New Delhi, April 7 () Indian pharma industry on Tuesday said there is enough stock of hydroxychloroquine in the country, and drug firms are ready to ramp up the production to meet domestic as well as export requirements.India has decided to partially lift the ban on the export of anti-malarial drug hydroxychloroquine in sync with its global commitment to deal with coronavirus pandemic.On March 25, India had banned the export of hydroxychloroquine in the midst of concerns that the drug could be used to fight COVID-19.India is the largest exporter of the drug.Officials said India would export the drug on a case-by-case basis after meeting all the domestic requirements."India manufactures 70 per cent of the world's supply of hydroxychloroquine. Companies like Zydus Cadila and IPCA are the major manufacturers of hydroxychloroquine in the country," Indian Pharmaceutical Alliance (IPA) Secretary General Sudarshan Jain told .The production capacity is sufficient to meet the current demand. Various scenarios are being assessed and it will be the endeavour to meet both the domestic demand and export obligation for Paracetamol and hydroxychloroquine," Jain said.The objective is to minimise speculative buying and hoarding in these trying times and ensure balance in distribution for patients and segment who need them, he added.In similar vein, Indian Drug Manufacturers' Association (IDMA) Executive Director Ashok Kumar Madan said: "India needs around 24 million tablets per year as on date for the three indications --- malaria, lupus and rheumatoid arthritis, it was being used".India currently has an annual installed capacity of around 40 metric tonne of active pharmaceutical ingredients (APIs) of hydroxychloroquine. With this capacity, "we can make around 200 million tablets of 200 mg," he added.So this indicates, that India currently has spare available capacity. Now, "we have to see how much would the country's requirements for the drug would grow for both treatment and prevention of COVID-19. The country has a production capacity of 40 tonnes of hydroxychloroquine (HCQ) every month, implying 20 crore tablets of 200 mg each. And since the drug is also used to auto-immune diseases like rheumatoid arthritis and lupus, manufacturers have good production capacities that can also be ramped up. Ipca Laboratories, Zydus Cadila and Wallace Pharmaceuticals are top pharma companies manufacturing HCQ in India. Recently, the Ministry of Health and Family Welfare had reportedly placed an order for around 10 crore tables of HCQ with Ipca laboratories and Zydus Cadila. The drug is not manufactured in developed nations such as the US because of non-existent malaria. Hydroxychloroquine is very similar to chloroquine, one of the oldest and best-known anti-malarial drugs, but with lesser side-effects. It can be bought over the counter and fairly inexpensive. But its purchase and use has been severely restricted as it was selectively used in the treatment of coronavirus because of its antiviral properties. India gets the active pharmaceutical ingredient (API) that is used to manufacture HCQ from China and supplies so far have been steady. As much as 70 per cent of all the APIs needed by India to manufacture drugs come from China. After Trump stated that the US could "retaliate" if India does not release stocks of the drug, Indian officials said exports of hydroxychloroquine and paracetamol will be allowed depending on availability of stock after meeting domestic requirements and existing orders. Last month, the US Food and Drug Administration (FDA) partially lifted a three-year-old 'import alert' on Ipca's two plants to import the medicine. Zydus Cadila has also received an order from the US. In fact, the risk of death was found to be higher in those treated with hydroxychloroquine than in the group that included patients not treated with the drug, according to the findings published in the preprint repository medRxiv. For the study, the researchers performed a retrospective analysis of data from patients hospitalised with confirmed SARSCoV-2 infection in all US Veterans Health Administration medical centres until April 11. Patients were categorised based on their exposure to hydroxychloroquine alone (HC) or with the antibiotic azithromycin (HC+AZ) as treatments in addition to standard supportive management for Covid-19. A total of 368 patients were evaluated. Rates of death in the hydroxychloroquine alone group was 27.8 per cent. The rate of death in the hydroxychloroquine and azithromycin group was 22.1 per cent and in the no hydroxychloroquine, it was even lower, 11.4 per cent, the results showed. The researchers found that the risk of ventilation was similar in the HC group and in the HC+AZ group, compared to the no HC group. "In this study, we found no evidence that use of hydroxychloroquine, either with or without azithromycin, reduced the risk of mechanical ventilation in patients hospitalised with Covid-19," said the study carried out by <keyword id="9092728" type="PERSON" weightage="20" keywordseo="joseph-magagnoli">Joseph Magagnoli</keyword> from Dorn Research Institute, Columbia VA Health Care System and colleagues. "An association of increased overall mortality was identified in patients treated with hydroxychloroquine alone. These findings highlight the importance of awaiting the results of ongoing prospective, randomized, controlled studies before widespread adoption of these drugs," the researchers concluded. Despite limited and conflicting data on the use of hydroxychloroquine in patients with COVID-19, the US Food and Drug Administration has authorised the emergency use of this drug when clinical trials are unavailable or infeasible. Hydroxychloroquine, alone or in combination with azithromycin, is being used in Covid-19 therapy based on anecdotal and limited observational evidence in several countries.

The United States has emerged as the global hotspot with 20,000 deaths and nearly 530,000 cases, According to Johns Hopkins University data.America has replaced Italy as the country with the highest number of COVID-19 deaths at 20,597, the data shows. Earlier this week, India at the request of President Donald Trump cleared the export of 35.82 lakh tablets of hydroxychloroquine to the US along with nine metric tons of active pharmaceutical ingredient or API required in the manufacturing of the drug."Supporting our partners in the fight against COVID-19. Hydroxychloroquine is used to treat malaria and rheumatoid conditions such as arthritis. In various studies, the drug has demonstrated antiviral activity, an ability to modify the activity of the immune system, and has an established safety profile at appropriate doses, leading to the hypothesis that it may also be useful in the treatment of COVID-19, the NIH said. The drug is not without risks as even short-term use can cause cardiac arrythmias, seizures, dermatological reactions, and hypoglycemia, it added. "Many US hospitals are currently using hydroxychloroquine as first-line therapy for hospitalized patients with COVID-19 despite extremely limited clinical data supporting its effectiveness," said Wesley Self, emergency medicine physician at Vanderbilt University Medical Center and PETAL Clinical Trials Network investigator leading the ORCHID trial. "Thus, data on hydroxychloroquine for the treatment of COVID-19 are urgently needed to inform clinical practice," he added. According to NIH, participants will be randomly assigned to receive hydroxychloroquine 400 mg twice daily for two doses (day one), then 200 mg twice daily for the subsequent eight doses (days two to five) or a placebo twice daily for five days. Trump said he spoke to Prime Minister Modi on Saturday morning and made a request to release Hydroxycholoroquine - an old and inexpensive drug used to treat malaria - for the US. "I called Prime Minister Modi of India this morning. They make large amounts of Hydroxychloroquine. India is giving it a serious consideration," Trump said at his daily news conference at the White House on Saturday. India's Directorate General of Foreign Trade on March 25 banned the export of Hydroxychloroquine but said that certain shipments on humanitarian grounds may be allowed on a case-by-case basis. With more than three lakh confirmed cases of coronavirus infection and over 8,000 fatalities, the US has emerged as the global epicentre of the deadly coronavirus disease to which there has been no cure. Trump said he would appreciate if India releases the amount of Hydroxychloroquine that the US has ordered. "And I said I would appreciate if they (India) would release the amounts that we ordered," he said, without mentioning that quantity of Hydroxychloroquine that has been ordered by US companies from India.
