Analysing endoscopy reports of 9,350 patients, the authors found that people having upper gastrointestinal conditions -- specifically, ulcers or other types of damage to the lining of the oesophagus, stomach, or upper part of the small <keyword id="700917" type="General" weightage="20" keywordseo="intestine" source="Orion">intestine</keyword> -- were far more likely to develop Parkinson's <keyword id="6361612" type="General" weightage="20" keywordseo="disease" source="Orion">disease</keyword> later in life. These findings, published in the Journal of the American Medical Association Network Open Researchers add to a growing body of evidence that ageing-related or neurodegenerative disease, long thought to originate in the brain, could begin in the gut. Gastrointestinal problems are known to be common in patients suffering from neurodegenerative disorders, the authors said. The researchers at the Beth Israel Deaconess Medical Center, US, said that gastrointestinal troubles experienced by patients of Parkinson's disease often appear up to two decades before symptoms such as tremors or stiffness in arms or legs, which interfere with one's movement and are usually the grounds for diagnosis. They said that digestive issues can involve constipation, drooling, difficulty in swallowing and a delayed emptying of the stomach. Constipation and difficulty in swallowing were strong risk factors related with a more than doubling of Parkinson's disease risk, the authors said. One of the possible biological mechanisms underlying these relationships between the gut and risk of Parkinson's disease could be problems in regulating dopamine, a brain chemical known to play a key role in digestion, they said. The authors also proposed that gastrointestinal conditions could trigger the building up of the protein 'alpha-synuclein', which is how Parkinson's disease presents in the brain. Future research could help understand these mechanisms better, they added.

Other examples of <keyword id="22817243" type="General" weightage="20" keywordseo="neglected-diseases" source="keywords">neglected diseases</keyword> include the West Nile, Zika and Chikungunya viruses. Mpox was discovered in 1958 (in captive monkeys, hence the original misnomer "monkeypox") and the first human case was identified in 1970. Then, for decades it was largely neglected by the scientific and public health communities, regarded as an uncommon infection in remote rural areas in tropical Africa, without relevance for the rest of the world. When a massive mpox outbreak hit developed countries in 2022, increased research funding led to a surge in scientific studies. On just one medical search engine, there's been more research produced since April 2022 than in the preceding 60 years. The 2022-23 global mpox outbreak happened despite repeated calls from African researchers for increased global investment in diagnostic, therapeutic and infection prevention tools for mpox. The WHO has now declared the current upsurge of mpox in central Africa a <keyword id="21952478" type="General" weightage="20" keywordseo="public-health-emergency" source="keywords">public health emergency</keyword> of international concern. This is the highest alert level for events that constitute a public health risk to other countries and requires a coordinated international response. Mpox's recent history is yet another reminder that an infectious disease in one corner of the world should not be regarded as someone else's problem, as it can suddenly start to spread fast and far. It also highlights global inequities in resource allocation and access to vaccines, diagnostics and treatments. These were made available in many industrialised countries and helped curb the global outbreak, but are still largely lacking in most of Africa. <h2>The 2022 outbreak: a total surprise</h2>The disease has been renamed "mpox" but the name of the virus, for now, remains "monkeypox" (MPXV). It is closely related to the smallpox virus. MPXV was considered a <keyword id="21311828" type="General" weightage="20" keywordseo="zoonotic-disease" source="keywords">zoonotic disease</keyword> endemic in parts of central and west Africa. It was acquired mainly through close contact with wild mammals, especially handling bush meat, but there was no sustained human-to-human transmission. Only very occasionally were cases seen outside the endemic areas, due to infected travellers or import of infected small mammals. This changed abruptly in 2022: a massive, rapidly evolving global outbreak caused over 99,000 laboratory-confirmed cases in 116 countries. At its peak in August 2022, over 6,000 cases were reported each week. This outbreak came as a total surprise: most cases were reported from non-endemic countries, mostly in men who have sex with men, who had become infected during recent sexual encounters. Even though most cases were clinically not particularly severe and the death toll stands at just over 200, the global outbreak was declared a public health emergency of international concern by the <keyword id="19896311" type="General" weightage="20" keywordseo="world-health-organization" source="keywords">World Health Organization</keyword> on 23 July, 2022. Fortunately, case numbers soon plummeted due to a combination of behavioural changes and vaccination in at-risk groups. Modern vaccines and antiviral drugs with activity against mpox were made available in many affected high-income countries. These had been developed and stockpiled in the US and Europe, mostly in preparation for a potential bioweapon attack using a poxvirus. The global outbreak in 2022 was caused by clade II of MPXV, which is endemic in west Africa and not as virulent as clade I MPXV, which so far has only been seen in the Congo Basin. That first mpox public health emergency of international concern was declared over in May 2023. Clade II MPXV infections are still occurring globally, but the worst seems over - for now. Complacency would be misguided, as illustrated by the current mpox outbreak that is gathering steam. <h2>Upsurge in Africa</h2>The African region is experiencing an upsurge in mpox cases, which started in 2023. As the continent which includes the areas where mpox has been endemic for a long time, Africa now presents a complex mosaic. The current clade I MPXV (formerly called Congo Basin strain) is more virulent than the clade II (west African) strain, resulting in a higher case fatality rate. The ongoing outbreak has its epicentre in South Kivu province, eastern DRC, and has the potential to fuel a large pandemic. It has a distinct epidemiological pattern with sustained chains of human-to-human transmission, often via the sexual route. It may have increased transmissibility (we don't know yet). The virus which causes it is the newly defined clade Ib lineage. It displays mutations that are the hallmark of human-to-human spread that is estimated to have been happening since September 2023. Case numbers are rising rapidly, even though many suspected cases are likely not tested and thus not counted as confirmed. Complicating matters, a commonly used test was found to miss infections with this lineage of the virus. It affects mostly adults. The case fatality rate is higher than it was in the 2022 global outbreak. Already, this outbreak has resulted in mpox cases occurring in several neighbouring countries, including some (like Kenya), with no previous record of mpox. The challenge is enormous. The eastern DRC is an area beset by multiple problems. This includes natural disasters, violence and infectious diseases including measles, cholera and poliomyelitis for the DRC. In recent years the second-largest Ebola outbreak ever took place in the wider area and, despite the availability of vaccines and treatments, posed considerable challenges. <h2>What needs to happen</h2>Equitable access to diagnostic tests, vaccines and antiviral treatments requires political commitment and financial investments. Scientific investigations are needed to learn more about exposure settings, transmission routes and clinical presentations. It's important to find the best ways to make these interventions. There is no doubt that a disease in one corner of the world can suddenly become a global heath threat. It's time the global health system woke up to this reality.

Mpox outbreak in Africa was neglected - it could now turn into the next global pandemic

Cases of malaria are considerably higher so far when compared to last year. Delhi has reported 363 malaria cases till this month while the figure stood at 294 in the corresponding month in 2023. As per a PTI report, the total malaria cases last year were 426. Most malaria cases are reported from the West Delhi zone. As for Chikungunya, 43 cases have been reported so far. This is nearly double than last year when the figure stood at 23. A total of 65 cases of the virus were reported last year. The most number of chikungunya cases have been reported from the South Delhi zone this year. Last week, the city witnessed a spike of nearly 250 cases of dengue in seven days. As per Municipal Corporation of Delhi's (MCD) data from January till September 21, a total of 1,229 dengue cases have been reported in the city with the death of a 54-year old registered in Lok Nayak Hospital, a senior official at the hospital had confirmed. Delhi reported 651 cases of dengue this month. Last month, a total of 256 dengue cases were reported in the city. The number of cases has become three-digit since August, it said. Last year, Delhi reported 3,013 dengue cases till the corresponding month, a manifold increase from 525 in 2022. A total of 9,266 cases were reported in the entire year while 19 dengue deaths were registered in the city last year. In September alone, the figure stood at 2,141 cases in 2023. The most number of cases this year are reported from the Najafgarg zone at 180. In a week, 45 cases have been reported in the area, the highest of all the zones besides the south zone where an equal number of cases were reported this week. The non-MCD areas governed by other agencies like NDMC, Delhi Cantt, and Railways, reported 312 dengue cases to the total tally, as per the data. To enforce mosquito control laws, the MCD has issued over 1.14 lakh legal notices and 39,338 challans to those who allowed mosquito breeding on their properties. Repeat offenders have been fined Rs 24.82 lakh, and police complaints have been filed against 9,241 people who didn't follow the rules, as per the data. The civic body also plans to increase fogging operations at major upcoming public events like Dussehra and Durga Puja and step up inspections and anti-larval measures in areas with high mosquito populations to curb the infectivity of the virus. Delhi reports over 300 dengue cases in a week; malaria and chikungunya cases on the rise

A new study has uncovered genes that could be related to <keyword id="21415597" type="General" weightage="20" keywordseo="heart-conditions" source="keywords">heart conditions</keyword> and <keyword id="20097972" type="General" weightage="100" keywordseo="Alzheimers-disease" source="keywords">Alzheimer's disease</keyword>, suggesting that the two could have similar origins. Researchers said that evidence increasingly links people having the heart condition with a quicker <keyword id="20097971" type="General" weightage="20" keywordseo="cognitive-decline" source="keywords">cognitive decline</keyword> and a 26% increased risk of dementia, affecting one's memory and thinking, and interfering with daily activities. Alzheimer's disease is the most common form of dementia. <keyword id="20849758" type="General" weightage="20" keywordseo="coronary-artery-disease" source="keywords">Coronary artery disease</keyword> is a condition in which blood flow to the heart is restricted due to narrowing of blood vessels. Even abnormal levels of fat in blood and inflammation could be possible shared risk factors between having coronary artery disease and developing Alzheimer's disease, the researchers at Edith Cowan University, Australia said. However, the processes underlying the link remain unclear. In this study, they employed genetic methods to examine the intricate relationship between heart disease and Alzheimer's disease. The team chose to study seven heart disease-related factors, such as coronary artery disease and angina, and 13 lipids, including cholesterol - both 'good' and 'bad'. Previous studies have long suggested lipids to play a role in the development of Alzheimer's disease, they said. "Our comprehensive analysis reveals a significant and positive global genetic correlation between Alzheimer's disease and three <keyword id="22817116" type="General" weightage="20" keywordseo="lipid-traits" source="keywords">lipid traits</keyword>: LDL ('bad' cholesterol), triglycerides, and total cholesterol," the authors wrote in the study published in the <keyword id="22817118" type="General" weightage="20" keywordseo="International-Journal-of-Molecular-Sciences" source="keywords" meta.keywordsubtype="org" meta.entityname="International Journal of Molecular Sciences" meta.hostid="261" meta.entityid="International Journal of Molecular Sciences">International Journal of Molecular Sciences</keyword>. This suggests that certain lipids, not all, may have a more direct genetic link with Alzheimer's disease, they said. Researchers found that the contribution of high temperatures to poor health and early death due to stroke has increased 72% since 1990 and is likely to continue increasing in the future, thereby highlighting how environmental factors can impact growing stroke burden. Further, for the first time, particulate matter or PM air pollution was found to have the same contribution as smoking towards causing a fatal form of brain bleed, according to the researchers forming the <keyword id="20691740" type="General" weightage="20" keywordseo="Global-Burden-of-Disease" source="keywords">Global Burden of Disease</keyword>, Injuries, and Risk Factors Study (GBD) group. The GBD study, the "largest and most comprehensive effort to quantify health loss across places and over time", is coordinated by the Institute for Health Metrics and Evaluation (IHME), University of Washington, US. Globally, the number of people experiencing a stroke for the first time rose to 119 lakh in 2021 -- up by 70% since 1990 -- while deaths related to stroke rose to 73 lakh, which was up by 44% since 1990, making the neurological condition the third leading cause of death after ischaemic heart disease (short supply of blood to heart) and COVID-19. They added that over three-fourth of those affected by strokes live in low- and middle-income countries. According to lead author Valery L. Feigin from Auckland University of Technology, New Zealand, the rapid growth in the number of people affected by stroke strongly suggested that <keyword id="23228151" type="General" weightage="20" keywordseo="stroke-prevention" source="keywords">stroke prevention</keyword> strategies currently used are not sufficiently effective. "New, proven effective population-wide and motivational individual prevention strategies that could be applied to all people at risk of having a stroke, regardless of the level of risk, as recommended in the recent Lancet Neurology Commission on Stroke should be implemented across the globe urgently," said Feigin. The researchers also estimated that stroke-related liabilities attributable to 23 modifiable risk factors, including air pollution, excess body weight, high blood pressure, smoking and physical inactivity, increased from 100 million years of healthy life lost in 1990 to 135 million in 2021. These risk factors are present in huge numbers in Eastern Europe, Asia and sub-Saharan Africa, they said. The authors also acknowledged substantial progress made in reducing the global stroke burden from risk factors linked to poor diet, air pollution and smoking. Poor health resulting from diets high in processed meat and low in vegetables declined by 40 % and 30%, respectively, while that due to PM air pollution and smoking fell by 20 % and 13%, respectively, they found. The results suggested that strategies for reducing exposure to these risk factors over the past three decades such as clean air zones and public smoking bans have been successful, the authors said. They called for implementing and monitoring the evidence-based recommendations set out in the 2023 World Stroke Organization-Lancet Neurology Commission on stroke for drastically reducing the global burden of stroke in the years to come and improving brain health and overall well-being of millions of people around the world. The recommendations include stroke surveillance programmes that monitor indicators of stroke such as incidence, recurrence, death rates and risk factors in a country, and care and rehabilitation services for people affected by stroke.

A new study by <keyword id="5069783" type="General" weightage="20" keywordseo="Centre-for-Chronic-Disease-Control" source="Orion">Centre for Chronic Disease Control</keyword>, India (CCDC) on cardiovascular risk reduction in South Asia has demonstrated significant long-term benefits of a multi-faceted quality improvement (QI) strategy, comprising of task sharing, and clinical decision support software (CDSS) with electronic health records, in managing type-2 diabetes in India and Pakistan. The study, published in the prestigious journal PLOS Medicine, highlights the sustained impact of QI interventions on <keyword id="21411276" type="General" weightage="100" keywordseo="diabetes-care" source="keywords">diabetes care</keyword> and associated complications over a median follow-up period of 6.5 years. The burden of diabetes Diabetes control remains a global challenge, leading to severe complications such as <keyword id="22908" type="General" weightage="20" keywordseo="heart-disease" source="Orion">heart disease</keyword>, <keyword id="25191" type="General" weightage="20" keywordseo="stroke" source="Orion">stroke</keyword>, <keyword id="16753" type="General" weightage="20" keywordseo="blindness" source="Orion">blindness</keyword> or visual impairment, and kidney and nerve damage. LMICs are often faced with challenges such as inadequate infrastructure and trained healthcare staff with digital literacy. In that respect, the study's findings are crucial for integrating care models sustainably and identifying relevant organizational barriers. Previous studies from high-income countries showed improvements in diabetes care, but their long-term impact on vascular complications and mortality was unclear. This study aimed to address the critical gap in data regarding the long-term effectiveness of QI strategies and their impacts on vascular complications in LMICs. Professor Nikhil Tandon, Department of Endocrinology and Metabolism, All India Institute of Medical Sciences, New Delhi, and senior author of the study explained the relevance of these findings in the context of national efforts to combat non-communicable diseases (NCDs). He said, "The <keyword id="22511956" type="General" weightage="20" keywordseo="National-Programme-for-NCDs" source="keywords">National Programme for NCDs</keyword> [NP-NCDs] operationalized by the Government of India has already a functional portal which provides an electronic health record functionality. Integrating the CDSS in this portal will provide significant incremental benefit to the impact of the National Programme, and bolster governmental efforts to battle the scourge of NCDs including diabetes and hypertension." Speaking about the findings, Prof. (Dr) Dorairaj Prabhakaran, Executive Director, CCDC, and one of the study authors, said, "Through the trial, we've demonstrated that a sustained quality improvement strategy can significantly enhance diabetes care and reduce complications in South Asia. For every 13 patients treated with this strategy, one major adverse cardiovascular event like heart attacks or strokes- or a microvascular event -like eye disease or decline in kidney function- was prevented, showcasing its profound impact on patient outcomes." Professor MK Ali, William H. Foege Distinguished Professor of Global Health, Emory University and one of the key authors of this article, said, "This study is an exemplar of trans-national collaboration for benefits of both, the global south and global north." Study Design and Findings The randomized clinical trial included 1,146 patients with poorly controlled type 2 diabetes across ten outpatient diabetes clinics from January 2011 through September 2019. Participants were randomized to receive either the QI strategy, which involved trained non-physician care coordinators and a clinical decision support software (CDSS) integrated into the electronic health record form, for physicians, or the usual care. Some of the key findings from the study include: <ul><li>At 6.5 years, patients in the QI strategy group were nearly twice as likely to achieve and maintain target levels for HbA1c (&lt;7%), systolic blood pressure (&lt;130mmHg), and LDL-cholesterol (&lt;100mg/dl), compared to the usual care group. </li><li> The incidence of first microvascular complications, such as diabetic eye disease, decline in kidney function, and nerve pain was notably 32% lower in the QI group. </li><li>There was a significant 28% reduction in the combined occurrence of microvascular and macrovascular events in the QI group.</li></ul> Importantly, these benefits were sustained only in clinics that continued the QI strategy for 6.5 years. With diabetes accounting for one in seven deaths in South Asia, the study provides unique insights into the sustainability of QI strategies in achieving diabetes care goals in low- and middle-income (LMICs) and the effects of these improvements on reducing vascular complications and diabetes-related deaths.
