Brain stroke is the second most common cause of death in <keyword id="3658" type="General" weightage="100" keywordseo="india" source="Orion">India</keyword> with one patient succumbing to the disease every four minutes, a top health expert flagged on Thursday. Padma Shri awardee Dr (Prof) MV <keyword id="272876" type="General" weightage="50" keywordseo="padma-srivastava" source="Orion">Padma Srivastava</keyword>, who is the most renowned neurologist in the country and is a Professor of Neurology at All India Institute of Medical Sciences, participated in the celebration of International Women's Day event at Sir Ganga Ram Hospital today. Delivering a keynote address at the event titled, "Stroke care and its primary preventive methods in poor resource settings in India", Dr <keyword id="5915" type="General" weightage="80" keywordseo="srivastava" source="Orion">Srivastava</keyword> said, "Stroke is the second most common cause of death in India. About 1,85,000 strokes occur every year in India with nearly one stroke every 40 seconds and one stroke death every 4 minutes." She further referred to the Global Burden of Diseases (GBD) and said that most incidents of stroke were recorded in the country. "India bore most of the burden of stroke with 68.6 per cent incidence of stroke. 70.9 per cent stroke deaths and 77.7 Disability Adjusted Life Years (DALYs) lost. These figures are alarming for India with many living in poor resource settings. Another alarming and important finding of the GBD 2010 stroke project is 5.2 million (31 per cent) strokes were in children aged less than 20 years. The stroke burden is greater in India and more so among younger and middle-aged people," she said. The health expert flagged the lack of necessary infrastructure to deal with the alarming data in the country. "In spite of these alarming figures, many Indian hospitals lack the necessary infrastructure and organization required to treat stroke patients quickly and efficiently and do not deliver adequate stroke care. The stroke services across the country especially in public sector hospitals are deficient in many aspects," Dr Srivastava said. The Padma Shri awardee further listed the solutions to boost the infrastructure needed to tackle the rising numbers. "One of the solutions for this deficiency in rich and poor resource settings in India is to adopt Telestroke models in poor resource settings. Implementation of Telemedicine / Telestroke facilities is an important step for bridging the economically and geographically challenged and underprivileged sections of the society," she said. This program also included inspirational talks by three distinguished faculty members of the hospital. The members included Dr Jayashree Sood, Chairperson, Institute of Anesthesiology, who spoke on how to maintain the balance between work and life, especially for women, <keyword id="16745456" type="General" weightage="50" keywordseo="prof-kusum-verma" source="Orion">Prof Kusum Verma</keyword>, Advisor Cytopathology who spoke on her experiences mitigating professional challenges and <keyword id="100507" type="General" weightage="50" keywordseo="padma-bhushan" source="Orion">Padma Bhushan</keyword> Dr Neelam Kler Chairperson of Department of Neonatology who talked about her belief in the saying 'Never say Never'. One patient succumbs to brain stroke every four minutes in India says expert

New Delhi, The phase-three human clinical trial of indigenously developed anti-coronavirus vaccine candidate Covaxin began at the AIIMS here on Thursday with Dr M V Padma Srivastava, the chief of Neurosciences Centre at the premier institute, and three other volunteers receiving the first dose. 'Covaxin' is being developed by <keyword id="149108" type="COMPANY" weightage="20" keywordseo="Bharat-Biotech">Bharat Biotech</keyword> in collaboration with the <keyword id="188563" type="ORGANIZATION" weightage="20" keywordseo="Indian-Council">Indian Council</keyword> of Medical Research (ICMR). Dr Srivastava was the first one to receive the shot, which would be given to around 15,000 volunteers at the AIIMS over the next few days, sources said. The first dose of 0.5 ml intramuscular injection was given to four volunteers. They were under observation for two hours and will be monitored for the next few days, a source said. When contacted, Dr Srivastava said, "Covaxin is the first indigenously-developed anti-coronavirus vaccine and on top of that, my institute is participating in the trial. I am honoured to be the first volunteer to receive the shot. I am happy to be a part of such a great cause. I am perfectly fine and I'm working." As part of the trial application, a dose of 0.5 ml would be given on day 0 and on day 28, sources said. The phase-three randomised double-blind placebo-controlled multi-centre trial would cover around 28,500 subjects aged 18 years and above. It would be conducted in around 25 sites across 10 states. The trial has already started at a few sites. The Bharat Biotech has been given the permission for conducting phase-3 human <keyword id="83911" type="ORGANIZATION" weightage="20" keywordseo="Clinical-trials">clinical trials</keyword> of the Covaxin from the Drugs Controller General of India (DCGI) The safety and immunogenicity data of the phase one and two trials were submitted to the central drug regulator. The Hyderabad-based firm, while applying for phase-three trial, stated that the vaccine was well-tolerated in all dose groups and no serious adverse events have been reported. The most common adverse event was pain at the injection site, which resolved transiently, the source said. Besides, Covaxin, four other vaccines are under different phases of clinical trial in India with the Serum Institute of India conducting phase-three trial of the Oxford-Astrazeneca COVID-19 vaccine while the indigenously developed vaccine by Zydus Cadila has completed phase-two clinical trial in the country. Dr Reddy's Laboratories will soon start combined phase two and and three clinical trials of the Russian COVID-19 vaccine Sputnik V in India. Also, Biological E. Ltd has started early phase 1 and 2 human trials of its COVID-19 vaccine candidate, officials had said last week.

	All three were in sublime touch last year and brought the country laurels in their respective sports. 
 Sania enjoyed the most sensational run, clinching nine titles -- including two Grand Slams last year with Swiss great Martina Hingis. The exceptional performance propelled her to the top of the <keyword id="935171" type="UNKNOWN" weightage="20" keywordseo="WTA-doubles-rankings">WTA doubles rankings</keyword>. 

	The 29-year-old is currently competing in the Australian Open in Melbourne, where she has reached the women's doubles quarterfinals with Hingis. 
 Saina has also been in spectacular touch, winning silver medals at the All England and World Badminton Championship last year besides clinching the India Open Super Series and <keyword id="2039194" type="UNKNOWN" weightage="20" keywordseo="Syed-Modi-Grand-Prix-Gold">Syed Modi Grand Prix Gold</keyword>. 

	Deepika, on the other hand, won a silver at the Archery World Cup Final besides bronze medals at the World Cup Stage II and the Asian Team Championships. 
 

	Saina, who is in the country to compete in this week's Syed Modi Grand Prix Gold Tournament, said the honour has left her surprised. Interestingly, the 25-year-old had expressed her displeasure at being ignored for the award last year. 
 
 

	"It is a great honour and I want to thank the Ministry for giving me the award. I am very happy with the way the last year has gone, I have been performing well. I reached World No.1 last year and now I am World No.2. I am sure this award will give me more motivation to do well further this year," Saina said. 
 

	"It came as a complete surprise to me. I had no clue. I did not expect at all. There was no news of it at all. When I think that I am just 25 and I have received this award, it makes me feel great. But it is still a long way to go." 
 

	Saina was left disappointed after the Sports Ministry decided to reject her application for the award in January last year, citing norms. 
 

	The ministry had picked two-time Olympic-medallist wrestler Sushil Kumar for the honour last year. 
 

	Saina was bestowed the Khel Ratna and Padma Shri in 2010 and also received the Arjuna Award in 2009. 
 

	The 21-year-old Deepika, meanwhile, had also won the team silver along with Laxmirani Majhi and Rimil Buriuly in the World Championships.

COVID-19 can affect almost all organs besides the lungs and the initial symptoms may be totally unrelated to chest complaints, according to experts at AIIMS here. They stressed that classification of cases into mild, moderate and severe categories based just on respiratory symptoms should be relooked to include other organ involvement. Experts from the institute, including its director Dr <keyword id="209402" type="PERSON" weightage="20" keywordseo="Randeep-Guleria">Randeep Guleria</keyword>, Dr MV Padma Srivastava, head of department of Neurology, Dr Ambuj Roy, Professor of Cardiology and Dr Neeraj Nischal, Associate Professor in the department of Medicine during their weekly '<keyword id="9832228" type="COMPANY" weightage="20" keywordseo="national-clinical-grand-rounds">National Clinical Grand Rounds</keyword>' organised in collaboration with NITI Aayog discussed various possible extra-pulmonary complications of COVID-19. Dr Guleria said eight months into COVID-19, a lot has been learnt and accordingly strategies are being changed from time to time. From what we thought of as a viral pneumonia has a lot of other manifestations which are beyond the lungs, he said. "As we have known more and more about COVID-19, we have realised it causes <keyword id="9832227" type="UNKNOWN" weightage="20" keywordseo="many-extra-pulmonary-manifestations">many extra pulmonary manifestations</keyword>. This is basically of the fact that this virus enters into cell through ACE2 receptors which although are present abundantly in upper airways and lungs, they are also present in many organs and thus other organs are also affected. "We have seen many patients who presented with features which are not been predominantly pulmonary but extra pulmonary manifestations," Dr Guleria said. He said though pulmonary manifestations continue to dominate as far as majority of COVID-19 cases are concerned, there is a significant number of patients who would present with manifestations which may be along with pulmonary manifestations or may be without pulmonary manifestations. "We as clinicians need to have a high index of suspicion during this pandemic -- when to suspect, treat and isolate these patients so that we can provide them good quality care," he stressed. The experts in the programme presented a number of cases in which the patients were labelled as asymptomatic or mild COVID but had serious life-threatening extra-pulmonary manifestations like stroke and heart blocks. "What started off as a viral pneumonia is now a multi-systemic disease. However, the jury is out whether SARS-COV2 is the culprit in these extra pulmonary manifestations or just an innocent bystander which happens to be at the wrong place at a wrong time," Dr Nischal said. "So the classification of COVID-19 into mild, moderate and severe cases based only on respiratory symptoms should be relooked into to incorporate other organ involvement," he said. Dr Nischal further underlined that management of such patients with other organ involvement should be as per existing guidelines of that particular complication. The doctor from the Medicine Department also highlighted the case of a 35-year old man who had headache and was vomiting but was found to have life threatening cortical vein thrombosis. When tested, he was found positive for COVID-19. He was asymptomatic for COVID-19 as per existing severity guidelines, Dr Nischal said. "There is a big spectrum of neurological manifestations which have been linked to COVID-19. In some patients, brain is involved and it may lead to clotting, resulting in stroke or can cause infection and lead to encephalitis or other complications which have nothing to do with lungs," Dr Padma said. Dr Ambuj's team presented the case of a patient who came with a very low pulse rate, detected COVID-19 positive and required initial support with some medicines to improve heart rate. "Normally pacemaker is put in such patients to improve their heart rate but based on experiences documented in literature, we realised some of these could be due to COVID-19, so a pacemaker was not put and her heart rate gradually improved with supportive treatment. "Sometimes, the electrical pulse system of the heart which gives rise to heart beat can be affected in COVID-19 and it is self-limiting and improves with time. So these patients who would otherwise normally require pacemaker may not ever need it. "However, more evidence is needed to be definitive about this as it is a new disease and limited information and experience regarding it is available as of now," Dr Roy said.
