<ul><li>The All India Institute of Medical Sciences (AIIMS) and the Indian Council of Medical Research (ICMR) have issued new guidelines for the treatment of patients with COVID-19.</li><li>Patients with moderate COVID-19, those with respiratory distress, should be put under hospital care until their oxygen saturation levels are at reasonable levels.</li><li>Here is the full list of guidelines by AIIMS and ICMR for moderate COVID-19 patients — simplified. </li></ul>The All India Institute of Medical Science (AIIMS) and the Indian Council of Medical Research (ICMR) have jointly issued new guidelines for the treatment of COVID-19 patients depending on the severity of a case — mild, moderate, or severe. And, in the case of moderate infections, India’s apex hospital has recommended that the concerned patient should be admitted into a hospital ward. <cmsannotation type="cmsannotation" annotationid="How to identify a patient with moderate COVID-19 infection?" comment="subhead" text="How to identify a patient with moderate COVID-19 infection?">How to identify a patient with moderate COVID-19 infection?</cmsannotation> According to AIIMS, a patient is classified as having a moderate case of COVID-19 if their respiratory rate is more than 24 breaths per minute and their SpO2 levels — level of oxygen saturation — are between 90% to 93% on room air. This means the patient is likely to be experiencing breathlessness and needs to be in hospital care. <cmsannotation type="cmsannotation" annotationid="Recommendations to help with oxygen support" comment="subhead" text="Recommendations to help with oxygen support">Recommendations to help with oxygen support</cmsannotation> The targeted levels of oxygen saturation, or SpO2, should be between 92% to 96%. For patients with chronic obstructive pulmonary disease (COPD), the recommended SpO2 level is between 88% to 92%. AIIMS and ICMR recommend a non-rebreathing face mask for oxygenation. All patients that require supplemental oxygen therapy should also be awake proning. This is when healthcare workers have patients lie on their stomachs to help with the oxygenation process. According to the guidelines, a patient’s position shoud be changed approximately every two hours. <cmsannotation type="cmsannotation" annotationid="Anti-inflammatory or immunomodulatory therapy" comment="subhead" text="Anti-inflammatory or immunomodulatory therapy">Anti-inflammatory or immunomodulatory therapy</cmsannotation> Immunomodulatory therapy treats diseases targeting the immune system and anti-inflammatory therapy reduces internal swelling. AIIMS and ICMR recommend giving 0.5 to 1 mg/kg of methylprednisolone, which is usually used to treat <a href="https://medlineplus.gov/druginfo/meds/a601157.html">severe allergic reactions</a>, divided over two doses — or an equivalent dose of dexamethasone, also used to treat severe allergic reactions — for five to 10 days as injections. Patients can be switched over to oral medication once they show improvement or are stable. <cmsannotation type="cmsannotation" annotationid="Anticoagulation" comment="subhead" text="Anticoagulation">Anticoagulation</cmsannotation> Anticoagulants, also known as blood thinners, are used to prevent blood clots in COVID-19 patients. The guidelines recommend that moderate COVID-19 cases should be given the conventional dose of prophylactic unfractionated heparin, normally used to treat deep vein thrombosis prophylaxis — blood clots, usually in the legs. They can also be given Low Molecular Weight Heparin (LMWH), like 0.5 mg/kg of enoxaparin every day. The nationwide Covid vaccination drive had commenced on January 16 and a total of 447 adverse events following immunisation (AEFI) were reported during the first two days, with most of them being minor along with three cases of hospitalisation. "Even if a person takes Crocin or Paracetamol, they may develop allergic reactions. There is nothing to worry about. There is no side-effect which would result in death," Dr Guleria said. He added, "Main side-effects are body ache, fever, pain at injection site which subside in 1-2 days and take place in less than 10 per cent of people. If the side-effects are severe, there could be skin rashes, nausea, difficulty in breathing." The AIIMS Director said that people must come forth to get vaccinated if they want <keyword id="3658" type="LOCATION" weightage="20" keywordseo="India">India</keyword> to come out of the Covid-19 pandemic, decrease the mortality rate, bring the economy back on track and reopen schools. 'Vaccine won't kill you': AIIMS Director allays fears

Indian vaccines offer lot of hope, due to various factors including a good safety signal, display of immunogenicity and capability of using the current storage infrastructure, said AIIMS Director Randeep Guleria in an exclusive interview with IANS. Recently, drug company <keyword id="44612" type="CITY" weightage="20" keywordseo="Pfizer">Pfizer</keyword> announced that its vaccine candidate for Covid-19 is more than 90 per cent effective at preventing infection. This was welcomed as a major step in the fight against the virus, which has unleashed havoc on the global economy. Guleria added that the Indian Covid-19 vaccine has a huge advantage over the Pfizer vaccine, which is required to be kept at a much lower temperature, and hence, cold storage for other vaccines can be used for the Indian vaccine too. Q: We have not seen any major issue with Indian vaccines at the trials stage, are you satisfied with progress made so far? A: Indian vaccines have a lot of hope and there are two or three reasons for that, one is that their safety signal is very good and immunogenicity is also being shown, and one is now looking at the results of Phase-3 trials, which will take some time, but I am hopeful we will have good amount of data because multi-centric studies is being done. We quickly will be able to generate some interim analysis to show in Phase-3 trials this vaccine can give good efficacy. A: What Pfizer has got is something very encouraging for other vaccines researchers also, because that shows there is a potential of good immunity by the vaccines. Therefore, other vaccines should also do well. Therefore, it is a big news for all vaccine developers. We can hope for a 90 per cent efficacy. The advantage for the Indian vaccine is that it is required to be kept at a much lower temperature. The Pfizer vaccine currently have to be kept at minus 70 degrees Celsius, therefore it will be very difficult to vaccinate people in lower- and middle-income countries. Because having a vaccine stored and transported at minus 70 degree Celsius not only needs huge investment but also needs huge planning, because most vaccines are not kept at that low temperatures. The Indian vaccines are going to be kept at a temperature of minus 20 degrees Celsius and this is available for most vaccines. As most vaccines a have cold storage facility, these can be used for this vaccine also. Therefore, the investment is not much. If we were to use a vaccine which require storage at minus 70 degrees Celsius, then one would require whole new cold chain development. I do not see that happening in lot of countries. Q: Is 90 per cent efficacy is being aimed at by Indian companies developing vaccine against Covid-19? A: We would like to have as much efficacy as one can. The WHO says if any vaccine has more than 50 per cent efficacy, it is good enough. We would definitely like to have an efficacy of 80-90 per cent. So, let us hope that is what we are able to achieve. The trial will take place in 525 subjects at various sites, including <keyword id="40458" type="COMPANY" weightage="20" keywordseo="AIIMS">AIIMS</keyword>, Delhi, AIIMS, Patna and Meditrina Institute of Medical Sciences, Nagpur. The Subject Expert Committee (SEC) on COVID-19 of the Central Drugs Standard Control Organization (<keyword id="6375144" type="COMPANY" weightage="20" keywordseo="cdsco">CDSCO</keyword>) on Tuesday deliberated upon Hyderabad-based Bharat Biotech's application seeking permission to conduct phase II/III clinical trials to evaluate the safety, reactogenicity and immunogenicity of Covaxin jabs in children aged 2 to 18 years. "After detailed deliberation, the committee recommended for conduct of proposed phase II/III clinical trial of whole virion inactivated coronavirus vaccine in the 2 to 18 years age group subject to the condition that the firm should submit the interim safety data of phase II clinical trial along with DSMB recommendations to the CDSCO before proceeding to phase III part of the study," a source said. Earlier the proposal was deliberated in the SEC meeting dated February 24 and the firm was asked to submit a revised clinical trial protocol.
