India vs. US: Healthcare industry

 Before the Budget 2017 is announced on 1st Feb, we wanted to compare the healthcare systems of two of the world's biggest democracies-the <keyword id="6356955" type="COUNTRY" weightage="20" keywordseo="united-states">United States</keyword> and India to highlight their differences and the changes we can make to improve the access and outcomes in our own country. According to <keyword id="43588" type="ORGANIZATION" weightage="20" keywordseo="World-Health-Organization">World Health Organization</keyword> (WHO) data for countries performing best in the healthcare sector, the US ranks 37, while India stands at 112.

Last year's Budget came under criticism for not addressing systemic problems adequately, emphasizing instead health insurance schemes and public-private partnerships. 
 
When it comes to outcomes, in India the life expectancy at birth is 63 years for men and 66 years for women, while that for the US is 76 years for men and 81 years for women. Children under the age of five are also a lot more likely to die in India than in the US.

New Delhi, Dec 19 () Shifa International on Thursday launched 'Shifa Care' platform in India to provide healthcare services and solutions to patients at their homes from a doctor of their choice, in a language and time preferred by them.
 The platform seeks to boost the healthcare ecosysytem in India and allows patients to connect with a doctor or multiple doctors for multi-disciplinary care in over 60 Indian languages from smart phone via video call, a top company official said.
 "The app will be available to patients from January 1, 2020. We have spend USD 2 million to develop this end to end healthcare ecosystem," Shifa International Founder and CEO Manish Chhabra told .
 The company already has 5,000 pre-registered doctors with it, he added.
 Shifa Care facilitates affordable and quality healthcare services across the rural rungs of the country apart from those in tier 1, 2 and 3 cities, the company said.
 "Starting with India, we aim to build an inclusive healthcare system, based on new-age technology, where there is no discrimination on the basis of caste, colour, creed or gender," Chhabra said.
 Shifa Care's ecosystem utilises Al (artificial intelligence), epidemiology and genetic sequence, all at one go driven by prediction, mitigation and Al algorithm, he added.
 "Our objective is to bring a paradigm shift in the healthcare segment from providing the reactive care to preventive care," Chhabra said.
 Shifa Care is completely compliant with HIPAA (Health Insurance Portability and Accountability Act - USA), he added.

There has been a significant ‘change of guard’ in the country. The new government is in place and anticipations are high that new economic policies will roll out sooner than later. After a long period of below-par performance, the ever-depreciating rupee has picked up pace and this may provide a major boost to trade ties with other countries. More progressive policies are expected to roll out, thanks to the new government’s initiatives. 
 
However, one cannot deny that rural health initiatives in India have been patchy, with the urban populace grabbing the largest chunk of the healthcare pie. For instance, 28% of the urban Indians have access to nearly 66% of healthcare initiatives while the rural majority struggles to access the remaining 34% benefits. 
 
India’s spending on healthcare is also dismal. It is just $43 (per capita) a year, compared to $85 in Sri Lanka, $240 in China and $265 in Thailand. In other words, India has been spending just 1.2% of its GDP over the past decade although it is now high time to increase the spending to at least 2.5%. 
 
But due to its urban approach, India has grown into a coveted healthcare destination, with liberal economic policies helping the healthcare sector grow better and faster. As specialty hospitals gun for private investments and corporatisation, the entire system tends to grow more hi-tech, efficient and professionally oriented – ensuring state-of-the-art facilities, speedy recovery and great hospital stay experience. In other words, India has been working towards a complete healthcare transformation for some time now. 
 
Over the past decade, even policy level changes have re-emphasized the vision of creating access to a minimum set of healthcare services for all. As for medical tourists seeking healthcare support in India and boosting several other industries in the process, the country offers some of the best facilities available in the world. 
 
The Indian healthcare system has seen a two-pronged growth strategy in the recent past. As the country is well known for its alternative and holistic healthcare systems like the Ayurveda, it has tried to tap the potential of both modern and traditional medicines to put in place a comprehensive healthcare system. 
 
 
Among the nations that are gaining popularity as the most coveted destinations for medical tourism, India is vying with countries such as Malaysia, Singapore and Mexico. A patient who visits India to seek medical treatment is also an important customer who will boost the local hospitality industry. In fact, when a person arrives here to avail of healthcare facilities, the average stay is 30 days and above, according to the data available. 
 
Confederation of Indian Industries (CII) also notes that about 150,000 medical tourists preferred India over other countries in 2005 although the sector enjoyed least support from the government. At that time, hospitals were not into branding in a big way and the recommendations or testimonies were few and far between. 
 
However, analysts who are following the trends in the healthcare sector are confident that the numbers will climb up to 3,200,000 by 2015, with major surgeries being carried out in areas such as bone marrow transplant; ocular disorders; liver, kidney and heart surgeries, and hip and knee replacement. 
 
At this juncture, it is critical to note that things had looked bright for India at the outset, but last year was not as growth-oriented as one would expect it to be. For one, private equity took a beating in terms of investments flowing into various industrial sectors. And the depreciating rupee brought PE investments in the healthcare sector down by 18.5% from the previous year. 
 
Recently, InternationalLiving.com conducted a global survey on retirement index and identified the countries with best healthcare systems in the world. Good policies, people-friendly facilities and affordability of healthcare services served as the parameters for this survey. France, Uruguay, Malaysia, Costa Rica and Mexico were rated among the best countries that provided the most evolved healthcare systems to their citizens. 
 
With healthcare and life sciences emerging as the major focus areas of investment and growth in the coming years, it is now imperative for the governments to take a huge leap in this direction. 
And the focus needs to shift from the urban-centred approach to rural areas which are lacking basic access to modern healthcare facilities till date.

Will India Witness A Boom In Healthcare
Sector?

India prides itself in being a galloping economy with great ideas and cultural diversity. 
 
However, it also stands out for its colossal neglect of some basic factors---such as health. 
 
For instance, Bill Gates recently made an embarrassing comparison of India with Ethiopia. The African country, he said, was reportedly terrible 10 years ago. However, it has become very good today, and continues to improve due to efficient surveillance systems. Worryingly, the world’s problem areas include parts of India, Pakistan, Nigeria and the DRC, he said. 
 
The <keyword id="102649" type="ORGANIZATION" weightage="20" keywordseo="Gates-Foundation">Gates Foundation</keyword> 
 To streamline processes, the Bill and <keyword id="142497" type="ORGANIZATION" weightage="20" keywordseo="Melinda-Gates-Foundation">Melinda Gates Foundation</keyword> recently launched the Child Health and Mortality Prevention Surveillance Network, or CHAMPS. It is putting some special focus on improving the structure and getting systems into place, said Bill Gates, to <a href="http://www.theatlantic.com/health/archive/2015/05/bill-gates-child-health-mortality-prevention-surveillance-network/392549/">The Atlantic</a>. 
 
For instance, it is not clear why neonatals die, yet their project is trying to probe the drivers of mortality for all children. The Gates announced that they are investing $75 million in a number of surveillance sites expected to collect information "about how, where and why children are getting sick and dying.” The plan will be spread first through six nations in Africa and South Asia, depending on field workers to take the autopsies of children who have died, especially in “beefed-up laboratories.” 
 
Disease surveillance system 
 The Integrated Disease Surveillance Programme (IDSP) was launched in India with the support of the World Bank in November, 2004, so that the country could detect and respond to disease outbreaks quickly. In March 2010 to March 2012, World Bank funds were made available for <keyword id="2858752" type="UNKNOWN" weightage="20" keywordseo="Central-Surveillance-Unit">Central Surveillance Unit</keyword> as well as nine identified states (Uttarakhand, Rajasthan, Punjab, Maharashtra, Gujarat, Tamil Nadu, Karnataka, Andhra Pradesh and West Bengal). The other 26 states were funded only by local budgets. 
 
The Surveillance Programme will continue well into the 12th Plan (2012-17) under the National Health Ministry, with an outlay of Rs 640 crore from the domestic budget. 
 
The World Bank’s focus was to strengthen the system for epidemic-prone diseases, improve central monitoring and coordination, improve state and district surveillance focusing on nine states, gear up and prevent influenza epidemics. 
 
Findings of the project 
 
The project seemed to have detected a few parameters for progress in surveillance as well as in identification and response, related to IDSP and also assessed the infrastructure and systems in place. A number of future goals have been evolved to improve the public health information and intervention. 
 
Only recently, it was found that ratings for the Integrated Disease Surveillance Project (IDSP) for India were just moderately satisfactory. The survey factored the risks to development outcome, bank and borrower performance. 
 
Unfortunately, Non-Communicable Diseases (NCDs), which should be a core, important public health function in India became part of the surveillance survey very late. The NCD risk factor was thus delayed and was improved only in some provinces, not countrywide. Ultimately, the country plans to integrate the NCD factor into the national programme. 
 
Inequalities 
 
The solutions to fight epidemics and diseases are distributed unevenly among different groups and communities, impacting socially and economically sensitive groups. Unfortunately, the inequitable system of distribution in India is glaringly evident in the implementation of the system. 
 
Take TDAP (Tetanus, diphtheria and pertussis), for instance. At first, the programme was not implemented with efficiency. It was restructured only in 2010 in the nine “best performing states”, selected on the basis of their performance as well as administrative and reporting efficiency. It was an ironical conclusion, as the states that needed support lagged behind the most, yet got the least central assistance. 
 
The twin outbreaks of dengue and chikungunya were a major public health concern and pain point a few years ago. The Aedes mosquito-borne chikungunya, with more than a million suspected cases in India, could be controlled by general hygiene and sanitation, but the disease was not shaken off due to the problems in the sanitation, hygiene and environment. 
 
Amazingly, the health minister at the time, Anbamoni Ramadoss, actually commented that the health ministry can't do much in the job! “(We need) involvement of general public, the associations and the civil society," he said. 
 
Efficient implementation 
 
The centre cannot wash off its responsibilities, but the execution is too erratic. For instance, Andhra Pradesh has noted that a special sanitation drive was launched, including massive disease surveillance, to improve the vaccination system in its state. In Gujarat, traditional medicines have been harnessed to fight the disease. 
 
The surveillance system reveals that a number of measures are being identified and put in place--for instance, deployment of trained personnel, appropriate technology as well as efficient administrative services. Establishing extensive laboratory networks are needed for careful investigation. 
 
So far, the ratings of the system have been just moderately satisfactory. Hence, the nation should gear up and streamline the administration, which is crying out for attention.

Good surveillance could be the lifeline of India’s healthcare system

The <a href="https://www.sciencedaily.com/releases/2018/05/180523091109.htm">study</a>, which uses a number between 0 to 100 to rank countries based on the quality and accessibility of healthcare, is based on 32 causes of death that can be prevented if proper healthcare is available in the region. The score which came up from 24.7 in 1990 is still lower than its neighbouring countries. India currently<a href="https://www.researchgate.net/publication/317036679_Healthcare_Access_and_Quality_Index_based_on_mortality_from_causes_amenable_to_personal_health_care_in_195_countries_and_territories_1990-2015_a_novel_analysis_from_the_Global_Burden_of_Disease_Study_"> ranks</a> below its neighbouring countries of Bhutan (53), Bangladesh (52), and Sri Lanka (73) in the index but it's above Afghanistan (32), and Pakistan (43).

 Advancements in technology over the past decades have resulted in the increase in quality and access to healthcare in India. <a href="http://www.business-standard.com/article/current-affairs/india-worse-than-bhutan-bangladesh-in-healthcare-ranks-145th-globally-118052400135_1.html">The country's</a> top performers include Kerala, Goa, Delhi and Himachal Pradesh. The Worst index ranks were given to Chhattisgarh and Jharkhand (34.7), Bihar (37), Odisha (36.3), Uttar Pradesh (34.9) and Assam (34). The Global average healthcare access and quality score increased from 42.4 in 2000 to 54.4 in 2016. Though the global average increased, the gap between the worst and the best country widened (29 in the Central African Republic and 95 in Andorra). Similar trends were seen in India where the gap between the highest and lowest scores of healthcare access and quality has also widened. Currently, the states of Uttar Pradesh, Chhattisgarh, Odisha, Bihar and Assam have a score below 38, whereas the union territory Goa and the state of Kerala have the highest score in 2016, exceeding 60 points. These regional <a href="http://www.business-standard.com/article/current-affairs/india-worse-than-bhutan-bangladesh-in-healthcare-ranks-145th-globally-118052400135_1.html">differences</a> in the index score can be due to multiple reasons, such as variations in,medical infrastructure, the quality of medical technologies available, physical access to healthcare facilities, and the availability of effective services and treatments. India needs to come a long way in healthcare access quality and technology in order to start with the global leaders. There is an urgent need to improve access and quality of healthcare. "These results emphasise the urgent need to improve both access to and quality of healthcare, otherwise health systems could face widening gaps between the health services they provide and the disease burden in their population," <a href="https://www.sciencedaily.com/releases/2018/05/180523091109.htm">says senior</a> author of the study Dr Rafael Lozano, Institute of Health Metrics and Evaluation at the University of Washington, USA. In countries where population is so high, the healthcare quality and access are increasing at a rate that is considerably below the required health need of the population.

India is 145th in healthcare access and quality even as global averages improve

Amongst all sectors in India, healthcare is one sector that has managed to both attract and deter startups from foraying into this segment. Some realise the enormous opportunities that the sector presents for growth, while there are many who are discouraged by the abysmal condition of the industry. <a href="http://www.businessinsider.in/searchresult.cms?query=Healthcare+startup">Healthcare startup</a> <a href="http://www.businessinsider.in/searchresult.cms?query=Practo">Practo</a> belongs to the former category. And there is a reason to why, its founder <a href="http://www.businessinsider.in/searchresult.cms?query=Shashank+ND">Shashank ND</a>, founder and CEO decided to venture into this particular segment. 
 
“Healthcare offers enormous opportunities. Healthcare is one sector that consumers across age groups, cities identify with. In fact, healthcare sector is likely to touch nearly $300 billion by 2020. So this is a sector, which has many problems, offers everyone ample opportunities to innovate,” <a href="http://www.businessinsider.in/searchresult.cms?query=Shashank+ND">Shashank</a> tells BI India in an exclusive interview. 
 
In fact, he bets big on the <a href="http://www.businessinsider.in/searchresult.cms?query=Startup+India">Startup India</a>, Standup India campaign and expects it to put startups in the limelight. “In a few decades, startups can improve the life of the country but also boost the economy of the country. The government and the prime minister leading from the front just show how important startups are for the country,” he says excitedly. 
 
So what does he expect from the initative? Well, he says that the focus remains on the policies. “We talked about how the policy makers can facilitate the growth of the industry and how we can be more effective with their support. I expect that the attitude of this government will become more attentive towards startups. They will start prioritizing startups in their priorities.” 
 
He adds, “I also expect that there will be a lot more clarity in the regulations because startups always work with the cutting-edge technology, innovation. This will clear the roadway for startups to grow in the economy.” 
 
Regulatory hurdles? Explaining his point, he notes that India has a very good domestic market which will continue to propel the growth of startups. But there are a lot of hurdles that startups will come up with, which the government needs to do away with. “An exodus of companies is happening in India because of regulatory hurdles. I believe that the government should take steps to retain companies in the country so that they can contribute to the economy,” he states. 
 
So what are these? Well, to begin with, <a href="http://www.businessinsider.in/searchresult.cms?query=Shashank+ND">Shashank</a> says startups are digitalizing the healthcare economy. “We are taking the doctors, patients and other stakeholders online. For instance, there are a lot of regulations that are not clear for online pharmacies, which I think makes it difficult for these players to be around for long.” 
 
He quickly adds, “My biggest grievance is there is no clarity in regulations when it comes to digitalization about healthcare sector such as diagnostics, pharmacies, which if digitalized could be very helpful. 
 
He also talks about the acute shortage of professional doctors, a challenge that has marred the growth of healthcare sector in rural India. “We are connecting doctors through internet and mobile, we can connect them with the patient anywhere in the country. But where it isn’t possible, there comes a huge gap. If the person in a rural setup can be given basic training and need not have a medical license but can have the requisite facilities where they can take care of the basic needs of the patient, it will reduce the stress on the healthcare setup in rural India. I think, we need de-skilling in healthcare sector,” he clarifies. 
 
Besides, protection of Intellectual Property Right especially in the healthcare sector is also a burning issue for him. “If IP is protected in India, the Indian companies will be valued a lot more and more companies will be willing to innovate and bring their products to the market,” asserts Shashank. 
 
So what is his advise to the young startupreneurs? “Well, innovate. Don’t copy what is happening in other markets. Be original and also be patient. Results don’t come overnight!”

New Delhi, Nov 28 () Healthcare industry body NATHEALTH on Thursday said it is working closely with the government to achieve the vision of providing universal health coverage in the country.
The body also suggested that corporate social responsibility (CSR) funds could be used to push the critical segments such as preventive health, skilling and digital technologies.
"We are working very closely with the government because unless we work together, universal health coverage to masses is just not going to happen," NATHEALTH President and Manipal Health Enterprises Chairman Sudarshan Ballal told reporters here.
He was speaking at the 6th Annual Summit of the NATHEALTH on the theme of 'Reimagining Collaboration to Strengthen India's Healthcare Growth Trajectory'.
The government and industry need to work together to realize the Prime Minister's vision of Health for All, he added.
Ballal also said that for transparency and ethical practices in the healthcare sector "we believe that we should have a code of ethics. Indian Medical Association (IMA), Ficci and many others have worked on this and we at NATHEALTH would like to facilitate so that the code of ethics is followed".
He also said that NATHEALTH would like to make India as a 'healthcare destination of the world'.
NATHEALTH Senior Vice President and Apollo Hospitals Group Vice Chairperson Preetha Reddy said: "Healthcare today is at an inflection point".
Both, the government and private sector have to work together to ensure that healthcare is accessible to the entire population of the country, she added.
More than anything else, "we need a world class brand for India and for that we have to give high quality good outcomes, so that the rest of world looks at India. There is a potential for people from other countries to come to India for healthcare, we just have to do more about it," Reddy said.
The importance for building trust between investors, policy makers and people seeking care - whether from India or from outside - cannot be undermined, she added.
The industry body also said it is willing to pool industry CSR and work with the government and development sector partners to test innovative delivery models and create replicable pathways for scale. AKT BAL

By Aditi Khanna London, Jul 15 () The year 2020 will witness an "inflection point" in the UK-India healthcare collaboration as the COVID-19 pandemic has brought collaboration in medicine to the centrestage, outgoing Consul General of India in Birmingham Dr Aman Puri has said. In an interview to on the eve of his departure from the West Midlands region of England, Puri reflected on the recently-launched West Midlands India Partnership (WMIP) to boost India-UK investment flows and flagged the healthcare sector as a particular focus of bilateral ties amid the coronavirus pandemic. "My sense is that 2020 will witness an inflection point in the UK-India healthcare collaboration. COVID-19 has brought healthcare centerstage, and the need for collaborations has been realised by all stakeholders more than ever before," said Puri, who organised annual India-UK healthcare conferences during his three-year tenure, resulting in several MoUs between key stakeholders. Puri has been the Consul General of India in Birmingham since 2017 and leaves the UK on Wednesday at the end of a three-year tenure to take charge as India's Consul General in Dubai. "Today, as the world is facing an unprecedented crisis due to the pandemic, the partnership between health systems assumes even greater significance. The Prime Minister of India, while announcing India's commitment of USD 15 million to the UK-led Global Vaccine Alliance (Gavi), said the pandemic, in some ways, exposed the limitations of global cooperation and that for the first time in recent history, humankind faces a clear common enemy," according to Puri. "Apart from this, there have been several collaborative efforts between British and the Indian healthcare systems, both in the public and the private sectors, in the areas of clinical research, vaccine development and exchange of best practices to manage COVID-19 pandemic," he said. The 44-year-old diplomat, who has previously been based in Brussels during his tenure in the European Union, believes Britain's exit from the EU and the pandemic-induced economic crisis will also throw up some opportunities within the India-UK sphere. "As a result of the economic crisis due to the pandemic, there is going to be a spike in distressed assets in both economies. Identifying complementarities between the UK and India, catalysing mergers and acquisitions will be a significant measure required to protect jobs, improve sustainability of business and promote long-term value creation," he said. Reflecting upon the momentum and impetus provided by trade missions from the West Midlands region to India over the past three years, the diplomat expressed the hope that the region is firmly on India's investment map. "I believe that the West Midlands region could be marketed better in India. London remains the preferred location for more than half of the Indian companies investing in the UK," noted Puri. "I strongly believe there is appetite and ample opportunity for Indian companies to do business in this region and vice-versa. Hopefully, the West Midlands India partnership (WMIP) will help to better broadcast opportunities which abound in this region," he said. The WMIP is a 'Special Purpose Vehicle' pushed forth by the Consulate of India in Birmingham, backed by both Indian and the UK governments, to attract jobs and tourists and promote two-way trade and investment. The setting up of the 'Guru Nanak Chair', supported by the government of India, at the University of Birmingham to mark the 550th birth anniversary of the founder of the Sikh faith last November, and the India Institute launched at the university a year earlier are among some of the other legacies of Puri's UK posting. "My vision and passion for India-UK collaborations and partnerships during my tenure in the UK has come to its fruition thanks to each and every individual and organisation that supported us. I hope the consulate will receive the continued support in the future and thereby help in strengthening India-UK relations," he said. 2020 will mark 'inflection point' in India-UK healthcare collaboration: Indian diplomat

India’s spending on the public <keyword id="6354750" type="UNKNOWN" weightage="20" keywordseo="health">health</keyword> care is estimated only at about 1.3 of its GDP. This is a thoroughly discouraging figure when compared to what the other BRICS countries are spending. Notably, Brazil, Russia and South Africa spend about 8.3 percent, 7.1 percent and 8.8 percent towards their public healthcare sector respectively.
