How to apply for a duplicate voter ID

Collect EPIC-002 form from the nearest electoral office.

Attach the supporting documents asked in the form and submit the application at the electoral office.

You will get a reference number to track the application status.

The electoral officer will verify your application and issue a duplicate voter ID card to you.

You will receive a notification when your card is ready and you can collect it in person from the electoral office.
