A new study by <keyword id="5069783" type="General" weightage="20" keywordseo="Centre-for-Chronic-Disease-Control" source="Orion">Centre for Chronic Disease Control</keyword>, India (CCDC) on cardiovascular risk reduction in South Asia has demonstrated significant long-term benefits of a multi-faceted quality improvement (QI) strategy, comprising of task sharing, and clinical decision support software (CDSS) with electronic health records, in managing type-2 diabetes in India and Pakistan. The study, published in the prestigious journal PLOS Medicine, highlights the sustained impact of QI interventions on <keyword id="21411276" type="General" weightage="100" keywordseo="diabetes-care" source="keywords">diabetes care</keyword> and associated complications over a median follow-up period of 6.5 years. The burden of diabetes Diabetes control remains a global challenge, leading to severe complications such as <keyword id="22908" type="General" weightage="20" keywordseo="heart-disease" source="Orion">heart disease</keyword>, <keyword id="25191" type="General" weightage="20" keywordseo="stroke" source="Orion">stroke</keyword>, <keyword id="16753" type="General" weightage="20" keywordseo="blindness" source="Orion">blindness</keyword> or visual impairment, and kidney and nerve damage. LMICs are often faced with challenges such as inadequate infrastructure and trained healthcare staff with digital literacy. In that respect, the study's findings are crucial for integrating care models sustainably and identifying relevant organizational barriers. Previous studies from high-income countries showed improvements in diabetes care, but their long-term impact on vascular complications and mortality was unclear. This study aimed to address the critical gap in data regarding the long-term effectiveness of QI strategies and their impacts on vascular complications in LMICs. Professor Nikhil Tandon, Department of Endocrinology and Metabolism, All India Institute of Medical Sciences, New Delhi, and senior author of the study explained the relevance of these findings in the context of national efforts to combat non-communicable diseases (NCDs). He said, "The <keyword id="22511956" type="General" weightage="20" keywordseo="National-Programme-for-NCDs" source="keywords">National Programme for NCDs</keyword> [NP-NCDs] operationalized by the Government of India has already a functional portal which provides an electronic health record functionality. Integrating the CDSS in this portal will provide significant incremental benefit to the impact of the National Programme, and bolster governmental efforts to battle the scourge of NCDs including diabetes and hypertension." Speaking about the findings, Prof. (Dr) Dorairaj Prabhakaran, Executive Director, CCDC, and one of the study authors, said, "Through the trial, we've demonstrated that a sustained quality improvement strategy can significantly enhance diabetes care and reduce complications in South Asia. For every 13 patients treated with this strategy, one major adverse cardiovascular event like heart attacks or strokes- or a microvascular event -like eye disease or decline in kidney function- was prevented, showcasing its profound impact on patient outcomes." Professor MK Ali, William H. Foege Distinguished Professor of Global Health, Emory University and one of the key authors of this article, said, "This study is an exemplar of trans-national collaboration for benefits of both, the global south and global north." Study Design and Findings The randomized clinical trial included 1,146 patients with poorly controlled type 2 diabetes across ten outpatient diabetes clinics from January 2011 through September 2019. Participants were randomized to receive either the QI strategy, which involved trained non-physician care coordinators and a clinical decision support software (CDSS) integrated into the electronic health record form, for physicians, or the usual care. Some of the key findings from the study include: <ul><li>At 6.5 years, patients in the QI strategy group were nearly twice as likely to achieve and maintain target levels for HbA1c (&lt;7%), systolic blood pressure (&lt;130mmHg), and LDL-cholesterol (&lt;100mg/dl), compared to the usual care group. </li><li> The incidence of first microvascular complications, such as diabetic eye disease, decline in kidney function, and nerve pain was notably 32% lower in the QI group. </li><li>There was a significant 28% reduction in the combined occurrence of microvascular and macrovascular events in the QI group.</li></ul> Importantly, these benefits were sustained only in clinics that continued the QI strategy for 6.5 years. With diabetes accounting for one in seven deaths in South Asia, the study provides unique insights into the sustainability of QI strategies in achieving diabetes care goals in low- and middle-income (LMICs) and the effects of these improvements on reducing vascular complications and diabetes-related deaths.

The opening game of the World Cup is scheduled to place at the same venue on October 5 with a match between defending champions England and New Zealand in Ahmedabad, the ICC announced on Tuesday. The final will also be held at the same venue on November 19, The first semi-final of the World Cup will be held on November 15 in Mumbai while the second semi-final will be played on November 16 at the Eden Gardens in Kolkata. India will open their campaign on October 8 against Australia at Chidambaram Stadium in Chennai. They will be playing a total of eight matches in the league stage. The much-awaited encounter between India and Pakistan will be the eighth meeting between the two sides at a men's World Cup. With their jinx broken in the T20 World Cup, Pakistan will be looking to emulate the same in the 50-over format on October 15. India and Pakistan have faced each other seven times previously - in 1992, 1996, 1999, 2003, 2011, 2015 and 2019. The only time the two sides did not meet since that first match in 1992 was in 2007, which was a disastrous campaign for both teams as they crashed out of the tournament in the opening round India have won all of the previous seven encounters, a record they extended to the T20 World Cups as well until recently. India, Pakistan to face-off at Ahmedabad's Narendra Modi Stadium this October

Two soldiers were killed and four injured in Pakistani firing at the Line of Control (<keyword id="6355098" type="UNKNOWN" weightage="20" keywordseo="LoC">LoC</keyword>) in the Naugam sector of North Kashmir's <keyword id="66021" type="CITY" weightage="20" keywordseo="Kupwara">Kupwara</keyword> district on Thursday, the army said. "Pakistan initiated an unprovoked ceasefire violation along LoC in Naugam Sector, Kupwara today morning by firing mortars and other weapons. Two soldiers were fatally injured. Four soldiers were injured, being evacuated. Befitting response is being given," the Army said. Earlier, one soldier was killed in the ceasefire violation by Pakistan in the Krishna Ghati sector along the LoC in Poonch on Wednesday night. He has been identified as Lance Naik Karnail Singh. There has been a spurt in ceasefire violations along the LoC in the Kupwara district of Kashmir valley and to the south of Pir Panchal in Rajouri and Poonch districts in Jammu with Pakistan targeting civilian areas and defence positions with long range mortars and small arms. The Army says it is giving a befitting response to all acts of aggression by Pakistan along the LoC. Defence Ministry pegs Pakistan's LoC ceasefire violations at 3,186 so far this year

Pakistan is setting up surface-to-air missile sites with the help of China in Pakistan occupied Kashmir (PoK), officials with the knowledge of the matter said on condition of anonymity. The Chinese and Pakistan armies are reconnoitering the areas along the disputed India-Pakistan border to set up additional military infrastructure. Indian Air Force Chief Air Chief Marshal R.K.S. Bhadauria, had said on Monday that Pakistan and China have increased their bilateral exercises in the recent past. "There is nothing to suggest collusive threat," he said on Monday during an annual press conference in Delhi and added that India is keeping a close watch. The Indian military has for long flagged the threat of two-front war. With the disputed borders with China and Pakistan active at the same point of time, the armed forces are stretched to the maximum. A top official said: "Chinese People's Liberation Army (PLA) troops were also seen in joint reconnaissance in forward areas at Deolian and Jura in PoK along with Pakistan's 12 infantry brigade as well as the Minimarg-based forces under Force Commander Northern Areas (FCNA), the division size formation of the Pakistan Army." The official said that construction work for installation of surface-to-air missile defence system is being carried out by the Pakistan Army and the PLA at Pauli Pir near Lasadanna Dhok in PoK. "Approximately 120 Pakistan Army personnel and 25 to 40 civilians are working on the construction site," said the source, adding that the control room for the system will be located at the headquarters of PoK. "Ten PLA troops, including three officers, will be deployed at the control room," the source said. "Similar constructions have also been reported at Chinar village and Chakothi village in Hattian Bala district of PoK," the source said. It was further noticed that a road is being constructed by engineers from China from Jaglot to Gauri Kot and is likely to be extended till Gultari. The PLA troops were also observed in general areas of Jaglot with the Minimarg-based 80 infantry brigade of the FCNA. Pakistan and China are reportedly building new missile sites along western border

Indian Army retaliated 'befittingly' to Pakistan's latest ceasefire violation in J&K's Poonch district

Pakistan is trying to step up infiltration and push in more terrorists into J&amp;K after failing to instigate the people of Kashmir following last year's August 5 decision of revoking Article 370 . GoC 19 Infantry Division Kashmir Major General Virendra Vats said this at a press conference in Baramulla, North Kashmir on Saturday. "Pakistan has been failing in its attempt to instigate the people of Kashmir ever since we had the momentous decision of August last year, they have been trying very hard, they also know the season is coming to an end very soon, three or four months is what they have, so they shall try their best to push in the terrorists," he said. The GoC said that the launch pads across the LoC are fully active and terrorists are making attempts to infiltrate into India. "The launch pads are fully occupied and if we have to hazard a guess it would be anything between 250 to 300 terrorists in the launch pads, he said. India's landslide victory through an unopposed inclusion into the <keyword id="138690" type="ORGANIZATION" weightage="20" keywordseo="United-Nations-Security-Council">United Nations Security Council</keyword> (UNSC) as a non-permanent member has irked the Imran Khan-led government in Pakistan, as angry reactions have started coming out of Islamabad, threatening India of dire consequences in case of any misadventure committed along the <keyword id="44759" type="ORGANIZATION" weightage="20" keywordseo="Line-of-Control">Line of Control</keyword> (LoC) and the Working Boundary (WB). Pakistan's Foreign Minister <keyword id="121648" type="PERSON" weightage="20" keywordseo="Shah-Mahmood-Qureshi">Shah Mahmood Qureshi</keyword> has warned India against any misadventure, threatening of a telling retaliatory response. "Beware, beware, remember February 2019 and be ready for a quick retaliation while casting an evil eye on us," he said during his address to the Upper House (Senate) of Pakistan. Qureshi lashed out at Indian Prime Minister Narendra Modi and Indian External Affairs Minister S. <keyword id="236448" type="PERSON" weightage="20" keywordseo="Jaishankar">Jaishankar</keyword>, asserting, "Pakistan can't be cowed down by the Indians." Qureshi also reiterated Islamabad's inclination towards Beijing in the recent deadly clashes in Ladakh between Indian and Chinese forces, stating that Indian soldiers were suffering casualties there. "There is a dramatic shift in the region. China has openly landed in the arena against India," he said. "Nepal, which was thought to be completely under India's influence, is now making claims on some disputed areas with India, while Sri Lanka and Bhutan also had their objections. Afghanistan also thinks India is impeding reconciliation process there," he added. Qureshi briefed the Senate stating that India is facing isolation and New Delhi is feeling the pressure. He also reiterated Islamabad's concerns over what they call a false-flag operation by New Delhi aimed at diverting attention of the global community from the situation in Jammu and Kashmir, which has remained under lockdown for almost a year now. Qureshi's aggressive remarks came at a time when the Imran Khan-led government is facing serious criticism from its opposition parties for failing to raise the issue of Kashmir at the UN platform and giving India a walkover to become a non-permanent member of the UN Security Council. While Pakistan voted against India's contention for the non-permanent membership, its vote didn't matter as a heavy majority voted in favour of New Delhi. "Though it was a secret balloting, Pakistan openly said that it had voted against India for last year's August 5 unconstitutional steps it took in occupied Jammu and Kashmir and for violating the UN Charter, UNSC resolutions and international norms," maintained Qureshi. Pakistan's anger over India's inclusion into the UNSC is understandable as its ongoing diplomatic campaign to attract support against the Modi government for its decision taken on August 5, 2019, abrogating Articles 370 and 35A, changing the special status of the state of Jammu and Kashmir into two Union Territories, in the United Nations has practically failed to yield results. Tensions at the Line of Control (LoC) have also intensified in the recent days as heavy exchanges of artillery firing have claimed lives of border security forces and civilians on both sides. Pakistan and India both have been lodging strong protests against the ceasefire violations, summoning senior diplomats of respective high commissions to record their protests. Tensions are expected to flare up in the coming days as August 5, 2020 inches closer, marking one year of the lockdown in Jammu and Kashmir, for which Islamabad is gearing up to stage protests, rallies and international conferences.
