A single flight plan for a journey from Los Angeles to Paris — passing over the UK — contained two waypoints with the same name.This confused the automatic flight planning system, causing both the primary and the backup to shut down within 20 seconds.The system can usually process 900 flight plans an hour during peak periods, but <a target="_blank" class="" href="https://www.businessinsider.com/air-traffic-control-problems-delays-travel-chaos-holiday-weekend-uk-2023-8">air traffic controllers had to process them manually</a> — reducing its capacity to just 60 an hour, the report found.It also identified numerous factors that meant it took longer to identify and rectify the problem.One such factor was that a Level 2 engineer systems expert was on call but couldn't intervene remotely because their login and password "could not be readily verified due to the architecture of the system."It took the engineer 90 minutes to reach the air traffic control site — just over three hours after the meltdown began.All the traffic regulations implemented due to the incident were lifted after a total of nine-and-a-half hours.Delays and cancellations had knock-on effects on airline schedules, causing further issues for the next week. The report estimated that airlines lost £65 million ($82 million) as a result.The report recommended that NATS, the UK's air traffic control provider, has a Level 2 engineer working on-site during busy travel periods.Its 34 recommendations include government legislation, requiring airports and airlines to review the support available for affected passengers, and for NATS to provide earlier notices of possible disruption.UK Transport Secretary Louise Haigh said, "My department will look to introduce reforms, when we can, to provide air travelers with the highest level of protection possible."A <a target="_blank" class="" href="https://www.businessinsider.com/ryanair-ceo-calls-air-traffic-control-boss-resign-flight-chaos-2024-9">NATS</a> spokesperson said in a statement that it welcomed the report and apologized "for the inconvenience passengers suffered because of this very unusual technical incident. It has consistently been among the 100 busiest airports in the world and was ranked <a target="_blank" href="https://www.worldairportawards.com/worlds-top-100-airports-2024/">as the world's 32nd-best by Skytrax in 2024</a>.On December 9, 2022, the morning shift controller arrived at work at 5:15 a.m. to find their night shift counterpart asleep at the console.The worker was discovered lying across two chairs with a blanket covering them, according to <a target="_blank" class="" href="https://www.atsb.gov.au/sites/default/files/2024-09/AO-2022-065%20%28Final%29.pdf">the report</a>. They were still wearing their headset, and the air situation display (ASD) screensaver was on.After ensuring there was no traffic in the area, the station was handed over to the oncoming controller.The report noted that the Brisbane Centre building was cooler at night, and it was considered normal practice for blankets to be used during night shifts.The ATSB's report blamed multiple night shifts in a short space of time, the worker's low workload, and an ineffective risk management system for the incident. It noted that the worker had 10 years of experience as a controller and had been in their current position for three years. However, due to last minute roster changes, they had worked their seventh night shift in nine days and had not taken the recommended rest period in between.There had been no traffic or scheduled flights in the airspace the controller was responsible for and a system supervisor had been on duty at the front desk of the center.However, the report found that cumulative fatigue was not being effectively managed at the center."Likely due to an underlying lack of resources within Airservices Australia, there was an over-reliance on tactical changes to manage the roster," investigators said.The ATSB said it would update its safety procedures and fatigue risk management regulations in response to the incident.Airservices Australia, which runs the Brisbane Centre, has increased the overall number of air traffic controllers and enhanced internal processes since the incident, according to the report. The ATSB will continue to monitor the issue.In the US, fatigue has been a growing safety concern as <a target="_blank" class="" href="https://www.businessinsider.com/airport-collision-close-call-solutions-sleep-runways-air-traffic-controllers-2024-2">shortages of air traffic controllers</a> have driven up the amount of overtime required for those that remain. Dwarfed by highrises, the East Indian-majority village has narrow streets lined with old quaint bungalows and crosses outside homes. With jazz music streaming in the air, walking around the area can transport you to a simpler time. While Bandra developed rapidly and became home to Bollywood's elite and a burgeoning street shopping hub, residents of Ranwar maintained their old bungalows. The coexistence, however, has come at a price, causing traffic issues and crowding. And now, plans are afoot to give the 300-year-old village a facelift while preserving the past. Talking to PTI, <keyword id="23283968" type="General" weightage="20" keywordseo="Johan-Dharmai" source="Orion">Johan Dharmai</keyword>, BJP secretary and a member of the East Indian community, said, "Locals want to preserve the heritage. One of the steps in the facelift plan is to close vehicle movement and build a walkway so that tourists can take a stroll and enjoy the architecture." Though many houses in Ranwar are now being redeveloped, Veronica Street has maintained its old-world charm. It has a square erected in 1866. However, honking rents the air on this street, as motorists see it as a shortcut to Bandra Reclamation and other areas. Mehboob Studio is just a stone's throw away, and senior citizens living on Veronica Street recall seeing actors Helen and Zeenat Aman around Ranwar. A beautification, restoration and conservation project is being initiated to improve safety, regulate traffic and create awareness about the centuries-old village. Mumbai BJP president and <keyword id="20238930" type="General" weightage="20" keywordseo="Bandra-West" source="keywords">Bandra West</keyword> MLA Ashish Shelar recently met locals to discuss ways to move the project forward. "The beauty of Bandra West is its 'gaothans'. The heritage needs to be protected and showcased. I have asked the government to allocate funds from the District Planning Development Committee (DPDC). <keyword id="23283965" type="General" weightage="20" keywordseo="Ranwar-Village" source="keywords">Ranwar village</keyword> will get a facelift. The work has begun on roads, CCTV cameras, street light illumination, parking and traffic plan will be in place with the help of locals and architects," Shelar told PTI. <keyword id="23283967" type="General" weightage="20" keywordseo="Architect-Samir-DMonte" source="keywords">Architect Samir D'Monte</keyword> has been working with the Ranwar Veronica Street Residents' Association since 2009, and they identified a range of problems that the area faced. D'Monte said improvements were made to open spaces and common areas, roads, sewage and drainage systems, street signage, street lighting and traffic and parking management. Apart from this, individual houses were restored, and an innovative fire system was also introduced. Locals say most residents are senior citizens and want to live peacefully without disturbances by motorists. Kaevan Umrigar of Khaki Tours, who works to create awareness about Mumbai's heritage, said Bandra comprised 23 gaothans that housed fisher folk, Kunbis (a farming community) and the agris community who had salt pans. "Ranwar was a farming village, and the area had paddy fields. Later, these paddy fields made way for urban settlements. These little villages are now trapped in the city. Now, only 10 to 15 gaothans in Bandra have survived," he said. But preserving heritage comes at a cost, and if a 'gaothan' comes under heritage guidelines, it becomes difficult to upgrade a home in the area or live a modern life, Umigar said, adding that heritage should be made attractive by tapping the tourism potential of these areas. 'Gaothans' in Mumbai and the larger Mumbai metropolitan region (MMR) are urban villages that house the city's original inhabitants. The Mumbai suburban district collector and Brihanmumbai Municipal Corporation (BMC) have been roped in for Ranwar's facelift plan. Umigar calls land scarcity a threat to 'gaothans', with only 100 left in the city. "When someone wants to develop gaothans, they want to do it vertically. There is a constant battle between heritage and development. They (villages) survive because the houses are tightly packed and on narrow lanes. So individual houses are not attractive for redevelopment, and parking becomes an issue," he said.

300-year-old Bandra village to get facelift to conserve its glorious past

However, a NOTAM — a notice to flight crews — said that Runway 29 was closed for takeoff until 5:45 a.m.In an <a target="_blank" href="https://archive.liveatc.net/kpwm/KPWM2-Twr-Jun-25-2024-0930Z.mp3">audio recording</a> published by LiveATC.net, the controller can be heard cautioning the Southwest pilots as the plane departs."There is a vehicle on the runway. The runway is closed," he said. Another person added: "I tried warning him."Six minutes later, ground operations asked the controller: "Did you ever get a hold of that Southwest plane once he's airborne or is he still kind of MIA in that regard?""We never talked to the aircraft," he replied.The Portland ATC tower doesn't officially open until 5:45 a.m. Instead, pilots are supposed to communicate on the common traffic advisory frequency (CTAF) so other pilots know what's going on.However, it appears that the Southwest pilots didn't talk on this radio frequency either.Per the ATC recording, the controller told ground operations: "You were on the runway and had to exit, and he wasn't even on CTAF."In a statement, Southwest said it is engaged with the NTSB and Federal Aviation Administration to understand what happened. "After departure, the aircraft continued safely to its destination," it added.Portland International Jetport did not immediately comment, noting the active NTSB investigation.The NTSB said in a statement that a preliminary report will be available in 30 days.The incident is one of several to involve a Southwest plane in recent months. On June 18, Flight 4069 was <a target="_blank" href="https://www.businessinsider.com/southwest-flight-drops-525-feet-above-town-prompting-faa-probe-2024-6">just 500 feet above the ground</a> while still nine miles away from the airport — prompting a low-altitude alert at air traffic control.Also this month, a <a target="_blank" href="https://www.businessinsider.com/southwest-pilot-nearly-crashed-flight-into-the-ocean-report-2024-6">Southwest pilot</a> accidentally sent his plane into a dive before it avoided crashing into the ocean near Hawaii.And in May, a Southwest 737 Max experienced a rare but serious stability issue known as a <a target="_blank" href="https://www.businessinsider.com/boeing-737-max-out-of-service-for-20-days-after-dutch-roll-2024-6">Dutch roll</a>. They include important safety information like one runway being farther away than another, or noting that pilots sometimes confuse runways.General aviation pilots account for 83% of wrong-surface events, the FAA said. But there have also been several recent instances involving commercial airlines.Last year, The New York Times found that <a target="_blank" href="https://www.businessinsider.com/airline-close-calls-happening-multiple-times-nyt-investigation-2023-8">near-misses between aircraft</a> were happening more often than previously thought.In January 2023, an American Airlines plane taxied onto the runway at JFK Airport while a Delta Air Lines flight was about to take off.And in July, an <a target="_blank" href="https://www.businessinsider.com/kentucky-woman-allegiant-air-flight-near-miss-florida-2023-7">Allegiant Air flight attendant was injured</a> when the plane suddenly shot upwards to avoid a nearby private jet at the same altitude.Despite the best efforts of pilots and <a target="_blank" href="https://www.businessinsider.com/becoming-air-traffic-controller-career-pros-cons-of-job-2022-1">air traffic controllers</a>, sometimes collisions do happen.Earlier in February, two JetBlue planes collided, with one plane's wing hitting the other's tail in a de-icing area at Boston Logan International Airport. It was the third plane collision at the airport within a year, although nobody was injured.The dangers were made clear in January when a <a target="_blank" href="https://www.businessinsider.com/japan-airlines-and-coast-guard-both-say-they-were-cleared-for-runway-2024-1">Japan Airlines A350 caught fire</a> after hitting a smaller plane on the runway at Tokyo Haneda Airport.<div type="image" title="Japan Airlines crash" height="2200" width="3300" msid="108182530" hostid="261" sizebytes="506972"></div>During a congressional hearing on February 5, the head of the FAA outlined its efforts to prevent such incidents.Mike Whitaker said <a target="_blank" href="https://www.businessinsider.com/faa-asking-airlines-be-more-vigilant-after-near-incidents-2023-3">close-calls</a> and runway incursions — when a plane is incorrectly situated on a runway — were his first area of inquiry when he took over as administrator last October.He said the FAA is looking into "improved airport signage, and runway and taxiway redesign."An FAA representative told Business Insider its plans included "technologies that provide capabilities to improve controller situational awareness and reduce runway incursions," such as a device that gives an audible and visual alert to controllers.In January, the National Transportation Safety Board released its findings about the plane collision at JFK Airport a year earlier. It said the captain was distracted and confused by instructions from air traffic controllers, while the co-pilot lost track of the plane's location.A collision was only avoided because the controller shouted at the pilots of the other plane to abort their takeoff."Overall, our data shows a recent downward trend in the rate of runway incursions," Whitaker said. "But to drive the number of runway incursions to zero, we must continue to focus on and invest in this priority."<h2>Mandatory overtime</h2>One major cause of near-misses is the <a target="_blank" href="https://www.businessinsider.com/air-traffic-controller-shortage-salary-requirements-plane-collisions-close-calls-2023-8">strained workload of air traffic controllers</a>.Rich Santa, president of the National Air Traffic Controllers Association, previously told Business Insider that many controllers are working six-day weeks and ten-hour days.And The Times reported that some <a target="_blank" href="https://www.businessinsider.com/air-traffic-controllers-workload-stress-alcohol-drugs-faa-close-calls-2023-12">controllers have turned to alcohol and sleeping pills</a> to cope with their grueling schedules.Long hours and irregular shifts also cause fatigue, which in turn leads to more problems at airports — as pointed out by Jennifer Homendy, chair of the National Transportation Safety Board, during a Senate hearing last November."<a target="_blank" href="https://www.businessinsider.com/air-traffic-control-staffing-shortage-causing-flight-cancellations-alpa-2022-6">Air traffic controllers</a> are being required to do mandatory overtime," she said. "It ends up leading to fatigue and distraction, which is exactly what we're seeing as part of these incident investigations. And it all just comes down to the shortage of staffing."A government audit released last June found 77% of critical ATC facilities are understaffed.Paul Rinaldi, a former NATCA president, told Forbes that <a target="_blank" href="https://www.forbes.com/sites/suzannerowankelleher/2023/06/26/faa-lacks-a-plan-for-air-traffic-control-shortage-per-government-report/">shortages began back in 2013</a> due to funding restrictions from budget sequestration."They closed the air traffic control academy," he said. "They looked at reducing hours and many air traffic facilities. They looked at closing and cutting more than 100 federal contract towers, and stopped most modernization projects."It's also a notoriously difficult job, so there are lots of training requirements. When the world was struck by the COVID pandemic, many training programs were paused, in turn delaying certification for controllers.The FAA is trying to combat the shortage in a variety of ways, like streamlining training programs.An FAA spokesperson said it is "accelerating air traffic controller hiring by moving to a year-round hiring track for experienced controllers from the military and private industry," and enhancing training with modernized simulators."We are continuing to work on our culture, processes, systems, and integration of safety efforts to maintain our stellar safety record," they added.<h2>Fatigue measures</h2>It's also put together a panel to review air traffic controller fatigue."The panel will examine how the latest science on sleep needs and fatigue considerations could be applied to controller work requirements and scheduling," Whitaker said during the congressional hearing.That includes "risks associated with controller fatigue resulting from shifting schedules and excessive overtime."For pilots and cabin crew, dealing with fatigue is more apparent because they travel through different time zones, unlike controllers whose biggest risk is exhausting work schedules.Boeing has developed an "alertness model" used for an app called CrewAlert, which anyone can download to assess fatigue risk.Brad Surak, Boeing's vice-president for digital aviation solutions, told BI at last year's Paris Air Show: "Its main purpose is for an aircrew who's operating on a complex schedule."They're changing multiple time zones, they're getting schedules disrupted, and the airline has to replan where they might be based," he added. "We don't just operate the schedule for economics, we take into account the <a target="_blank" href="https://www.businessinsider.com/wizz-air-ceo-safety-comments-pilots-should-fly-tired-2022-6">fatigue level of the crew</a>."Now it looks like controllers' sleep schedules are set to receive more attention too.
