times of infection. "Broad-spectrum antiviral drugs, antibodies, and vaccines are needed to combat the current pandemic and those that will emerge in the future," the researchers said. In addition to SARS-CoV-2, studies have shown that remdesivir is effective against a vast array of coronaviruses, including other bat viruses that could emerge in the future in humans. "We want remdesivir and other drugs to be useful both now and in the future," the study authors wrote. Recently, a study published in the journal Clinical Infectious Diseases, estimated that remdesivir could prevent the deaths of as many as 13,647 South Africans by December this year. Earlier this month, the European Commission had authorised remdesivir for the treatment of coronavirus, making it the first drug authorised at the European Union level for the treatment of Covid-19. Remdesivir is an effective coronavirus treatment claims study

<keyword id="6359469" type="CITY" weightage="100" keywordseo="Ranbaxy">Ranbaxy</keyword> has filed a complaint against the US Food and Drug Administration (<keyword id="6354656" type="ORGANIZATION" weightage="100" keywordseo="FDA">FDA</keyword>) in an American federal court for revoking approvals to market generic versions of patented drugs — Astra Zeneca's anti-ulcer pill Nexium and Roche's anti-viral Valcyte. 
 
In its petition, Ranbaxy said that the FDA had exceeded its jurisdiction by revoking the approvals which were granted a few years ago. Ranbaxy called the FDA's decision "arbitrary, capricious and contrary to law." It added, "FDA has no power to correct an alleged 'mistake' it made six years ago". 
 FDA had withdrawn its approvals, saying it had made a mistake in granting them. Ranbaxy had sought FDA’s approval for the drugs manufactured at its <keyword id="203483" type="CITY" weightage="100" keywordseo="Paonta-Sahib">Paonta Sahib</keyword> plant, which the FDA had blacklisted in 2008. 
 
The FDA, while revoking its approvals, said, “Ranbaxy forfeited its eligibility for 180-day exclusivity for generic Valcyte but was silent on whether the company had also surrendered its exclusivity rights for generic Nexium. Ranbaxy in its plea has assumed that its exclusivity for Nexium is most likely to meet a similar fate.”

Ranbaxy Sues US FDA For Withdrawing Approval For Its Drugs

The first batch of DRDO-developed anti-Covid drug 2-DG (2-deoxy-D-glucose) was launched on Monday to treat patients suffering from the deadly disease. The medicine comes in powder form and can be taken with water. Defence Minister Rajnath Singh and Health Minister Harsh Vardhan released the first batch of the 2-DG, an anti-Covid-19 therapeutic application of the drug developed by the Institute of Nuclear Medicine and Allied Sciences (INMAS), a lab of <keyword id="152819" type="COMPANY" weightage="20" keywordseo="Defence-Research-and-Development-Organisation">Defence Research and Development Organisation</keyword> (DRDO) in collaboration with Dr Reddy's Laboratories (DRL). Rajnath Singh handed over the anti-Covid drug to his cabinet colleague Harsh Vardhan who handed over it to Delhi All India Institute of Medical Sciences (AIIMS) director Randeep Guleria at an event here. The Defence Minister would be later distributing around 10,000 doses of the drug to a few hospitals in the national capital. The Drugs Controller General of India (<keyword id="172241" type="COMPANY" weightage="20" keywordseo="DCGI">DCGI</keyword>) granted emergency use nod for the drug last week. The drug could be a game-changer in the battle against pandemic as it helps in faster recovery of hospitalised patients and reduces <keyword id="11257268" type="UNKNOWN" weightage="20" keywordseo="oxygen-dependence">oxygen dependence</keyword>. Clinical trial results have shown that the drug helps in faster recovery of hospitalised patients and reduces supplemental oxygen dependence. Higher proportion of patients treated with 2-DG showed <keyword id="9043621" type="COMPANY" weightage="20" keywordseo="RT-PCR">RT-PCR</keyword> negative conversion in Covid patients. The medicine was found to be safe for Covid-19 patients in phase 2 trials, conducted between May and October last year. It was found to be effective in cutting short the hospital stays of Covid patients and reducing their supplemental oxygen dependence. During the first wave of the pandemic in April last year, INMAS-DRDO scientists conducted laboratory experiments with the help of <keyword id="395778" type="COMPANY" weightage="20" keywordseo="Centre-for-Cellular-and-Molecular-Biology">Centre for Cellular and Molecular Biology</keyword> (CCMB), Hyderabad, and found that this molecule works effectively against SARS-CoV-2 virus and inhibits the viral growth. Based on these results, the Drugs Controller General of India (DCGI) <keyword id="170943" type="COMPANY" weightage="20" keywordseo="Central-Drugs-Standard-Control-Organisation">Central Drugs Standard Control Organisation</keyword> (CDSCO) permitted Phase-II clinical trial of 2- DG in Covid-19 patients in May 2020. The DRDO, along with its industry partner DRL, Hyderabad, started the clinical trials to test the safety and efficacy of the drug in Covid-19 patients. In Phase-II trials (including dose ranging) conducted during May-October 2020, the drug was found to be safe in Covid-19 patients and showed significant improvement in their recovery. Phase-II was conducted in six hospitals and Phase IIb (dose ranging) clinical trial was conducted at 11 hospitals all over the country. Phase-II trial was conducted on 110 patients.
