Nursing Officer Harish Kumar Kajla was suspended with immediate effect on Monday night in view of the April 22 incident of disrupting OT patient services. The union is seeking immediate revocation of suspension of Kajla and stopping of all kinds of retaliative measures against union executives and union members of main OT. "In response to your unilateral decision to suspend Harish Kajla, president of AIIMS Nurses Union, without even citing proper reasons, the union has called an emergency executive meeting and decided to go on indefinite strike from 8 a.m., 26/4/2022 demanding immediate revocation of suspension of Harish Kumar Kajla and stopping of all kinds of retaliative measures against union executives and Union members of main OT", said nurses union in a letter to AIIMS director Randeep Guleria. In a statement, the union said: "The union has always been receptive and open to expressing our genuine concerns and version throughout this issue. But unfortunately, we were neither called, nor contacted via any communication which forced us to go on an indefinite strike to safeguard the basic rights of our members." AIIMS nurses' union goes on indefinite strike after suspension of its president

Following a modified Atkins diet high in fat and low in carbohydrates and taking medication may reduce seizures by over half in people with tough-to-treat epilepsy, according to a study by researchers at the <keyword id="6365231" type="General" weightage="80" keywordseo="all-india-institute-of-medical-sciences" source="Orion">All India Institute of Medical Sciences</keyword>. The modified Atkins diet is a combination of the Atkins diet and a ketogenic diet that includes food items such as soy products, heavy cream, butter and oils, leafy green vegetables, and animal protein including eggs, chicken, fish, and bacon. While the ketogenic diet has been shown to be effective in reducing seizures, its stringent requirements and restrictions can make it difficult to follow, the researchers said. "For people with drug-resistant epilepsy, or those who have been unable to find an effective treatment to reduce seizures, it's encouraging to see that there are lifestyle changes that can be combined with standard drug therapy to reduce the number of seizures," said <keyword id="2194193" type="General" weightage="80" keywordseo="manjari-tripathi" source="Orion">Manjari Tripathi</keyword>, from AIIMS <keyword id="6358777" type="General" weightage="20" keywordseo="new-delhi" source="Orion">New Delhi</keyword>. "Our study found that this combination may reduce the chance of seizures by more than half," said Tripathi, author of the study published in the journal <keyword id="72938" type="General" weightage="20" keywordseo="neurology" source="Orion">Neurology</keyword>. The study involved 160 adults and adolescents who had epilepsy for more than 10 years on average and had at least 27 seizures per month despite trying an average of four antiseizure medications at the maximum tolerated dose. They were randomly assigned to receive either standard drug therapy alone or drugs plus the modified Atkins diet over six months. Participants logged their seizures and meals. They were given food lists, sample menus and recipes. Carbohydrate intake was restricted to 20 grams per day. <keyword id="914284" type="General" weightage="80" keywordseo="us-federal" source="Orion">US Federal</keyword> dietary guidelines recommend between 225 and 325 grams of carbs per day. After six months, researchers found that 26 per cent of people who had both drug therapy and followed the modified Atkins diet had more than a 50 per cent reduction in seizures compared to only three per cent of the people who had drug therapy alone. Four people in the diet group were free of seizures by the end of the study, while no one in the medication-only group was seizure-free, the researchers found. The study also looked at the quality of life, behaviour and side effects at six months. The group that had drug therapy and followed the modified Atkins diet showed an improvement in all areas compared to the group that had drug therapy alone, the researchers said. They noted that 33 per cent of the participants did not complete the study due to poor tolerance of the diet, lack of benefits or the inability to follow up in part due to COVID-19. However, the researchers said tolerance of the modified Atkins diet was better than what is seen with the ketogenic diet. "While the modified Atkins diet may be an effective treatment in controlling seizures, further research is needed to identify genetic biomarkers and other factors associated with the response to this diet," Tripathi added. "This may improve patient care by encouraging targeted precision based earlier use of this diet," she added. The researchers acknowledged a limitation in the study that seizures were self-reported or reported by caregivers, so some of them may not have been reported at all. High fat, low carb diet along with medicines can cut seizures by half: AIIMS study

Two doses of <keyword id="9485939" type="General" weightage="20" keywordseo="Covaxin" source="Orion">Covaxin</keyword> are 50 per cent effective against symptomatic COVID-19, according to the first real-world assessment of India's indigenous coronavirus vaccine published in <keyword id="456544" type="General" weightage="20" keywordseo="The-Lancet" source="Orion">The Lancet</keyword> Infectious Diseases journal. The results of an interim study recently published in The Lancet showed that two doses of Covaxin, also known as BBV152, had 77.8 per cent efficacy against symptomatic disease and present no serious safety concerns. The latest study assessed 2,714 hospital workers at the <keyword id="6365231" type="General" weightage="20" keywordseo="All-India-Institute-of-Medical-Sciences" source="Orion">All India Institute of Medical Sciences</keyword> (AIIMS) in Delhi, from April 15 May 15, who were symptomatic and underwent RT-PCR test for COVID-19 detection. Researchers noted that the Delta variant was the dominant strain in India during the study period, accounting for approximately 80 per cent of all confirmed COVID-19 cases. Covaxin, developed by Hyderabad-based <keyword id="149108" type="General" weightage="20" keywordseo="Bharat-Biotech" source="Orion">Bharat Biotech</keyword> in collaboration with the National Institute of Virology, Indian Council of Medical Research (NIV-ICMR), Pune, is an inactivated whole virus vaccine administered in a two-dose regimen, 28 days apart. In January this year, Covaxin was approved for emergency use in India for people aged 18 and above. The World Health Organization (WHO) added the vaccine to its list of approved emergency use COVID-19 vaccines earlier this month. The latest study was conducted during India's second COVID-19 surge and in healthcare workers who were primarily offered Covaxin. "Our study offers a more complete picture of how BBV152 (Covaxin) performs in the field and should be considered in the context of COVID-19 surge conditions in India, combined with the possible immune evasive potential of the Delta variant," said Manish Soneja, Additional Professor of Medicine at AIIMS New Delhi. "Our findings add to the growing body of evidence that rapid vaccine rollout programmes remain the most promising path to pandemic control while public health policies must continue to include additional protective measures, such as mask-wearing and social distancing," Soneja said in a statement. The COVID-19 vaccination centre at AIIMS New Delhi exclusively offered Covaxin beginning January 16 this year to all of its 23,000 employees. Researchers evaluated the effectiveness of the vaccine against symptomatic RT-PCR confirmed SARS-CoV-2 infection. Of the 2,714 employees in the study population, 1,617 people tested positive for SARS-CoV-2, the virus that causes COVID-19, and 1,097 tested negative. Positive cases were matched to negative RT-PCR tests (controls). The odds of vaccination with Covaxin were compared between cases and controls and adjusted for occupational exposure to COVID-19, previous SARS-CoV-2 infection, and infection dates. The study found that the vaccine effectiveness against symptomatic COVID-19 after two doses of Covaxin with the second dose administered 14 or more days before undergoing RT-PCR testing was 50 per cent. The effectiveness of two vaccine doses remained stable over the seven-week follow-up period, the researchers said. The adjusted vaccine effectiveness of the first dose, estimated after seven and 21 days, was low, which is consistent with the performance of other preventives against the Delta variant, they said. "Findings from the study confirm previous research indicating that two doses of BBV152 are required to achieve maximum protection and that all vaccine roll-out plans must follow the recommended dosing schedule," said Parul Kodan, Assistant Professor of Medicine at AIIMS New Delhi. "More research is needed to better understand how these findings translate to BBV152's effectiveness against delta and other variants of concern, especially related to severe COVID-19 infection, hospitalisation, and deaths," Kodan said. The authors acknowledge that the vaccine <keyword id="12595121" type="General" weightage="20" keywordseo="effectiveness-of-Covaxin" source="Orion">effectiveness of Covaxin</keyword> estimated in this study is lower than the efficacy reported by the recently published phase 3 trial. They noted that several factors may be responsible for the lower vaccine effectiveness in the latest study. The researchers said this study population only included hospital employees who may have a higher risk of exposure to COVID-19 infection than the general population. The research was conducted during the peak of India's second wave of COVID-19 with high test positivity rates for both hospital employees and residents of Delhi, they said. Prevalence of circulating variants of concern, especially Delta, may have also contributed to the vaccine's lower effectiveness, according to the researchers. The authors acknowledge several limitations to their study. The study does not estimate the vaccine effectiveness against hospitalisation, severe disease, and death, which require further assessment, they noted. Also, the study was not designed to estimate vaccine effectiveness for different time intervals after vaccination or to determine if vaccine effectiveness changed over time, the researchers added. Covaxin shows 50% effectiveness against symptomatic COVID-19 in real-world assessment, says Lancet study

"The global burden disease published in Lancet shows that in 2019, air pollution was the fourth leading cause of mortality and third for disability-adjusted life years. Air pollution is a silent killer which makes it difficult for the policymakers to understand," he said. "We must focus more on the chronic effects of pollution such as diabetes, lower respiratory tract disease, lung cancer, stroke or neonatal deaths - there is enough data to say that around 20 to 40 per cent of deaths in these diseases are caused by air pollution," Guleria said. He was speaking at the second Good Air Summit organised by the Integrated Health &amp; Wellbeing (IHW) Council. Guleria also said that more than smoking tobacco, pollution contributes to developing diseases such as lung cancer, <keyword id="73190" type="COMPANY" weightage="20" keywordseo="COPD">COPD</keyword> and heart disease. "These are acute effects," he added. The AIIMS chief also shared that within a few days of spike in air pollution levels, the influx of patients increased in the hospital's OPDs. "Experience in AIIMS shows that whenever there is a spike in air pollution, the number of patients in OPDs increases in the next 5-6 days," Guleria said. Union Minister for Environment, Forest and Climate Change, Prakash Javadekar, also attended the summit. Accepting that pollution is a sensitive issue and a serious challenge, he expressed hope that the newly-constituted <keyword id="10260083" type="COMPANY" weightage="20" keywordseo="commission-for-air-quality">Commission for Air Quality</keyword> will reduce pollution in the national capital. "We introduced the idea of measuring air quality and brought the concept of AQI. The Sameer app of the Central Pollution Control Board provides hourly update of the national Air Quality Index (AQI). I am hopeful that the Commission for Air Quality Management will reduce pollution further," Javadekar stated. Meanwhile, Kamal Narayan, CEO, IHW, complained that air pollution did not receive the kind of response it required either from the government or the citizens, despite being an evident health emergency. "The data from 'State of Global Air 2020' shows that 1.67 million or 17 lakh people die annually due to air pollution that comes to around 4,700 deaths per day. Which means around half a million more people fell prey to air pollution last year than the number of lives claimed by Covid-19 virus till date, globally. However, despite being a super-killer and an evident health emergency, air pollution did not get the emergency response either from the system, or from the people," he said. Commenting on the issue, Swatanter Kumar, retired Supreme Court judge and former chairperson of <keyword id="2462566" type="COMPANY" weightage="20" keywordseo="The-National-Green-Tribunal">the National Green Tribunal</keyword> (NGT), said that protection of the environment is a constitutional obligation of both the government and the citizens. "Protecting the environment is a constitutional obligation for the citizens and a constitutional duty of the governments. We don't need more laws but better implementation of the existing frameworks. We need holistic, pragmatic and practical solutions for intergenerational equity and examine the sources of cleaner energy as well," he said.
