Now, longevity doctors are scrambling to get people to slow down."In our clinic, we are de-prescribing," Dr. Andrea Maier, a leading longevity doctor who runs both a private and a public longevity clinic in Singapore, told Business Insider. "We first have to diagnose what's wrong, what somebody needs, and that might differ."Maier, also a professor of medicine and functional aging at the National University of Singapore, is one of several longevity medicine doctors who told BI that they are recommending patients <a target="_blank" class="" href="https://www.businessinsider.com/supplement-expert-doesnt-take-supplements-eats-vegan-2023-11">stop taking many of the supplements</a> they have learned about online."People think that more helps more, and it's not the case," Dr. Evelyne Bischof, who practices longevity medicine in China, Switzerland, and Israel, told BI. "There are interactions, there are side effects."Bischof and other longevity medicine practitioners say the trend toward overdoing it has been supercharged in recent years by more aggressive and flashy online marketing of longevity supplements. Ads on Facebook, books by longevity influencers, along with <a target="_blank" class="" href="https://www.businessinsider.com/equinox-40000-membership-optimize-longevity-health-years-2024-5">gyms</a> and <a target="_blank" class="" href="https://www.businessinsider.com/rich-people-obsessed-longevity-luxury-hotels-iv-drips-mri-scans-2024-10">resorts</a> are touting antiaging fixes.According to a recent <a target="_blank" href="https://www.mckinsey.com/industries/consumer-packaged-goods/our-insights/the-trends-defining-the-1-point-8-trillion-dollar-global-wellness-market-in-2024">McKinsey survey</a>, demand for healthy aging products is soaring; 70% of American consumers across all age groups are spending more in this category than they used to post-pandemic.Doctors like Maier and Bischof say supplements can slowly build up to dangerous health issues, impacting vital organs like the kidneys and the liver or interacting with other drugs and supplements in toxic combinations.Increasingly, they are seeing concerning results from blood, urine, and other tests on patients who take many different products.<h2 id="ac30069c-c621-4f87-835b-6cece95b1fb0">Overdoing supplements can hurt your health</h2><div type="image" title="hand with lots of different supplements in it" height="3449" width="5142" msid="115111811" hostid="261" sizebytes="403768"></div>In the past, patients typically hadn't invested in healthy aging supplements on their own. They would learn about them for the first time from their <a target="_blank" href="https://www.businessinsider.com/what-is-a-longevity-clinic-executive-physical-2024-5">longevity medicine</a> doctor, said Bischof, who serves patients at hospitals in Shanghai and Tel Aviv, as well as in her own VIP concierge longevity medicine business.Things have shifted since the pandemic. In the public clinic where she works in Israel, she estimates that 20% of patients come in with a laundry list of longevity supplements in their regimen, most of them too highly dosed."Five years ago, it was really the opposite," Bischof said. "It was trying to convince a patient to actually take a supplement — besides the vitamins."In her practice, she has seen through clinical testing how supplements can build up in vital organs. One 40-year-old patient who "really overdid" his longevity supplement doses had a biological age four years older than his actual age, as measured by his blood. He also had suboptimal kidney function.He didn't want to quit taking the pills (Bischof did not name the products), but his subsequent blood tests were only looking worse and worse, and his biological age kept creeping up.Eventually, Bischof was able to convince the patient to stop taking his longevity supplements. "And, of course, the <a target="_blank" href="https://www.businessinsider.com/what-is-biological-age-is-reverse-aging-possible-2023-2">biological age</a> reversed," she said.Others have noticed the same trend.Pharmacologist Myriam Merarchi, founder and CEO of the Swiss fertility and biological age testing company Beyond Genomix, sees a lot of "biohackers" wanting to test their telomeres, since telomere health is tightly correlated with aging. Popular new products touted on TikTok and Instagram include <a target="_blank" href="https://www.businessinsider.com/what-is-nad-longevity-supplement-hollywood-uses-prevent-aging-2024-7">NAD boosters</a> (a popular antiaging supplement among the Hollywood elite), <a target="_blank" href="https://www.businessinsider.com/nestle-and-loreal-are-betting-on-urolithin-a-longevity-supplement-2024-10">urolithin A</a> (ostensibly boosts cellular health, improves muscle strength, and slows aging), and coenzyme Q10 (a popular antioxidant for fertility and aging).Doctors are also seeing a lot of alpha-ketoglutarate (AKG). The compound, being <a target="_blank" href="https://pubmed.ncbi.nlm.nih.gov/37217632/">studied clinically</a> as a potential antiaging supplement, is one of the main ingredients in a new $49 "Longevity Mix" sold by tech biohacker Bryan Johnson.Dietician Naras Lapsys, chief clinical officer at Chi Longevity, a private clinic in Singapore, says many people are piling these newer pills on top of older, popular longevity supplements. That could be resveratrol (used for heart health) or spermidine (touted as being good for rejuvenating cells)."If you're taking a longevity supplement, there's no evidence to suggest at all taking one is good and therefore two is even better and three is even better again," Lapsys said. "A good starting point is to strip down to a lesser number and start measuring."Someone who's dehydrated regularly might want to think twice before taking a supplement like NMN, which can <a target="_blank" href="https://www.biorxiv.org/content/10.1101/2024.04.09.588624v1.full.pdf">build up in the kidneys</a> and cause inflammation. CoQ10 can make blood thinners less effective, and resveratrol might <a target="_blank" href="https://www.mskcc.org/cancer-care/integrative-medicine/herbs/resveratrol">help hormonal cancers</a> like breast cancer thrive and proliferate."If you are just taking supplements because a book has told you or an influencer has told you, if we think about levels of evidence, that sits pretty low," Lapsys said. "Test, don't guess."<h2 id="5a439981-0ef7-46e4-98d0-bdda1f648469">Experts recommend a personalized, data-driven approach</h2>All of this is deeply personal. One patient might benefit from getting more calcium, while another could benefit from ditching their B vitamins."I understand that not everybody can get a doctor," Bischof said. "We're trying to say, just educate yourself on what could be the side effect and how — a little bit — to track it."For patients who are excited about longevity supplements, Bischof recommends cycling them, taking one for a few months, and then pausing instead of taking them continuously throughout the year.When you are taking a new kind of longevity supplement, you should keep tabs on how it is affecting your health in specific ways. Is your <a target="_blank" href="https://www.businessinsider.com/vo2-max-explained-what-is-good-how-improve-2024-3">VO2 max</a> improving? Do you have fewer colds during flu season than you used to? These are the kinds of simple check-ins that can help determine if a supplement is doing something for you."Have at least one marker that you can follow, an objective marker, to make sure that it's actually helping you," Bischof said. "Do not take something where you think it might help. You need to have an objective measure that will confirm that it works for you at this dose, at this frequency, at this age, in your current situation."

Longevity doctors are scrambling to get patients to stop taking flashy, expensive supplements

<ul class="summary-list"><li>Quant giant D.E. Shaw has hired people from non-traditional finance backgrounds for decades.</li><li>The firm's head of recruiting told BI he looks for certain traits when hiring, including "learning agility."</li></ul>It was 2011, and Tim Willenken had just spent the past 12 years — four as an undergrad and eight in a doctoral program — preparing to be a philosophy professor.There was just one problem: He didn't want to be in academia anymore.He "had no idea where to start" looking for a job, knowing just about the professions he had been exposed to in graduate school — doctors, lawyers, and professors, he told Business Insider.To learn about what else was out there, he went through his Facebook friends list and called people working at companies in roles he hadn't heard of before. He eventually spoke to an old college friend he knew through debate club who had previously worked at D.E. Shaw."That was the first time I'd heard of the firm," said Willenken, a senior vice president in the D.E. Shaw's corporate development unit. "I think I'd had one conversation about what a hedge fund was when I applied." D.E. Shaw, started in 1988 by now-billionaire computer scientist David Shaw in New York, is remarkably secretive even for the guarded world of hedge funds, but what little has leaked paints the manager as being on the <a target="_blank" class="" href="https://www.businessinsider.com/first-half-quant-returns-from-de-shaw-two-sigma-renaissance-2024-7">cutting edge</a> of quant finance.While working at the $60 billion fund is now the goal for many finance-focused students at <a target="_blank" class="" href="https://www.businessinsider.com/de-shaws-summer-internship-recruitment-interview-tips-2023-4">top universities</a> and a destination for quants, the manager has also maintained its long history of hiring non-traditional talent. Early hires included crime fiction writer Charles Ardai and Amazon founder Jeff Bezos, and the manager now boasts on its website that employees come "by way of academia, Silicon Valley, or an NGO."Other hedge funds are also hoping to convince academics, technologists, and others to join their ranks instead of staying in their field or working in Big Tech as they grow and compete to have best-in-class talent rosters. These offers are not just down to compensation, which is handsome at hedge funds.The firm's head of recruiting, Jeremy Reff, a former English major at Harvard who has worked at the fund for nearly two decades, is in charge of continuing and improving on this tradition of non-traditional hires. While some who join D.E. Shaw may require additional training or a longer runway to get up to speed, he said, the return on these hires outstrips the costs."There are benefits to having a beginner's mindset," he said, and the firm's expansion into different investment strategy types, such as <a target="_blank" class="" href="https://www.prnewswire.com/news-releases/the-d-e-shaw-group-raises-combined-1-1-billion-for-two-new-funds-focused-on-private-market-opportunities-301746589.html">private investing</a> and discretionary global macro, can be attributed to the curious nature of these hires.  The reality, though, is that a lot of roles don't have exact equivalents to other parts of the industry, so people from outside finance are not on that much different footing as those with more traditional finance backgrounds, he said. "We are looking for traits and attributes more than one particular skill," he said.<h2 id="1ec22262-b503-4604-bafe-92fe9e0ecb78">Mentorship, prerequisites, and 'learning agility'</h2>While the firm is happy to invest ample time in training talented recruits, certain roles still require certain skills. For example, knowing how to program is a must if you're applying to be a <a target="_blank" class="" href="https://www.businessinsider.com/wall-street-top-technology-projects-goldman-blackstone-morgan-stanley-2024-8#d-e-shaw-s-generative-ai-approach-encourages-programmers-to-build-their-own-tools-4">software engineer</a>, Reff said.The firm's learning and development team is structured to get people up to speed on financial markets and where the firm sits within them. The rotational associates program lets new hires work for different teams in different roles so new employees can get a sense of where they fit best.The real learning happens on the job though."The only real way to learn is to get your hands dirty," Willenken said.Reff's job is to find people who can keep up. The interview process is designed to test potential at the firm, including creativity and "learning agility.""There are thousands of indicators for success that we use," he said. "No one is hired solely based on their résumé."   The firm used to consider <a target="_blank" class="" href="https://nymag.com/intelligencer/2019/09/david-e-shaw-college-donations.html">standardized test scores</a> as part of its hiring process but has phased them out over the last few years. "They aren't predictive of success," Reff said.The résumés are generally reviewed blindly, meaning names are not attached to the qualifications and experience.   The process has changed significantly since Reff was hired in 2006, two years after graduating from undergrad; he recalled speaking to his interviewer for a while about the Spanish novel "Don Quixote." Now, it's a more structured process, incorporating industry best practices with the firm's internal metrics and "secret sauce."  "We don't get it 100% correct, of course, and are always tinkering with it," he said.<h2 id="067dcf50-7ee4-431a-b455-7435b3922f71">Academics, doctors, and veterans</h2>The end result is that driven people find their place in the firm, one way or another. Ana Díaz Rivero, a quantitative researcher, "never had an interest in finance," but wanted to leave academia after getting her doctorate in physics.Her role allowed her to "continue pursuing academic research without being in academia," she said."A lot of the skills and tools we use are very similar. The principles behind how we do research are very similar," she said.For those who have joined the firm after working in a non-finance field, incorporating past abilities and skills into their current roles was a big selling point.For some, the biggest carryover from their former careers is habits they learned, like Lindsay Temes's military-trained punctuality and organizational skills. Shaw and found it was a place where she "would be valued and heard."  She overcame some initial hesitancy about financial services thanks to employees from non-traditional backgrounds who stressed that her role wasn't going to be "static."  A former doctor for the UK's National Health Service, Ali said that "the day-to-day is certainly different than working in medicine," but the jump to finance wasn't as daunting as it can seem from the outside.<div type="image" title="Dr. Omar Ali smiles while sitting in a conference room" height="1525" width="2000" msid="114980199" hostid="261" sizebytes="230880"></div>He said firms that focus on their employees' talents and not on fitting people into specific boxes make the transition easy."My real value is not my ability to navigate a balance sheet, it's my practical expertise in the field and my background," he said.<h2 id="900d026f-556a-4784-8b67-9629236daaba">Don't want to 'win' on compensation</h2>Hedge funds are known for paying their employees well, especially compared to fields like academia or government work.Job postings show positions such as a software developer making $200,000 a year, and a summer internship on the fundamental research team for college students is offering $18,000 a month in salary, plus overtime, along with a $10,000 signing bonus, a $10,000 housing allowance, and thousands of dollars in stipends for "self-study" materials and technology equipment.But Reff said, "I never want to win on solely compensation," and those who have joined from outside finance say they were drawn to the firm for a number of reasons beyond pay.Díaz Rivero and Ali knew of D.E. Shaw Research, a computational drug discovery company led by Shaw, which put the hedge fund on their radar. Willenken, the doctor of philosophy, appreciated that his role included the parts of academia he enjoyed most without the siloed approach academic research roles typically require.Timelines are tighter and there's more collaboration on the corporate development team, he said, and "the questions are a little more concrete than in philosophy." But he is using the same muscles to search for answers to questions that don't have easy answers.   "There's not a lot of low-hanging fruit. If there was an obvious answer, someone would have thought of it by now," he said.Reff has a shorthand he uses with potential recruits to describe the firm: "These are a bunch of people who really want to get things right."  "I think that's very attractive to people in a lot of different fields," he said.

The Karnataka State Consumer Disputes Redressal Commission has directed the hospital and two <keyword id="20000183" type="General" weightage="20" keywordseo="doctors" source="keywords">doctors</keyword> to pay Padmavathi, a resident of Jayanagar, Rs 50,000 as litigation expenses. It has also directed <keyword id="22497673" type="General" weightage="20" keywordseo="New-India-Assurance-Co-Ltd" source="keywords" meta.keywordsubtype="org" meta.entityname="New India Assurance Co Ltd" meta.hostid="261" meta.entityid="New India Assurance Co Ltd">New India Assurance Co Ltd</keyword>, which had issued a policy covering the hospital expenses, to pay her Rs five lakh for "the professional and <keyword id="21130249" type="General" weightage="20" keywordseo="medical-negligence" source="keywords">medical negligence</keyword> caused." The then 32-year-old woman underwent the surgery on September 29, 2004 at the <keyword id="22497672" type="General" weightage="20" keywordseo="Deepak-Hospital" source="keywords" meta.keywordsubtype="org" meta.entityname="Deepak Hospital" meta.hostid="261" meta.entityid="Deepak Hospital">Deepak Hospital</keyword>, with the two doctors allegedly performing the hernia operation. On completion of the surgery, her appendix was also removed, according to the Commission's order last month. Immediately, on the next day, she complained of severe pain for which she was prescribed some pain killers and assured that it is post surgery discomfort and that it will get cured. She suffered acute stomach and back pain for several years and was also admitted in the same hospital twice later, the order noted. Padmavathi later approached another private hospital here in 2010 and during a scan, it was noticed that some foreign body existed in the abdominal and back portion of her body and it was suggested to her to get it (surgical needle) removed. She then underwent surgery and the 3.2 cm surgical needle was removed following which she approached the consumer forum with a complaint the following year. The forum noted that the complainant was aged about 32 years, when she underwent all these surgeries and removal of the surgical needle. "Definitely she suffered severe pain and inconvenience till removal of the surgical needle," the order stated. Hence, she is entitled to get "global compensation" of Rs five lakh and the insurance company (New India Assurance Company Limited) is directed to pay the amount while the two doctors are liable to pay litigation expenses of Rs 50,000 to the complainant, it further added.

Doctors said subsequent news reports that she was in remission because of alternative therapies were potentially misleading and dangerous.Macpherson, 60, who is also the founder of wellness company WelleCo, told <a target="_blank" href="https://www.womensweekly.com.au/news/elle-macpherson-now/">The Australian Women's Weekly magazine</a> in an interview published on Monday that she is in remission from breast cancer despite refusing the conventional treatments of <a target="_blank" href="https://www.businessinsider.com/what-is-preventative-chemotherapy-how-it-works-2024-3">chemotherapy</a>, mastectomy, radiation, and hormonal therapy her doctor suggested.She instead opted for what she described as "an intuitive, heart-led, holistic approach."Macpherson said: "In traditional terms, they'd say I'm in clinical remission, but I would say I'm in utter wellness."News outlets around the world reported that Macpherson overcame cancer with alternative therapies.However, Macpherson told the outlet that she had a lumpectomy, a conventional treatment that removes <a target="_blank" class="" href="https://www.businessinsider.com/breast-cancer-risk-screening-age-2023-1">breast cancer </a>along with a small amount of the healthy tissue surrounding it. This led to her 2017 diagnosis of a type of breast cancer known as HER2-positive estrogen-receptive intraductal carcinoma.<h2 id="3cf33fad-0891-45c4-96b5-dd9ce951ecbd">Macpherson's lumpectomy might have treated her breast cancer</h2>A lumpectomy in itself can be an appropriate standard medical treatment for the condition, professor Chris Pyke, a cancer surgeon, breast cancer risk quantification expert, and the director of medical services at the Mater private hospitals in Brisbane, Australia, <a target="_blank" href="https://www.theguardian.com/society/article/2024/sep/04/elle-macpherson-breast-cancer-treatment-australian-womens-weekly-misinformation-risk">told</a> The Guardian.Pyke added that Macpherson's form of cancer, in many cases, could more accurately be described as a type of non-invasive precancer that, if left untreated, has the potential to turn into invasive cancer, although the chances are low."It's quite possible that just removing that lump all by itself could have been sufficient treatment," Pyke said.Macpherson also said she had a previous cancer scare in 2014 when a lump she found in her breast turned out to be a benign cyst. It prompted her to seek out a naturopath, a type of alternative-medicine practitioner, and to found her wellness brand.Dr. Liz O'Riordan, a breast cancer surgeon and advocate for breast cancer awareness, wrote on X in response to the interview: "The potential dangers of choosing alternative treatment for breast cancer can increase your risk of dying by six times. Or "invasive ductal cancer," an invasive form, which is often treated with chemotherapy to prevent recurrence."Without Elle making a comment or her surgeon commenting to explain exactly what type of cancer she has, we don't really know," O'Riordan said.Ceinwen Giles, the CEO of Shine Cancer Support, a charity that supports people who receive a cancer diagnosis in their 20s, 30s, and 40s and who is also in remission, told Business Insider news reports not emphasizing that Macpherson had surgery could be harmful to vulnerable people."There is a lot of stuff out there, particularly on the internet, that I think preys on vulnerable people, and I've been there myself, where you're desperately looking for something that will make you better," she said."I worry with that kind of thing is that people will see it and think, 'oh, well, I'm not going to have any traditional treatments. I'm just going to go down a holistic route because it worked for her,'" Giles said.<div type="image" title="Elle Macpherson" height="2081" width="2775" msid="113069112" hostid="261" sizebytes="303684"></div><h2 id="caa7e7aa-ef87-43cf-8f29-2965f7dc3732">Alternative treatments are not backed by science</h2>Macpherson's holistic treatment involved renting a home in Arizona for eight months, where she was under the care of her primary doctor, who specializes in integrative medicine, an approach that uses both conventional and alternative therapies, as well as naturopath, holistic dentist, osteopath, <a target="_blank" class="" href="https://www.businessinsider.com/guides/health/are-chiropractors-doctors">chiropractor</a>, and two therapists.She told The Weekly that she devoted "every single minute to healing myself."Complementary therapies, such as yoga, therapy, and herbal medicine, are used by some people with cancer alongside medical treatment as they can help them cope with their symptoms better, the charity <a target="_blank" href="https://www.cancerresearchuk.org/about-cancer/treatment/complementary-alternative-therapies/about/difference-between-therapies">Cancer Research UK</a> says on its website.There are many reasons someone might want to try alternative therapies, the charity said, but there is no scientific or medical evidence to show that they can cure cancer. In fact, they might cause harmful side effects or interfere with medical treatment, it said.Patients are also not informed about the possible risks, survival rates, and side effects as they would be with conventional treatment. "That's my worry with alternative medicine. Our nanny also arrives around this time, helping wrangle our two toddlers and baby while I try to get ready and help them prepare for the day.Once our second child was born, we decided a nanny would be helpful for our family. Parenting is not an individual sport.<h3 id="6b867e7c-a20d-4cdc-a5ef-4289073c7792">8 a.m. - 8:30 a.m.:</h3>After breakfast, either my mother or I drop off the toddlers at school. The newborn stays home with our nanny.<h3 id="ec7d99c7-37a8-4e10-abb8-3d9e2713dd3f">9 a.m. - 12 p.m.:</h3>My husband and I work from home every weekday. My workday begins with meetings. I oversee all our providers, offering close supervision and support on their cases. I also focus on product updates and refining the member experience.<h3 id="8df79240-54c5-4433-9ad8-72ab4a0cee7e">12 p.m. - 1 p.m.:</h3>Lunchtime is a brief break from work, where I try to relax and spend time with my baby. It's my time to recharge and recenter.<h3 id="d203c4b3-e441-464b-beb7-cc815bcec4ea">1 p.m. - 5 p.m.:</h3>The afternoon is filled with more provider consultations, clinical research, creating social media content, or meetings. My husband picks the kids up from school.<h3 id="27b042ac-beb5-4820-96c5-4e222c78bf8f">5 p.m. - 7 p.m.:</h3>Dinner time is family time — I block my work calendar to be fully present during these hours. We eat together, talk about our day, and unwind.Our nanny leaves around this time, and my mom rests in her room. The evening usually involves playing with the toddlers and caring for the baby, but by this point, I'm pretty exhausted.I try to remind myself that these years will go by quickly. This is sometimes when I yearn for the days to be longer so I could spend more hours with the kiddos. Guilt tends to creep in, and I wish I had more to give.<h3 id="1cf989b4-869d-48f6-b2ec-cfd2a9975940">7 p.m. - 7:30 p.m.:</h3>Our baby goes down first, and then my husband and I put the toddlers to bed, usually with bedtime stories or some quiet time.<h3 id="25d878b7-7961-4f54-aaf0-2b8c87def575">8 p.m. - 10 p.m.:</h3>I might spend this time with my partner, catch up on a book, or relax. This is also when I sometimes lose track of good <a target="_blank" class="" href="https://www.businessinsider.com/guides/health/sleep-hygiene">sleep hygiene</a> and end up scrolling through Instagram or watching reality TV. On an ideal night, I would prefer to do another 10-minute meditation, and I try when I can, but it's hard.There have been some very tough times, especially when our third child was born, but we also know it's the best time of our lives.<h2 id="6f990ed4-9b65-42ee-8dea-f89135fd0f0d">Communication and planning are essential</h2>My husband and I use shared calendars to keep track of our schedules, which helps us stay coordinated and avoid conflicts. We have occasional epic fails, like forgetting to leave on time to pick up our kids from school or camp.I know my husband doesn't feel guilty. I think it's partially because he's a man, but he also feels very comfortable with all of our different caretakers and that part of the work he does outside our home benefits our family and children.<h2 id="5806e438-54f7-4875-894f-f3a888237ca9">I'm not afraid to ask for help and set boundaries</h2>I don't hesitate to delegate tasks and ask for help. I couldn't have three children if I didn't have other adults in my home helping me mother my children as well as mothering me.I also have boundaries between work and family time. We have a rule not to check work emails while with our children. We've also created family traditions, like weekend outings, to make special memories together.<h2 id="18f4897c-e0e6-4f64-add4-ebde0d3c45d3">Making time for myself is important</h2>I try to meditate daily, journal when I remember to, and stick to an exercise routine that works for me, but I accept that things won't be perfect.There are days when everything feels chaotic, my house doesn't look Instagram-ready, and the balance feels off. Learning to let go of perfectionism and find joy in the moment has been key.My cofounder and my husband are my two critical partners. We each hold the other accountable. The same month, the powerful healthcare conglomerate, which owns the nation's largest <a target="_blank" href="https://www.businessinsider.com/one-medical-patients-affected-unitedhealthcare-mount-sinai-dispute-2024-3">health insurer</a>, got the green light to take over a decades-old independent Oregon <a target="_blank" href="https://www.oregonlive.com/business/2024/03/oregon-health-officials-give-emergency-ok-to-corvallis-clinics-acquisition-by-insurance-giant-unitedhealth-group.html">medical practice</a>, adding another 100 healthcare providers to its roster. It picked up a 400-doctor <a target="_blank" href="https://www.fiercehealthcare.com/payers/unitedhealth-quietly-scoops-new-york-physician-group-crystal-run">group</a> in New York about a year before.These are just the purchases we know about; there could be many more. Physician practice deals often go unannounced and get little attention from regulators, but they add up. Near the end of 2023, <a target="_blank" href="https://www.businessinsider.com/optumhealth-ceo-plans-to-make-clinics-a-100-billion-business-2021-4">UnitedHealth's Optum</a>, the business unit housing its clinics and surgery centers, said it employed or was affiliated with 90,000 doctors. That's a whopping 10% of all doctors in the US.Though it's the hungriest, <a target="_blank" href="https://www.businessinsider.com/optumhealth-ceo-plans-to-make-clinics-a-100-billion-business-2021-4">UnitedHealth</a> is just one buyer in the market for physicians. Little by little, big businesses have taken over healthcare in the US. Hospitals have been scooping up medical practices for decades to create giant health systems. Insurance companies fought back with their own clinic purchases. Private equity firms increasingly staked claims to nursing homes and groups of specialists such as emergency physicians and dermatologists.And in the last few years, tech companies and retailers like Amazon, CVS Health, and Walgreens added thousands of doctors to their payrolls when they bought <a target="_blank" href="https://www.businessinsider.com/cvs-walgreens-amazon-primary-care-acquisition-strategy-2024-3">primary-care chains</a>.Nearly 78% of US doctors are employed by hospitals or other corporate entities, such as insurers and PE firms, according to <a target="_blank" href="https://www.physiciansadvocacyinstitute.org/PAI-Research/PAI-Avalere-Study-on-Physician-Employment-Practice-Ownership-Trends-2019-2023">research</a> from consultancy Avalere and the Physicians Advocacy Institute. For the first time, more medical practices were owned by corporate entities (30%) than hospitals (28%) as of January 2024, the research found.There's mounting evidence that this trend toward corporatization could harm patients. Research has found that prices go up when hospitals and private-equity firms buy medical groups. Sometimes, care gets <a target="_blank" href="https://www.nber.org/papers/w30928">worse</a>, but there's still much unknown about how corporate ownership affects medical care, especially when it's a giant like UnitedHealth or CVS doing the buying.Some doctors and experts in the industry fear corporate owners could prioritize their investors and profits ahead of patients, and doctors could lose the ability to make their own decisions about what's best for a patient."The big guys aren't cheaper. They don't deliver better customer service, they aren't more timely, none of that. They're just able to negotiate higher prices," said Farzad Mostashari, the CEO of Aledade, a company that aims to help primary care practices stay independent.The federal government is starting to pay attention. In March, it <a target="_blank" href="https://www.ftc.gov/news-events/news/press-releases/2024/03/federal-trade-commission-department-justice-department-health-human-services-launch-cross-government">launched</a> an inquiry into PE firms and other corporate owners' "increasing control over healthcare" and how this might threaten patients' access to affordable and quality care.The feds also zeroed in on UnitedHealth, <a target="_blank" href="https://www.wsj.com/health/healthcare/u-s-launches-antitrust-investigation-of-healthcare-giant-unitedhealth-ff5a00d2">reportedly</a> facing an antitrust investigation by the US Justice Department into how its medical group acquisitions affect consumers and competitors. News of the probe surfaced before a <a target="_blank" href="https://www.businessinsider.com/change-healthcare-cyberattack-small-mental-health-practices-payments-money-2024-3">cyberattack on UnitedHealth's</a> technology business, Change Healthcare, caused hospitals and clinics nationwide to forgo billions in payments — raising more questions about UnitedHealth's chokehold on the healthcare industry.<h2>Doctors are hot commodities</h2>It's tough to lump the many buyers of medical practices together, as they're pursuing different strategies. But, in essence, they're all hiring doctors for their relationships with patients, who bring opportunities for more profits. <a target="_blank" href="https://www.businessinsider.com/gen-z-millennials-giving-up-medical-care-cant-afford-costs-2024-2">US healthcare spending</a> reached $4.5 trillion in 2022, representing almost a fifth of the national GDP.Hospitals rely on doctors — especially those who provide primary care — to refer patients for lucrative procedures or surgeries. <a target="_blank" href="https://www.businessinsider.com/unitedhealth-stock-exec-medical-costs-rising-2023-6">Insurers like UnitedHealthcare</a> and CVS' Aetna are required by federal law to spend most of the money they collect in premiums on medical care. Still, they can pocket more of those premiums by sending patients to clinics they own.Pharmacies hope that buying medical practices will give more prescriptions to fill and get more customers to buy over-the-counter products at the front of the store."They all realize that patients will follow what their doctors tell them to do or where their doctors tell them to go," said Chas Roades, a longtime healthcare consultant. "So it allows them to both drive traffic to a downstream profit model that they're interested in growing, and I think it also is a way to get consumers to trust them."Meanwhile, independent doctors have a tough time going it alone. Reimbursement is shrinking, and small practices don't have the ability to <a target="_blank" href="https://www.ama-assn.org/system/files/2022-prp-practice-arrangement.pdf">negotiate higher rates</a> as big hospital systems do. Plus, running a modern medical practice is expensive, requiring investments in staffing, technology, and electronic health records. <a target="_blank" href="https://www.businessinsider.com/ai-chatbots-tech-doctors-medicine-healthcare-system-empathy-quality-email-2023-6">The pandemic</a> made things even more challenging, spurring many doctors to sell their private practices.<h2>The quiet takeover of physician practices</h2>Much of the takeover of physician practices has slid under the radar. That's because acquisitions valued below <a target="_blank" href="https://www.ftc.gov/enforcement/competition-matters/2024/02/new-hsr-thresholds-filing-fees-2024">$119.5 million</a> don't have to be reported to federal regulators, and many practice purchases don't meet that threshold. Publicly traded UnitedHealth, for instance, rarely announces its purchases of medical groups and surgery centers."We're really flying blind here with respect to understanding how many acquisitions by which entities and what geographies and what specialties," said Yashaswini Singh, an assistant professor at Brown University who studies healthcare consolidation.This lack of disclosure has made consolidating doctor practices difficult to study. Research into private equity's effects on healthcare is just emerging, and there's little to no academic, peer-reviewed research on how other corporate owners' acquisitions affect healthcare prices and quality, Singh said.Studies, including Singh's <a target="_blank" href="https://jamanetwork.com/journals/jama-health-forum/fullarticle/2795946">research</a>, have shown, however, that PE ownership leads to steeper prices and more utilization, meaning patients may have more frequent doctor's appointments. Together, that drives healthcare costs higher. Research also suggests that PE ownership of nursing homes and hospitals <a target="_blank" href="https://jamanetwork.com/journals/jama/article-abstract/2813379?guestAccessKey=e0cef9be-d55c-4bcf-8892-412af8f24355&amp;utm_source=For_The_Media&amp;utm_medium=referral&amp;utm_campaign=ftm_links&amp;utm_content=tfl&amp;utm_term=122623">worsens the quality</a> of care. One <a target="_blank" href="https://www.nber.org/system/files/working_papers/w28474/w28474.pdf">paper</a> found that patients who went to nursing homes owned by private equity firms had a 10% greater chance of dying.And yet, private equity companies are rapidly expanding their presence in healthcare. A 2023 <a target="_blank" href="https://www.antitrustinstitute.org/wp-content/uploads/2023/07/AAI-UCB-EG_Private-Equity-I-Physician-Practice-Report_FINAL.pdf">study</a> found that one private equity firm owned 30% of the market share for physician practices in over a quarter of metropolitan areas across the US. A single PE firm owned over half the market share in some areas.<h2>'An unsatisfying way of practicing medicine'</h2>Acquisitions by other corporate giants might not produce the same outcomes as PE firms, said Zirui Song, an associate professor of healthcare policy and medicine at Harvard Medical School who studies private equity in healthcare. PE firms are unique in holding on to their acquisitions for only a few years and saddling their acquired companies with debt and fees. Owners who hang on to their medical groups might be more motivated to improve services, he said.Anecdotally, though, many doctors don't like working for corporate overlords, Roades said. Older physicians who own equity in their medical practice get a nice payday when they sell, but the younger ones left behind are closely managed and measured in terms of cost and productivity, he added.It's "an unsatisfying way of practicing medicine. We're seeing a growing number of disgruntled early and mid-career physicians in these groups who find themselves suddenly working for large corporations, and this is not what they wanted to do with their lives," he said.After a few years working at Optum-owned Oregon Medical Group, Dr. Nick Jones opened a <a target="_blank" href="https://www.businessinsider.com/direct-primary-care-a-no-insurance-healthcare-model-2017-3">direct primary care practice</a>, a type of clinic that charges a monthly fee instead of billing insurance. He said there's a sense of powerlessness for primary-care doctors in the traditional healthcare system — whether they're employed by Optum, a large hospital system, a private-equity firm, or a nonprofit clinic.With thousands of patients to treat, they face a mountain of administrative paperwork and have no control over their schedules. These problems stem from the fact that primary-care doctors are <a target="_blank" href="https://kffhealthnews.org/news/article/compensation-pay-primary-care-shortage-solution/">underpaid</a> in the traditional "<a target="_blank" href="https://www.commonwealthfund.org/publications/newsletter-article/moving-health-care-system-away-fee-service">fee-for-service</a>" healthcare system, in which doctors are paid for every visit, he said.While he felt supported at his previous Optum-owned clinic because of his office manager, "the financial underpinnings and the economics of medicine" made it unsustainable and stressful, Jones said. "There was a three or four month wait for patients to see me. There was still too much on our plate to do, and [during the pandemic] there weren't enough medical assistants or case managers or nutritionists or other support to make 20 patients a day feel like the right thing to do."<h2>Doctors worry corporate giants put profits over patients</h2>The insurers and retailers say care is better and cheaper when integrated under one roof, connected to the health plan, pharmacy, or other provider organizations.For its part, Optum says it wants to improve the health and well-being of people and lower the cost of care with integrated care teams and better technology. It says it's helping clinics transition to providing <a target="_blank" href="https://www.businessinsider.com/optumhealth-ceo-plans-to-make-clinics-a-100-billion-business-2021-4">"value-based"</a> or "accountable" care, which means that its doctors get paid fixed fees upfront that, in theory, encourages them to keep patients healthy and order fewer unnecessary tests.Optum said in a statement it continues "to focus on supporting physicians, advance practice clinicians, and nurses in closing gaps in care and helping the health system perform better for everyone."A spokesperson pointed to a <a target="_blank" href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2799376">study</a> by Optum published in JAMA Network Open in December 2022 that found seniors in value-based care arrangements were less likely than seniors in traditional payment models to visit the emergency room, or be admitted at the hospital for COPD, asthma, stroke, or heart attack.But there are potential dangers. Patients may end up with fewer choices as bigger companies edge out competitors.California health system Emanate Health sued Optum in 2023 for anticompetitive behavior. It alleged that Optum canceled its contracts with Emanate hospitals and steered patients away after Emanate refused to agree to certain terms. Among the terms: Emanate primary-care doctors would serve only Optum patients, and Optum would get first dibs if Emanate decided to sell its primary-care practices.Care might also feel less personal. Some <a target="_blank" href="https://www.businessinsider.com/one-medical-patients-affected-unitedhealthcare-mount-sinai-dispute-2024-3">One Medical patients</a> complained that since the chain folded into Amazon, their appointments were shortened, and certain features for seniors, like having health coaches available during patient visits, went away, according to a report from <a target="_blank" href="https://www.washingtonpost.com/technology/2024/02/28/amazon-health-one-medical/">The Washington Post</a>.<h2>Patients could end up spending less time with doctors and face higher costs</h2>Dr. Mitch Li, an emergency physician who started Take Medicine Back, a group that opposes corporate owners pushing into healthcare, said patients will spend less time with their doctors and potentially face higher costs from predatory billing practices."The corporate practice of medicine is the reason why <a target="_blank" href="https://www.businessinsider.com/gen-z-millennials-giving-up-medical-care-cant-afford-costs-2024-2">healthcare costs</a> are out of control," Li said.Above all, some doctors fear having to make decisions that benefit the owner's bottom line, even though they may not be the best option for a patient.Dr. Michelle Cooke, a primary-care physician, previously practiced at an Atlanta clinic owned by a hospital system. Though the system was not-for-profit, it didn't operate like that, and over time, the job became unbearable, she said.She had more than 3,000 patients assigned to her care — <a target="_blank" href="https://www.jabfm.org/content/jabfp/29/4/496.full.pdf">far more</a> than typical — and appointments were too short, especially for the chronically ill, she said. The hospital system would prioritize lucrative physicals for <a target="_blank" href="https://www.businessinsider.com/cigna-selling-medicare-advantage-business-insurers-higher-costs-slower-growth-2024-2">Medicare patients</a> rather than seeing patients with an urgent need."It really got to the point where I feel like I was working to feed the corporation," Cooke said. "It really wasn't incentivizing what the patients needed."Cooke has since opened a direct primary care practice, where she sees just a few hundred patients and can spend more time with them. She said she's found that she prescribes less medication now because she has time to educate patients on lifestyle habits and nutrition.<h2>The federal government and states are dialing up the scrutiny</h2>The Federal Trade Commission, the US Department of Justice, and the Department of Health and Human Services are <a target="_blank" href="https://www.wsj.com/articles/federal-agencies-probe-private-equity-profiteering-in-healthcare-2ba4c23a">stepping up scrutiny</a> of healthcare deals, particularly those backed by private-equity firms. The agencies also signaled that they want to look at smaller deals that typically wouldn't trigger an antitrust review.Several states have also increased their oversight of local healthcare consolidation. Some state lawmakers are also trying to strengthen <a target="_blank" href="https://www.nejm.org/doi/full/10.1056/NEJMp2306904#f1">laws prohibiting</a> corporations from practicing medicine. Most states have these bans on the books, but companies have found various ways around them.It's unclear if any of these efforts will meaningfully slow the corporatization of healthcare. And, to be sure, some physician practices may not have survived without an infusion of capital from a corporate buyer.A bill introduced by Oregon State Rep. Ben Bowman would have closed certain <a target="_blank" href="https://www.opb.org/article/2024/02/22/oregon-lawmakers-could-limit-corporate-ownership-of-medical-practices/">loopholes</a> and put more distance between corporate entities and their medical practices. Bowman told Business Insider he wanted to prevent big businesses from eroding doctors' autonomy."A physician who went to medical school who took an oath — their highest ethical duty is to care for their patients. Whether it's a private equity firm or a large corporation, their highest ethical duty is to generate a return on investment for their shareholders.
